RESUMEN

PURPOSE: To investigate cross-sectional associations between dietary patterns and cognitive functioning in elderly free of dementia. METHODS: Data of 389 participants from the German DELCODE study (52% female, 69 ± 6 years, mean Mini Mental State Score 29 ± 1) were included. The sample was enriched with elderly at increased risk for Alzheimer's disease (AD) by including participants with subjective cognitive decline, mild cognitive impairment (MCI) and siblings of AD patients. Mediterranean and MIND diets were derived from 148 Food Frequency Questionnaire items, and data-driven patterns by principal component analysis (PCA) of 39 food groups. Associations between dietary patterns and five cognitive domain scores were analyzed with linear regression analyses adjusted for demographics (model 1), and additionally for energy intake, BMI, other lifestyle variables and APOe4-status (model 2). For PCA-derived dietary components, final model 3 included all other dietary components. RESULTS: In fully adjusted models, adherence to Mediterranean and MIND diet was associated with better memory. The 'alcoholic beverages' PCA component was positively associated with most cognitive domains. Exclusion of MCI subjects (n = 60) revealed that Mediterranean and MIND diet were also related to language functions; associations with the alcoholic beverages component were attenuated, but most remained significant. CONCLUSION: In line with data from elderly population samples, Mediterranean and MIND diet and some data-derived dietary patterns were related to memory and language function. Longitudinal data are needed to draw conclusions on the putative effect of nutrition on the rate of cognitive decline, and on the potential of dietary interventions in groups at increased risk for AD.

RESUMEN

O objetivo deste estudo foi identificar as principais doenças de felinos na região sul do Rio Grande do Sul. Foram revisados os protocolos de necropsia e das amostras biológicas de felinos encaminhados ao Laboratório Regional de Diagnóstico da Faculdade de Veterinária da Universidade Federal de Pelotas (LRD/UFPel), no período de 1978 a 2018. Nesse período foram recebidas 1633 amostras de felinos, sendo 363 (22%) entre os anos de 1978 e 1999 e 1270 (78%) entre os anos de 2000 e 2018. Com relação aos diagnósticos, 457 felinos (28%) apresentaram tumores benignos ou malignos, sendo os tegumentares e os mamários os mais frequentes. As doenças bacterianas, fúngicas, virais, parasitárias, sem agente definido e as intoxicações totalizaram 554 casos (33,9%), destacando-se a esporotricose, com 12,8% dos diagnósticos. Concluiu-se que, na região sul do RS, o encaminhamento de felinos para diagnóstico aumentou significativamente após o ano 2000, comprovando que a espécie passou a ter maior importância como animal de companhia. Concluiu-se, também, que as neoplasias têm papel relevante entre as doenças de felinos e que a esporotricose é uma das mais importantes zoonoses na região.(AU)


The goal of this paper was to identify the main disease affecting felines in the southern region of Rio Grande do Sul. The necropsy protocols and feline biological materials submitted to the Regional Diagnostic Laboratory of the Veterinary College of the Federal University of Pelotas (LRD / UFPel) were reviewed, from 1978 to 2018. During this period 1633 feline samples were received, 363 (22%) between 1978 and 1999 and 1270 (78%) between 2000 and 2018. 59% of felines did not present a defined breed. As for diagnoses, 457 felines (28%) presented benign or malignant tumors, the most common being the integumentary and mammary tumors. Bacterial, fungal, viral, parasitic or undefined agent infections and intoxications were observed in 554 cases (33.9%), especially sporotrichosis with 12.8 % of the diagnoses. It was concluded that in southern RS the referral of cats for diagnosis increased significantly after the year 2000, proving that they became more significant as companion animals. It was also concluded that neoplasia play a relevant role among feline diseases, and that sporotrichosis is one of the most important zoonoses in the region.(AU)

Lungworms are a common finding in seals and fur seals around the world. However, from existing records, the biogeographical distribution of filaroid helminths appears to be restricted, and these parasites are endemic in only certain areas and species, mainly in the Northern Hemisphere. The occurrence of infection in pinniped species in the Southern Hemisphere is scarce. The objective of this work is to verify the prevalence of lungworms in Arctocephalus australis in waters off the southern coast of Brazil. Twenty subadult specimens of A. australis found recently dead on the southern coast of Brazil were necropsied and their lungs were examined. Parasitic cysts were found in only one specimen (prevalence of 5%). The helminths were morphologically identified as Parafilaroides normani (Metastrongyloidea: Filaroididae). This helminth species has been reported in pinnipeds from Australia, New Zealand and South Africa. This is the first record of P. normani in A. australis and for the western South Atlantic, providing additional data regarding the biogeographic distribution of the parasite.

