RESUMEN
We report a case of congenital infiltrating lipomatosis of the face (CILF) with right TMJ ankylosis causing asymmetry and reduced mouth opening. The management involved soft tissue debulking combined with a right TMJ arthroplasty and is explained in detail. A review of the relevant literature revealed the rarity of this condition. The diagnosis of CILF remains challenging. The currently accepted treatment strategy is less aggressive as facial asymmetry tends to recur. Infiltration of the facial structures from adipose tissue requires several surgical procedures in most cases. Bony hypertrophy on the affected side has been a common finding in reported cases. There have been no previous reports of TMJ ankylosis associated with this condition. CILF is a benign condition with a good long term prognosis. After 2 years of follow up our patient, has maintained full function despite facial asymmetry.

This article deals with the microbiological aspects of infections, the principles and role of antibiotic use and the general management of the patient suffering from an odontogenic infection. Clinical aspects of the most common presentations will be addressed to understand the organized management of these challenging patients. (Part one appeared in Oral Health, May, 1995).

Traumatic injuries to the midface are not nearly as common in Canada as they are in the United States, but practitioners must still be prepared for the evaluation and treatment of patients presenting with midface fractures. This case report demonstrates the experience one individual who presented with the clinical signs and symptoms of midfacial trauma. Cases of this nature can be confidently managed by an oral and maxillofacial surgeon. A review provides knowledge regarding anatomic features, classification schemes and the diagnostic and therapeutic decisions encountered with the treatment of zygomatic complex fractures. The forward projection of the zygoma causes it to be frequently injured secondarily to blunt trauma of the midface, at the expense of protecting the orbit.

Liposarcoma of the head and neck region is extremely rare. An additional case of pleomorphic liposarcoma of the buccal mucosa is reported, representing an even rarer combination. The importance of combined radiation and radical surgical therapy as a means of improving survival is emphasized.

A case involving primary lymphoma of the mandible is presented. The difficulties in diagnosis of this particular pathology are highlighted. The features of primary lymphoma of the mandible are described based on a review of the literature. Malignant lymphoma must be considered in the differential diagnosis of unexplained dental pain, swelling, ulceration, and radiographic evidence of rarefaction.

A case of rhabdomyosarcoma of the maxillary sinus is reported. A brief review of the origin, histomorphology, classification, clinical presentation, behavior, prognosis, and treatment is given relative to occurrence in the maxillary antrum. Misinterpretation of the microscopic findings can delay the histologic diagnosis. An error in tissue diagnosis may be minimized by awareness of a chance encounter with this lesion.

The clinical and histopathological features of 224 new cases of peripheral giant cell granuloma were studied. In addition, the data from 956 previously reported cases in the literature were evaluated and compared with the present series. Females were found to be more commonly affected than males. All age groups were found to be affected with a relative predilection from 40 to 60 years of age. The mandible was more often affected than the maxilla. Clinically, the lesions were red in color, had a smooth surface and were rubbery or with a soft consistency. The most characteristic histologic features included a non-encapsulated highly cellular mass with abundant giant cells, inflammation, interstitial hemorrhage, hemosiderin deposits, mature bone or osteoid.

2 cases of oral submucous fibrosis have been reported. In each case, the diagnosis was confirmed histologically, and the treatment instituted was identical. Excision of buccal fibrotic bands was carried out, with subsequent placement of skin grafts. The patients were followed postoperatively to monitor results and particularly, to screen for malignant changes. The literature concerning oral submucous fibrosis is reviewed.

This paper presents the results of animal studies on the interface between 6 aluminum, 4 vanadium, titanium alloy and bone. 7 New Zealand white rabbits were anaesthetised and a new design of threaded implant inserted into the tibial metaphysis of each leg. 3 further animals had fine lathe turnings inserted in the same site. The animals were sacrificed 7 weeks later and the implants removed together with the surrounding bone. After suitable processing of the specimens, they were subjected to scanning and transmission electron microscope studies. Compact bone was found in close proximity to the surface of the metal. With higher magnifications, the bone was found to be in intimate contact with the metal. There was no suggestion of the presence of cells or uncalcified collagen fibres between the bone and the implant. At the highest magnification, there was the appearance of a homogenous transition from the bone to the metal, suggesting a biological adhesion.
