RESUMEN

In the western mountainous region of Khyber Pakhtunkhwa Pakistan at the Shangla district, we found Physalis ixocarpa for the first time, not yet reported from Pakistan. Physalis ixocarpa was unidentified and has no ethnobotanical record in the flora of Pakistan. It is a member of family Solanaceae and having a close relation with Solanum tuberosum and Lycopersicon esculentum. The stem is prostrate with a dichotomous pattern of branches having leaves flower and fruits. Leaves are smooth, ovate and the margins of leaf blade dentation are poorly developed. The average length and width of the leaves are 6.50 and 3.61 cm respectively. P. ixocarpa grows to the length of 4-5 feet and an annual herb. The flowers of the plants are yellow in color and having purple color spots on the petals which are star-shaped. The round berry fruits are surrounded by persistent calyx and purple in color. The fruits are the 3-6cm in diameter. The plants are found in the different localities of district Shangla especially in Bar and Koz Kana. The life cycle of reporting plant is started in May and completed in November.

The aim of this study was to investigate the impact of heat stress on production performance and oxidative stress in different plumage colors of Japanese quail. For this purpose, a total of 100 birds were used in this study. The 25 birds belonged to Wild-type (n=25, grey), Tuxedo (n=25, black), Golden (n=25, yellow) and Recessive white (n=25). The birds were reared for 42 days in an environmentally controlled room at 39°C and relative humidity of 60-65%. The body weight, body weight gain (g/bird/day), and feed conversion ratio were not different between the groups (p>0.05). However, the feed intake (g/bird/day) of the Wild-type had a higher value than the Tuxedo (black) group counterparts between 15 and 21 days different (p⟨0.05). There was no significant effect of heat stress on the carcass traits (p>0.05). Spleen weights were different between the groups (p⟨0.05). The yellow group had the highest spleen weight. The highest MDA level was found in the Recessive White variety, followed by Wild-type (grey), Golden (yellow) and Tuxedo (black), respectively. However, there were no statistical differences amongst the groups (p>0.05). There was also no statistical significance in glutathione (GSH) and superoxide dismutase (SOD) levels (p>0.05). The heat shock protein 70 kDa (HSP70) level was significantly different between the groups (p⟨0.001). The highest percentage was observed in the Golden (5.06%) and the lowest in the White (1.43%) variety. There was no superior color variety of Japanese quail regarding fattening performance and carcass traits. It is conceivable that when considering the stress response of the different colors, the Golden group is more sensitive to stress due to the hepatic and cellular level of HSP70.

The use of medicinal plants as an unconventional health treatment is gaining considerable recognition and popularity worldwide. The current study was designed to inspect five medicinally important species (such as Mentha longifolia (L.) Huds., Mentha piperita L., Mentha spicata L., Ocimum basilicum L., and Rosmarinus officinalis L.) of Lamiaceae, collected from district Peshawar, through ethnoecological, phytochemical, and elemental analyses. Biological spectra expressed that therophytes (60%) were the dominant life-form class, while nanophyll (60%) was the leading class among leaf size. The ethnobotanical profile showed that all the species were medicinal and ornamental (100%) each, while 60% were used in spices. Quantitative analysis for the macro- and microminerals confirmed the presence of 13 elements (C, N, O, Mg, K, P, S, Ca, Al, Si, Fe, Cl, and Na), which were present in varying amounts from species to species. The methanol extract of leaf samples was used for the analysis of phytochemical constituents such as saponins, flavonoids, tannins, terpenoids, phlobatannins, steroids, and anthraquinones. The medicinal potential of these plants was correlated with the presence of these phytochemicals. Due to the presence of active constituents, the plants had high potential in antifungal, antidiuretic, antioxidant, and anti-inflammatory activities.

