OBJECTIVES: Encephalitis is a severe neurological syndrome for which herpesvirus and enteroviruses are the most common etiological agents. Arboviruses, a wildly diverse group of pathogens, are also critical epidemiological agents associated with encephalitis. In Brazil, little is known about the causative agents of encephalitis. METHODS: We conducted a hospital surveillance for encephalitis between 2020 and 2022. Molecular (RT-PCR and qPCR) and serological (virus-specific IgM and viral antigens) techniques were performed in cerebrospinal fluid and serum samples obtained from study participants. RESULTS: In the 43 participants evaluated, the etiologic agent or the presence of IgM was detected in 16 (37.2%). Nine (20.9%) cases were positive for chikungunya virus (CHIKV), three (7.0%) for dengue virus, two (4.7%) for human adenovirus, one (2.3%) for varicella-zoster virus, and one (2.3%) for enterovirus. Whole-genome sequencing revealed that the CHIKV identified belongs to the East/Central/South African lineage. CONCLUSION: Herein, CHIKV is a common pathogen identified in encephalitis cases. Our results reinforce previous evidence that chikungunya represents a significant cause of encephalitis during CHIKV outbreaks and epidemics and add to existing information on the epidemiology of encephalitis in Brazil.

Dengue fever is among the most significant public health concerns in Brazil. To date, the highest number of Dengue notifications in the Americas has been reported in Brazil, with cases accounting for a total number of 3,418,796 reported cases as of mid-December 2022. Furthermore, the northeastern region of Brazil registered the second-highest incidence of Dengue fever in 2022. Due to the alarming epidemiological scenario, in this study, we used a combination of portable whole-genome sequencing, phylodynamic, and epidemiological analyses to reveal a novel DENV-1 genotype V clade and the persistence of DENV-2 genotype III in the region. We further report the presence of non-synonymous mutations associated with non-structural domains, especially the NS2A (non-structural protein 2A), as well as describe synonymous mutations in envelope and membrane proteins, distributed differently between clades. However, the absence of clinical data at the time of collection and notification, as well as the impossibility of monitoring patients in order to observe worsening or death, restricts our possibility of correlating mutational findings with possible clinical prognoses. Together, these results reinforce the crucial role of genomic surveillance to follow the evolution of circulating DENV strains and understand their spread across the region through inter-regional importation events, likely mediated by human mobility, and also the possible impacts on public health and outbreak management.

This work presents a one-step synthesis methodology for preparing a hydrochar (HC) doped with TiO2 (HC-TiO2) for its application on the degradation of crystal violet (CV) using UV and visible radiation. Byrsonima crassifolia stones were used as precursors along with TiO2 particles. The HC-TiO2 sample was synthesized at 210 °C for 9 h using autogenous pressure. The photocatalyst was characterized to evaluate the TiO2 dispersion, specific surface area, graphitization degree, and band-gap value. Finally, the degradation of CV was investigated by varying the operating conditions of the system, the reuse of the catalyst, and the degradation mechanism. The physicochemical characterization of the HC-TiO2 composite showed good dispersion of TiO2 in the carbonaceous particle. The presence of TiO2 on the hydrochar surface yields a bandgap value of 1.17 eV, enhancing photocatalyst activation with visible radiation. The degradation results evidenced a synergistic effect with both types of radiation due to the hybridized π electrons in the sp2-hybridized structures in the HC surface. The degradation percentages were on average 20% higher using UV radiation than visible radiation under the following conditions: [CV] = 20 mg/L, 1 g/L of photocatalyst load, and pH = 7.0. The reusability experiments demonstrated the feasibility of reusing the HC-TiO2 material up to 5 times with a similar photodegradation percentage. Finally, the results indicated that the HC-TiO2 composite could be considered an efficient material for the photocatalytic treatment of water contaminated with CV.

