RESUMEN

In a multicenter study of 30 patients treated with ciprofloxacin (mean daily dosage, 1000 mg per day; mean duration of treatment, 10.6 days) for a variety of infections, 14 were microbiologically proven. Of these, bacteriologic cure and/or improvement resulted in 92.3% of cases. For all 30 infections clinical cure and/or improvement resulted in 88.9% of cases. A total of ten infections were classified as chronic. Overall there were 5/30 (16%) adverse reactions (ADRs), however, only two were definitely related to ciprofloxacin therapy. For three ADRs the relationship to therapy was uncertain. Therapy with ciprofloxacin was discontinued in one (3.3%) patient because of adverse effects. Four patients elected to continue ciprofloxacin therapy despite mild side effects.

RESUMEN

A study was carried out to evaluate the histologic changes of meniscus perforation repair associated with dermal grafts to the temporomandibular joint of cynomolgus monkeys (Macaca fascicularis) following surgical perforation of the meniscus. Dermal grafts were studied histologically at 3-week, 6-week, 3-month, and 6-month intervals. Results showed early attachment of the dermal graft to the meniscus, followed by gradual incorporation of the graft into the meniscus and subsequent return of the meniscal-graft complex to a normal meniscal architecture.

RESUMEN

Etomidate, a nonnarcotic, nonbarbiturate hypnotic agent, was assessed in a group of 20 patients requiring general anesthesia for outpatient oral surgical procedures. Changes in mean blood pressure, heart rate, and transcutaneous oxygen tension (PtcO2) were examined following the intravenous administration of etomidate for the induction and maintenance of general anesthesia. Clinical evidence of pain on injection, myoclonic muscle activity, apnea, nausea, and emesis were documented. A postoperative questionnaire evaluated levels of amnesia and acceptance of the drug by the patient and surgeon. No significant (P less than 0.05) change in PtcO2 occurred during etomidate infusion; however, a statistically significant but clinically insignificant change did occur in mean blood pressure and heart rate. Although myoclonic muscle activity, pain on injection, and nausea and vomiting were documented, the subjective evaluation of this agent by patient and surgeon was favorable.

RESUMEN

A prospective randomized double-blind study was conducted to evaluate two prophylactic antibiotic regimens in a population of 40 patients undergoing intraoral orthognathic surgery. All patients received one intramuscular dose of 600,000 units procaine penicillin G and 400,000 units aqueous penicillin G preoperatively. Two million units aqueous penicillin G were administered intravenously every three hours intraoperatively and postoperatively three hours after the last intraoperative dose. One group of 20 patients then received 2 million units aqueous penicillin G intravenously every four hours for a total of 12 doses (two days) postoperatively. The second group of 20 patients received a placebo intravenously every four hours for a total of 12 doses (two days) postoperatively. Three infections occurred in the group of patients receiving the placebo postoperatively and none in the group of patients receiving the antibiotics postoperatively.

RESUMEN

The use of ketamine in adults has been complicated by untoward hallucinatory emergence reactions. The addition of nitrous oxide and diazepam to subanesthetic doses of ketamine may be effective in decreasing these reactions. Adult patients were sedated for outpatient oral and maxillofacial surgery procedures using nitrous oxide, diazepam, and either fentanyl or ketamine. Postoperative questionnaires were used to evaluate hallucinatory reactions, amnesia, and patient acceptance. Comparison of the results of patients receiving ketamine and those receiving fentanyl revealed no statistical differences in hallucinatory reactions or patient acceptance. Amnesia was more complete in the ketamine than the fentanyl group.