PURPOSE: To investigate the postoperative gastrointestinal complaints and their effects on the satisfaction level of patients after laparoscopic Nissen fundoplication (LNF). MATERIALS AND METHODS: Over a 7-year period, 553 patients who underwent "floppy" LNF were evaluated for preoperative and postoperative complaints. For this purpose, a set of questions derived from gastroesophageal reflux disease-health-related quality-of-life questionnaire (GERD-HRQL) was used. A P-value of <0.05 was considered to show a statistically significant result. RESULTS: The present study included 215 patients with a mean follow-up of 60 months. Reflux-related symptoms [regurgitation (17.7%), heartburn (17.2%), and vomiting (3.7%)] and nonspecific symptoms [bloating (50.2%), abdominal pain (15.3%), and belching (27%)] showed a significant decrease (P<0.001) after the surgery. Inability to belch (25.1%) and early satiety (29.3%) were the newly emerged symptoms. The percentage of patients with flatulence increased from 23.3% to 38.1% after LNF. There was no significant difference for dysphagia (25.6%) and diarrhea (15.3%) in the postoperative period. Of the patients, 15.3% had recurrent preoperative complaints and 9.8% were using drugs for that condition. Satisfaction level and preference for surgery were 82.8% and 91.6%, respectively. There was no significant difference in GERD-HRQL score according to body mass index. CONCLUSIONS: This is the first study in which postoperative reflux-related and nonspecific gastrointestinal complaints are analyzed together for a long follow-up period. We found a significant decrease in many reflux-related and nonspecific symptoms. Although some disturbing complaints like inability to belch, early satiety, and flatulence emerged, the preference for surgery did not change.

BACKGROUND: The most common cause of procedural failure in cardiac catheterization using the transradial approach is radial artery spasm. The aim of this study was to compare the procedural success rate of the 6­Fr Glidesheath Slender with the 6­Fr standard sheath in transradial coronary angiography and intervention. METHODS: Patients who underwent percutaneous coronary angiography via the transradial approach through placement of a 6-Fr Glidesheath Slender or a 6-Fr standard sheath for primary radial access were prospectively enrolled in the study. RESULTS: The study included 200 cases: 76 patients undergoing percutaneous coronary angiography with the Glidesheath Slender and 124 patients with the standard sheath. Failed procedures were recorded for 19 patients (9.5%), defined as inability to complete the procedure via the primary access. There was no difference in the percentage of failed cases between the Glidesheath Slender and standard sheath groups (10.5% vs. 8.9%, OR = 1.21, p = 0.8). More cases of spasm were observed in the Glidesheath Slender group compared with the standard sheath group, which was not statistically significant (7.9% vs. 5.7%, OR = 1.43, p = 0.56). Smoking, hyperlipidemia, and age influenced the procedural outcome in the Glidesheath Slender group, while body mass index, sex, and smoking impacted the procedural outcome in the standard sheath group. CONCLUSION: There is no difference in procedural success rates, as defined by the ability to complete the procedure via primary radial access, between the 6­Fr Glidesheath Slender and the 6­Fr standard sheath. Our study suggests that the patient characteristics that elevate the risk of procedural failure for Glidesheath Slender may differ from those for the standard sheath.

Biochar (BC) has been reported to improve growth and drought resistance in many plants. However, adequate information on the drought resistance mechanism mediated of BC on Phragmites karka, a bioenergy plant, is not available. The impact of BC addition (0%, 0.75% and 2.5%) on plant growth and physiology of P. karka under drought was assessed. Soil water-holding capacity and soil water content were significantly improved with 0.75% BC as compared with the un-amended controls. This resulted in improved plant performance under drought conditions. An increase of parameters, such as plant fresh and dry biomass, root to shoot ratio and root mass fraction, was paralleled by an increase of chlorophyll content, net photosynthesis rate and water use efficiency of plants. Plants treated with 0.75% BC experienced less oxidative stress due to higher photosystem II efficiency and stimulated activity of antioxidant defense systems. Our results demonstrate that soil amendment with 0.75% BC allow the potential energy plant P. karka to grow in an arid habitat.

