RESUMEN

PURPOSE: To establish the effectiveness of vitrectomy and gas tamponade for treating retinal detachments due to peripheral retinal breaks with an associated macular hole and to discover the status of the macular hole at long-term follow-up. METHODS: Twenty-three consecutive patients with combined peripheral break and macular hole retinal detachments were treated by pars plana vitrectomy. The main outcome measures were reattachment of the retina and status of the macular hole. RESULTS: Seventy-eight percent of the operations were successful in reattaching the retina initially, improving to 87% after two patients had another operation. Three patients declined further surgery. Long-term follow-up of macular hole status was possible in 16 cases. Closure rate was 31%. CONCLUSION: Pars plana vitrectomy with gas tamponade is an effective method of treating this form of retinal detachment. Some macular holes close after this surgery.

RESUMEN

PURPOSE: To establish the role of preoperative subconjunctival mydricaine and diclofenac 0.1% in maintaining mydriasis during vitrectomy. METHODS: Fifty-seven patients were entered into the study. All were given cyclopentolate 1% and phenylephrine 2.5% preoperatively. Each patient was randomly allocated to one of three groups. In Group 1, patients received mydricaine by subconjunctival injection and diclofenac 0.1% topically preoperatively. In Group 2, patients received only subconjunctival mydricaine. Group 3 patients received only topical diclofenac preoperatively. Pupil diameter was measured with calipers before and at the end of the operation. RESULTS: There was no statistically significant difference in the change in pupil size between Groups 2 and 3. In all patients in Group 1 (who received both subconjunctival mydricaine and diclofenac preoperatively), pupil size was either maintained or increased after vitrectomy. This result was statistically significant when compared with the other groups (for Group 1 versus Group 2, P<0.005; for Group 1 versus Group 3, P<4.7x10(-06)). CONCLUSION: Topical diclofenac is useful for maintaining pupil size during vitrectomy only when used in conjunction with subconjunctival mydricaine, especially in patients in whom prolonged surgery is anticipated.

A retrospective, hospital-records-based study of neonates screened for retinopathy of prematurity (ROP) was undertaken to determine whether the inclusion criteria for screening could have been safely altered to reduce the numbers of babies screened whilst not missing any stage III disease. Babies from six neonatal intensive care units in Birmingham were screened by a single examiner. Between November 1989 and November 1995, 1611 babies were examined and 1429 of these fell within the inclusion criteria of current guidelines for ROP screening produced by the Royal College of Ophthalmologists and the British Association of Perinatal Medicine--any baby < or = 1500 g birthweight or < or = 31 weeks gestational age. Thirty-nine (39) babies developed stage III ROP of which 31 (2.2%) were from Birmingham. In addition 8 babies with stage III ROP were referred from elsewhere. All 39 babies with stage III ROP had a birthweight < or = 1250 g or a gestational age of < or = 29 weeks, but 2 fell outside one criterion, indicating the need for both to be used. Had these inclusion criteria been utilised during this period, then 30% fewer babies would have been examined (432 of 1429). The importance of using both birthweight and gestational age as inclusion criteria is discussed, and the dangers of altering the indications for national screening on the basis of one study population is emphasised.

This prospective study investigated whether low-dose ionising radiotherapy preserved vision and caused membrane regression in patients with age-related subfoveal neovascular membranes (SFNVMs) or vascularised pigment epithelial detachments (PEDs) and relatively good initial visual acuities. Twenty-five patients with initial Snellen acuities of 6/24 or better were treated with low-dose external beam radiotherapy. Of the patients with SFNVMs, visual acuities were maintained or improved in 58% at 6 months and 53% at 1 year. Neovascular membrane size was assessed by image analysis and showed some regression in 47% and 41% at 6 and 12 months respectively. These results suggest that patients with SFNVMs and good vision may benefit from radiotherapy, faring better than previous reports of the natural history of this condition. Conversely, patients with vascularised PEDs did not appear to benefit from radiotherapy. Only 17% maintained their vision at 1 year and 33% suffered retinal pigment epithelial tears. The results from patients with SFNVMs and good initial vision, excluding those with vascularised PEDs, are encouraging however, any benefit from this treatment needs to be proven by controlled trials with long follow-up.

AIMS/BACKGROUND: The aetiology of Fuchs' heterochromic uveitis (FHU) is unknown although it can occur in combination with a number of different ocular conditions. Five patients with FHU who show an association with sarcoidosis were studied. METHODS: Four patients with clinical signs compatible with FHU who had elevated serum angiotensin converting enzyme levels (sACE), and a fifth case with a normal sACE and a positive Kveim test were described. RESULTS: All five cases had iris nodules, two later developed mutton fat keratic precipitates, and one had peripheral retinal periphlebitis. Of the four cases with elevated sACE, one had respiratory function test abnormalities and an abnormal chest x ray compatible with pulmonary sarcoidosis. Another had a chorioretinal scar and developed intermediate uveitis 2 years after presentation. CONCLUSIONS: In all of these cases a diagnosis of FHU may represent a specific secondary ocular response to sarcoidosis rather than a primary idiopathic uveitis syndrome. Although FHU remains a clinical diagnosis, routine uveitis investigations should still be performed in this group of patients.