During these past years, several studies have provided serological evidence regarding the circulation of West Nile virus (WNV) in Brazil. Despite some reports, much is still unknown regarding the genomic diversity and transmission dynamics of this virus in the country. Recently, genomic monitoring activities in horses revealed the circulation of WNV in several Brazilian regions. These findings on the paucity of genomic data reinforce the need for prompt investigation of WNV infection in horses, which may precede human cases of encephalitis in Brazil. Thus, in this study, we retrospectively screened 54 suspicious WNV samples collected between 2017 and 2020 from the spinal cord and brain of horses with encephalitis and generated three new WNV genomes from the Ceará and Bahia states, located in the northeastern region of Brazil. The Bayesian reconstruction revealed that at least two independent introduction events occurred in Brazil. The first introduction event appears to be likely related to the North American outbreak, and was estimated to have occurred in March 2013.The second introduction event appears to have occurred in September 2017 and appears to be likely related to the South American outbreak. Together, our results reinforce the importance of increasing the priority of WNV genomic monitoring in equines with encephalitis in order to track the dispersion of this emerging pathogen through the country.

RT-PCR testing data provides opportunities to explore regional and individual determinants of test positivity and surveillance infrastructure. Using Generalized Additive Models, we explored 222,515 tests of a random sample of individuals with COVID-19 compatible symptoms in the Brazilian state of Bahia during 2020. We found that age and male gender were the most significant determinants of test positivity. There was evidence of an unequal impact among socio-demographic strata, with higher positivity among those living in areas with low education levels during the first epidemic wave, followed by those living in areas with higher education levels in the second wave. Our estimated probability of testing positive after symptom onset corroborates previous reports that the probability decreases with time, more than halving by about two weeks and converging to zero by three weeks. Test positivity rates generally followed state-level reported cases, and while a single laboratory performed ~90% of tests covering ~99% of the state's area, test turn-around time generally remained below four days. This testing effort is a testimony to the Bahian surveillance capacity during public health emergencies, as previously witnessed during the recent Zika and Yellow Fever outbreaks.

Brazil experienced a large dengue virus (DENV) epidemic in 2019, highlighting a continuous struggle with effective control and public health preparedness. Using Oxford Nanopore sequencing, we led field and classroom initiatives for the monitoring of DENV in Brazil, generating 227 novel genome sequences of DENV1-2 from 85 municipalities (2015-2019). This equated to an over 50% increase in the number of DENV genomes from Brazil available in public databases. Using both phylogenetic and epidemiological models we retrospectively reconstructed the recent transmission history of DENV1-2. Phylogenetic analysis revealed complex patterns of transmission, with both lineage co-circulation and replacement. We identified two lineages within the DENV2 BR-4 clade, for which we estimated the effective reproduction number and pattern of seasonality. Overall, the surveillance outputs and training initiative described here serve as a proof-of-concept for the utility of real-time portable sequencing for research and local capacity building in the genomic surveillance of emerging viruses.