BACKGROUND: Online programs targeting lifestyle have the potential to benefit brain health. We aimed to develop such a program for individuals with subjective cognitive decline (SCD). These individuals were reported to be at increased risk for dementia, and report both an intrinsic need for brain health information and motivation to participate in prevention strategies. Co-creation and user-evaluation benefits the adherence to and acceptance of online programs. Previously, we developed a prototype of the online program in co-creation with the users . OBJECTIVES: We now aimed to evaluate the user-experiences of our online lifestyle program for brain health. DESIGN: 30-day user test; multi-method. SETTING: Participants were recruited in a memory clinic and (online) research registries in the Netherlands (Alzheimer Center Amsterdam) and Germany (Center for memory disorders, Cologne). PARTICIPANTS: Individuals with SCD (N=137, 65±9y, 57% female). MEASUREMENTS: We assessed user-experiences quantitatively with rating daily advices and usefulness, satisfaction and ease of use questionnaires as well as qualitatively using telephone interviews. RESULTS: Quantitative data showed that daily advices were rated moderately useful (3.5 ±1.5, range 1-5 points). Participants (n=101, 78%) gave moderate ratings on the programs' usability (3.7±1.3, max 7), ease of learning (3.6±1.9) and satisfaction (4.0±1.5), and marginal ratings on the overall usability (63.7±19.0, max 100). Qualitative data collected during telephone interviews showed that participants highly appreciated the content of the program. They elaborated that lower ratings of the program were mainly due to technical issues that hindered a smooth walk through. Participants reported that the program increased awareness of lifestyle factors related to brain health. CONCLUSIONS: Overall user-experience of the online lifestyle program was moderate to positive. Qualitative data showed that content was appreciated and that flawless, easy access technique is essential. The heterogeneity in ratings of program content and in program use highlights the need for personalization. These findings support the use of online self-applied lifestyle programs when aiming to reach large groups of motivated at-risk individuals for brain health promotion.

Foi realizado um estudo retrospectivo dos casos suspeitos de aborto por herpesvírus equino observados na região sul do Rio Grande do Sul entre 1978 e 2016. Foram revisados os protocolos de necropsia do Laboratório Regional de Diagnóstico da Faculdade de Veterinária da Universidade Federal de Pelotas resgatando-se os dados epidemiológicos, lesões macroscópicas e histológicas de cada caso. Foram observados dois surtos da enfermidade com prevalência entre 5,7% e 50% nos diferentes estabelecimentos, e dois casos individuais. Em todos os casos foram enviados fragmentos de órgãos fetais formolizados. Histologicamente, em todos os casos foram observados focos de necrose no fígado, pulmão e baço e presença de corpúsculos de inclusão acidofílico em hepatócitos, células epiteliais pulmonares e leucócitos. A imuno-histoquímica utilizando anticorpo policlonal comercial para herpesvirus equino-1 (EHV-1) revelou marcação positiva em todos os casos. Além disso, foi extraído DNA dos tecidos emblocados em parafina dos casos e submetidos à técnica de nested-PCR seguida de sequenciamento genômico dos amplicons em duas amostras. Estes achados indicam, que EHV-1 deve ser considerado como diagnóstico diferencial em casos de aborto em equinos no sul do Rio Grande do Sul.(AU)

The aim of this study was to evaluate the effect of pST injections on metabolism, testicular size, and sperm characteristics in young boars. Sixty 22-day old piglets were divided into two groups: pST (n=30) and Control (n=30). The pST group was submitted to pST injections (90µg/kg body weight) every three days up to 330 days of age. Blood collections were performed weekly. Testicular weight was measures at 22, 82, 142, 202 and 365 days of age. Libido and fresh semen characteristics were evaluated between 150 and 210 days of age. Semen characteristics were also evaluated during a 72h storage period (15ºC). Testosterone, albumin, and phosphorus blood concentrations were higher in the pST group (P<0.05). The pST group had a higher IGF-I concentration in seminal plasma (P=0.05) and higher testicular weight (P<0.001) compared to the Control group. The pST group had higher ejaculate volume (P<0.001), total sperm count (P=0.047) and number of inseminating doses/ejaculate (P=0.047). During the 72h storage period, the pST group had a lower number of morphological alterations (P<0.001) compared to the Control group. In sum, pST injection in young boars increased testosterone concentration, testicular size, and sperm quality.(AU)


