RESUMEN

The engraftment of a skin sheet together with transplantation of dermal equivalent was studied on rats. The skin sheet was taken as a source of epithelization material in unhealing wound. The process of wound healing was evaluated by changes of matrix metalloproteinases (MMP) activity levels in wound fluid. It was shown that results of skin sheet transplantation could be predicted by monitoring of wound fluid MMP-2 and MMP-9 activity levels. It was determined that skin sheet engraftment appeared at medium values of wound fluid MMP-2 and MMP-9, and transplanted skin sheet was lysed at high and low values.

RESUMEN

One of the problems in wound treatment optimization is the necessity of an effective and objective method of laboratory wound process monitoring. In present study the current wound process was estimated by changes in MMP-2 and MMP-9 activities in the wound fluid. An original model was used in this work to study correlation of morphological structure of the wound with the activities of MMP-2 and MMP-9 in wound fluid at various types of wound process. Protein fractions of the coelomic liquid from regenerating sea star Asterias rubens were used as the substances changing the wound process. The correlation of wound process with MMP-2 and MMP-9 activities in wound fluid was revealed on the basis of correlation analysis.

RESUMEN

Investigation of the cell (osteocytes, fibroblasts and keratinocytes) adhesion and penetration in pores of the titanium pylon in vivo on the laboratory animals (Wistar rats) has been performed. The titanium pylon has been implanted in bone of a rat's thigh residuum. Electronic scanning and morphological analysis demonstrated the certain integration of the pylon with the surrounding tissues. The latter opens a possibility for development of direct skeletal attachment of limb prostheses.

RESUMEN

Modeling in vivo (on Wistar rats) was carried out by the method of complete integration of the prosthesis of an amputated extremity with a residuum; in this procedure the prosthesis is fixed to the residuum with a metal pylon one end of which is implanted into the bone of the residuum, while the other end traverses the residuum tissues and skin 5-7 cm above the residuum surface. This procedure includes not only successful implantation in the residuum bone, but also the possibility that the pores in the metal pylon can be filled with skin cells in the area of the pylon which traverses the tissues of the residiuum. The porous titanium pylon was implanted into the bone of experimental animals with amputated extremities. Penetration of bone and skin cells into the pores of the studied material was demonstrated, which provides tighter fixation in the bone and shows promise for the development of a natural cutaneous barrier to infection.

RESUMEN

The article is dedicated to a new method of treatment of pancreatic cancer, developed by researchers of St. Petersburg Research Institute of Roentgenology and Radiotherapy. The method consists in slowing down blood flow in the tumor by means of selective embolization of its arteries using a mixture of the chemiotherapeutic agent gemcitabine (Gemzar) in the oily radio-paque medium Lipodol Ultrafluid. Upon entering microcirculatory vessels, the agent diffuses into pancreatic tissue. This tumor chemoinfiltration provides prolonged contact between tumor tissue and the chemoembolization agent, thus allowing strong antitumor effect without high systemic toxicity. Experimental studies on 12 dogs have proved that oil arterial chemoembolization with lipiodol does not cause morphological changes in the pancreas, and is only manifested by short and reversible hyperenzymemia.

RESUMEN

A simultaneous study of wound proteolytic activity and morphological picture of the first stages of wound healing on rat deep wound model has been shown. The process of wound healing can be evaluated by dynamics of matrix metalloproteinase activities in wound fluid. Changes in activities of different matrix metalloproteinases correlate with different stages of healing. Implantation of polychlorvinyl camera in the wound makes it possible to obtain the volume of wound fluid sufficient for a complex evaluation of healing at the initial stages of wound process.

RESUMEN

Vascular anastomotic thrombosis is one of the most frequent complications after segmental transplantation of the pancreas (STP). We propose a new type of vascular anastomosis to reduce the rate of vascular thrombosis following STP. For this purpose we used double arterial-double venous anastomosis (DADVA). Four different types of vascular anastomosis used for STP were studied and compared. The rate of vascular anastomotic thrombosis was evaluated in correlation with the type of vascular anastomosis used. Traditional vascular anastomosis (TVA) was complicated by vascular thrombosis in 51.9% of cases. DADVA reduced rate of vascular thrombosis to 2.6% (P < 0.01).