PURPOSE/METHOD: We examined a pregnant patient who had symptomatic white-centered retinal hemorrhages. Severe preeclampsia rapidly developed despite the patient not having hypertension, proteinuria, or edema at initial examination. RESULTS/CONCLUSIONS: Retinal hemorrhages gradually resolved without treatment and visual acuity returned to 20/20 six months after emergency delivery of a healthy infant. Retinal or choroidal vascular abnormalities in pregnant patients may be caused by early preeclampsia and immediate obstetric referral is indicated.

We undertook a prospective slit lamp examination of 56 eyes which had undergone routine phacoemulsification, in order to determine the incidence of metallic intraocular foreign bodies. Characteristic small reflective particles were noted in 86%, with a mean of 5.1 particles per affected eye. No accompanying clinical problems were seen. The used phacoemulsification needles were examined with colour photography and scanning electron microscopy. A characteristic pattern of wear was observed, corresponding to an area under the plastic irrigating sleeve rather than at the tip. This suggests cavitation erosion rather than instrument contacts as the cause. Titanium alloy contains 6% aluminium, 4% vanadium and 0.25% iron. All its constituents may corrode and leach from the surface of the metal when in contact with body fluids and have been implicated in numerous adverse biological processes.

The results of a retrospective analysis of 49 eyes (40 patients) that underwent penetrating keratoplasty for keratoconus are presented. All grafts had the same size trephine for both host and donor corneas. Ninety-eight per cent of eyes achieved a post-operative visual acuity of 6/12 or better; 43% of eyes attained this level of acuity unaided, although more than half of these required spectacles or contact lenses to reach their best corrected acuity. The average post-operative spherical ametropia was -0.5 dioptre (SD 2.97 dioptres) and the average post-operative cylinder was -3.8 dioptres (SD 2.63 dioptres). Three eyes (6%) required keratorefractive surgery to reduce astigmatism. Wound integrity was satisfactory using the same (size) trephine for both host and donor, with no patient suffering a post-operative wound leak or iris prolapse. The reduction of post-operative myopia and astigmatism is discussed.

The aim of this study was to determine whether capsulotomy size influences visual performance. Snellen visual acuity and forward light scatter (light scattered towards the patient's retina, but out of the focussed retinal image) measurements using simple computer graphics based on van den Berg's technique were used to measure visual performance. Twelve patients were studied: 4 had small central capsulotomies through undilated pupils and 8 had wide capsulotomies through dilated pupils. The two groups were matched for age and pre-laser Snellen acuity. Following treatment, both groups had equal improvements in Snellen acuity. There was only a significant (p < 0.001) improvement in forward light scatter readings in the group who received wide capsulotomies. Measurements of forward light scatter are more sensitive than Snellen acuity testing in demonstrating loss of visual performance in patients with media opacities. It is recommended that pupils are dilated prior to Nd:YAG capsulotomy if forward light scatter from capsule remnants and the consequent glare disability are to be minimised.

A prospective, randomised study was performed on 52 patients undergoing routine limbal extracapsular cataract surgery to compare the effect of 10/0 nylon and 9/0 elastic polypropylene sutures on post-operative astigmatism. Both the magnitude and the axis of the astigmatism were measured by keratometry and refraction pre-operatively and at regular intervals for 3 months post-operatively. Both groups showed an increase in the magnitude of astigmatism immediately post-operatively. The polypropylene group showed a more rapid decrease in astigmatism over the first 7 days and subsequent stabilisation, although the only significant (p < 0.05) difference in astigmatism between the two groups was at 1 week post-operatively. Both groups showed 'with-the-rule' astigmatism immediately post-operatively, although more of the polypropylene group developed 'against-the-rule' astigmatism by the end of the study. Despite an early reduction in post-operative astigmatism in the polypropylene group, early spectacle prescribing would not be recommended because of the shift in the axis of astigmatism. In addition, the polypropylene group had a 4 times greater incidence of iris prolapse compared with the nylon group.

RESUMEN

Five children with an orbital recurrence of retinoblastoma have been successfully treated by a combination of excision biopsy of the tumour mass, radical orbital radiotherapy, and systemic chemotherapy. Nine previous children, consecutive with the five presented here, died from disseminated retinoblastoma after failure of earlier treatment programmes for orbital recurrence. An aggressive therapeutic approach is justified by this improvement in survival.
