RESUMEN

Background: Computer-aided design/computer-aided manufacturing (CAD/CAM) resin ceramics allow easier milling than glass ceramics but are suspected to be more stainable. Although Photoshop® is widely used for picture analysis, its potential for shade selection has not been properly assessed. Aim: Purpose primary: To evaluate the color stability of three CAD/CAM resin ceramics and Lithium Disilicate. Secondary: to compare the color evaluation between a spectrophotometer (Vita Easyshade compact) and Photoshop software. Materials and Methods: Three CAD/CAM resin ceramic materials (n = 10) and a fourth group of lithium disilicate were used. Half of each group were thermocycled (5°C and 55°C; 3000 cycles). All samples were immersed in colored beverages (coffee, tea and red wine) for 30 days. Values were obtained by spectrophotometry and photographs analyzed using Photoshop software. The parameters measured were CIEL*a*b, and the color difference (ΔE) was analyzed. A mixed model test was used to compare the results through time and materials (α = 0.05). The comparison between the spectrophotometer and Photoshop results was performed using the bivariate Pearson's correlation test. Results: Lithium disilicate glass ceramic exhibited less color change (ΔE = 14) than resin ceramics (15.7 < ΔE < 18.7). The least change was noted with GC Cerasmart (ΔE = 15.7) followed by Vita Enamic (ΔE = 17*) and Brilliant Crios (ΔE = 18.7*). Spectrophotometer and Photoshop values showed low correlations. Conclusions: Resin ceramics may suffer from color change in clinical use. Photoshop is technique sensitive; pictures are easily affected by the light conditions and camera settings.

RESUMEN

Tribology is the discipline concerning the application of friction, lubrication, and wear concepts of interacting surfaces in relative motion. A growing interest has developed in tribology application in medical biomaterials, such as resin composites used in restorative dentistry. Yet, the keywords "tribology" and "biotribology" are little applied in the pertinent publications. The aim of this scoping review was to offer an overview of tribology application in dental composites research and to identify knowledge gaps and address future research. A literature search was conducted on Pubmed and Scopus databases and the studies investigating the tribological behavior of resin composites were included for qualitative synthesis. The majority of studies on dental tribology were published in the research areas of mechanical engineering/nanotechnology and differed in several methodological aspects. The preponderant engineering approach and the lack of standardized testing make the laboratory findings poorly informative for clinicians. Future research should focus on the tribological behavior of dental materials composites by means of an integrated approach, i.e., engineering and clinical, for improving development and advancement in this field of research.

INTRODUCTION: This study assessed the prevalence of radix entomolaris and 2 canals at the distal aspect of mandibular first molars among different geographic regions by means of cone-beam computed tomographic imaging. METHODS: Precalibrated observers from 23 worldwide geographic locations followed a standardized screening protocol to assess 5750 cone-beam computed tomographic images of mandibular first molars (250 per region), gathering demographic data and recording the presence of radix entomolaris and a second canal at the distal aspect of teeth. Intra- and interrater reliability tests were conducted and comparisons among groups were performed using proportions and odds ratio forest plots. The significance level was set at 5%. RESULTS: The results of intra- and interrater tests were above 0.79. The prevalence of radix entomolaris varied from 0.9% in Venezuela (95% confidence interval [CI], 0%-1.9%) to 22.4% in China (95% CI, 17.2%-27.6%). Regarding the proportion of a second distal canal, it ranged from 16.4% in Venezuela (95% CI, 11.8%-21.0%) to 60.0% in Egypt (95% CI, 53.9%-66.1%). The East Asia subgroup was associated with a significantly higher prevalence of an extra distolingual root, whereas the American subgroup, the American native ethnic group, and elderly patients were linked to significantly lower percentages of a second canal at the distal aspect of teeth. No significant differences were noted between male or female patients. CONCLUSIONS: The overall worldwide prevalence rates of radix entomolaris and a second canal at the distal aspect of the mandibular first molar were 5.6% and 36.9%, respectively. The East Asia geographic region and Asian ethnic group had a higher prevalence of a second distal root.