Coastal salt marsh plants employ various combinations of morphological and physiological adaptations to survive under saline conditions. Little information is available on salinity tolerance mechanisms of Halopeplis perfoliata, a C3 stem succulent halophyte. We investigated the growth, photosynthesis and antioxidant defence mechanisms of H. perfoliata under saline conditions (0, 150, 300 and 600 mM NaCl) in an open greenhouse. Optimal shoot succulence, projected shoot area and relative growth rate were obtained in the low (150 mm NaCl) salinity treatment, while growth was inhibited at the highest salinity (600 mm NaCl). The CO2 compensation point and carbon isotope composition of biomass confirmed C3 photosynthesis. Increases in salinity did not affect the photosynthetic pigment content or maximum quantum efficiency of PSII of H. perfoliata. Assimilation of CO2 (A) also remained unaffected by salinity. A modest effect on some gas exchange and photochemistry parameters was observed at 600 mm NaCl. With increasing salinity, there was a continual increase in respiration, suggesting utilisation of energy to cope with saline conditions. Under 300 and 600 mm NaCl, there was an increase in H2 O2 and MDA with a concomitant rise in AsA, GR content and CAT activity. Hence, H. perfoliata appears to be an obligate halophyte that can grow up to seawater salinities by modulating photosynthetic gas exchange, photochemistry and the antioxidant defence systems.

In the present study, a simple analytical method is proposed for determining tryptophan, and method is validated on some cereal and legume samples. In the method alkaline hydrolysis of proteins was used due to the destruction of tryptophan structure during acid hydrolysis. Following alkaline hydrolysis (120°C for 12h), hydrolysates are filtered through ashless filter paper and pH values are adjusted with hydrochloric acid solution. Separation and detection of tryptophan are performed on a reversed-phase column with fluorescence detection within 10min by using a mobile phase of acetonitrile and acetate buffer of pH 6.3 (1:9, v/v). For determination of tryptophan content, the procedure described in the study offers an alternative analysis method by enabling high speed analysis and the use of simple extraction process to the other available methods.

BACKGROUND: The antioxidative effects of the traditional grape-based beverage, hardaliye, were investigated with a 40-day randomized controlled clinical trial on 89 healthy adults. Subjects were randomly divided into three groups: high hardaliye (HH), low hardaliye (LH) and control group. HH and LH groups consumed 500 mL and 250 mL hardaliye per day, respectively, and the control group did not consume any hardaliye. Dien conjugate (DC), malondialdehyde (MDA), vitamin C, total antioxidant capacity (TAC) and homocysteine concentrations were measured in fasting blood samples collected at baseline and after intervention. RESULTS: Significant decreases in DC, MDA and homocysteine concentrations were observed in HH and LH groups (P < 0.001) after intervention, whereas the control group showed no change. The reduction in homocysteine was significantly different between HH and LH groups (P < 0.001), except for DC and MDA. TAC and vitamin C were slightly increased; however, the change was not statistically significant. CONCLUSION: Dietary supplementation with hardaliye affect the MDA, DC and homocysteine levels in blood, possibly due to the presence of antioxidant compounds. Dose response was only observed for homocysteine. Further studies need to be performed to assess the effects on antioxidant capacity.

