RESUMEN

Duplication or polydactyly of the thumb is among the most common congenital deformities that involve the hand. The purpose of this presentation is to review the Mayo Clinic experience with thumb duplication and to present preferred operative procedures to achieve the best clinical outcome and esthetic appearance for the hand. We propose that a combination or "best of parts", utilizing the components of both duplicates with retention of the ulnar duplicate as the base for reconstruction will provide the most functional clinical result. A clinical grading system and analysis of results from our personal experience during two different time frames provides the scientific support for our conclusions that combination procedures are preferred to simple excisions or to central joining of parts as in the Bilhaut-Cloquet procedure.

Sixty-eight patients underwent radial recession osteotomy for avascular necrosis of the lunate were retrospectively evaluated after an average follow-up period of 52 months. Twenty-five patients had undergone 1 or more additional procedures concurrently for treatment of Kienböck's disease. Pain diminished in 93% of patients, grip strength improved, and wrist motion was preserved; 75% of patients continued in their original occupations, including heavy labor. Surgical complications were uncommon. Four patients subsequently underwent salvage procedures, including 2 total wrist arthrodeses. Three of these 4 patients were receiving workers' compensation. One third of patients demonstrated lunate healing after joint leveling. Preliminary results suggest that concomitant lunate revascularization or vascularized bone grafting may improve the radiographic result. With rare exceptions, radial recession osteotomy relieves pain and improves function in Kienböck's disease.

Trispiral tomography enhances the staging of Kienböck's disease and aids in surgical planning. The clinical records, plain x-rays, and trispiral tomograms of 105 patients with Kienböck's disease were reviewed. When tomograms were used, upward revision of the classification stage was indicated in 73% of patients with stage I or stage II disease and in 10% of those with stage III disease. On tomograms, 91% of patients had lunate fractures, compared with 55% on plain films. The most common lunate fracture seen on trispiral tomograms was a transverse shear fracture that represented lunate collapse; the next most common was a midcoronal fracture that may be displaced, causing fragment extrusion palmarly or dorsally. The most common instability pattern was nondissociative proximal row flexion, seen in stage III. Indices of carpal collapse and ulnar translation may be useful in following up patients, but values vary widely among patients.

Radiopalmar opening wedge osteotomy was used in 11 female patients (12 wrists) with Madelung's deformity who ranged in age from 9 to 31 years. Five patients met the radiographic criteria for generalized dyschondrosteosis. Clinically, there was dissatisfaction with the appearance of the wrist. The principal motion deficits were in radiocarpal extension and forearm pronosupination associated with varying degrees of discomfort with sustained activity. All had a decreased radioulnar angle, lunate subluxation shown radiographically, and various degrees of dorsal subluxation of the ulnar head. Indications for surgery were pain, limited motion, cosmetic appearance, and progressive deformity in two immature patients. Biplanar corrective opening wedge osteotomy of the radius was performed with iliac crest graft. Three patients had an added ulnar recession. Fixation was with a plate or Kirschner wire. Supplementary distracting external fixation was used in six cases. The follow-up period averaged 48 months. Pain relief in all patients was satisfactory. Range of motion and grip strength were not influenced. The radioulnar angle improved by 11 degrees, but lunate subluxation was minimally changed. Length of the forearm and bowed appearance of the forearm were improved. Reduction of the radioulnar joints was improved but remained incongruous.

A retrospective study of 45 patients with symptoms of wrist pain and weakness with clinical evidence of instability is presented. These patients had normal intrinsic interosseous ligaments demonstrated arthrographically or surgically. They were followed for a minimum of 18 months and an average of 5.8 years. All patients demonstrated instability of the proximal carpal row which is referred to as carpal instability non-dissociative (CIND). Seven patients were treated non-operatively and 38 were treated operatively. Surgical management consisted of soft tissue reconstruction directed at the area of instability in 34 cases, joint levelling osteotomy in six, three of whom also had soft tissue reconstructions, and mid-carpal fusion in one. The overall good and excellent results were a disappointing 56% and there was no significant difference between the non-surgical and surgical groups. The best results were found in the ulna-minus CIND patients who underwent a joint levelling osteotomy, with 83% good and excellent results. CIND is associated with extrinsic ligamentous laxity; however, significant difficulty exists in locating the precise areas of maximum pathology. This accounts for the unpredictable results of treatment. As our understanding of the pathomechanics of CIND improves, the treatment will become more specific and the results should improve.