O objetivo deste estudo foi determinar o efeito da administração de pST sobre o metabolismo, o tamanho testicular e a qualidade espermática de cachaços jovens. Foram usados leitões com 22 dias de idade, divididos em dois grupos: pST (n=30) e controle (n=30). O grupo pST foi submetido a injeções de pST (90µg/kg de peso vivo) a cada três dias até 330 dias de idade. Peso testicular foi avaliado aos 22, 82, 142, 202 e 365 dias de idade. Libido e qualidade do sêmen fresco foram avaliados entre 150 e 210 dias de idade. Qualidade espermática foi avaliada durante refrigeração (15ºC) por um período de 72 horas. Concentrações sanguíneas de testosterona, albumina e fósforo foram maiores no grupo pST (P<0,05). O grupo pST apresentou maior concentração de IGF-I no plasma seminal (P=0,05) e maior peso testicular, quando comparado ao grupo controle (P<0,001). O grupo pST apresentou maior volume espermático (P<0,001), concentração espermática (P=0,047) e número de doses espermáticas por ejaculado (P=0,047). Durante o período de 72 horas de refrigeração, o grupo pST teve menor número de patologias espermáticas (P<0,001). Assim, conclui-se que a administração de pST aumenta a concentração sanguínea de testosterona, o tamanho testicular e a qualidade espermática de cachaços jovens.(AU)

BACKGROUND: Persistent vertical diplopia may occur after cataract surgery as a rare complication of retro- or parabulbar anesthesia. This is probably caused by structural changes in the muscles, altering muscular elasticity and function and thus complicating setting of the dosage for corrective strabismus surgery. The aim of our study was to investigate the effect of strabismus surgery in this specific motility disorder. METHODS: The findings from 15 consecutive patients (six women, nine men, median age 76 years), who had undergone initial strabismus surgery in our eye clinic between 2007 and 2010 due to vertical diplopia following cataract surgery, were investigated retrospectively. In all cases, cataract surgery had been performed under retro- or parabulbar anesthesia. RESULTS: Preoperatively, all affected eyes (five right eyes, ten left eyes) showed hypotropia with elevation deficiency and overaction of the inferior rectus muscle and/or superior oblique muscle on down-gaze. The median vertical deviation in primary position was 9.1 deg (min. 4.6, max. 24.7), measured with the alternate prism cover test, and 8 deg (min. 3.5, max.18) at the tangent screen of Harms. In all cases, the inferior rectus muscle was recessed 3 to 6 mm (median 3.5 mm). On the first day after surgery, the median angle of squint in primary position was 2.3 deg (min. 0, max. 10.2), when measured with the alternate prism cover test, with a mean dose-effect relationship of 1.8 ± 0.7 deg angle reduction per millimetre recession (median 1.9 deg/mm). In the postoperative period, eight patients examined after 2 to 20 months (median 3.5 months) showed a median vertical deviation of 5.7 deg (min. 1.7, max. 11.3), with a mean dose-effect relationship of 1.7 ± 1.3 deg/mm (median 1.8 deg/mm), but the values ranged widely. Four patients were not examined but interviewed by telephone. There was no feed-back from three patients. Six of 12 follow-up patients had no complaints, three had prisms to correct a persisting angle, and three patients needed further squint surgery. CONCLUSIONS: The efficacy of inferior rectus muscle recession for correction of hypotropia following cataract surgery with local anesthesia ranged widely. In this condition, operating on one muscle is a good option for correction of squint angles of less than 12 deg. Squint angle enlargement can occur in the postoperative course, and may necessitate further surgery.