BACKGROUND AND OBJECTIVE: In this study, we aimed to investigate whether the combination of low-dose (0.25 mg kg(-1)) midazolam premedication with parental presence can effectively reduce anxiety at induction as well as provide a smoother emergence. METHODS: Institutional ethics committee approval and informed consent from one of the parents were obtained prior to the study. Sixty ASA grade I or II children undergoing surgery were enrolled in the study. Children were randomized to receive either 0.5 mg kg(-1) midazolam orally (group M) or 0.25 mg kg(-1) midazolam orally with parental presence (group MP) or parental presence alone (group P). The child's anxiety and sedation scores were evaluated as 1-4 points on the Anxiety Scale and as 0-4 points on the University of Michigan Sedation Scale (UMSS), respectively, at the entrance to the operating room and for tolerance to the face mask. Heart rate, the mean arterial blood pressure and O2 saturation (%) were assessed at repeated intervals before and after induction. At the end of surgery, the child's Anxiety Scale score, UMSS score, Observer's Pain Scale (OPS) score and FLACC score were also assessed. RESULTS: There were no differences between groups in demographic variables and duration of surgery or anaesthesia. Mean blood pressure changes were similar between groups at measured intervals, but the heart rate was higher in group M before and after induction of anaesthesia (P<0.05). UMSS score was greater in both midazolam groups (groups M and MP) in the preoperative period (P<0.05). Anxiety Scale scores for anxiolysis were higher in groups M and MP than in group P (less anxious and more sedated) at 20 min after premedication, at the entrance to the operating room and at mask tolerance time points (P<0.05). During recovery there was no significant difference in sedation, recovery scores or behavioural anxiety assessment between groups, Anxiety Scale score, UMSS score, FLACC score, Visual Analog Scale (VAS) score and Observer's Pain Scale score in groups MP, M and P. CONCLUSION: Preoperative administration of midazolam 0.5 mg kg(-1) for premedication alone, without parental presence at induction, and that of low-dose midazolam 0.25 mg kg(-1) for premedication with parental presence at induction are both equally effective in reducing separation anxiety and providing a smooth emergence. However, parental presence alone, without midazolam for premedication, is not an adequate approach for this outcome. If the environment for parental presence is convenient, the dose of midazolam may be reduced and induction and emergence conditions may still be of high quality.

We evaluated the morbidity, complications and outcomes in 42 patients who underwent abdominal paravaginal defect repair (PDR) for vaginal wall prolapse due to a paravaginal defect and the prolapse of other pelvic organs. Out of a total of the 42 patients, 32 underwent PDR and concomitant pelvic reconstructive procedures, and 10 patients underwent PDR plus anti-incontinence surgery and concomitant pelvic reconstructive procedures. The mean patient age was 44.5 years. The mean follow-up period was 40 months. One bladder injury and two hemorrhages occurred intraoperatively. The cure rate of anterior vaginal wall prolapse was 92.9%. Of the 20 patients with urodynamic stress incontinence (USI) who underwent PDR alone, the rate of recurrence of USI was 20%; however, there was no recurrence in the 10 patients who underwent PDR plus the anti-incontinence procedures. Paravaginal repair is a safe and effective procedure for the surgical correction of a paravaginal defect, but has limited applicability in the surgical correction of USI.

The aim of the present study is to investigate a possible association between osteoarticular involvement of brucellosis and HLA-B27 antigen. Standard microtoxicity technique method was used for detection of HLA-B27 antigen in 78 patients with brucellosis and 100 healthy donors. Osteoarticular complications were defined in 25 of 78 patients (32.1%). Overall frequency of HLA-B27 in the study group was 14.1% (11/78) and 16% (4/25) in patients with osteoarticular involvement, while it was 8% (8/100) in the control group. Although HLA-B27 frequency in osteoarticular group was slightly higher than both of the other two groups, there was no significant difference (p > 0.05). Conclusively, we could say that further studies including a larger number of patients with osteoarticular involvement of brucellosis may need to be performed in order to find a possible association.

We report the case of a 25-year-old man with multiple bilateral hydatid cysts of the brain in whom we were able to assess the growth rate of the cysts on repeated examination. On average, the cysts increased in diameter by 1 cm per month.

Chordomas are rare, insidious, and slow-growing but locally aggressive, malignant tumors originating from the primitive notochord and localized along the axial skeleton. Between 1988 and 1993, four patients with sacrococcygeal chordomas were treated at the Ankara Numune State Hospital. There was a median 17-month interval between the onset of symptoms and diagnosis. In all patients, a posterior approach was used. In addition, two patients were treated with adjuvant radiation therapy. There were no serious intra- or postoperative complications. Early diagnosis and surgery plus radiation therapy seem to provide the best results in terms of survival.

