RESUMEN

Examination of 26 patients with infectious-and-allergic myocarditis and of 12 cases with myocarditic cardiosclerosis, performed with the aid of radionuclide ventriculography revealed signs of impairement of left ventricular systolic function, with both end-systolic volume and the mean myocardial fiber circular shortening rate measurements showing a downward tendency. A relationship was established between alterations in systolic function of the heart and clinical pattern of the medical condition.

RESUMEN

The disturbances in the cardiac left ventricular contractility have been shown to be much graver in complete blockade of the right bundle of His as evidenced by the examination of 35 patients who had a history of myocardial infarction, and data from the two-year follow-up. The data obtained should be considered in determining time for the patients fo return to their ordinary activities and become functional members of society.

RESUMEN

Rehabilitation treatment is evaluated in 55 patients (long-term results) with a history of myocardial infarction residing in the Chernobyl atomic station zone. It was established that the course of the disease in persons who participated in liquidation of the Chernobyl disaster was characterized by a more frequent development of reinfarctions as compared with the Kiev population. Patients residing in the zone neighbouring the Chernobyl station developed more frequently cardiac failure. This requires detailed examination of these patients during their return to work.

RESUMEN

Overall 235 patients with a history of uncomplicated myocardial infarction were examined. Of these, in 79 patients (33%), myocardial infarction developed in the presence of arterial hypertension. To predict the efficacy of rehabilitation treatment, postinfarction angina pectoris, arterial hypertension, the size and localization of the injury, the status of the coronary and myocardial reserves were taken into consideration. The data obtained confirmed the prognostic significance of arterial hypertension in the estimation of the efficacy of rehabilitation therapy. Arterial hypertension contributes to an unfavourable course of CHD and to a decrease of the work fitness of the patients who suffered myocardial infarction. Estimating the long-term efficacy of rehabilitation treatment, it is necessary to bear in mind not only the coronary reserves, but also the myocardial reserves, its integral index--the total ejection fraction and regional myocardial contractility.

RESUMEN

Two-year follow-up of 65 patients with myocardial infarction complicated by an aneurysm of the aorta using unifactorial mathematical analysis revealed signs allowing before returning of the patients for working activity to single out groups with a reduced working capacity lasting two years after myocardial infarction. The prognosis of reduction of the working capacity in patients with a history of myocardial infarction complicated by an aneurysm of heart depended on the presence of stenocardia before or after myocardial infarction and arterial hypertension. It was established that the initial values of threshold loads (at the level of 50 Wt) were important in determining the long-term working prognosis.

RESUMEN

Age-related effects on the function of restoration in 254 patients with a history of myocardial infarction were investigated. It was found that the age effect influenced tolerance to physical load, the values of which are not always related to the character of the sustained infarction of the myocardium.

RESUMEN

The results of a 2-year follow-up of 400 patients with prior myocardial infarction (MI) and bicycle ergometric findings of intracardiac hemodynamic parameters and oxygen supply during exercise tests, and the criteria for tetrachoric index significance within the confidence intervals were used to reveal the factors predicting the efficacy of rehabilitative treatment. To predict decreased working capacity in the long-term period is determined by the extent of lesion after myocardial reinfarction, while angina pectoris and arterial hypertension are of significance in patients without reinfarction. A role of dynamic indices was defined in predicting decreased working capacity. Further lowered working capacity may be judged from the changes occurring in threshold exercise capacity and left ventricular end-diastolic volume in the first 6 months of postinfarction.

RESUMEN

A two-year follow-up of 400 patients with prior myocardial infarction and usage of regression and discriminant analysis algorithms provided a number of linear models to predict changes in the patients' working capacity over the observed period. A variety of models depending on the feasibility of a laboratory to apply instrumental tools for examination suggested their wide utilization. The advantage of the models is that a small set of signs is used, no computer engineering is required. The models may be used in patients with different clinical disease (uncomplicated, repeated or cardiac aneurysm-complicated myocardial infarction). The modelling provided accurate prediction at 84-100% levels.

