RESUMEN

Inhibition of the receptor activator of NF-κB ligand (RANKL) is a novel therapeutic option in the treatment of osteoporosis and related diseases. The aim of this study was to evaluate bone metabolism and structure in pigs after RANKL inhibition. 12 growing pigs were assigned to 2 groups with 6 animals each. The OPG group received recombinant human OPG-Fc (5 mg/kg IV) at day 0, the control group was given 0.9% NaCl solution. Serum levels of OPG-Fc, calcium (Ca), phosphorus (P), and bone turnover markers were evaluated every 5 days, and pigs were euthanized on day 20. Serum OPG-Fc concentration peaked at day 5 and coincided with significantly decreased Ca, P, and bone turnover markers. By day 15, measureable OPG-Fc serum levels could only be detected in 2/6 animals. With OPG-Fc clearance starting at day 10, serum Ca and P concentrations were not different between the 2 groups. TRACP5b, P1CP, and BAP levels significantly decreased by 40-70% relative to vehicle controls in the OPG-Fc group between days 5 and 10, indicating that pharmacologic concentration of OPG-Fc led to systemic concomitant inhibition of bone formation and resorption in young growing pigs. Dual X-ray absorptiometry data derived from the proximal femur did not differ between the 2 groups. µCT analysis of selected bone sites demonstrated an OPG-Fc-induced improvement of specific bone architectural indices and bone mineralization.

A 15-month-old 25 kg female spayed American Staffordshire Terrier was examined to determine the cause of a six month period of lameness of the left pelvic limb. Signs of severe pain were evident on hyperextension and simultaneous internal rotation or abduction of the left hip joint; crepitus or instabiltiy was not detected. The diagnosis of avulsion fracture of the lesser trochanter was based on radiographs, computed tomography (CT) and magnetic resonance imaging (MRI), supported by initial findings such as lameness and hip joint pain in extension. The dog was treated with strict confinement for eight weeks followed by a gradual increase in activity; non-steroidal anti-inflammatory drugs were given for one month. Three months later, the dog was clinically sound and unrestricted activity was resumed. Soundness was confirmed on follow-up 18 months after injury. CT and especially MRI were powerful diagnostic tools to identify chronic iliopsoas strains with an avulsion fracture of the lesser trochanter. Complete recovery ensued following conservative treatment only, despite the proximal diplacement of an avulsed segment of the lesser trochanter. Thus, traumatic injury to the iliopsoas muscle and to the lesser trochanter should be included as differential diagnosis in cases of hind leg lameness.

Radiographic features of the region of the trochanteric fossa were evaluated bilaterally on 175 ventrodorsal radiographs of canine hip joints and findings were compared to anatomic specimens. The fossa showed a variable radiographic appearance, which was affected by positioning and anatomic diversity. On a small number of radiographs (n = 12), the trochanteric fossa was clearly defined as a circular area of radiopacity surrounded by a radiolucent halo. This appearance was significantly influenced by medial rotation of the femur (P < 0.001). On some anatomic specimens an irregular protuberance representing the site of attachment of the gemelli, internal and external obturator muscles was seen within the fossa, surrounded by a variable number of small openings, which were thought to be nutrient foramina. Dissection identified a small number of minor branches of the medial circumflex femoral artery entering these.

An anatomic study of the equine digit using magnetic resonance imaging (MRI) was performed. Seventeen isolated forelimbs and one hindleg of nine warmblood horses were imaged in transverse, sagittal, and dorsal planes with a 1.5 Tesla magnet using T1-, T2- proton density-weighted spin echo sequences as well as T2 gradient echo sequences. One scan plane in each horse was compared with corresponding anatomic and histologic sections. The best imaging planes to visualize various anatomic structures were determined. Fibrocartilage was visualized in the insertion of the deep digital flexor tendon and the suspensory ligament as well as in the distal sesamoidean ligaments. The correlation of MRI images with anatomic and histologic sections confirmed that all of the anatomic structures in the equine digit could be evaluated in PD and T2 studies.

Elbow joints of a random sample of 59 cats were investigated by means of X-rays and dissection as well as histologically and by making cuts through synthetic resin-embedded joints. Bone-dense shadow craniolateral to the elbow joint was observed in X-ray images of 23 limbs (about 20%). Furthermore 8 of these 23 elbow joints (about 7%) showed signs of arthrotic alteration. Histologic examination revealed one or more centers of metaplasia of varying status of development of the annular ligament of the radius. Alterations varied from chondroid metaplasia to mineralisation and ossification. They turned out to be located in the area where the annular ligament of the radius crosses the tendon of origin of the supinator muscle and are considered to be a reaction of tough connective tissue to mechanical stress. Compressive forces seem to be less responsible than tensile stress. As soon as bone-dense shadow is observed in X-ray images, alterations have grown so far as the annular ligament of the radius as well as the tendon of origin of the supinator muscle are affected.

RESUMEN

Three patients developed renal failure a few days after onset of sulfinpyrazone administration. In two the renal dysfunction was reversible, while the third died of a second myocardial infarction. The clinical picture of renal failure was uncharacteristic. It is recommended that renal function be tested two to four days after starting sulfinpyrazone, and to discontinue the drug immediately if there is a rise in blood urea nitrogen and creatinine. Since in two of the patients the creatinine values were elevated before sulfinpyrazone had been administered, it is clear that even minor pointers to impaired renal function should be considered as contraindications to the use of sulfinpyrazone.

RESUMEN

A case report concerning a 75 year old female patient whose treatment of multifocal ventricular extrasystoles and a ventricular tachycardia after an acute myocardial infarction consisted of multiple drugs, one of them being the antiarrhythmic substance Propafenon, is given. The patient developed a profound, but reversible, granulocytopenia, which disappeared nine days after discontinuing of most of the drugs. With the exemption of Propafenon all of the drugs were reinstituted after the remission: The not anymore needed ones only for a few days, all others until now, without ill effect. The patient is doing fairly well one year after this episode.
