RESUMEN

BACKGROUND AND AIM: Probiotics are live microorganisms that confer a health benefit on the host when administered in adequate amounts. In the present study, the putative probiotic strain was identified from the gut of Drosophila melanogaster and assessed for its protective effect in inflammatory bowel disease. METHODS: Active probiotics were screened from the Drosophila melanogaster gut by the selection criteria of gastric juice tolerance, hydrophobic property, antimicrobial potential, adhesion, and invasion properties. The active probiotics were identified by 16s rDNA sequencing and the effect of these active probiotics was evaluated in a Dextran sulphate sodium (DSS)-induced mice model by estimating inflammatory markers and histopathological changes. RESULTS: Nine Gram-positive and bile salt tolerant bacterial isolates were obtained from the gut samples. The isolates PTH 2, PTH 4, and PTH 7 clearly showed significant activity in antimicrobial potential, hydrophobic (>74%) property, and intestinal juice tolerance. Among these, PTH 7 was selected for further studies due to its significant low-invasion ability and it proved capable of reducing the secretion of proinflammatory cytokines. The 16s rDNA studies revealed that PTH 7 was Myroides pelagicus. Administration of M. pelagicus to the DSS-induced colitic animals significantly suppressed myeloperoxidase, ALP, and malondialdehyde levels, and also lowered levels of proinflammatory cytokine expression. Further, the recovery from the disease by the probiotic treatment was supported by histopathological and macroscopic observation. The treated animals did not show any adverse signs in their physiology or behavior. CONCLUSION: Myroides pelagicus successfully prohibited inflammatory markers and acted as a potent probiotic. Future studies with this stain might prove its efficacy as a drug for the management of colitis.

Streptomyces strain isolated from the soil sediment was studied for its in vitro α-glucosidase and antioxidant properties. Morphological characterization and 16S rRNA partial gene sequencing were carried out to confirm that the strain Loyola AR1 belongs to genus Streptomyces sp. Modified nutrient glucose broth was used as the basal medium for growth and metabolites production. Ethyl acetate extract of Loyola AR1 (EA-Loyola AR1) showed 50% α-glucosidase inhibition at the concentration of 860.50 ± 2.68 µg/ml. Antioxidant properties such as total phenolic content of EA-Loyola AR1 was 176.83 ± 1.17 mg of catechol equivalents/g extracts. EA-Loyola AR1 showed significant scavenging activity on 2,2-diphenyl-picrylhydrazyl (50% inhibition (IC50), 750.50 ± 1.61 µg/ml), hydroxyl (IC50, 690.20 ± 2.38 µg/ml), nitric oxide (IC50, 850.50 ± 1.77 µg/ml), and superoxide (IC50, 880.08 ± 1.80 µg/ml) radicals, as well as reducing power. EA-Loyola AR1 showed strong suppressive effect on lipid peroxidation (IC50, 670.50 ± 2.52 µg/ml). Antioxidants of ß-carotene linoleate model system reveals significantly lower than butylated hydroxyanisole.

We sought to determine the rate of healthcare-associated infection (HCAI), microbiological profile, bacterial resistance, length of stay (LOS) and excess mortality in 12 ICUs of the seven hospital members of the International Infection Control Consortium (INICC) of seven Indian cities. Prospective surveillance was introduced from July 2004 to March 2007; 10 835 patients hospitalized for 52 518 days acquired 476 HCAIs, an overall rate of 4.4%, and 9.06 HCAIs per 1000 ICU-days. The central venous catheter-related bloodstream infection (CVC-BSI) rate was 7.92 per 1000 catheter-days;the ventilator-associated pneumonia (VAP) rate was 10.46 per 1000 ventilator-days; and the catheter-associated urinary tract infection (CAUTI) rate was 1.41 per 1000 catheter-days. Overall 87.5% of all Staphylococcus aureus HCAIs were caused by meticillin-resistant strains, 71.4% of Enterobacteriaceae were resistant to ceftriaxone and 26.1% to piperacillin-tazobactam; 28.6% of the Pseudomonas aeruginosa strains were resistant to ciprofloxacin, 64.9% to ceftazidime and 42.0% to imipenem. LOS of patients was 4.4 days for those without HCAI, 9.4 days for those with CVC-BSI, 15.3 days for those with VAP and 12.4 days for those with CAUTI. Excess mortality was 19.0% [relative risk (RR) 3.87; P < or = 0.001] for VAP, 4.0% (RR 1.60; P=0.0174) for CVC-BSI, and 11.6% (RR 2.74; P=0.0102) for CAUTI. Data may not accurately reflect the clinical setting of the country and variations regarding surveillance may have affected HCAI rates. HCAI rates, LOS, mortality and bacterial resistance were high. Infection control programmes including surveillance and antibiotic policies are a priority in India.

