RESUMEN

ObjectiveTo explore the possible mechanism of pediatric tuina therapy in treating anorexia nervosa. MethodsTotally 120 children with anorexia nervosa were randomly divided into a tuina group and a medication group, with 60 cases in each group. Sixty healthy children undergoing physical examinations were recruited as the healthy control group. Children in the tuina group received traditional pediatric tuina therapy, while those in the medication group received orally chewed Jianwei Xiaoshi tablets. Each treatment course lasted for 7 days, with a 1-day interval between courses, and a total of 4 courses were administered. Before and after treatment, seven indicators including gastric motility frequency, gastric area, gastric area 30 minutes after drinking, anterior-posterior diameter and area during gastric fundus dilation, anterior-posterior diameter and area during gastric fundus contraction were measured using a color Doppler ultrasound diagnostic instrument in children from the healthy control group, tuina group, and medication group. Additionally, gastric emptying rate at 30 minutes, changes in anterior-posterior diameter and area during gastric fundus contraction, and changes in gastric area were compared. ResultsThis study ultimately included 60 healthy children in the control group, 59 children in the tuina group, and 51 children in the medication group. Compared with the control group at baseline, the gastric area and the anterior-posterior diameter and area during gastric fundus contraction increased, while the gastric emptying rate, gastric motility frequency, and changes in anterior-posterior diameter during gastric fundus contraction decreased in both the tuina group and medication group, with only a decrease in gastric area during gastric fundus contraction observed in the tuina group (P<0.05). Compared with baseline, after treatment, the gastric emptying rate, gastric motility frequency, and changes in anterior-posterior diameter and area during gastric fundus contraction increased in the tuina group, while the gastric area and area during gastric fundus contraction decreased 30 minutes after treatment; in the medication group, gastric motility frequency and changes in anterior-posterior diameter during gastric fundus contraction increased, while the area during gastric fundus contraction decreased (P<0.05 or P<0.01). Compared with the medication group after treatment, the gastric area decreased 30 minutes after treatment, while the gastric emptying rate and gastric motility frequency increased (P<0.05). ConclusionThe possible mechanism of pediatric tuina therapy in treating anorexia nervosa is to promote gastric motility and gastric emptying, thereby improving gastrointestinal dysfunction in children.

RESUMEN

Danggui Liuhuangtang is the 47th of the 100 famous classical formulas published by the National Administration of Traditional Chinese Medicine, and is known as the holy medicine for night sweat. By bibliometrics, the authors collected the ancient books on Danggui Liuhuangtang and screened out 269 valid data, involving 156 ancient books of traditional Chinese medicine. The analysis on the historical origin, disease syndromes, pathogenesis, composition, dosage, preparation, usage, and processing of Danggui Liuhuangtang found that this famous classical formula originated from Secret Book of the Orchid Chamber (《兰室秘藏》) written by LI Dongyuan, and is composed of Angelicae Sinensis Radix, Rehmanniae Radix, Rehmanniae Radix Praeparata, Phellodendri Chinensis Cortex, Scutellariae Radix, Coptidis Rhizoma and Astragali Radix. It has the functions of nourishing Yin, reducing fire, consolidating exterior and stopping sweating, and mainly treats night sweat due to Yin deficiency and fire exuberance. In the later generations, disease syndromes are mostly treated based on LI Dongyuan's theory, and have expanded to more than 30 kinds (339 in total), among which night sweat (208) was the most, accounting for 61.36% of the total disease syndromes, followed by spontaneous sweating (38), accounting for 11.21%. Additionally, it was found that Danggui Liuhuangtang was widely used in modern clinical practice for various disease syndromes. Among them, endocrine disease (77, 28.21%) was predominant, followed by gynecological disease (48, 17.58%), and pediatric disease (24, 8.79%). Although Danggui Liuhuangtang treats many disease syndromes, their pathogenesis was always yin deficiency and fire exuberance. Through the systematic excavation of the ancient books on Danggui Liuhuangtang and the analysis of its modern clinical application, this paper probed into the historical evolution and confirmed the key information of the formula, providing detailed literature basis for the research and development application of famous classical formulas.

RESUMEN

<p><b>OBJECTIVE</b>To explore the improvement of hand motion function,spasm and self-care ability of daily life for stroke patients treated with floating-needle combined with rehabilitation training.</p><p><b>METHODS</b>Eighty hand spasm patients of post-stroke within one year after stroke were randomly divided into an observation group and a control group, 40 cases in each one. In the two groups, rehabilitation was adopted for eight weeks,once a day,40 min one time. In the observation group, based on the above treatment and according to muscle fascia trigger point, 2~3 points in both the internal and external sides of forearm were treated with floating-needle. The positive or passive flexion and extension of wrist and knuckle till the relief of spasm hand was combined. The floating-needle therapy was given for eight weeks, on the first three days once a day and later once every other day. Modified Ashworth Scale(MAS), activity of daily life(ADL, Barthel index) scores and Fugl-Meyer(FMA) scores were used to assess the spasm hand degree,activity of daily life and hand motion function before and after 7-day, 14-day and 8-week treatment.</p><p><b>RESULTS</b>After 7-day, 14-day and 8-week treatment, MAS scores were apparently lower than those before treatment in the two groups(all P<0. 05), and Barthel scores and FMA scores were obviously higher than those before-treatment(all P<0. 05). After 14-day and 8-week treatment, FMA scores in the observation group were markedly higher than those in the control group(both P<0. 05).</p><p><b>CONCLUSION</b>Floating-needle therapy combined with rehabilitation training and simple rehabilitation training could both improve hand spasm degree, hand function and activity of daily life of post-stroke patients, but floating-needle therapy combined with rehabilitation training is superior to simple rehabilitation training for the improvement of hand function.</p>