PURPOSE: To investigate the effects of root canal irrigants, dentin surface treatment, and timing of the restoration on the microtensile bond strength (µTBS) of a universal adhesive to deep coronal dentin. MATERIALS AND METHODS: Ninety (90) intact molars were grouped according to the irrigation protocol: group 0 (saline, control group); group 1: 5.25% sodium hypochlorite (NaOCl), 17% ethylenediaminetetraacetic acid (EDTA) followed by 5.25% NaOCl; group 2: 18% etidronic acid (HEDP) + 5.25% NaOCl in a single mixture. Groups 1 and 2 were divided into 2 subgroups (n = = 10) according to the time of restoration: immediate (subgroup A) and after 7 days (subgroup B). Four other groups (n = 10) were created according to the dentin surface pretreatment and received an immediate restoration: group 3: 5.25% NaOCl - 17% EDTA and AH Plus Cleaner; group 4: 5.25% NaOCl - 17% EDTA and 100% ethanol; group 5: 5.25% NaOCl + 18% HEDP and AH Plus Cleaner; group 6: 5.25% NaOCl + 18% HEDP and 100% ethanol. µTBS testing was carried out for each subgroup and interfaces were analyzed through SEM. The µTBS was analyzed with Welch statistics and the post-hoc Games-Howell test for multiple comparisons (p < 0.05). RESULTS: Statistically significant differences were observed between test groups and control groups (p < 0.001), except for group 3 (p = 0.349). No significant differences were recorded between groups 1A (27.5 ± 8.7 MPa), 2A (32.6 ± 14.5 MPa) and 3 (27.3 ± 14.3 MPa). Group 6 (48.6 ± 11.6 MPa) exhibited the highest µTBS among all tested groups. For groups 1 and 2, significantly higher values were recorded for subgroups 1B (39.3 ± 7.6 MPa) and 2B (43.8 ± 8.3 MPa) compared to subgroups 1A (27.5 ± 8.7 MPa) and 2A (32.6 ± 14.5 MPa). CONCLUSION: When using NaOCl as root canal irrigant, it is advisable to delay the placement of a post-endodontic-treatment restoration.

INTRODUCTION: The presence of multiple root canals is an important morphologic aspect of mandibular premolars. This study aimed to perform a worldwide analysis on the prevalence of a lingual canal in mandibular premolars and to evaluate its influence on patients' demographics in 23 countries using cone-beam computed tomographic images. METHODS: Observers from 23 countries were instructed to evaluate cone-beam computed tomographic images of 300 first and 300 second premolars (13,800 teeth) regarding the presence of a lingual canal, canal configuration, and data related to patients' ethnicity, age, and sex following a standardized screening methodology. Intra- and interrater evaluations were performed using the Cohen kappa test and intraclass correlation coefficient. Proportion and odds ratio forest plots were calculated in order to compare groups. Statistical significance was set at 5%. RESULTS: Both kappa and intraclass correlation coefficient values were above 0.60, and the percentage of agreement was 94.9% (first premolar) and 97.8% (second premolar). A significant statistical difference was observed between the worldwide proportion of a lingual canal in mandibular first (23.8%; range, 12.0%-32.7%) and second (5.3%; range, 1.0%-15.3%) premolars (P < .05). Asians and patients over 60 years old were associated with the lowest proportions of a lingual canal (P < .05), whereas Africans and younger groups were associated with the highest proportions (P < .05). The prevalence of a lingual canal in males (27.9%) was higher than females (20.0%) for the first premolar only (P < .05). Males were associated with 1.533 and 1.597 higher odds of presenting a lingual root canal in the first and second premolars, respectively. CONCLUSIONS: The worldwide proportion of a lingual root canal was 23.6% and 5.3% for the first and second premolars, respectively. Ethnicity, geographic region, age, and sex had an influence on the outcomes.

The aim is to evaluate the influence of remaining tooth substance and post-endodontic restoration on fracture strength of endodontically treated maxillary incisors. 150 maxillary central incisors were divided into three groups, Group 0, intact teeth; Group 1, removal of distal wall; Group 2, removal of mesial and distal walls, and further into two subgroups A0,A1,A2 and B0,B1,B2 according to post-endodontic restoration (post/no-post), then loaded to fracture. Interactions among variables and intergroup significance were tested with two-way ANOVA and Kruskal Wallis's tests (p≤0.05). Tukey's test was applied for multiple comparisons. Statistically significant differences were found between groups B1-A1, and B2-A2, but they were not found between B0-A0. Intragroup analysis showed statistically significant differences in both groups post/no-post with decreasing dental substance. Fiber post placement causes an increase in fracture strength and a reduction of irreparable fractures in endodontically treated maxillary central incisors that lost at least one wall.

PURPOSE: The aim of this study was to compare the influence of the number of coronal walls and post-endodontic restorations on the mechanical strength of 165 recently extracted endodontically treated maxillary premolars. METHODS: The teeth were divided into 3 control (no post) and 3 test groups according to the number of residual walls. Each test group was divided into subgroups according to the type of post-endodontic restoration (single, oval, and multi-post techniques). Samples were prepared conforming to the assigned subgroup. A universal loading machine applied a load parallel to the longitudinal axis of the teeth, thus simulating physiological occlusion. ANOVA and the Kruskal Wallis test were used for comparisons (P ≤ 0.05), and Tukey's test for multiple comparisons. RESULTS: For intact teeth, significant differences were found among all 3 subgroups, with single post showing the highest values. For 3 residual walls, oval post resulted in significantly lower values than single and multi-post systems. For 2 residual walls, the multi-post technique showed almost twice the resistance of oval post restorations. CONCLUSION: In oval canals the use of a single or multi-post technique increased post-and-core resistance even in intact teeth, whereas oval fiber posts showed no improvements. Multi-post design improved fracture resistance mostly in maxillary premolars lacking both marginal ridges.

