RESUMEN

The objective of this study was to clarify the difference in thumb deformity between our case with ring chromosome 4 and thumb polydactyly, in which ulnar side was small and had no active motion, and other cases of ring chromosome 4, by analyzing previous reports. Our case had bilateral atypical thumb polydactyly, which was classified as type 4 on the right side and type 5 on the left side, according to the Japanese Society for Surgery of the Hand: Modified International Federation of Societies for Surgery of the Hand classification. Of the 39 cases in previous reports, 20 (51.3%) had thumb deformities. Among them, 19 cases (95%) showed radial ray deficiency and only 1 case (5%) showed bilateral thumb polydactyly. Clinodactyly was seen in 5 cases (12.8%). Foot deformities were present in 6 cases (15.4%). We discussed about thumb polydactyly with and without chromosomal deformities.

BACKGROUND AND AIMS: It is generally thought that bleeding from a hemangioma is difficult to stop. With development of the long pulse dye laser (LPDL), it has become possible to treat hemangioma with a large blood vessel diameter. Thus, it is effective in treating infantile hemangioma and pyogenic granuloma. MATERIALS AND METHODS: Five patients who visited our hospital from July 2015 to July 2017 due to hemorrhagic hemangioma were treated using a flash lamp excitation pulse dye laser with parameters of 7 mm spot size, 3 msec pulse width, fluence 12-14J/cm2, DCD 30 msec, and delay 30 msec. RESULTS: The bleeding not only stopped, but the raised lesion was flattened in all cases. CONCLUSIONS: LPDL is effective for both infantile hemangioma and pyogenic granuloma. It not only stops bleeding, but also treats the vascular lesions.

PURPOSE: To discuss an optimal surgical approach for impalpable testis in children, our own treatment results and those reported in the literature were reviewed. MATERIALS AND METHODS: Seventy-two impalpable testes were diagnosed in 68 patients: unilateral in 64 patients and bilateral in 4 patients. All patients underwent surgical exploration at the ages of 6 to 140months (median, 15months). The inguinal canal was initially explored, and abdominal exploration was performed with laparoscopy when an extra-abdominal testis was not identified. In addition, articles regarding surgical exploration for impalpable testis, published over the last 20years, were retrieved and the results were examined. RESULTS: Testes were detected by inguinal exploration in 28 of 72 (39%) impalpable testes: intracanalicular in 22 testes and at the internal inguinal ring (peeping or low abdominal testis) in 6 testes. All these testes were treated by conventional inguinal orchidopexy. Laparoscopic exploration was performed in 44 (61%) impalpable testes, and 4 (5.6%) high abdominal testes were detected and treated by two-stage Fowler-Stephens orchidopexy. Vanishing or absent testis was the final diagnosis in the remaining 40 testes (55.6%). The literature review showed that the ratios of intra- and extra-abdominal testes were lower in the articles that reported the results of inguinal or scrotal exploration than in those of laparoscopic exploration, although the difference was not significant. CONCLUSIONS: Considering the relatively low incidence of high abdominal testis, we recommend to start with inguinal exploration for impalpable testis. When an extra-abdominal testis is not detected, transinguinal laparoscopic exploration should be indicated. LEVEL OF EVIDENCE: Treatment study, Level IV.

Spectroscopic characteristics of a spiral flow inductively coupled plasma (ICP), which could be sustained stably at 9 L min(-1) of Ar plasma gas flow rate with 1.5 kW RF forward power, were studied for axially viewing ICPOES. The emission intensity profile, excitation temperature and plasma robustness were evaluated, and were similar to those of the standard ICP. The background and emission intensities of elements as well as the excitation behavior for both atom and ion lines were also examined and compared to those of the standard ICP. Since the spectroscopic characteristics of the spiral flow ICP were similar to those of the standard ICP, it could be used as a new low gas flow ICP in axially viewing ICPOES.

The expression of cell surface antigens and bcl-2 protein was examined with flow cytometry in 30 lymphoma samples including 28 lymph nodes and others, from January, 2001 to May, 2003. Cases studied included the following: follicular lymphoma (FL), 11; diffuse large B-cell lymphoma (DLBCL), 12; diffuse small B-cell lymphoma, 1; mantle cell lymphoma, 1; MALT lymphoma, 1; precursor-B lymphoblastic lymphoma, 1; T-lymphoblastic lymphoma, 1; blastic NK-cell lymphoma, 1; and unknown, 1. In addition, cytogenetic analysis, fluorescence in situ hybridization (FISH), and a real-time polymerase chain reaction assay were performed. Strong bcl-2 expression was observed in 91% of FL, and 17% of DLBCL. On the other hand, strong bcl-2 expression was found in all 9 cases of FL with t(14;18), all two cases of DLBCL with t(14;18), one unknown case with t(14;18), and one FL case with t(11;14). Our results support that strong bcl-2 expression as demonstrated with flow cytometry is correlated with FL (p<0.0001), and lymphomas with the t(14;18) (p<0.0001).
