RESUMEN

Recently there has been observed a significant increase in frequency of erectile dysfunction in Poland. Contemporary methods of the therapy of this disturbance are based not only on psychotherapy, oral drugs and other non invasive methodes, but on intracavernous injection of vasodilatory agents as well. The objective of the study was to evaluate the efficiency of intracavernous quick self-injection of prostaglandin E1 implementing automatic "SELFINJECT" device. Authors achieved 93% of positive results. That method seems to be safe and well tolerated.

The influence of frequent ejaculation on sperm quality in 96 men with oligozoospermia, asthenozoospermia or oligoastenozoospermia was investigated. Ejaculates were produced by each patient in short period of time (4-6 hours). In oligozoospermic group (n = 31) semen characteristics were similar between two ejaculates. In asthenozoospermic group (n = 27) the motility and the total motile sperm count increased significantly in the second ejaculate (32.1% 5.6 vs. 48.0% +/- 7.1 and 36.7 mln +/- 5.6 vs. 47.8 mln +/- 7.6 respectively). Similarly in oligoasthenozoospermic group (n = 38) the motility and the total motile sperm count increased significantly in the second ejaculate (28.1% +/- 5.3 vs. 44.2% +/- 6.2 and 7.1 mln +/- 1.2 vs. 11.3 mln +/- 2.7 respectively). In three groups, pooling two ejaculates increased significantly the total motile sperm count by 58.1%-136.6% comparing to first ejaculate. Pooling two sequential ejaculates from subfertile men is a simple method to increase the quality of sperm for assisted reproduction techniques.

RESUMEN

Intrauterine insemination (IUI) is a widely accepted method of infertility treatment in case of oligospermia. Sperm requires special preparation to avoid applying of active ingredients and abnormal cells to uterine cavity. In this paper authors compared the efficiency of IUI after swim-up sperm preparation on Menezo B2 medium, with IxaPrep technic sperm preparation. In our material there were no significant differences in received results in relation to sperm preparation method.

A retrospective analysis of 301 twin deliveries managed at The Second Department of Obstetrics and Gynecology, Warsaw Medical Academy, from January 1, 1986 to December 31, 1995 was undertaken in order to investigate the impact of the mode of delivery and twin presentation on neonatal outcome. Vertex presentation of both twins was the most common with an incidence of 47.5% followed by vertex-non-vertex (27.6%) and nonvertex presentation of the first twin (24.9%). 186 (61.8%) patients delivered vaginally, while 115 (38.2%) women underwent caesarean section including 6 operations performed after the vaginal delivery of the first twin. In vertex presentation of the first twin and breech second twin there was no significant difference in neonatal outcome measured by 5-minute Apgar score and birth trauma incidence between second twins delivered vaginally and second twins delivered by caesarean section. Vaginal delivery with internal podalic version of the second twin in vertex-transverse presentations was related to increased risk of lower 5-minute Apgar score and increased risk of birth trauma occurrence compared to caesarean section. Time interval between vaginal delivery of twins had no significant impact on neonatal outcome.