AIM: The aim of this study was to compare the adaptation of complete denture base (CDB) manufactured by three different techniques: conventional, milling, and three-dimensional (3-D) printing. MATERIALS AND METHODS: A master cast was duplicated to create 60 gypsum casts. Twenty casts (n = 20) were attributed to each group. In the computer-aided design and computer-aided manufacturing (CAD/CAM) groups (milling and 3-D printing), the 40 gypsum casts reserved for these two groups were scanned. An STL file was obtained and a master CDB was designed and then fabricated according to each technique. In the conventional group, a polyvinyl siloxane putty mold was obtained from the milled CDB, and this mold was used to fabricate 20 conventional denture bases by compression molding using the silicon-gypsum technique in a bronze flask. The inner surfaces of the obtained 60 CDB were scanned and superimposed over their corresponding master cast. Deviation analyses were calculated using digital subtraction technique. Five functional areas (posterior palatal seal, anterior border seal, crest of the ridge, maxillary tuberosities, and palate) were selected to evaluate the variations in CBD adaptation. RESULTS: Based on the results and color maps of all selected regions, milling technique offers the best adaptation. The crest of ridge in the conventional technique showed the least adaptation and the posterior palatal seal in the 3-D printing technique showed the best adaptation. CONCLUSION: Within the limitations of this study, the CAD/CAM fabrication techniques seem to offer better adaptation of CDB compared to the conventional fabrication technique. Milled CDBs presented the most homogeneous distribution of adaptation, yet the 3-D printing process seems a promising techniques that needs to be addressed and perfected. CLINICAL SIGNIFICANCE: The CAD/CAM technologies can help overcome many limitations related to conventional impressions and therefore should be well investigated to improve the edentulous patient's quality of life.

AIM: The aim of this study was to assess if the curing mode and the etching mode could affect the push-out bond strength of different post cementation systems using two universal adhesives. MATERIALS AND METHODS: A total of 100 single-rooted teeth were divided into Prime & Bond Elect (PBE), Prime & Bond Active (PBA), and Prime & Bond XP (PBXP) as a control. The PBE, PBA, and PBXP were used in the self-etch (SE) and etch and rinse (E&R) mode. Post cementation was performed using Core X flow used in dark-cure and in light-cure. Data were analyzed using the one-way ANOVA test and post hoc Bonferroni and Dunnet tests. The Student's t test was performed to find significance between two independent groups. RESULTS: Bond strength was significantly influenced by the adhesive strategies. The PBE and PBA obtained higher values when used in the SE mode (p < 0.001). Light-cured groups obtained significantly higher values (p < 0.001) compared to dark-cured groups. The post space region also had a significant effect on the bond strength; the apical third recorded lower values in all groups (p < 0.001). CONCLUSION: The PBA and PBE universal adhesives obtained higher PBS values when used in the SE mode and followed by the light-curing of resin cement. CLINICAL SIGNIFICANCE: Universal adhesives represent a good alternative to the conventional total-etch adhesive system for fiber post cementation. Since they perform better in SE, they would be recommendable in clinical practice, as they can be used with a simplified technique.

PURPOSE: The purpose of this study was to evaluate the effect of surface treatments on zirconia-resin bonding and the effect of aging on bond durability for one year. METHOD: Three hundred and twenty zirconia blocks were divided into 4 equal study groups. Group 1 (control): as-sintered, group 2: (GB): grit-blasted, group 3: (LAS): laser-etched, group 4: (SIE): selective infiltration etching. Composite cylinders were bonded to the zirconia with resin cement and ceramic primer. Aging was performed following 3 different aging protocols: thermocycling, storage in distilled water, or storage in an enzymatic esterase solution. Micro-shear bond strength test (µSBS) was recorded using a universal testing machine. µSBS values were analyzed using two-way Analysis of Variance followed by Tukey post-hoc tests. Level of significance was set at 0.05. RESULTS: GB, LAS and SIE groups showed significantly higher values when compared to control. Groups GB, LAS and SIE reported a significant decrease up to 50% in µSBS after water storage and enzymatic degradation, while control group reported a 90% decrease. Failure analysis showed mainly adhesive failure for control group, while the percentage of cohesive failure in resin cement was higher in SIE group compared to GB and LAS groups. CONCLUSION: Water aging and esterase solutions played a significant role by increasing bond degradation. A minimum of one-year water and esterase storage medium should be used to evaluate the durability of the bond between resin cement and zirconia.