RESUMEN

Extension block splinting (EBS) at the proximal interphalangeal joints of the fingers is a common technique for both primary treatment of reduced dorsal dislocations/fracture-dislocations at that joint and as a rehabilitation method following open reduction of such injuries. It is seldom realized that the method is a classic example of two orthopedic principles: stable arc splinting and early protected motion. As a primary treatment technique, following reduction of the dislocation, EBS is exemplary for an ideal group of cases, competitive for a marginal group of cases, and often unsatisfactory for a questionable group of cases. Demarcation between the groups is made, and the technique of EBS is reviewed.

From a series of 56 patients with triangular fibrocartilage injury, 33 patients with peripheral rim tears not associated with instability of the distal radioulnar joint were identified by arthrography or arthroscopy. Open repair of the peripheral tear produced 11 excellent, 15 good, 6 fair, and 1 poor result (grading based on a Mayo modified Green-O'Brien wrist score). Ulnar recession improved surgical exposure and corrected ulnar variance in 11 patients. A dorsal approach for repair of radial and anterior peripheral rim tears was used in 28 patients. Repair of peripheral tears restored functional integrity to the triangular fibrocartilage, and good to excellent results are reported in 26 of the patients treated.

Traumatic carpal instability implies that the normal relationships of the radiocarpal and mid carpal joint are distorted either statically or during dynamic stress. Normal kinematics of the carpus are dysfunctional. The causes of carpal instability are dissociation of the intercarpal ligaments on either side of the lunate, a so-called scapholunate dissociation or a luno-triquetral dissociation. Carpal instability nondissociative is generally due to a laxity or attenuation of the intrinsic ligaments of the carpus and are associated with deformity of the distal radius. Ulnar translation of the carpus on the distal radioulnar articular surfaces occurs with shear stretching of the origins of the radiocarpal ligaments. The radial styloid attenuation of the ligaments may result in abnormal motions of the carpal bones going from ulnar to radial deviation at which time a catch-up click may occur. Carpal instabilities are usually associated with malalignment of the lunate with respect to the longitudinal axes of the radius and capitate and tends to assume an extended position with scaphoid fractures or injuries to the scapholunate ligament, a palmar flexed position with injuries to the luno-triquetral area or the ulnar capsule.

RESUMEN

This article reviews the outcome of injury of the hand in 25 patients treated at our institution by the open wound technique: wide débridement, drainage, open packing, and delayed closure. The results obtained are superior to those reported in the literature: Among the patients, 84% of the involved hands or fingers were salvaged, 64% had essentially normal hand function at the time of final follow-up, and 92% were able to return to their previous jobs. We propose that aggressive treatment by the open wound technique improves the outcome of high-pressure injection injuries of the hand.

The treatment of scapholunate dissociation remains controversial. Although good to excellent results have been reported using a variety of surgical techniques, they have often not been confirmed by other authors. Limited wrist mobility and the likelihood that degenerative changes will progress appear to be inherent in most approaches. The ideal treatment is to restore the normal anatomy and, with it, the function of the wrist. Destroying a normal joint by even a limited fusion as a means to provide stability to an abnormal joint is counter to our usual approach to disease. It is for that reason that more reliable soft-tissue reconstructive procedures are necessary. Those interested in arthroscopy are pursuing endoscopic methods of repair, but, at present, little progress is apparent. The physician who solves this problem with an uncomplicated and reliable surgical solution will have indebted us all.

RESUMEN

A review of the relatively small body of literature about digital nerve compression injuries has been supplemented by the author's experience and interest of over 40 years. The principal lesions involved are of two types: (1) fibrosis about, around, and in the nerve; and (2) reactive hyperplasia of the nerve support elements and sometimes of special end organs, Pacinian corpuscles, and even of the nerve fibers themselves. Nerve signs and symptoms often accompanied by a mass in chronic cases are diagnostic. The most important features of the topic are the recognition by all of the risk elements, embodied in many personal and professional devices and techniques. These risk elements are present in all parts of our society, including home and occupational, avocational, and medical situations. Treatment is best provided by awareness of risk and avoidance of risk activities, particularly repetitive risk activities. Surgery is preferably avoided, but when indicated it is effective. It consists principally of lysis and protective positioning of the damaged nerve or, on occasion, of excision of terminal or end-organ lesions. More information must be gathered about these exceedingly common but notably under-reported problems.