INTRODUCTION: The aim of this study was to compare the intraoperative and early postoperative outcome of trabeculectomy with mitomycin C in glaucoma patients undergoing general anaesthesia compared to those undergoing subconjunctival anaesthesia. METHODS: The perioperative results of consecutive glaucoma patients receiving a trabeculectomy with mitomycin C either under general anaesthesia (group 1, n = 60) or in subconjunctival anaesthesia (group 2, n = 60) were analysed in a retrospective study. All surgical procedures were performed by one surgeon and only one eye of each patient with no history of previous conjunctival surgery was included in the study. RESULTS: No significant peri- and postoperative differences concerning period of hospitalisation, intraocular pressure, filtration-bleb bleedings, hyphaema, frequency of laser suture lysis, hypotony, chorioidal detachment or revision surgery could be shown between the two groups. Considering all patients together, there was a significant correlation between the occurrence of postoperative filtering bleb bleedings and the absolute number of topical antiglaucomatous substances used prior to surgery. CONCLUSIONS: The perioperative risk profile of penetrating glaucoma surgery with subconjunctival anaesthesia seems to be similar to that of procedures under general anaesthesia. For prevention of postoperative filtering bleb bleedings, the administration of topical antiglaucomatous drugs should be stopped before surgery.

INTRODUCTION: The aim of this study was to differentiate the pressure-reducing results following trabecular aspiration combined with small incision cataract surgery in eyes with pseudoexfoliation depending on preoperative pressure and medication score. PATIENTS AND METHODS: A retrospective analysis was made of 104 exfoliative eyes of 104 patients who underwent combined phaco/trabecular aspiration. Success was defined as a relative reduction of IOP of at least 20 %, absolute IOP of ≤ 20 mmHg, stable or reduced medication score and a lack of any further pressure-reducing surgery. RESULTS: The success rate was 0.68 and 0.64 after 1 and 2 years. In eyes with medically uncontrolled preoperative IOP (> 20 mmHg) the IOP dropped significantly from 25.4 ± 4.3 mmHg to 17.0 ± 3.4 mmHg after two years. In eyes with preoperative controlled IOP the pressure lowering effect was low (17.1 ± 2.3 to 15.9 ± 2.3 mmHg) with a significant reduction of medication. CONCLUSIONS: Combined cataract surgery and trabecular aspiration is a reasonable option in exfoliative eyes with IOP values in the low twenties in order to achieve a reduction of topical medications and to reach a stable IOP level.

Twenty-one cases of pythiosis in horses (n = 10), dogs (n = 9) and cattle (n = 2) were investigated. The aetiology in all cases was confirmed by immunohistochemistry. Data related to the clinical course and outcome and localization of the lesions were obtained from pathology reports. The equine lesions consisted of fibrotic tissue with multiple, often coalescing, areas of immature granulation tissue encircling eosinophilic cores. Affected dogs had gastrointestinal and/or cutaneous lesions with either or both of a granulomatous/pyogranulomatous or necrotizing eosinophilic inflammatory reaction. In cattle, cutaneous lesions were characterized by multifocal to coalescing granulomas with surrounding fibrosis. The number of intralesional hyphae, the distribution of hyphae, the presence of angioinvasion and the nature of the local inflammatory reactions were associated with the different types of lesions observed.

We report the case of a 75-year-old patient diagnosed with severe aortic stenosis and two-vessel coronary artery disease. Due to multiple comorbidities including chronic renal insufficiency, stroke and pulmonary hypertension (EuroSCORE: 34%; STS mortality risk: 14.9%), he was not a candidate for conventional aortic valve surgery. He underwent a novel hybrid treatment approach combining off-pump CABG and transapical aortic valve implantation via a median sternotomy. Extracorporeal circulation could be entirely avoided.

BACKGROUND: There is an increasing interest in assessing the quality of life as an additional guide for therapeutic choices, especially in patients with chronic diseases, e.g. glaucoma. Most standardized questionnaires are available in English making them inapplicable for daily use in non-English-speaking environments. The "Glaucoma Quality of Life 15 Questionnaire" (GQL-15) is a test designed for glaucoma patients in English and was translated into German in order to analyze its practicability in a daily clinical setting. PATIENTS AND METHODS: The GQL-15 was handed out to glaucoma patients during the hospital stay. The questionnaires were analyzed retrospectively and correlated to clinical indices, such as age, visual acuity, visual field, parametric mean deviation (MD) and frequency of pharmacologic treatment. RESULTS: A total of 31 patients were evaluated. It could be demonstrated that the quality of life summary scores correlated with visual field loss. Evaluation of subscale scores of visual function revealed that glare and dark adaptation were correlated with glaucoma severity especially in the early stages of the disease. CONCLUSIONS: The German translation of the GQL-15 was well accepted by glaucoma patients. In our analysis disability of vision-related activities as expressed in the translated GQL-15 correlated with glaucoma severity. This first use of the translated GQL-15 encourages further studies in German glaucoma patients and their quality of life.

