RESUMEN

Objective:To analyze and explain the influences of the professional background of the directors of public hospitals on the economic operation of the hospital,and to provide research support for training compound hospital management talents and improving the relevant policies of public hospital director management.Methods:The fixed effect model was used to conduct a quantitative analysis based on the data of individual directors and hospital economic performance of public hospitals in Guangdong from 2005 to 2018.Results:Compared with hospitals whose directors without clinical medical backgrounds,the medical income,the number of admissions and medical expenditure of the hospitals whose directors with clinical medical backgrounds were lower,and turnover of bed rate was higher,the differences were statistically significant(P<0.05).Compared with hospitals where the directors had not obtained a degree related to management and did not participate in management training,the asset-liability ratio and management cost of the hospital where the directors had obtained a degree or participated in the training were lower,and the liquidity ratio was higher,with statistical significance(P<0.05).Conclusion:For the economic operation indicators closely related to clinical work,directors with clinical medical background have a more significant impact because of their advantages in clinical front-line learning and work experience.For the economic operation indicators closely related to asset management,administration and cost management,directors with management learning experience can better reveal and exert their advantages in applying advanced management concepts,methods and tools,and have a more significant impact.

RESUMEN

Objective:To study reflux characteristics of patients with endoscopic negative heartburn and their manifestation under probe-based confocal laser endoscopy (pCLE) based on the Rome Ⅳ standard.Methods:Thirty-six endoscopic negative outpatients with typical heartburn at the Department of Gastroenterology of the Second Affiliated Hospital of Baotou Medical College from September 2020 to March 2021 were included, and underwent 24-hour multichannel intraluminal impedance-pH monitor and pCLE. According to Rome Ⅳ diagnostic process, patients were divided into non-erosive reflux disease (NERD) group ( n=16), reflux hypersensitivity (RH) group ( n=8) and functional heartburn (FH) group ( n=12). The Gerd-Q scale score, 24-hour pH monitoring results and microstructure changes under pCLE were compared among the three groups. Results:There was no significant difference in the total score, positive symptom score, negative symptom score or positive influence score of Gerd-Q scale among the three groups ( P>0.05). DeMeester score [28.45 (20.08, 34.53)] and acid reflux times (24.88±9.05) in the NERD group were significantly higher than those in the RH group [7.30 (3.90, 11.38), P<0.001; 13.63±5.76, P=0.003] and FH group [6.90 (4.80, 9.73), P<0.001; 7.42±8.32, P<0.001]. But there was no significant difference between the RH group and the FH group ( P>0.05). The diameter of intra-papillary capillary loop (IPCL) (18.68±2.12 μm) and dilation of intercellular space (3.95±0.97 μm) in the NERD group were significantly higher than those in the RH group (13.91±1.99 μm, P<0.001; 2.97±0.55 μm, P=0.006) and FH group (13.83±2.00 μm, P<0.001; 2.31±0.54 μm, P<0.001), but there was no significant difference between the RH group and the FH group ( P>0.05). The number of IPCL in the NERD group, RH group and FH group were 2.0 (1.00, 2.75), 2.0 (1.00, 2.75) and 1.5 (1.00, 2.00), respectively with no significant difference ( P=0.697). Conclusion:Gerd-Q scale is not suitable for differential diagnosis of patients with endoscopic negative heartburn. Compared with functional esophageal diseases (RH and FH), acid reflux and mucosal microstructure changes are of more important pathogenic significance in NERD.

RESUMEN

Objective To investigate the chemical constituents of the fruit pedicels of Panax ginseng. Methods The compounds were purified by silica gel column chromatography and preparative reverse-phase high performance liquid chromatography. Their structures were elucidated on the basis of spectroscopic analyses. Results Twenty-eight compounds were isolated from the total extract of the fruit pedicels of P. ginseng. They were identified as 20(S)-protopanaxatriol (1), 20(S)-protopanaxadiol (2), 20(S)-dammar-24- ene-3-one-6α,12β,20-triol (3), (20S,23E)-dammar-23-ene-3β,6α,12β,20,25-pentol (4), (20S,24R)-dammar-26-ene-3β,6α,12β,20,24- tetrol (5), 20(R)-dammar-3-one-6α,12β,20,25-tetrol (6), ginsenoside Rk2 (7), 20(R)-dammar-24(25)-epoxy-3β,6α,12β,20- tetrol (8), ginsenoside CK (9), 20(S)-dammar-3β,6α,12β,20,25-pentol (10), ginsenoside Rh4 (11), 20(S)-dammar-3β,12β,20,25-tetrol (12), 20(S)-dammar-3β,6α,12β,20,24-pentol (13), 20(R)-dammar-3β,6α,12β,20,25-pentol (14), pseudoginsenoside RT5 (15), ginsenoside Rh1 (16), ginsenoside Rh3 (17), 20(S)-dammar-3β,12β,20,25-tetrol-3-O-β-D-glucopyranoside (18), 20(S)-isoginsenoside Rh3 (19), ginsenoside Rg1 (20), ginsenoside Y (21), ginsenoside Rd (22), ginsenoside la (23), 20(S)-dammar-3β,12β,20,25-tetrol-3-O-β-D-glucopyranosyl-(1→2)-O-β-D-glucopyranoside (24), ginsenoside Rg2 (25), notoginsenoside Fe (26), ginsenoside Re (27), and ginsenoside Rb1 (28), respectively. Conclusion Among them, compounds 4-6, 8, 10, 12, 13, and 21 are isolated from P. ginseng for the first time.