BACKGROUND: We investigated how often blind placement of peripherally inserted central catheters (PICCs) through the antecubital veins results in a correct tip location in relation to carina and evaluated the inter-observer agreement in locating the tip of PICCs in plain radiography with digital imaging. METHODS: In this study, 202 suitable chest radiographs with PICCs out of 803 patients were identified. An initial audit on the tip of these catheters in relation to carina was done by a consultant anaesthetist and was recorded as the first observer. The same sets of CXRs were examined by a consultant radiologist and the tips were identified and recorded as the second observer. Inter-observer agreement was assessed. RESULTS: In 75 of 202 (37%), PICCs had a central tip location in relation to the carina. Fifty-five of 131 (42%) right-sided catheters had a central location compared with 20 of 71 (28%) of the left-sided catheters. The tip position for right-sided catheters was most frequently centrally located whereas the tip for left-sided catheters was most commonly positioned in the ipsilateral innominate vein. There was excellent agreement between the observers in reporting the tip of PICCs at all positions (kappa=0.87) including central locations (kappa=0.83). CONCLUSIONS: Right antecubital PICCs are more likely to be placed in the central location in relation to the carina. PICCs inserted through the left antecubital veins need to be pushed further down to aim for a central location. Inter-observer variability in identifying the tip of PICCs is least with the introduction of digital imaging.

Early and small hepatocellular carcinoma (HCC) rarely presents with biliary ductal invasion. We present a case of early HCC presenting with biliary invasion, in the absence of a mass on computed tomographic scanning. The patient, a hepatitis B carrier, was initially diagnosed to have "liver fluke infection" after a "leaf-like structure" was found within the right hepatic duct on endoscopic retrograde cholangiopancreatography (ERCP). The specimen was retrieved with a Dormia basket. Histology revealed HCC. This report highlights an unusual case of early and small HCC presenting with biliary ductal invasion. A high index of suspicion has to be entertained in the background of hepatitis B regardless of the atypicality of presentation.

RESUMEN

This is a prospective observational study of 100 consecutive patients who were at risk for developing acute lung injury and were admitted into the surgical intensive care unit. We found a highly significant correlation between an increase in serum albumin levels and a fall in lung injury score and vice versa (r = -0.51, P = 0.000). A highly significant association was also found between mortality, fall in serum albumin levels, rise in lung injury score, and a higher simplified acute physiology score at admission (P = 0.000).

As it is quick and easy to prepare sperm with SpermPrep column, this method was assessed in the andrology laboratory for IUI preparations, as an alternative to Percoll, since Percoll is not available in market. This paper evaluates sperm prepared by SpermPrep method and compares them to those prepared by MiniPercoll for matched asthenozoospermic specimens. Spermatozoa prepared with MiniPercoll were of significantly higher quality, when compared with those prepared by SpermPrep, in terms of computer-assisted sperm motion parameters, recovery of motile fraction, morphology, hypoosmotic swelling and nuclear stability as assessed by sodium dodecyl sulphate exposure. SpermPrep did not appear to be equivalent to MiniPercoll and therefore could not be widely applied to asthenozoospermic specimens. It may be used on an individual basis, depending on the initial semen sample. Although other density gradient methods such as colloidal silica particles coated with silane appear to be equivalent to Percoll, they need to be critically evaluated with respect to their batches or different sources of vendors and be compared to Percoll as a standard method.

The clinical presentation, diagnosis and treatment of a series of 18 cases of neuroblastoma is reviewed. About half the patients were below the age of 2 years. Fever, loss of weight and appetite appeared to be the common manifestations, underscoring the vague and non-specific symptomatology of such a grave childhood disorder. Unusual presentations included paraplegia, chronic diarrhoea, lower limb edema and respiratory distress. Management consisted of radical surgery with adjunctive chemotherapy where indicated. It is emphasised that in resectable lesions every attempt must be made to remove tumour involved tissue as thoroughly as possible. With this approach 50% of patients followed up to 4 years have survived. Comparison of characteristics of survivors against non-survivors showed that the only significant difference was in the state of the disease--survivors having generally earlier state tumours.
