RESUMEN

This is a case report about a 66-year old man with recurrent pulmonary embolisms. Phlebography revealed a monstrous venous aneurysm of the right chest wall with several venous dilatations and calcification as the source of the embolism. Radiological embolisation and surgical ligation of the large aneurysmatic neck were performed. No further embolism episodes were registered under anticoagulation during the 23 months of the postoperative follow-up investigation.

"In 1970, as in previous decades, the U.S. Bureau of the Census's household enumeration began with a lead question as to who was the ¿head of household'. With the resurgence of feminism, this concept was challenged as an ambiguous concept which implied an authority structure imputed by the Bureau but not measured, and offensive to many people. This paper tells the story of successful feminist mobilization in the 1970s that led to the removal of this concept from the U.S. decennial censuses beginning with 1980."

RESUMEN

This study provides the most recent national estimates of the prevalence of employment during nonstandard hours (evenings, nights, or rotating hours) and on weekends. It also examines in a multivariate context the relevance of job and family characteristics as determinants of such employment, separately for men and for women. The findings support the contention that the demand for employment during nonstandard hours and weekends is pervasive throughout the occupational hierarchy, but particularly in service occupations and in personal service industries and for both men and women. Gender differences exist, however, in the relevance of family factors. Being married reduces women's but not men's likelihood of employment during nonstandard hours, and the presence of children affects women's but not men's hours and days of employment. (The direction of the effect for women depends on the children's age.) Implications of these findings are discussed.

RESUMEN

An inhibitor against blood clotting factor IX was observed as a very rare finding in case of an eight year old boy suffering from haemophilia B. By reason of a haemarthros a replacement therapy was successful done using FEIBA (Factor Eight Inhibitor Bypassing Activity). The haemostatic characterization of the inhibitor and its influence by therapy will be presented.

In view of the universal metabolic importance of zinc in the organism, it was the purpose of the present work to determine the concentrations of zinc in serum, of amino acids and ammonia in plasma of patients with liver cirrhosis, and investigate that correlations might exist between these substances. The study involved 18 patients with decompensated liver cirrhosis without coma and eleven with coma. The subjects with normal livers were used as controls. While confirming known data (reduced zinc levels, imbalance of plasma amino acids, hyperammonaemia in chronic liver diseases) the findings also revealed correlations between the above substances. A negative correlations existed between zinc and ammonia. Decreases in zinc serum levels were accompanied by increases in plasma ammonia concentrations in hepatic coma (p less than 0.05). Plasma levels of amino acids did not correlate with serum zinc concentrations.

RESUMEN

The experiment of a self-injury by permanent intoxication with the coumarin derivative Warfarin is described. Various isolated haemostatic defects as differential-diagnostically demarcated causes for the leading symptom decrease of the value of the thromboplastin time which concern the effect of coumarin are mentioned. It is referred to the coagulation-analytic and chemical-toxic methods of proof of coumarins.

RESUMEN

For the laboratory control of a heparin therapy thrombin time, partial thromboplastin time and activated recalcification time are used. On account of distinct differences in the heparin sensitivity of these reactions an indication-related application is necessary. The ability of evidence and the possibility of establishing test-specific therapeutic regions are restricted by differences caused by reagents, individual variability and influence by accompanying haemostasiological changes. The own approach, taking into consideration the so-called heparin resistance, it presented.

RESUMEN

Pathologic inhibitors of blood coagulation as cause of acquired haemostatic failure are rare. We report about a 23 years old primigravida with a life-threatening haemorrhage post partum. Analysis of coagulation parameters showed the presence of inhibitor to factor VIII. We reacted successfully with cyclophosphamid and cryoprecipitated factor VIII. References to diagnostic and therapy of pathologic clotting factor inhibitors are described.