Factors responsible for the efficacy of rehabilitative treatment of patients with a history of myocardial infarction were defined on the basis of the 2-year follow-up of 235 patients who suffered myocardial infarction, bicycle ergometry data, studies into intracardiac hemodynamics, oxygen supply in exercise tests, and criteria for the significance of the tetrachoric indicator with regard to the confidence limits. In patients who suffered uncomplicated myocardial infarction, prediction of the decreased work fitness in the long-term period is determined by the presence of angina pectoris and arterial hypertension. It has been established that whether will be lowered in future can be judged from the changes seen in the magnitude of the liminal power and the end diastolic volume of the left ventricle during the first 6 months following myocardial infarction.

Altogether 129 patients were investigated 3 months after the onset of myocardial infarction (MI). Left ventricular diastolic function was assessed using a set of parameters of radionuclide ventriculography, including fractions of filling of the first and second thirds of the diastole, increments of volumes in the phases of fast and slow filling, a maximum rate of filling and the time of achieving it. Marked disorders of isovolumetric relaxation (indices of filling of the first third of the diastole) as well as changes of volumetric compliance (a ratio of increments of volumes of filling of the first and second thirds of the diastole) were revealed in the patients. A degree of disorder of left ventricular diastolic function and MI localization showed no correlation, however more noticeable changes were observed in the group of patients with the anterio-apical localization of myocardial lesion as compared to patients with lesions of posterior-lateral localization.

Results are described on the effect of limitation of the infarction zone on the course of IHD during the first year of observation in 320 patients with a history of myocardial infarction (MI). The clinical course of the disease and changes of tolerance of physical load were studied in two groups of patients. In one of the groups limitation of the infarction zone was realized during the first ten days. It was found that limitation of the infarction zone is of importance only in the acute period of MI reducing the frequency of fibrillations and lethal outcomes. Limitation of the infarction zone did not effect the course of IHD during the first postinfarction period. Infarction in the involved zone showed no increases in frequency.

RESUMEN

Paired bicycle ergometry [correction of veloergometric] testing was carried out in 94 patients with a history of myocardial infarction before and after single intakes of 40 mg of obsidan and 200 mg of labetalol. The effect of these agents on the increase of tolerance to physical load was evaluated. It was found that obsidan and labetalol increased the tolerance of these patients to physical loads. The positive effect was realized due to negative chronotropic effect of obsidan and reduction of the systemic arterial pressure under the effect of labetalol.

A total of 93 patients with myocardial infarction were investigated 3-4 mos. after the beginning of disease by the method of radionuclide ventriculography. Total and regional ejection fractions (for 7 zones), left ventricular functional volumes (end-systolic, end-diastolic and stroke volumes), rates of contraction and expulsion were determined. A certain level of left ventricular pump function in patients after myocardial infarction was determined by the ratio of the number of hypokinetic segments and the expression of a decrease in their contractility and normo- and hyperkinetic segments which are the main compensatory factor. Variations in the structure of formation of the total ejection fraction with relation to a site of myocardial infarction were revealed. Compensatory significance of hyperkinetic zones is decreased with a growing degree of expression of contraction asynergia.

RESUMEN

On the basis of veloergometric testing and discriminant analysis carried out in 178 patients with a history of myocardial infarction a formula was designed for preventive evaluation of the course of the disease for a two-year period. This method allows to single out patients with an unfavourable course of the disease even in case of satisfactory tolerance of physical loads during initial examination. The prognosis efficacy was 75%.

The use of an expanded set of parameters of radionuclide ventriculography with 99mTc-pertechnetate, characterizing myocardial function, made it possible to specify radionuclide semiotics of myocardial infarction, to identify 4 cardiodynamic types, and to establish correlation between a degree of cardiodynamic changes and a site of lesion, and the relationship with exercise tolerance.

RESUMEN

Results are reported of rehabilitation treatment of patients with a history of myocardial infarction (53 died, 65 are living for 10 years during this observation). Follow-up studies indicate that duration of life following myocardial infarction is determined not only by the size of infarction after the first coronary catastrophe but also by the frequency of reinfarctions and progression of the atherosclerotic process.

A 2-year follow-up of 200 myocardial infarction patients with regard to clinical findings, intracardiac hemodynamics, oxygen and hemodynamic provision of loading tests has furnished evidence on validity of the proposed scheme of the patients long-term monitoring survey. The scheme is adjusted for season, functional class, follow-up duration and permits reduced number of 2-year follow-up examinations of patients with a history of myocardial infarction.