A pandemia da Covid-19 tem se apresentado como um dos maiores desafios sanitários desse século. Em dezembro de 2019, na China, o agente etiológico foi identificado como um novo coronavírus, nomeado SARS-CoV-2. No Brasil, o primeiro caso confirmado da Covid-19 ocorreu em fevereiro de 2020 e, no mês seguinte, a Secretaria da Saúde do Estado da Bahia (Sesab) confirmou o primeiro caso na Bahia.O Laboratório Central de Saúde Pública Prof. Gonçalo Moniz (Lacen-BA) centralizou o diagnóstico laboratorial para confirmação dos casos suspeitos de Covid-19 dos 417 municípios baianos, utilizando a técnica de RT-PCR. Este estudo tem como objetivo identificar e analisar as não conformidades das amostras suspeitas de Covid-19 encaminhadas ao Lacen-BA. Trata-se de um estudo descritivo, cujos dados foram obtidos por meio de consulta aos relatórios de amostras e exames em desacordo, disponíveis no sistema Gerenciador de Ambiente Laboratorial (GAL), gerados mensalmente, no período de abril a outubro de 2020. Para garantir a qualidade das amostras recebidas, foram definidos critérios de aceitação/rejeição de amostras e criado o formulário de notificação de não conformidades, assegurando a rastreabilidade das amostras de Covid-19. Através de relatórios diários do sistema GAL, selecionou-se os nove principais motivos de não conformidades, sendo o mais frequente "requisição cancelada pela gerência do GAL devido à expiração do prazo de triagem", com 72,8% dos registros. A inserção da padronização de processos na etapa pré-analítica permite trabalhar com segurança, garantindo a qualidade da amostra a ser processada e, consequentemente, um resultado fidedigno, dentro do prazo acordado.
The Covid-19 pandemic is one of the greatest health challenges of this century. In December 2019, in China, the etiologic agent was identified as a new coronavirus, named SARS-CoV-2. In Brazil, the first case of Covid-19 was confirmed in February 2020 and, in the following month, the Department of Health of the State of Bahia (Sesab) confirms the first case in the state. The Central Public Health Laboratory Prof. Gonçalo Moniz (Lacen/BA) centralized the laboratory diagnosis to confirm the suspected cases of Covid-19 of the 417 municipalities of the state, using the RT-PCR technique. This study aims at identifying and analyzing the non-conformities of the suspected samples of Covid-19 sent to Lacen-BA. This is a descriptive study whose data were obtained by consulting there reports of samples and exams in disagreement, available in the Laboratory Environment Manager (GAL) system, generated monthly, from April to October,2020. To guarantee the quality of the samples received, acceptance / rejection criteria for the samples were defined and a form for the notification of non-conformities was created, ensuring the traceability of the Covid-19 samples. Daily reports from the Laboratory Environment Manager system based the selection of nine main reasons for non-conformities, among which "requisition canceled by the management of the GAL due to the expiration of the screening period" was present in 72.8% of the records. Process standardization, in the pre-analytical stage, allows working with security, guaranteeing the quality of the sample to be processed and a reliable result within the established period.
La pandemia del Covid-19 se ha presentado como uno de los desafíos de salud más grandes de este siglo. En diciembre de 2019, China identificó el agente etiológico del nuevo coronavirus llamado SARS-CoV-2. En Brasil, se notificó el primer caso del Covid-19 en febrero de 2020 y, al mes siguiente, la Secretaría de Salud del Estado de Bahía (Sesab) confirmaba el primer caso en Bahía. El Laboratorio Central de Salud Pública Prof. Gonçalo Moniz (Lacen/BA) centralizó el diagnóstico de laboratorio para confirmar los casos sospechosos del coronavirus de los 417 municipios de Bahía, mediante la técnica de RT-PCR. Este estudio tiene como objetivo identificar y analizar las no conformidades de las muestras sospechosas del Covid-19 enviadas al Lacen/BA. Este es un estudio descriptivo cuyos datos se obtuvieron consultando los informes de muestras y pruebas en desacuerdo disponibles en el sistema Laboratory Environment Manager (GAL), generados mensualmente, de abril a octubre/2020. Con el fin de garantizar la calidad de las muestras recibidas, se definieron criterios de aceptación/rechazo de las muestras y se elaboró un formulario para la notificación de no conformidades, asegurando la trazabilidad de las muestras. Por medio de informes diarios del sistema Laboratory Environment Manager, se seleccionaron nueve principales causas de no conformidades, de las cuales la más frecuente fue "requisición cancelada por la gerencia del GAL por vencimiento del período de cribado" con el 72,8% de los registros. La inserción de la estandarización de procesos en la etapa preanalítica permite trabajar con seguridad, garantizando la calidad de la muestra que procesar y, en consecuencia, un resultado confiable dentro del plazo acordado.

