RESUMEN

BACKGROUND: Clinicians caring for neonates with congenital heart disease encounter challenges with ethical implications in daily practice and must have some basic fluency in ethical principles and practical applications. METHODS: Good ethical practice begins with a thorough understanding of the details and narrative of each individual case, examination via classic principles of bioethics, and further framing of that translation into practice. RESULTS: We explore some of these issues and expand awareness through the lens of a case presentation beginning with fetal considerations through end-of-life discussions. CONCLUSIONS: We include specific sections that bring attention to shared decision-making, research ethics, and outcomes reporting. We review empirical evidence and highlight recommendations.

BACKGROUND: Chemotherapy-induced peripheral neuropathy is a common and often severe side effect from many chemotherapeutic agents, with limited treatment options. There is no literature on the use of topical cannabinoids for chemotherapy-induced neuropathy. CASE PRESENTATIONS: The current manuscript presents a case series of patients presenting in oncology clinics at Sutter Health, CA and Mayo Clinic, Rochester, MN from April 2019 to December 2020 with chemotherapy-induced peripheral neuropathy who used topical creams containing the cannabinoids delta-nine-tetrahydrocannabinol (THC) and/or cannabidiol (CBD). CONCLUSIONS: This case series suggests that topical cannabinoids may be helpful for patients with chemotherapy-induced peripheral neuropathy. This paper also discusses the potential mechanisms of action by which topical cannabinoids might alleviate established CIPN symptoms. A randomized placebo-controlled trial using a standardized product is planned to study the actual efficacy of such treatment.

BACKGROUND: Cardiac tumors are uncommon in the pediatric population. When present, cardiac manifestations stem from the tumor causing inflow or outflow obstruction. While common in adults, cardiac myxomas presenting with generalized systemic illness or peripheral emboli especially with no cardiac or neurological symptoms are rare in children. CASE PRESENTATION: We report a case of a previously healthy adolescent girl who presented with a 6-month history of constitutional symptoms and a purpuric rash with no cardiac or neurologic symptoms, found to have a cardiac myxoma. CONCLUSIONS: A vasculopathic rash in the setting of atrial myxomas has been shown be a precursor to significant morbidity and mortality. Due to the rarity of this entity, the time elapsed from onset of non-cardiac symptoms until diagnosis of a myxoma is usually prolonged with interval development of irreversible neurological sequelae and death reported in the literature. Therefore, we highlight the importance of including cardiac myxomas and paraneoplastic vasculitis early in the differential diagnosis for patients presenting with a purpuric rash and systemic symptoms.

RESUMEN

There are several etiologies of abdominal pain during pregnancy, including an extremely rare: the intestinal volvulus. Have been reported about 78 cases in the literature of intestinal volvulus complicating pregnancy, we reported this case occurred in our hospital and to assess the difficult diagnosis of intestinal volvulus in pregnancy.

BACKGROUND: Cervical and endometrial measurement in the gynecological and obstetric patients is of vital importance. There is no consensus for the correct way in which should be made the measurement, more than anything is for an opinion of some experts. OBJECTIVE: To determine whether there are differences in measurement of endometrial or sagittal cross section of the uterus and cervix in the measurement of fractional linear fashion or along the cervical canal. MATERIAL AND METHODS: Using a transvaginal ultrasound, was measured in a transverse and longitudinal endometrial lining. And by fractional measuring along the cervical canal and a linear manner from the internal to the external. RESULTS: We studied a total of 63 patients. The mean endometrial transversal measurement in a cross was 7.1. mm (SD +/- 3.3) The mean endometrial longitudinally measured were: 7.9 mm (SD +/- 3.4). The mean cervical measurement was fractionally 3.3 cm (SD +/- 0.4) Mean cervical linear measurement was 3.9 cm (SD +/- 0.4). Using student's t test where the value of p in the endometrial measurement was 0.0871 and p value in cervical measurement was 0.009, the latter being statistically significant. CONCLUSIONS: With respect to the measurement of the endometrial lining, there is no significant difference do any of the two different techniques. However, measurement of the cervix, another significant difference (p = 0.009), so it should be further investigated which of these two techniques is the right way to establish more accurate diagnoses.
