RESUMEN

A 32-year-old woman underwent laparotomy and right salpingo-oophorectomy for a right adnexal mass. Examination of the specimen revealed a 77 g well-encapsulated tumor that was diagnosed as a mixed germ cell-sex cord-stromal tumor with annular tubules. Sixty-seven months after an uneventful recovery, a large recurrence was treated by total abdominal hysterectomy, left salpingo-oophorectomy, removal of peritoneal implants, lymphadenectomy, omentectomy, and chemotherapy. Histologically, the neoplasm was identical to the one found in the right ovary. To our knowledge, no similar case has been previously reported.

RESUMEN

Small-bore (22- or 23-gauge) needles were used to aspirate 458 lung masses. Sensitivity for the detection of malignancy by this method was 96.6% (312 of 323 patients); accuracy was 98.7%. Surgical confirmation was available for comparison in over half of patients with malignancy. Histologic reclassification of malignancy occurred in only 6.1% of patients, with significant misclassification (misdiagnosis of small cell carcinoma) occurring in only two instances. One hundred thirteen of 117 nonmalignant conditions were properly categorized, with an overall specificity of 96.6%. No major complication occurred. While several recent studies have stressed the advantage of using larger needles, to overcome the limitation of smaller aspiration needles that provide only cytologic material, small-needle aspiration appears to be a safe, reliable, and accurate means for diagnosing lung lesions.

RESUMEN

Eighteen Holstein heifer calves were allotted to two groups. Calves in one group were dehorned electrically at 8 wk of age whereas the control group remained horned through 12 wk of age. Feed intake and growth rate did not differ between the two groups. Blood constituents varied with age, nutrition of calf, and dehorning. Dehorning influenced cortisol in blood plasma with high concentrations (25 versus 6 ng/ml) measured for 1 h after dehorning. Electrical dehorning at 8 wk of age presents no long-term stress to calves.

RESUMEN

Pyostomatitis vegetans is a rare and unusual disorder of the oral cavity, characterized by erythema and edema of the mucosa and numerous small, superficial yellow pustules. Its significance lies in its association with inflammatory disease of the bowel, either ulcerative colitis or Crohn's disease. We report here a patient with pyostomatitis vegetans who, upon medical workup, was discovered to have asymptomatic Crohn's disease. The clinical features, histopathology, and treatment of pyostomatitis vegetans, and its purported relationship to pemphigus vegetans of Neumann are discussed.

Paraffin sections of submandibular, sublingual, minor salivary, and parotid glands from ten human autopsy cases were stained with a battery of ten lectins conjugated to horseradish peroxidase. Variable affinity for one or another lectin between mucous cells in a gland evidenced cellular heterogeneity in mucin production. Mucous cells of a given type of gland varied among individuals, but for a single individual appeared markedly but not completely similar from one type of salivary gland to another. The individual variation related, in part, to the ABO blood group and secretor status of the individual. For mucous cells in secretors of blood group A and B all antigens stained strongly for the presence of terminal alpha-N-acetylgalactosamine or alpha-galactose, respectively. Mucous cells in AB secretors contained both antigens, whereas those of O (H) secretors lacked both. Mucous cells of three presumed nonsecretors, two of whom were immature infants and possibly too young to produce ABO antigen, failed to stain. Mucous cells in glands from the presumed nonsecretors, however, revealed a staining pattern consistent with the presence of Lea antigen. Mucous cells of nonsecretors stained with Lotus tetragonolobus agglutinin but not with Ulex europeus I agglutinin, whereas mucous cells of ABO secretors stained with both lectins. This difference in lectin binding indicated that sites reactive only with Lotus tetragonolobus agglutinin contain 1----4 linked fucosyl residues and sites stained by both lectins contain fucose linked 1----2 to the oligosaccharide. Staining of mucous cells of nonsecretors with Pisum sativum agglutinin indicate that either the lectin binds to internal N-acetylglucosamine of Lea substance or the mucous cells contain an N-glycosidic glycoprotein of the type thought to bind this lectin. Serous cells stained less strongly than mucous cells and differed in lectin affinities from one type of gland to another in an individual. Staining of serous cells of a given gland varied markedly among different subjects. This individual variability did not relate to blood group as terminal sugars demonstrative of A or B blood group antigens were not detected in any serous cells. Serous cells in the submandibular glands from the two immature infants were unreactive with all lectin conjugates. Secretions in parotid and submandibular serous cells generally contained a higher content of fucose than those in sublingual serous cells, which contained higher levels of a terminal galactose-sialic acid dimer. Some but not other cells of striated and interlobular ducts of submandibular glands of one subject stained for alpha-N-acetylgalactosamine.

RESUMEN

Although approximately 50 cases of nodular pulmonary amyloidosis have been described in the literature, few of them have involved bilateral confluent disease. An exceptional case with disease extensive enough to cause death by respiratory insufficiency is reported. A remarkable degree of calcification in the subpleural areas, with osseous and chondroid metaplasia, characterized the amyloid deposits. Amyloid was confined to the respiratory tract except for contiguous areas (vertebral body cortex, epicardial fat, and hilar lymph nodes). A review and update of the literature since 1974 is included.