Descrevem-se dois surtos de intoxicação por Senecio spp. em ovinos a campo no sul do Rio Grande do Sul. A enfermidade ocorreu no ano 2006 em duas propriedades que estavam severamente invadidas pela planta e com acentuada escassez de forragem. A morbidade nos dois surtos variou de 10 por cento a 9,5 por cento nas duas propriedades, respectivamente. Na primeira propriedade os animais eram nascidos e criados na área e na outra os ovinos haviam sido adquiridos alguns meses antes do surto. Os sinais clínicos caracterizaram-se por emagrecimento progressivo, apatia, fraqueza, icterícia e fotossensibilização. Macroscopicamente, o fígado dos ovinos necropsiados estava aumentado de tamanho, amarelado, com superfície capsular irregular e vesícula biliar aumentada de tamanho e com edema da parede. Ao corte no fígado havia nódulos bem delimitados, esbranquiçados com 1-3mm de diâmetro. Havia, ainda, ascite e discreto edema de mesentério. Microscopicamente, as lesões hepáticas eram semelhantes em todos os fígados e caracterizadas por megalocitose, fibrose periportal, presença de hepatócitos tumefeitos, vacuolizados e de hepatócitos necróticos distribuídos aleatoriamente pelo parênquima hepático, hiperplasia das células dos ductos biliares e presença de pseudo-inclusões nos núcleos de hepatócitos. Encefalopatia hepática (status spongiosus) foi observada em todos os ovinos necropsiados. O diagnóstico foi baseado na epidemiologia, sinais clínicos, lesões macroscópicas e histológicas observadas. Concluiu-se que, apesar de a intoxicação por Senecio spp. ser pouco frequente em ovinos e essa espécie animal controlar eficientemente a planta, isso, aparentemente, depende da lotação utilizada e surtos da intoxicação podem ocorrer quando esta lotação é menor que 0,2 ovinos por hectare e há carência de forragem acentuada.


Two outbreaks of Senecio spp. poisoning in grazing sheep in southern Rio Grande do Sul are described. The disease occurred in 2006 on two farms that were heavily infested with Senecio spp. and had shortage of forage. The morbidity in both outbreaks was 10 percent and 9.5 percent, respectively. In one case the sheep were born and had grown up on the farm. On the other, the sheep flock was introduced into the farm some months before the outbreak. Clinical signs included progressive emaciation, apathy, weakness, jaundice and photodermatitis. At necropsy, the sheep had an augmented yellowish and firm liver with irregular capsular surface, and an extended and edematous gallbladder. There was distension and edema of the gallbladder. The cut surface the liver showed well-circumscribed whitish nodules measuring 1-3mm in diameter, which were randomly scattered throughout the hepatic parenchyma. There was also edema of mesentery and ascitis. Histopathological findings of the liver included hepatomegalocytosis, bile duct hyperplasia and periportal fibrosis. There were also swollen hepatocytes, randomly scattered necrotic hepatocytes and nuclear pseudo inclusions. Hepatic encephalopathy (status spongiosus) was observed in the brain in all necropsied sheep. The diagnosis of Senecio poisoning was based on epidemiological data, clinical signs, necropsy findings and histological lesions. It was concluded that despite infrequent, Senecio spp. poisoning in sheep may occur when animals are placed in areas heavily invaded by the plants. Although sheep can efficiently control Senecio spp. in paddocks, this apparently depends on the stocking rate, and outbreaks may occur when stocking rate is less than 0.2 sheep per hectare and when there is severe shortage of forage.

