RESUMEN

The nursing care of a group of moderately demented patients (n = 11) in a nursing home was improved as a result of an education of the staff. Thus the patients were subjected to an increased emotional and intellectual stimulation during the ordinary daily care and participated in group sessions twice a week. In order to evaluate the effects of the treatment, psychological parameters and cerebrospinal fluid (CSF) HVA (homovanillic acid), 5-HIAA (5-hydroxyindole acetic acid) and HMPG (4-hydroxy, 3-metoxyphenylglucol) were quantified before and after a 2-month treatment period. A group of similar patients (n = 13) in another nursing home constituted a control group. The ratings of concentration, absent mindedness and recent memory showed a more favorable development in the treatment group than in the control group where an intellectual deterioration was evident. Restlessness was rated higher in the treatment group after the treatment period, while the psychological testings showed no significant changes between the groups. CSF HVA concentrations increased in the treatment group and decreased in the control group (P less than 0.05). No change was evident in 5-HIAA or HMPG concentrations in either group. The results suggest that environmental factors influence biochemical markers of transmitter activity which thus possibly may be of etiological importance in dementia.

Immunostimulation in the central nervous system (CNS) measured as abnormal intrathecal immunoglobulin production or activated lymphocytes in the cerebrospinal fluid (CSF), was found in 22 of 25 HIV seropositive patients. All of 11 patients with symptomatic HIV infection and nine of 14 asymptomatic patients had an increased IgG index or a Tourtellotte's production number indicating CNS infection. The amount of intrathecal immunoglobulin was not correlated to the severity of disease since five of six AIDS patients had only slightly elevated values, while some patients without AIDS had a high intrathecal immunoglobulin production. Activated lymphocytes and plasma cells in CSF were frequently found in symptomatic HIV patients without AIDS as well as in asymptomatic HIV seropositive patients. Macrophages and activated monocytes were observed in CSF from AIDS patients. The results indicate that HIV has a high neurotropicity and is spread to CNS in all stages of the infection.

RESUMEN

A manic-depressive woman was unresponsive to treatment with lithium and neuroleptics during a period of mania. Additional levothyroxine treatment of her primary hypothyroidism resulted in rapid and complete recovery from her mania.

A longitudinal population study of 1,462 women, aged 38-60 years at entry, was begun in Gothenburg, Sweden, in 1968-1969. This paper deals with the first 12-year follow-up period. In univariate analysis, peak expiratory flow showed a significant negative correlation with the 12-year incidences of myocardial infarction, electrocardiographic changes suggesting ischemic heart disease, stroke, and death. The association with incidence of myocardial infarction, stroke, and death remained in multivariate analysis, and was independent of age, body height, body mass index, adipose tissue distribution, chest deformity, history of pulmonary disease, smoking habit, serum cholesterol concentration, serum triglyceride concentration, systolic blood pressure, diabetes, and physical activity. The findings indicate that measuring peak expiratory flow is a simple procedure to identify women with an increased risk of cardiovascular disease or death. It seems urgent to find out what preventive measures might be taken as a consequence of these observations.

A 12-year longitudinal population study of 1462 women, aged 38-60, was carried out in Gothenburg, Sweden in 1968-69. Women with high initial serum triglyceride values had a higher 12-year incidence of myocardial infarction, stroke and total mortality than the others. The findings for serum triglycerides persisted for myocardial infarction, stroke and total mortality after adjustment for other possible risk factors for ischaemic heart disease such as age, systolic blood pressure, smoking, indices of obesity and serum cholesterol, while serum cholesterol did not predict any end-points studied when taking other risk factors including serum triglycerides into account.

A longitudinal population study of 1462 women aged 38-60 was carried out in Gothenburg, Sweden, in 1968-9. In univariate analysis the ratio of waist to hip circumference showed a significant positive association with the 12 year incidence of myocardial infarction, angina pectoris, stroke, and death. The association with incidence of myocardial infarction remained in multivariate analysis and was independent of age, body mass index, smoking habit, serum cholesterol concentration, serum triglyceride concentration, and systolic blood pressure. The relation between the ratio of waist to hip circumference and the end points of myocardial infarction, angina pectoris, stroke, and death was stronger than for any other anthropometric variable studied.
