RESUMEN

In this prospective, multicentre cohort study, we analysed specific prognostic factors and the impact of timing of highly active antiretroviral therapy (HAART) on disease progression and death among 625 human immunodeficiency virus (HIV)-1-infected, treatment-naïve patients diagnosed with an AIDS-defining disease. HAART was classified as early (<30 days) or late (30-270 days). Deferring HAART was significantly associated with faster progression to a new AIDS-defining event/death overall (p 0.009) and in patients with Pneumocystis jiroveci pneumonia (p 0.017). In the multivariate analysis, deferring HAART was associated with a higher risk of a new AIDS-defining event/death (p 0.002; hazard ratio 1.83; 95% CI 1.25-2.68). Other independent risk factors for poorer outcome were baseline diagnosis of AIDS-defining lymphoma, age >35 years, and low CD4(+) count (<50 cells/µL).

Se presentan los resultados multidisciplinarios de cuatro casos de mioepiteliomas de glándula salival diagnosticados por biopsia por aspiración con aguja fina y resecados posteriormente. Histológicamente dos casos fueron benignos y dos malignos. Los extendidos celulares de las variedades benignas contenían una población uniforme de células discretamente alargados con citoplasma bipolar y membranas celulares conspicuas. El núcleo fue pequeño y oval con cromatina homogénea. Las células se disponían en grupos y nidos sobre un fondo proteináceo. Se observaron células aisladas adyacentes que presentaban citoplasma eosinófilo, granular y claro con núcleo central, redondo e hipercromático. En uno de los extendidos se observó numerosos capilares con células endoteliales prominentes y fondo con matrix condroide. Dos casos presentaron cambios citológicamente malignos con un fondo necrohemorrágico, pleomorfismo, nucléolo prominente, hipercromatismo y mitosis atípicas ocasionales. Las reacciones de inmunohistoquímica demostraron positividad para proteína S-100 y vimentina y focalmente para citoqueratina. Los hallazgos de la microscopia electrónica correspondieron a células mioepiteliales.

Colonic carcinomas with minimal or no glandular differentiation are a heterogeneous group of neoplasms which differ in their histologic appearance, clinical features, prognosis and molecular characteristics. Since 1990, we prospectively identified 11 patients with a predominantly solid (nonglandular) adenocarcinoma of the colon for which the term medullary adenocarcinoma of the colon (MAC) is proposed. The clinical, histological, histochemical, and immunohistochemical features of these neoplasms were studied. All patients with MAC were women with tumors in the cecum or proximal colon. Histological analysis showed nests or trabeculae of regular small to medium-sized cells with moderate amounts of eosinophilic cytoplasm; some cells contained mucin vacuoles. The nuclei had an open chromatin pattern and exhibited prominent nucleoli. Lymphatic permeation was present in most cases. Immunohistochemical reactions were positive for cytokeratin, carcinoembryonic antigen, and epithelial membrane antigen. Despite its histological resemblance with endocrine tumors, MAC is negative for endocrine markers. Of the eight patients for whom follow-up is available, four patients (two Dukes B and two Dukes C) are alive and well 1 to 4 years after surgery, one patient (Dukes C) died of tumor, one patient is alive with liver metastasis 4 years after surgery, and two patients died in the postoperative period. MAC appears to be a distinctive clinicopathologic entity. This tumor should be distinguished from other more aggressive, nonglandular tumors of the colon.

The authors describe the case of a fifty-nine-year-old white man, previously in good health, who initiated his present illness with acute episode of enterocolitis characterized by mild fever and, in the next eight hours, twenty-four episodes of watery diarrhea, nausea and vomiting, as well as generalized sweating and severe weakness secondary to hypovolemia and electrolyte disorder. These complications were corrected in seventy-two hours in the intensive care unit. Two days later, when the patient was stable hemodynamically, under cardiac monitoring and with normal laboratory studies including serum electrolytes, he developed electrocardiographic changes characterized by trifascicular block (prolonged P-R interval, complete right bundle branch block [CRBBB] and left posterior hemiblock [LPH]) with a cardiac rate of thirty beats per minute, for which a temporary pacemaker was inserted. Endomyocardial biopsy showed histopathologic signs of myocarditis and the immunologic study of the cardiac tissue revealed positive polymerize chain reaction (PCR+) with the presence of antitoxine choleric antibodies (AcTCA). After three weeks, the same conduction disturbances remained, for which a permanent pacemaker was inserted. On top of intravenous fluid replacement and electrolyte supplements, the patient was managed with tetracycline 2 g a day for one week and sulfamethoxazole-trimethoprim 800/160 mg a day for two weeks. The purpose of this study is to present a rare and very well-documented myocarditis by cholera in a patient with enteric disease, in whom several cardiac complications occurred.

The Mexican Health Office (SSA) promotes the use and progeny of the appropriate technology for the bacteriological quality evaluation and disinfect the water by means of the in situ progeny of the clorus and ozonus gases when needed, for the human consumption in suburban and rural communities. The clorus water disinfecting conventional methods by many reasons are useless, even in urbanized cities. CEDAT has built and design a group of apparatus, called: "Water treatment whole equipment for small communities", and it's build up by: Membrane Filter, Double Chamber Incubator, ultraviolet Light Sterilizer and a clorus and ozonus progenic gas electrolytic cell with CA and CD power box and a Venturi Tube. The electrolytic cell raw material is the sodium chlorine (salt) water mixed. The evaluation of these equipment was made at the Manuel Gea González Medical Doctor General Hospital. During 38 days there was a daily measure of chlorine sewage in six different parts of the hospital. It was used the cellulose membrane filter technic with a 0.45 mm porus and a sowing in M-Endo and M-FC Broth cultivation was in a 37 and 44 centigrade incubation. The first 10 days chlorine sewage was found, the last 28 weren't measurable. Total coliforms in 23 of 42, 7 days samples and excrement coliforms in 5 of 18, 3 days samples, were developed during the 38 days period. The electrolytic cell was settled for the water gas measure up in its way to the reservoir. Afterwards chlorine sewage was measured from 0.2 mg/l to 1 mg/l in all the sample parts. No germ kind was discovered during the 10 days sampling.(ABSTRACT TRUNCATED AT 250 WORDS)
