RESUMEN

OBJECTIVE: To evaluate the implementation of Xpert-MTB/RIF®, as an early diagnosis technique, in a rural area of Ethiopia. METHODS: Data were retrospectively collected from those patients over 13 years of age who were requested to take the Xpert MTB/RIF® test in a rural hospital located 45 km from the reference laboratory, during the first 3 years of its implementation (2015, April -2018, April). RESULTS: A total of 306 patients older than 13 years were evaluated, in 85 (27.8%) there was an error in the processing of the test and the result was not obtained. Of the 221 samples with results, the median time between obtaining the sample and receiving the result was 21 days and 42 of them were positive (19%, 95% CI: 14.2-24.9%). The sample with the highest diagnostic yield was adenopathy (88.8%; [8/9]; p<0.001). CONCLUSIONS: There are more bacteriological diagnoses with Xpert-MTB/RIF®, but with a delay in obtaining the result and its main objective, which is early diagnosis, is not achieved.

RESUMEN

A rare case of primary hydatid cyst of the mesentery is described. Though cystic echinococcosis infestation can occur in any part of the body primary peritoneal involvement is rare. The mechanism of primitive peritoneal infection by the parasite is still unclear. The hypotheses which can explain this rare primary localization, the sensitivity and the specificity of the serological and instrumental tests and the therapeutic management of this atypical lesion are discussed.

RESUMEN

The Authors report a case of primary non-Hodgkin's lymphoma of the testis. Although lymphoma is a rare neoplasm, it is commonest tumor in the elderly age. Surgical therapy is essential but some times it can be associated to radio-chemotherapy.

RESUMEN

The Authors report a case of intestinal bleeding due to lipoma of the epatic flexure. Lipomas are the commonest mesenchymal benign tumors that can be found in the colon and are second as frequence only to the adenomatous polyps. They are most frequent in the right colon and shown an opposite distribution in comparison with adenocarcinomas and adenomatous polyps. When tumor size is greater than 3 cm, lipomas become symptomatic. Radiological and endoscopic investigations provide orientative elements for diagnosis. In lipomas less than 2 cm in diameter an endoscopic treatment is feasible.

RESUMEN

The Authors have reported a case of retroperitoneal leiomyosarcoma. The retroperitoneal localization is quite unusual and early diagnosis is difficult. Only surgery operation and radio-chemotherapy can improve the prognosis. Tumor size is the major prognostic factor.

The Authors report a case of Vater's ampulla apudoma and after having examined the characteristics of these neoplasms they discuss clinical presentation, diagnostic and treatment problems of islet cell adenomas. They review the literature and make some remarks.

RESUMEN

Personal experience in the treatment of 93 cases of cancer of the large bowel and rectal localization is reported 32 patients (A group) received blood transfusions, 61 patients (B group) hadn't any transfusions or autotransfusions. Postoperative morbidity was 34.37% in A group and 14.75% in B group. The Authors stress the basic importance of autotransfusions as well in patients surgically treated for colorectal cancer.

RESUMEN

Lipomas are an uncommon cause of small-bowel obstruction. The symptoms are intermittent and nonspecific. The Authors report a case in which intussusception was diagnosed with echography and the obstruction was treated with resection. They conclude that lipomas must always be considered in the differential diagnosis of intrabdominal neoplasms and in intestinal obstructions.
