RESUMEN

PURPOSE: To evaluate the development of astigmatism after cataract extraction depending on the kind of incision MATERIAL AND METHODS: Astigmatism was examined in 107 eyes of 92 patients, who underwent cataract extraction, mostly with IOL's implantation. In 52 eyes phacoemulsification with tunnel incision, measuring 5 mm in 27 and 8 mm in 25, was performed. In the control group of 55 eyes incision with scleral flap measured 2/5 of the corneal circumference. In cases with 5 mm incisions no sutures were used, in 8 mm incisions wounds were closed with single sutures and in control group the double continuous sutures were applied. Astigmatism was measured in two days, one week, four weeks and 3 months after surgery. RESULTS: Post-operative astigmatism decreased gradually and after 3 months was average 0.14 D in the eyes without sutures, 0.68 D in those with single ones and 1.78 D in the control group.

RESUMEN

The anterior chamber tamponade with Healon and 2.5% solution of hydroxymethyl-cellulose has been applied in 10 patients, aged 9-65 years, during antiglaucomatous surgery connected with interscleral dislocation of iris. The tamponade prevented the rupture of blood vessels of iris. Bleeding connected with the cutting of blood vessels was stopped by filling the anterior chamber with viscoelastic substance under increased pressure. It was possible to preserve the anterior chamber during surgery. The properties of Healon and 2.5% solution of hydroxymethyl-cellulose were similar.

RESUMEN

Forty one patients aged 29 to 82 years were subjected to cataract extraction in ambulatory conditions. Fifty eyes were operated, among them 33 with implantation of a posterior chamber artificial lens. In cases of a lack of opacities in the cortex layers their structure was loosened before the operation by a YAG laser. The lens was extracted mainly by phacoemulsification. The ambulatory procedure did not result in any undesirable consequences. The profits of operation performed in such a way were stressed. Attention was called to the fact that by prudent qualification of patients and a careful selection of surgical methods the risk of this kind of treatment is not greater than in stationary conditions.

RESUMEN

The dependence of hydrodynamics on the topography of trabeculectomy performed simultaneously with cataract extraction was checked in 35 eyes with cataract and glaucoma. The eye was opened by a three-surface incision with a broad scleral flap. The specimen were examined in a microscope. In 21 eyes the whole trabeculum together with Schlemm's canal were excised, in 12 eyes the anterior band. In the specimen from 2 eyes no elements of the trabeculum could be found. The dependence of the regulation of the IOP on the geography of the trabeculectomy was established. Accepting the filtering mechanism of the trabeculectomy one may assume that the applied method of incision promotes the formation of the outflow++ ways of the aqueous in a high percentage of cases.

RESUMEN

The authors performed 41 operations of pterygium with transplantation of a free epithelial flap from the bulbar conjunctiva in order to cover the defect after the removal of the pathological tissue. The results of 35 operations had been controlled in the period of one to ten years. In 25 cases the operation was the first intervention, in 9 cases the second or third in a recurrent pterygium treated by other methods. In case of necessity the semilunar fold was reconstructed. A recurrence took place in one case after 6 months. The cause of the recurrence was the collection of the flap together with the fibrous layer of the conjunctiva. In the remaining cases a good and lasting effect was achieved, as well from an anatomical as from a cosmetic point of view. The results were compared with the results achieved by various authors by means of another methods.

RESUMEN

The intrascleral peripheral iridencleisis was performed in 42 cases of severe advanced primary, secondary and developmental glaucoma. Normalization of the intraocular pressure was attained in 71.4% of eyes. The operation did not cause any decrease of the visual acuity; no essential complications were noted.