A Covid-19 é uma doença infecciosa causada pelo novo coronavírus, denominado SARS-CoV-2, que causou um surto de pneumonia viral incomum em pacientes em Wuhan, na China, no final do ano de 2019. O vírus se disseminou pelo mundo em grandes proporções, atingindo o status epidemiológico de pandemia. Diante desse cenário, que afetou toda a Federação brasileira, o Laboratório Central de Saúde Pública Professor Gonçalo Moniz (Lacen-BA) tem exercido papel fundamental no diagnóstico da Covid-19 e na vigilância genômica do SARS-CoV-2. Nesse sentido, este estudo tem como objetivo descrever as estratégias implementadas pelo Lacen-BA para ampliar a capacidade diagnóstica e atender a demanda da rede SUS-BA no contexto da pandemia da Covid-19. Trata-se de um estudo descritivo-observacional, orientado por um modelo lógico sustentado em quatro dimensões: parque tecnológico, metodologias analíticas, descentralização do exame e monitoramento de indicadores de resultados. As iniciativas de gestão possibilitaram ampliação da capacidade instalada e operacional, mediante modernização da estrutura física, renovação do parque tecnológico, reorganização dos fluxos e processos de trabalho, aporte de novas tecnologias analíticas e estruturação de dashboard para monitorar indicadores e subsidiar o processo decisório. O Lacen-BA, enquanto coordenador da Rede Estadual de Laboratórios de Saúde Pública e sistema de apoio da Rede de Atenção à Saúde (RAS), constitui-se então em estruturas policêntricas essenciais para o diagnóstico descentralizado e regionalizado da Covid-19, contribuindo para a integração sistêmica das ações e serviços no contexto da regionalização da saúde, de modo a garantir a universalidade do acesso e integralidade dos cuidados aos usuários do SUS.
Covid-19 is an infectious disease caused by the new coronavirus, called SARS-CoV-2, which caused an outbreak of unusual viral pneumonia in patients in Wuhan, China, at the end of 2019 and spread across the world, in large proportions, reaching the epidemiological status of a pandemic. Considering this epidemiological scenario that affected the entire Brazilian Federation, the Central Laboratory of Public Health Professor Gonçalo Moniz (Lacen-BA) has played a fundamental role in the diagnosis of Covid-19 and the genomic surveillance of SARS-CoV-2. In this sense, this study aims at describing the strategies implemented by Lacen-BA to expand the diagnostic capacity to meet the demand of the SUS-BA network, in the context of the Covid-19 pandemic. This is a descriptive-observational study, guided by a logical model based on four dimensions: technological park, analytical methodologies, decentralization of the exam and monitoring of result indicators. The management initiatives enabled the expansion of the installed and operational capacity by modernizing the physical structure, renewing the technological park, reorganizing workflows and processes, providing new analytical technologies, structuring the dashboard to monitor indicators and support the decision-making process. The Lacen-BA, as coordinator of the State Public Health Laboratory Network and support system of the Health Care Network (RAS), constitutes essential polycentric structures for the decentralized and regionalized diagnosis of Covid-19, which can contribute to the systemic integration of actions and services in the context of regionalization of health to guarantee the universality of access and comprehensive care to SUS users.
El covid-19 es una enfermedad infecciosa causada por el nuevo coronavirus, llamado SARS-CoV-2, que provocó un brote de neumonía viral inusual en pacientes en Wuhan, China, a fines de 2019, y que se extendió por el mundo, en grandes proporciones hasta alcanzar el estado epidemiológico de pandemia. Ante este escenario epidemiológico que afectó a Brasil, el Laboratorio Central de Salud Pública Profesor Gonçalo Moniz (Lacen-BA) ha jugado un papel fundamental en el diagnóstico del covid-19 y la vigilancia genómica del SARS-CoV-2. En este sentido, este estudio tiene como objetivo describir las estrategias implementadas por Lacen-BA para ampliar la capacidad de diagnóstico y atender la demanda de la red SUS-BA, en el contexto de la pandemia del Covid-19. Este estudio es descriptivo-observacional, guiado por un modelo lógico con base en cuatro dimensiones: parque tecnológico, metodologías analíticas, descentralización del examen y seguimiento de indicadores de resultado. Las iniciativas de gestión permitieron ampliar la capacidad instalada y operativa al modernizar la estructura física, renovar el parque tecnológico, reorganizar los flujos y procesos de trabajo, brindar nuevas tecnologías analíticas y estructuración del cuadro de mando para monitorear indicadores, y apoyar la toma de decisiones. Lacen-BA, como coordinador de la Red Estadual de Laboratorios de Salud Pública y sistema de apoyo de la Red de Atención a la Salud (RAS), constituye estructuras policéntricas imprescindibles para el diagnóstico descentralizado y regionalizado del Covid-19, que pueden contribuir a la integración sistémica de acciones y servicios en el contexto de la regionalización de la salud, a fin de garantizar la universalidad del acceso y la atención integral a los usuarios del SUS.

PURPOSE: To describe the implementation of a postfracture care program in a private hospital in Colombia, the results achieved after the program's first year, and the challenges encountered. METHODS: A cross-sectional descriptive study of the first year's outcomes. The program was implemented following best practices described in the "Capture the Fracture" framework. We assessed the management of fractures before the launch of the program. A multidisciplinary group was established to collaborate on the diagnosis and treatment of patients with osteoporotic fractures. A full-time program coordinator was appointed. We analyzed the program's clinical outcomes and limitations. RESULTS: One-hundred and ninety patients were included in the study, with an average age of 76.7. Hip fracture was the most frequent one (33.6%). After the first year of implementing the program, 39.4% of patients received osteoporosis treatment, with an adherence rate of 73%. The incidence of subsequent falls was 5.8% and 1% for new fractures. CONCLUSIONS: The implementation of a program for patients' care with fragility fractures is challenging for healthcare institutions. The role of a full-time coordinator is critical for the proper operation of such programs.