RESUMEN

Klippel-Trenaunay syndrome is a rare congenital malformation characterized by a large angiomatous nevus; hypertrophy of soft tissue or overgrowth of bone, or both; and venous varicosities. The cases of 108 patients who had a diagnosis of this syndrome between 1956 and 1990 were reviewed. One hundred and twenty-six anomalies were found in twenty-nine patients, each of whom had one to thirteen malformations of the fingers or toes, or both. The ratio of female patients to male patients was approximately two to one. Twenty-six patients had macrodactyly affecting one to six digits; nine, syndactyly involving two or more digits; five, metatarsus primus varus; two, clinodacytly; two, polydactyly; one, camptodactyly; and one, a congenital trigger finger. Thirty-three of the 126 anomalies were in extremities that had no nevi, varicose veins, or generalized hypertrophy.

The current status of cumulative trauma disorder of the upper limb is examined from the standpoint of its long history, its current controversies, and the philosophic basis of the present laws, which control management of the condition. The future needs that should permit cumulative trauma disorders to be better understood, better managed, and better legislated are also addressed.

Six men had concurrent scapholunate dissociation and Kienböck's disease, a finding suggestive of a common cause. Five patients attributed the onset of wrist pain to a single traumatic event. Three had x-ray evidence of scapholunate dissociation before the onset of lunate osteonecrosis. Biomechanical factors that may be of significance are ulnar minus variance, lesser compliance of the triangular fibrocartilage, ulnar translation of the carpus at impact with shear fracture through the lunate, and disruption of the scapholunate interosseous membrane occurring under similar stress. Lunate osteonecrosis may depend on a susceptible vascular pattern or intraosseous injury or both. The development of lunatomalacia complicates an already unstable wrist. Treatment options vary according to the clinical stages of each condition, although contrasting treatments have not established optimal care.

Uncontrolled maternal diabetes frequently results in congenital anomalies. This report describes an infant of a mother with diabetes born with thumb hypoplasia and contralateral thumb duplication without other associated anomalies. Maternal diabetes should be part of the etiologic differential diagnosis of thumb anomalies. The concomitant occurrence of thumb duplication and hypoplasia, apparently as a result of the same systemic insult, suggests a close relationship between hypoplasia and duplication.

Nine patients with the diagnosis of unilateral Preiser's disease were seen between 1970 and 1987. The mean age of four male and five female patients was 37 years (range, 20 to 70 years). The diagnosis was based on radiographic evidence of sclerosis, fragmentation, erosion, and collapse of the proximal pole of the scaphoid. Onset was usually insidious but two had a preexisting radial hypoplasia, a third had a modest scaphoid malunion, and a fourth had a recent fall. Treatment consisted of scaphoid excision and silicone rubber prosthetic replacement in three, debridement of necrotic bone fragments in one, and different periods of cast support and observation in the remaining five. No relationship to ulnar variance was seen. Mean follow-up was 6.9 years. Of the scaphoid implants, two had subluxated, but only one was painful. In the more conservatively treated group, pain and function were only modestly limiting in four, who returned to their original occupations as did the one treated by curettage of necrotic bone in the proximal pole. One patient (case 4) was severely incapacitated because of associated severe congenital anomalies of the lower extremities. Polyaxial tomography permitted a better assessment of the degree of involvement and carpal alignment. Preiser's disease is a rare affliction of the carpal scaphoid which may involve the entire bone in avascular changes but primarily leads to fragmentation and collapse of the proximal pole. A conservative approach to treatment is favored based on this experience.

Helping parents to decide what is best for their child is a necessary portion of medical management for the injured, anomalous, or sick child. It involves collecting information about, consideration of, and giving advice concerning much more than medical details. Certain steps are suggested in offering this help: (1) obtaining comprehensive medical, socioeconomic, and psychologic information; (2) packaging this information to inform fully and satisfy the parents/surrogates of the child; (3) confirming the family's informed participation in the treatment decision; and (4) reinforcing that decision with a very supportive and communicative perioperative period.
