RESUMEN

Functional Endoscopic Sinus Surgery (FESS) is now undoubtedly the choice procedure for the surgical treatment of chronic sinusitis and nasal polyposis after failure of medical treatment. Between January 1990 and December 1992, 51 patients with chronic sinusitis and 44 with nasal polyposis who underwent FESS were evaluated (170 ethmoidectomies). Preoperative evaluation is obtained through personal scaling combined with a CT of the sinuses, whereas postoperative assessment is based on the same scoring together with an endoscopic evaluation. Good functional results and no significant differences between the two types of pathology are obtained after a 1 year follow-up.

RESUMEN

A patient presenting a recent spontaneous C.S.F. leak was endoscopically treated. Several materials were used to seal off the fistula. The literature on the etiology and treatment of C.S.F. leaks is presented.

RESUMEN

Electrophysiological features in acoustic neurinoma are reviewed. A case of an acoustic neurinoma with a normal BERA is reported. The authors conclude that no isolated electrophysiological sign is able to disclose a retrocochlear hearing loss. They insist on the necessity to interpret an unilateral hearing loss on basis of an integration of a complete clinical, paraclinical and electrophysiological testing.

RESUMEN

20 cases of uvulopalatopharyngoplasty performed on patients suffering from sleep apnea syndrome have been studied with a pre- and post-operative polysomnographic recording. Eleven patients (55%) responded to this therapy and showed a significative decrease of their post-operative apnea-hypopnea index. An increase of the mean oxygen saturation was noticed during sleep. On the contrary, the sleep quality did not improve. Age, weight, apnea index are not predictive factors influencing the result of UPPP. Even if UPPP has a relatively poor success rate it still should be proposed to patients suffering from sleep apnea syndrome as it remains the simplest and best tolerated surgical treatment.

We tested the availability of the commercial oto-acoustic device "ILO88" on adults and neonates. We describe a simple method to quickly obtain reliable results with this material. We also tested the possibility of recording evoked oto-acoustic emissions on ears with grommets tubes.

RESUMEN

The authors report an observation of Wegener's disease, clinically isolated as a secretory otitis media on the left side followed by a right sudden deafness. Resistance to the usual treatment and the evolution of the disease with association of pulmonary manifestation and fever have lead to the diagnostic of Wegener granulomatosis confirmed by ANCA test and renal punction.

RESUMEN

In our study, 71 normal-hearing ears from adults and 39 from neonates (n = 110) were tested by the advanced echo test technique devised and described by Kemp (ILO 88). Almost all ears (97% and 95%) showed emissions. This material is convenient for clinical routine in adults and for neonatal screening.

RESUMEN

A study realised among 60 children by means of a probe tube microphone allowed us to determine the mean ear canal resonance by the newborn and the young children. This is noticeably less than the adult one. The ear canal reaches its adult physiology after one year, so it implies a somewhat different auditory sensibility by the newborns than by the adults, particularly in the 2000-4000 Hz range. We suggest that the A.B.R. wave I latency and thresholds modifications during children's first year of life are (at least partially and in the majority of cases) an ear canal physiological variation effect and that different calibrations should be used for newborns screening test and for adults purposes.

RESUMEN

We recommend the association of scintigraphy and sialography in salivary glands pathology. The obtained results of both methods are very often complementary or convergent. In case of sialadenitis or dry mouth, it happens that results diverge, but these differences can be explained if one remembers the anatomo-physiological alterations. To prevent a false positive result of atrophy. The scintigraphy must always be realized before sialography. N.B. "Scintigraphy" means isotopic sialography. "Sialography" means conventional sialography.