Yellow fever virus (YFV) causes a clinical syndrome of acute hemorrhagic hepatitis. YFV transmission involves non-human primates (NHP), mosquitoes and humans. By late 2016, Brazil experienced the largest YFV outbreak of the last 100 years, with 2050 human confirmed cases, with 681 cases ending in death and 764 confirmed epizootic cases in NHP. Among affected areas, Bahia state in Northeastern was the only region with no autochthonous human cases. By using next generation sequence approach, we investigated the molecular epidemiology of YFV in NHP in Bahia and discuss what factors might have prevented human cases. We investigated 47 YFV positive tissue samples from NHP cases to generate 8 novel YFV genomes. ML phylogenetic tree reconstructions and automated subtyping tools placed the newly generated genomes within the South American genotype I (SA I). Our analysis revealed that the YFV genomes from Bahia formed two distinct well-supported phylogenetic clusters that emerged most likely of an introduction from Minas Gerais and Espírito Santo states. Vegetation coverage analysis performed shows predominantly low to medium vegetation coverage in Bahia state. Together, our findings support the hypothesis of two independent YFV SA-I introductions. We also highlighted the effectiveness of the actions taken by epidemiological surveillance team of the state to prevented human cases.

Abstract. In the face of the coronavirus pandemic (COVID-19), in hospital and emergency units, there is low availability of mechanical respirator for patients in need of this support, greatly improving the survival rate. In these situations, there is a need for simpler equipment, easy access, low cost, and fast manufacturing. In this study, a 3D prototype transport respirator was developed using as a model the Takaoka 600 Mini Respirator, national technology from the 1950s. The influence of adjustable parameters of the respirator was evaluated to understand it is functioning: maximum and minimum lung pressure; respirator intake pressure; respiratory rate; inspiratory and expiratory time according to the sensitivity of the mini respirator; and pressure and flow of O2 line intake. The increase in sensitivity led to an increase in maximum and minimum pulmonary pressure, decreased inspiratory and expiratory time, with margins of 1/1, 1/2, 1/3 inspiratory/expiratory time ratio (I/E ratio). The intake flow of O2 varied proportionally with the pressure of air intake into the respirator, with its increase leading to an increase in respiratory rate, without major influences on lung pressure and the I/E ratio. The O2 line intake pressure without major influences on lung pressure, showing and I/E ratio >1 in values below 3.5 kPa x 100. In conclusion, it was possible to obtain a pulmonary ventilator-dependent only on positive O2 flow, compact and effective for patient transport, and in cases of emergencies with control of maximum pressure and respiratory rate offered to the patient. Among the parameters evaluated for this respirator, an line pressure of O2 from 3.5 kPa x 100, sensitivity between 3 and 5, a flow of 5 to 15 L/min is recommended.