BACKGROUND: Blood flow in the pancreas before and after transplantation has not been studied sufficiently. Blood flow in the pancreatic transplant as a function of type vascular anastomosis used has not been explored. AIM: The objective of our study was to study blood flow in the intact pancreas prior to harvesting and at different time periods after transplantation. Two different types of vascular anastomosis were used for segmental transplantation of the pancreas. Blood flow in the transplanted pancreas was compared in this two experimental groups. METHODS: Study was conducted on 61 mongrel dogs. Autotransplantation of a pancreatic segment was performed to the left iliac vessels. There were two randomized experimental groups. In the first group (26 animals) traditional vascular anastomosis was used to revascularize pancreatic graft. Proximal end of splenic artery was anastomosed to iliac artery in "end to side" manner. Portal end of splenic vein was anastomosed to iliac vein in "end to side" manner as well. Splenic ends of splenic artery and vein were ligated. In the second group (35 animals) double arterial double venous anastomosis was used. For this purpose, after completing traditional vascular anastomosis, second arterial anastomosis was created distal to the first one between distal end of splenic artery and iliac artery in "end to side" fashion. Second venous anastomosis was performed distal to the first one between splenic end of splenic vein and iliac vein in "end to side" fashion. Blood flow in the pancreatic segment was measured using direct blood flow measurement method and electromagnetic flowmetry. After transplantation of pancreatic segment, blood flow was measured in the pancreatic graft 3 min, 1 h, 1 and 6 months following transplantation. RESULTS: Blood flow in the pancreas before harvesting was 0.69 +/- 0.01 ml/min/g. Blood flow in the pancreatic transplant in traditional vascular anastomosis group was higher than blood flow in the normal pancreas (1.13 +/- 0.05 ml/min/g after one month and 1.00 +/- 0.05 ml/min/g after six months (p < 0.01)). Blood flow in the double arterial double venous anastomosis group was not statistically significantly different from blood flow in the normal pancreas at 1 and 6 months after transplantation (0.70 +/- 0.02 and 0.67 +/- 0.02 ml/min/g accordingly). CONCLUSION: Double arterial double venous anastomosis provided more physiological blood flow to the graft, than traditional vascular anastomosis.

Twenty-nine patients with type I diabetes mellitus were followed up for at least a year after intramuscular allotransplantation of cultured fetal pancreatic islet cells. The patients were divided into 2 groups with different types of proteinuria. Group 1 (prenephritic stage) consisted of 16 patients with transitory proteinuria and group 2 (nephritic stage)-of 13 with constant proteinuria. Serum beta 2-microglobulin (beta 2 mg) was radioimmunoassayed before transplantation and 7 weeks, 1, 3 to 6 months, and over a year after it. The levels of beta 2 mg appreciably varied in the two groups. In group 2 they were increased on day 7 but later virtually did not differ from the control. In group 1 one patient developed a decreased beta 2 mg level starting from the first up to the sixth months after allotransplantation. The results indicate that diabetic nephropathy may be partially reversed by allotransplantation and hence, this method is recommended for patients with transitory proteinuria.

Antinuclear antibodies were detected and studied by indirect immunofluorescence in the sera of patients with insulin-dependent diabetes mellitus (IDDM) subjected to allotransplantation of pancreatic islet cell culture (PICT). Continuous cell lines L-41 (derivative of J-96) and HEp-2, a sensitive substrate for the detection of antinuclear antibodies, were used after enzyme treatment. Antinuclear antibodies were detected in 19.0% of diabetics in comparison with 1.5% in healthy volunteers.PICT evidently did not influence these antibodies, although a rise of hemagglutinins against sheep red cells after PICT was previously observed. The data indicate that the nuclear antigens of HEp-2 and L-41 against which antinuclear antibodies are directed are different from those found in human fetal pancreatic islet cells which have been implanted. A possible NAg maturity deficiency in fetal tissue may be another cause of their failure to affect the antinuclear antibodies.

The authors measured IgG, IgM, IgA, and natural sheep hemagglutinin (NSH) before and after fetal pancreatic culture allotransplantation to patients with insulin-dependent diabetes mellitus (IDDM). Patients with IDDM were found to have higher NSH levels than healthy donors. Secondary transplantations caused a more pronounced increase of NSH level than the initial ones.

Within the first 6 weeks after kidney transplantation the diagnostic and prognostic importance of some characteristics of partial renal function was studied in 48 patients with postoperative acute renal failure (ARF), precrisis, acute rejection of the transplant, and a crisis-free course of the postoperative period. It was established that the numerical data and the diagnostic importance of the characteristics under study are determined by the degree of the preceding ischemic lesion of the kidney. Some functional criteria of the crisis irreversibility and the effect on its temporary parameters of postoperative ARF were defined.
