RESUMEN

The authors analyze x-ray findings in 194 patients with colonic diverticulosis and its inflammatory complications. Findings of preoperative irrigoscopy were correlated with dates of morphological investigations. Roentgenomorphological and functional changes were assessed in different clinical types of colonic diverticulosis. Correlation was received between this changes and clinical sings of disease. Authors determined risk-groups of inflammatory complications of colonic diverticulosis.

RESUMEN

The authors analyze x-ray findings in 194 patients with colonic diverticulosis and its inflammatory complications. Clinical, x-ray, and morphologic studies helped the authors to specify the x-ray semeiotics of diverticulitis, paraintestinal infiltrates, intestinal fistulas. An optimal complex of radiological methods to detect each of these complications has been developed. The authors claim that irrigoscopy supplemented, if necessary, with simultaneous fistulography, or oral enterography is the priority and basic method for the diagnosis of inflammatory complications of colonic diverticulosis.

RESUMEN

Surgical treatment of chronic colonic stasis is still a complicated and difficult for solution problem. The undertaken study characterizes one of the causes of chronic constipation in adults--the hypoganglionic variant of Hirschsprung's disease whose clinical course is marked by a prolonged period of compensation while the complex diagnosis is extremely difficult. The severe disorders of the motor-evacuation function of the large intestine make surgery necessary; operation leads to recovery of 96% of patients.

RESUMEN

Eighty-six patients were subjected to complex examination in the immediate postoperative period to check the condition of intestinal anastomoses. In 58 (67.4%) patients inflammation in the zone of the anastomosis was found to be negligible and produced no specific clinical manifestations; 28 (32.6%) patients had marked inflammatory changes in the region of the anastomosis. This condition, interpreted as anastomositis, is favourable for the development of incompetence of the sutures of the anastomosis, which was diagnosed in 4.7% of patients. Timely diagnosis of anastomositis and application of a complex of nonoperative measures is conducive to reduction of the frequency of postoperative complications and mortality.

RESUMEN

A current approach to the perfection of the diagnosis of colonic tumors is proposed in patients at risk by means of introduction into the ambulatory health care system (polyclinics, diagnostic centers) of standardized colonic fluorography followed by endoscopy in necessary cases. The main organizational and methodological principles of the use of colonic fluorography were worked out as a method of screening to reveal colonic tumors. It is suggested that such new elements as the "prescopic stage" realized outside the radiation zone, automated distant air inflation into the colon and so forth may be introduced in colonic fluorographic examination. Experience gained with over 2065 colonic fluorographies on a special fluorograph and 1120 examinations on routine x-ray diagnostic units fitted with the amplifier and a photochannel (79 cases of colonic carcinoma and 89 benign tumors were identified) evidences that the program of making colonic fluorography enables one to significantly improve the diagnosis of colonic tumors.

RESUMEN

An analysis of healing the intestinal anastomoses in 52 patients after restorative-reconstructive operations on the colon is presented. In 36.5% of the cases a complicated course of the early postoperative period was observed: anastomositis--in 28.8% and incompetence of the anastomosis sutures--in 7.7%. Such complications were mainly observed in patients older than 60 years of age with a concomitant pathology, with a one-trunk colostoma and when forming low colorectal anastomoses.

RESUMEN

An examination of 1525 patients has established that the main clinical symptoms of the perifocal inflammatory process are: hyperthermia of different degrees, leukocytosis and higher ESR, pain in the rectum area and in the abdomen, tenderness and pasty tissues in the tumor area. The roentgenological symptoms are: the presence of fistulas, considerable dilatation of presacral space not corresponding to the tumor size, pathological "cellularity" of the paraintestinal tissues.