Objetivo: investigar os casos de febre maculosa, verificar os aspectos relacionados à assistência à saúde e caracterizar o perfil epidemiológico desses e do ambiente de infecção. Métodos: estudo descritivo dos casos confirmados de FM entre janeiro e novembro de 2017. Foi elaborado um formulário semiestruturado para coleta dos dados. Para as análises epidemiológicas, foram utilizados os programas EpiInfoTM 7 e Qgis® 2.18.1. Foram calculadas frequências, medidas de tendência central, dispersão e indicadores de oportunidade de assistência. Nos ambientes prováveis de infecção, foram realizadas coletas de carrapatos que foram identificados e submetidos à pesquisa de Rickettsia do grupo da FM. Resultados: foram confirmados 15 casos de FM; desses, a maioria foi do sexo masculino (93%) com média de 42 anos de idade; 10 evoluíram para óbito. Todos apresentaram febre, 13(87%) cefaleia e 12(80%) mialgia. Apresentaram mediana e média de evolução para óbito pela doença de 6 dias (1 a 9), oportunidades de hospitalização, tratamento, diagnóstico e notificação de 4,5(0 a 8); 7(0 a 26); 9(DP±6) e 33(DP±45) dias, respectivamente. Para assistência à saúde, 14(93%) casos procuraram atendimento mais de duas vezes, com tempo de internação de 4,5 dias (0 a 8 dias). O hospital foi o serviço procurado, exclusivamente, no quarto atendimento. Dengue e FM foram as hipóteses diagnósticas mais frequentes. Foram coletados 250 espécimes de carrapatos em quatro municípios, sendo em um município identificado Amblyomma sculptum naturalmente infectados. Conclusões: foram assinaladas condições assistenciais pouco sensíveis à ocorrência da FM, o que, provavelmente, colaborou para ocorrência de óbitos, além da necessidade de sensibilização de equipes de saúde e vigilância quanto à ocorrência da FM na região.
Objective: investigate the cases of spotted fever, verify aspects related to health care and characterize the epidemiological profile of these and of the environment of infection. Methods: A descriptive study of confirmed cases of SF between January and November 2017. A semi-structured form for data collection was developed. EpiInfoTM 7 and Qgis® 2.18.1. Frequencies, measures of central tendency, dispersion and indicators of opportunity of assistance were calculated. In the probable environments of infection ticks were collected, identified and submitted to Rickettsia research of the SF group. Results: Fifteen SF cases were confirmed; of these, the majority was male (93%) and averaged 42 years old; 10 evolved to death. All presented fever, 13 (87%) headache and 12 (80%) myalgia. They presented an average of evolution to death by SF of 6 days (1 to 9), opportunities for hospitalization, treatment, diagnosis and notification of 4.5(0 to 8); 7(0 to 26); 9(DP± 6) and 33(DP± 45) days respectively. For health care, 14(93%) cases sought care more than twice and were hospitalized for 4.5-days (0 to 8 days). The hospital was the only service exclusively sought in the fourth service. Dengue and SF were the most frequent diagnostic hypotheses. A total of 250 specimens of ticks were collected in four counties, and in one municipality Amblyomma sculptum was identified as naturally infected. Conclusions: It was pointed out that assistance conditions were not very sensitive to the occurrence of SF which probably contributed to the instance of deaths as well as the need for sensitization of health and surveillance teams regarding the appearance of SF in the region.

Objetivo: investigar surto de toxoplasmose no município de Gouveia-MG, no período entre fevereiro e maio de 2015. Método: foram realizados estudos descritivo e caso-controle, com análise multivariada (regressão logística) e nível de significância de 5%. Definiu-se como caso o indivíduo com toxoplasmose aguda, confirmada laboratorialmente (IgM e/ou IgG reagentes e baixa avidez de IgG), e controle o indivíduo susceptível (IgM e IgG não reagentes). Resultado: o estudo contemplou 50 casos e 68 controles, sendo a maioria dos casos do sexo masculino (43 pacientes, 86%). Os casos tiveram 10 vezes mais chance de tomar água procedente do Manancial (IC95%: 2,8-37,4; p=0,01), 5,1 vezes mais chance de ingerir linguiça (IC95%: 1,2-20,4; p=0,02) e 4,5 vezes mais chance de ser do sexo masculino (IC95%: 1,2-16,7; p=0,02), quando comparados aos controles. Conclusão: foi confirmado um surto de toxoplasmose aguda, em Gouveia - MG, de transmissão hídrica e/ou alimentar. A maior ocorrência no sexo masculino pode estar relacionada a hábitos de risco, envolvidos na transmissão da doença. Foram propostas medidas que resultaram na diminuição e no controle dos casos da doença.
Objective: to investigate the outbreak of toxoplasmosis in the municipality of Gouveia-MG, between February and May 2015. Methods: Descriptive and case control study, with multivariate analysis (logistic regression) and level of significance of 5%. For the purpose of the study, a case was defined on an individual with laboratory which confirmed acute toxoplasmosis (IgM and/or IgG with low IgG avidity) and a control was a susceptible individual (nonreactive IgM and IgG). Results: The study comprised 50 cases and 68 controls, with most cases being male (43 patients, 86%). When compared to controls, cases had 10 times greater odds of having drunk water from Stream A (IC95%: 2,8-37,4; p=0,01), 5,1 times greater odds of having ingested sausage (IC95%: 1,2-20,4; p=0,02) and 4.5 times greater odds of being of male sex (IC95%: 1,2-16,7; p=0,02). Conclusion: A water/foodborne outbreak of acute toxoplasmosis was confirmed in Gouveia-MG, affecting predominantly men. This may be associated with habits regarding disease transmission. Measures that resulted in the reduction and control of cases of this disease have been proposed.