Foi realizado um estudo retrospectivo de casos de raiva paralítica em bovinos na área de influência do Laboratório Regional de Diagnóstico (LRD) da Faculdade de Veterinária, Universidade Federal de Pelotas (UFPel), ocorridos em 1978-2007. Foram estudados também 11 surtos de raiva observados em 2008-2010, na mesma região. Neste período morreram 42 animais de um total de 686 sob risco. A idade dos animais foi de 1-6 anos e o curso clínico de 4-14 dias. No estudo retrospectivo de 1978-2007 foram identificados 77 surtos ou casos isolados de raiva paralítica em bovinos. A morbidade em todos os surtos diagnosticados em 1978-2010 variou de 0,37 por cento a 20 por cento. Vinte e quatro casos ocorreram no outono, sete na primavera, 14 no verão e 16 no inverno. O diagnóstico foi realizado pela epidemiologia, sinais clínicos e lesões histológicas observadas. No estudo dos casos de raiva paralítica em bovinos ocorridos a partir de 2008, o diagnóstico foi confirmado pela técnica de imuno-histoquímica utilizando anticorpo policlonal anti-virus rábico. Em dois destes casos não foi observada meningoencefalite não-supurativa, porém a imuno-histoquímica demonstrou a presença do antígeno viral. Esta técnica é uma importante ferramenta para o diagnóstico de raiva, devendo ser utilizada em todos os casos suspeitos nos quais não se evidenciam lesões de encefalite.


A retrospective study of paralytic rabies in cattle in southern Rio Grande do Sul, Brazil, diagnosed from 1978 to 2007 by the Regional Diagnostic Laboratory (LRD) of the Veterinary School, Federal University of Pelotas (UFPel), with 77 outbreaks or isolated cases of paralytic rabies in cattle, is reported. A study of 11 outbreaks of rabies, observed from 2008 to 2010 in the same region, where 42 cattle aged 1-6 years died from a total of 686 at risk, with a clinical course of 4 to 14 days, was also made. The morbidity of all outbreaks diagnosed from 1978-2010 ranged from 0.37 percent to 20 percent; 24 cases occurred in autumn, 7 in spring, 14 in summer, and 16 in winter. The diagnosis was achieved by epidemiology, clinical signs and histological lesions. Immunohistochemistry using rabies virus polyclonal antibody was positive in all cases. In two cases non-suppurative meningoencephalitis was not observed, and the diagnosis was confirmed by immunohistochemistry. This technique is an important tool for the diagnosis of rabies and should be used in all suspected cases in which no evidence of encephalitis is observed.

PURPOSE: Preferred hemodialysis (HD) access is an autologous fistula. Vascular grafts are used in patients with vessels unsuitable to accomplish an arteriovenous fistula (AVF). It is recommended that most current grafts mature in situ for 2-3 weeks before being accessed. Graft complications occur because the structure was not designed for the trauma of repeated cannulation. This study graft has a different structure that enables early use. Its design minimizes weeping through the graft walls and is meant to endure repeated access, thus minimizing the use of HD catheters. The purpose of this study was to show that the Flixene™ graft can be safely placed in patients where fistulas have failed and can be cannulated in 24-72 hr, while maintaining patency rates similar to other polytetrafluoroethylene (PTFE) grafts on the market. Flixene™ configuration should also reduce the incidence of pseudoaneurysms and seromas. METHODS: A prospective two-center study placed 33 grafts in 33 patients; graft efficacy, post-operative complications, and patency were evaluated. Ease of cannulation and dialysis center complications related to early cannulation were documented. Six month follow-up data was analyzed. RESULTS: Successful access was achieved in all 33 patients within 72 hr (29 patients within 24 hr). Overall primary patency at 6 months was 49%; primary-assisted patency at 6 months was 80%. No pseudoaneurysms or seromas were documented at 6 months. Complications were typical of graft access. CONCLUSION: Early cannulation was successful in all patients. Primary and secondary patency rates at 6-months were equivalent to other data reported on PTFE grafts. Flixene™ successfully prevented pseudoaneurysm and seroma formation at 6 months of prospective follow-up. This graft is a better last-resort option for patients who cannot receive a fistula, compared to double-lumen cuffed catheters.

RESUMEN

Applanation tonometry has been the gold standard in clinical ophthalmology for more than fifty years. The most popular factor is central corneal thickness that is now routinely considered in glaucoma management. However, other individual features of the cornea can also play a key role for the interpretation of the applanation values. Other factors influencing applanation tonometry that have been well known for decades include tear film, fluorescein illumination etc., and should be kept in mind. According to the available literature the absence of a correct calibration cannot be neglected.