RESUMEN

Spinal metastasis plays an important role in the survival rate and general condition of cancer patients. In this paper, we present 70 patients with metastatic spinal tumors, diagnosed and surgically treated in the Departments of Neurosurgery and Orthopedics of Ankara Numune State Hospital between 1984 and 1993. Our clinical observations suggest that the survival rate is influenced by the type of the primary lesion, pathology, and the patient's preoperative physical and neurological status.

Hydatid cysts of central nervous system are rare and comprise only 2% to 3% of all hydatid cysts reported. Orbital localization is very uncommon and has been reported less than 1% of all hydatid diseases. The primary treatment of hydatid disease is surgical. The most important complication of the surgical treatment is secondary hydatidosis due to spillage of the cyst contents. Because of the difficulties of the orbital localization, total extirpation of the cysts without rupture is almost impossible. Preventing spontaneous rupture of the cysts during surgery and postoperative antihelmintic treatment should be taken into consideration in these cases. This study includes four cases who underwent surgery for orbital hydatid cysts. Radiological characteristics, operative technique and postoperative medical therapy are discussed.

RESUMEN

Esthesioneuroblastoma is an uncommon malignant neoplasm that arises from olfactory mucosa, often with intracranial extension. A case report of the youngest victim in the literature (2-year-old boy) is presented.

Brain abscess is probably the least common manifestation of tuberculous infection of the central nervous system; meningitis and tuberculoma are much more common. A case of tuberculous brain abscess in a 23-year-old man with previous history of Tbc meningitis is presented. The computerized tomographic scan demonstrated a unilocular space-occupying lesion in the left thalamic region, surrounded by a thick hyperdense enhancing rim. It is suggested that a relatively long clinical history and previous Tbc meningitis history together with the appearance of a thick-walled abscess-like lesion on the CT scan may indicate the diagnosis of a tuberculous brain abscess. Only after neurosurgical removal of the abscess, the pathohistological examination reveal tuberculous etiology of the abscess. The patient later died from aspiration bronchopneumonia. Only 28 instances of tuberculous abscess have been reported in the literature.

RESUMEN

1. Activities of glucose 6-phosphate dehydrogenase (EC 1.1.1.49), 6-phosphogluconate dehydrogenase (EC 1.1.1.44), isocitrate dehydrogenase (EC 1.1.1.42), malate dehydrogenase (EC 1.1.1.37), malate dehydrogenase (decarboxylating) (EC 1.1.1.40), and pyruvate carboxylase (EC 6.4.1.1) were determined in subcellular fractions of mammary gland from rabbits during pregnancy, at different stages of lactation and during weaning. The results were compared with those obtained in similar experiments with rat mammary gland. 2. Three bases of expression of the activity of enzymes in the particle-free supernatant fraction of mammary gland were compared. During lactation, activity expressed per mg. of particle-free supernatant protein (uncorrected for milk protein) correlated well with that expressed per mug. of DNA phosphorus. The disadvantages of expressing activities per g. wet wt. are discussed. 3. The major differences between the two tissues were: (a) neither malate dehydrogenase (decarboxylating) nor a soluble form of pyruvate carboxylase could be detected in rabbit mammary gland at any stage of the lactation cycle; (b) isocitrate dehydrogenase increased in activity during lactation in rabbit mammary gland, but not in that of the rat. 4. Pyruvate carboxylase in the mitochondrial fraction of rabbit mammary gland, and in both the mitochondrial and the soluble fractions of rat mammary gland, did not change in activity during lactation. 5. For each tissue, the NADP-dependent dehydrogenases studied had a high activity at all stages of the lactation cycle compared with the rate of fatty acid synthesis at mid-lactation. The significance of these results is discussed with respect to the supply of NADPH via NADH.