We report an ongoing measles outbreak in Manaus, Amazonas state, Brazil. As at 3 November 2018, 1,631 cases were confirmed corresponding to an incidence of 75.3 per 100,000 inhabitants; all five sanitary districts presented confirmed cases. Reintroduction of measles virus in Manaus is likely related to the current outbreak in Venezuela and due to recent decline in measles vaccine coverage. Given the current scenario, prevention and control measures should target individuals aged 15-29 years.

Although invasive infections and mortality caused by Candida species are increasing among compromised patients, resistance to common antifungal agents is also an increasing problem. We analyzed 60 yeasts isolated from patients with invasive candidiasis using a PCR/RFLP strategy based on the internal transcribed spacer (ITS2) region to identify different Candida pathogenic species. PCR analysis was performed from genomic DNA with a primer pair of the ITS2-5.8S rDNA region. PCR-positive samples were characterized by RFLP. Restriction resulted in 23 isolates identified as C. albicans using AlwI, 24 isolates as C. parapsilosis using RsaI, and 13 as C. tropicalis using XmaI. Then, a group of all isolates were evaluated for their susceptibility to a panel of previously described killer yeasts, resulting in 75% being susceptible to at least one killer yeast while the remaining were not inhibited by any strain. C. albicans was the most susceptible group while C. tropicalis had the fewest inhibitions. No species-specific pattern of inhibition was obtained with this panel of killer yeasts. Metschnikowia pulcherrima, Pichia kluyveri and Wickerhamomyces anomalus were the strains that inhibited the most isolates of Candida spp.(AU)

OBJETIVO: to investigate the occurrence of GBS in the Metropolitan Region of Recife, PE, Brazil, 2015. METHODS: this was a descriptive study using data from the Hospital Information System, National Pharmaceutical Services Management System and interviews; GBS cases were classified according to Brighton criteria and prior infection according to laboratory and clinical criteria. RESULTS: in 2015, the number of GBS hospitalizations had a threefold increase in comparison to 2014. We investigated 44 confirmed or probable GBS cases, of which 18 had symptoms of Zika infection up to 35 days before the occurrence of GBS, mainly rash; one case was laboratory-confirmed for Zika virus infection and one death was registered. CONCLUSION: the findings reinforce a possible relationship between GBS and Zika infection, given the absence of increased GBS occurrence in previous dengue epidemic years, absence of chikungunya transmission records, presence of clinical manifestations compatible with infection and a laboratory confirmation.

The number of substances nominally listed in the prohibited list of the World Anti-Doping Agency increases each year. Moreover, many of these substances do not have a single analytical target and must be monitored through different metabolites, artifacts, degradation products, or biomarkers. A new analytical method was developed and validated for the simultaneous analysis of peptides and organic molecules using a single sample preparation and LC-Q-HRMS detection. The simultaneous analysis of 450 target molecules was performed after cleanup on a mixed-mode solid-phase extraction cartridge, combined with untreated urine. The cleanup solvent and reconstitution solvent were the most important parameters for achieving a comprehensive sample preparation approach. A fast chromatographic run based on a multistep gradient was optimized under different flows; the detection of all substances without isomeric coelution was achieved in 11 minutes, and the chromatographic resolution was considered a critical parameter, even in high-resolution mass spectrometry detection. The mass spectrometer was set to operate by switching between positive and negative ionization mode for FULL-MS, all-ion fragmentation, and FULL-MS/MS2 . The suitable parameters for the curved linear trap (c-trap) conditions were determined and found to be the most important factors for the development of the method. Only FULL-MS/MS2 enables the detection of steroids and peptides at concentrations lower than the minimum required performance levels set by World Anti-Doping Agency (1 ng mL-1 ). The combination of the maximum injection time of the ions into the c-trap, multiplexing experiments, and loop count under optimized conditions enabled the method to be applied to over 10 000 samples in only 2 months during the 2016 Rio Summer Olympic and Paralympic Games. The procedure details all aspects, from sample preparation to mass spectrometry detection. FULL-MS data acquisition is performed in positive and negative ion mode simultaneously and can be applied to untargeted approaches.