BACKGROUND: The aim of this study was to evaluate the concordance of intraocular pressure (IOP) measurements in the supine glaucoma patient using a Perkins applanation tonometer (PAT) compared to the iCare® rebound tonometer (RT), a hand-held device requiring no local anaesthesia. METHODS: 73 left eyes of 73 glaucoma patients were included in this consecutive case study and measured both by supine Perkins applanation tonometry and by right lateral posture rebound tonometry in the supine position (RLP). The patients were divided into three subgroups dependent on IOP (SG-1: 0 - 15 mmHg, SG-2: 16 - 22 mmHg, SG-3: more than 23 mmHg). RESULTS: The mean deviation between RT and PAT was 2.6 ± 4.0 mmHg, the 95 % confidence interval was -5.3 to 10.4 mmHg. 69 % of the measurements showed deviations within 3 mmHg between the two devices. Deviation was smallest in SG-2, and largest in SG-3. CONCLUSIONS: Rebound tonometry is comfortable to use even in supine patients. RT measurement agreed overall significantly with those of Perkins applanation tonometry, generally overestimating PAT measurement. In high IOP values, RT did not correlate as well with PAT as in moderate IOP levels.

BACKGROUND: Hereditary optic neuropathies lead to a decrease in visual acuity corresponding to a reduction of the ganglion cell layer and the nerve fibre layer. The most common neuropathies, Leber hereditary optic neuropathy (LHON) and autosomal dominant optic atrophy (ADOA), are mitochondrial disorders. CASE REPORT: We describe two cases, one with LHON and one with ADOA, and demonstrate the decrease in thickness of the ganglion cell layer using spectral domain OCT (optical coherence tomography). RESULTS: In the case with LHON, the ganglion cell layer and the nerve fibre layer showed a considerable reduction from the initial occurrence of symptoms to the 1? years follow-up examination. In the patient with ADOA, the atrophy of the ganglion cell layer and the nerve fibre layer was clearly visible on OCT corresponding to the poor visual acuity of the patient. CONCLUSIONS: Spectral domain OCT instruments allow for high resolution imaging and quantitative analysis of the various retinal layers. Using this technology, it is possible to distinguish between retinal disorders of the outer retinal layers and disorders of the inner retinal layers, like in our cases with LHON and ADOA.

BACKGROUND: A comparison of intraocular pressure (IOP) in cases of infantile glaucoma during general anesthesia was carried out by measurement with the iCare rebound tonometer (RBT) and a handheld applanation tonometer (Perkins). METHODS: A total of 45 eyes from pediatric patients with childhood glaucoma were included in this prospective trial. Bland-Altman plots and linear regression were used for statistical analysis. RESULTS: In almost two thirds of the eyes the difference between RBT and Perkins was 2 mmHg or less. A systemic bias of 1.96 mmHg and a 95% confidence interval of -3.35 to 7.26 was detected. Linear regression of the Bland-Altman data showed a proportional error (gradient=0.16; r(2)=0.23; p<0.01). CONCLUSIONS: The iCare rebound tonometer is useful during general anesthesia in cases of childhood glaucoma. It provides comparable IOP values to applanation tonometry with a tendency to record higher values.

BACKGROUND: Joubert syndrome (JS) belongs to the ciliopathies and is a mostly autosomal recessively inherited disease (in the case of OFD1 mutations, JS is an X-linked trait). It is characterised by midbrain-hindbrain malformations with developmental delay, hypotonia and ataxia and a broad spectrum of other facultative findings. The aim of our study was to examine the ophthalmological and neuro-ophthalmological features of JS in our patients and to compare our findings to those of other studies. METHODS: In a retrospective study we evaluated the ophthalmological and neuro-ophthalmological findings of 9 consecutive patients who met the diagnostic criteria of JS. RESULTS: All patients had abnormalities of ocular motility, 4/9 used head thrusts to shift gaze (oculomotor apraxia OMA). In 6/8 patients, the optokinetic reflex (OKN) was absent. Furthermore, 8/9 children showed nystagmus, mostly see-saw nystagmus. Manifest strabismus was found in 8/9 while 3/9 had a retinopathy with either abnormal ERG and/or fundus appearance with or without visual impairment. Chorioretinal colobomata were present in 5/9 cases. Two patients showed a unilateral congenital ptosis, one a facial nerve paresis. CONCLUSIONS: The early neuro-ophthalmological findings in JS are not pathognonomic, but may lead to the diagnosis of JS. The syndrome should be suspected in patients with nystagmus, especially see-saw nystagmus, and abnormal OKN and/or OMA, and/or colobomata of the fundus, and further paediatric examinations should be initiated.