Zinc is essential to numerous metabolic processes in the organism, multiform symptoms being found especially in deficiencies. In addition to nutritional factors, diseases such as cirrhosis of the liver. Crohn's disease and chronic renal diseases are relevant in this context. In the present work, serum zinc levels were investigated in 109 patients with various chronic liver diseases. The lowest serum zinc concentrations were seen in patients with decompensated hepatic cirrhosis with coma. Patients with decompensated alcoholic cirrhosis had lower zinc levels as subjects with nonalcoholic cirrhosis. None of the groups exhibited a significant change in serum zinc levels during the treatment period. Laboratory data (such as transaminases, thromboplastin time, alkaline phosphatase, total proteins) did not correlate with the serum zinc concentrations. The concentration of plasma ammonia, however, appeared to be inversely related to the serum zinc levels. Thus, patients with coma had maximum ammonia and minimum zinc levels.

RESUMEN

Pathological inhibitors of the coagulation which are recognized by characteristic laboratory findings may cause an increased inclination to haemorrhage, may be clinically mute or also may effect an increased inclination to thrombosis (lupus inhibitors). It is reported on patients with rare coagulation inhibitors, a pathologic antithrombin with normal reptilase time and an antithromboplastin with protracted effect, in which thromboses appeared. The occurrence of thromboses in these patients up to now does not allow the assumption of an effect of these inhibitor substances furthering thrombosis, but only the statement that despite coagulation-analytically recognizable inhibition effects the development of a thrombosis is not prevented. A prophylactic factor substitution in operative interventions is not indicated.

RESUMEN

Arterial embolism or thrombosis are very rare complications of the fibrinolytic therapy of deep venous thrombosis. The characteristics symptoms of these illness are the acute arterial failure of the concerned extremity during venous thrombolysis. The diagnosis take place by angiography, by ultrasonics, and in particular cases by the skin thermographic method. If the arterial occlusion is not operable, the thrombolysis by urokinase is discussed. This seems to be the last possible method to support the extremity.

This study builds on an earlier finding from the May 1980 Current Population Survey that one-third of full-time dual-earner couples with children in the United States include at least one spouse who works other than a regular day shift. Using the same data source, the relevance of husbands' and wives' job characteristics (occupation and industry) on their shift work status are considered, and the nature of the association between husbands' and wives' work shifts is explored. Four alternative models are initially posed and tested with log-linear analysis; these models vary in the extent to which a spouse's shift work status is contingent upon the job characteristics of a husband or wife. None of these models fit the data. A modification of the simplest of the four models (using forward selection) is the best fitting model. It is not symmetrical: the wife's shift is contingent upon both her occupation and industry and that of her husband, but the husband's shift is contingent only upon his occupation and industry. There is a relationship between husband's and wife's shift, the direction of which depends upon the occupations of both spouses. This study demonstrates the importance of taking a "couple" perspective on shift work among married persons, and the need for models that include interaction effects.

In a 1980 sample of U.S. nonfarm households with children and with both spouses employed full time, one-third of the couples included at least one spouse who worked other than a regular day shift. In about one-tenth of the couples the spouses worked entirely different shifts with no overlap in hours. These findings are linked to an earlier study which showed a high prevalence of child care by employed fathers whose wives were employed in certain occupations.

PIP: The purpose of this study is to examine factors associated with mother's age at first birth, specifically to examine the process whereby age at menarche may affect the timing of the first birth. A sample of 541 black and white women aged 15-29 at time of their first birth, was drawn from the birth records of residents of Brooklyn, the Bronx, and Queens, in New York City. Mean age at menarche was 12.7 for blacks, and 12.3 for whites; the mother's education was only weakly associated with age at menarche. There seems to be a direct relationship between age at menarche and fertility only for black women. Age at menarche may be a crude indicator of fecundity, but does not seem to influence the timing of the first birth. However, there is some evidence that for girls who did not use contraception at the time of their first sexual experience, early age at menarche may be related to early age at first birth. Age at menarche does influence the timing of dating, and consequently the timing of motherhood, for black women only. Age at first date is correlated with age at first sexual intercourse especially for blacks.^ieng
