The transmission of microorganisms via hands is a critical factor in healthcare-associated infections (HAIs), underscoring the importance of rigorous hand hygiene. The rise of antimicrobial-resistant microorganisms, driven in part by the overuse of antibiotics in clinical medicine, presents a significant global health challenge. Antimicrobial soaps, although commonly used, may exacerbate bacterial resistance and disrupt skin microbiota, posing additional health risks and environmental hazards. Essential oils, with their broad-spectrum antimicrobial properties, offer a promising alternative. This study evaluates the antimicrobial activity of essential oils against various bacterial and fungal strains, including multidrug-resistant isolates. Using a range of in vitro and in vivo antimicrobial assays, including minimal inhibitory concentration (MIC), minimal bactericidal concentration (MBC), and minimal fungicidal concentration (MFC), the essential oils were tested against a broad spectrum of pathogens. Additionally, the chemical composition of the oils was analyzed in detail using gas chromatography-mass spectrometry (CG-MS). Clove, oregano, and thyme oils demonstrated potent inhibition of all tested ATCC bacterial strains, with MIC values ranging from 3.125 to 50 µL/mL. These oils also showed significant activity against multidrug-resistant Escherichia coli and Pseudomonas aeruginosa strains. Notably, clove oil exhibited remarkable efficacy against fungal strains such as Aspergillus fumigatus and Trichophyton rubrum, with MIC values as low as 1.56 µL/mL. Synergy tests revealed that combinations of clove, oregano, and thyme oils yielded significantly lower MIC values than individual oils, indicating additive or synergistic effects. The formulation of a soap incorporating clove and oregano oils demonstrated efficacy comparable to synthetic antiseptics in vivo. These findings highlight the exceptional antimicrobial potential of essential oils, mainly clove and oregano, against resistant microorganisms, offering a viable alternative to conventional antimicrobial agents.

Introduction: Healthcare-associated infections (HAIs), also known as nosocomial infections or hospital-acquired infections, begin within 48 hours of hospitalization, within 30 days after hospital discharge, or 90 days after undergoing surgical procedures. Objective: The study aimed to describe the compliance with the five moments for hand hygiene (HH) of the healthcare workers (HCWs) in a hospital. Methods: A prospective observational study was conducted from June 1 to 30,2020 in a pediatric hospital. HCWs compliance with the five moments of HH was registered by direct observation blindly to the participants, using the fact sheet for HH of the WHO. In the rows, the five moments of contact with the patients were recorded: before touching a patient, before clean aseptic procedure, after body fluid exposure risk, after touching a patient and after touching patient surroundings. The actions performed, also was registered: hand washing, alcohol hand friction, omission, or use of gloves. Data were analyzed in SPSS V 21. The protocol was approved by the institutional review board. Results: During the study period, 2.595 observations to 104 HCWs were made. They were pediatric residents 38.5 %, nurses 32.7 % and pediatricians 28.8 %. A global compliance with the five moments of HH of the participants were 64.5% (1673/2595). Before touching a patient, the adherence was 86,9%. The nurses adhered in 69%, pediatrician in 68.6 % and the pediatric residents in 57.2%. Discussion: The global percentage of compliance with the five moments of HH of the medical and nursing staff of the pediatric hospital was 64.5%. Nurses had the highest percentage of adherence. Handwashing compliance was higher before contact with the patients.
Introducción: Las infecciones asociadas a la atención médica (IAAS), también conocidas como infecciones nosocomiales o infecciones adquiridas en el hospital, comienzan dentro de las 48 horas posteriores a la hospitalización, dentro de los 30 días posteriores al alta hospitalaria o 90 días después de someterse a procedimientos quirúrgicos. Objetivo: El estudio tuvo como objetivo describir el cumplimiento con los cinco momentos para la higiene de manos (HH) de los trabajadores de la salud (TDS) en un hospital. Metodología: Se realizó un estudio observacional prospectivo del 1 al 30 de junio de 2020 en un hospital pediátrico. La adherencia de los TDS con los cinco momentos de HH fue registrada por observación directa de manera ciega a los participantes, utilizando la hoja informativa para HH de la OMS. En las filas se registraron los cinco momentos de contacto con los pacientes: antes de tocar a un paciente, antes de un procedimiento aséptico limpio, después de exponerse a fluidos corporales, después de tocar a un paciente y después de tocar el entorno del paciente. También se registraron las acciones realizadas: lavado de manos, fricción con alcohol en gel, omisión o uso de guantes. Los datos fueron analizados en SPSS V 21. El protocolo fue aprobado por el comité de ética institucional. Resultados: Durante el período de estudio, se realizaron 2,595 observaciones a 104 TDS. Eran residentes pediátricos en un 38.5%, enfermeras en un 32.7% y pediatras en un 28.8%. La adherencia global a los cinco momentos de HH de los participantes fue del 64.5% (1673/2595). Antes de tocar a un paciente, la adherencia fue del 86.9%. Las enfermeras se adhirieron en un 69%, los pediatras en un 68.6% y los residentes pediátricos en un 57.2%. Discusión: El porcentaje global de cumplimiento con los cinco momentos de HH del personal médico y de enfermería del hospital pediátrico fue del 64.5%. Las enfermeras tuvieron el mayor porcentaje de adherencia. El cumplimiento del lavado de manos fue mayor antes del contacto con los pacientes.

The use of jewelry among healthcare professionals poses a risk of cross contamination due to potential bacterial accumulation and spread. Through a mixed-method design, this study first analyzed the implications of healthcare professionals wearing jewelry on patient care biosafety as well as on the residual bacterial load of hands and rings after hand hygiene. Firstly, an observational prevalence study to verify whether nursing professionals wear personal accessories during healthcare assistance was carried out. Second, an experimental design involving intentional contamination and hygiene of the hands, with and without a ring, was conducted. The bacterial load of both hands and rings was measured by counting colony forming units. The observational study showed that nursing workers frequently wear jewelry during healthcare assistance. Nonetheless, the experimental study did not indicate differences in bacterial contamination between hands with and without a ring, despite the hand hygiene procedure applied. In conclusion, many nursing workers wear jewelry in the workplace. Although hands with and without a ring exhibited similar microbial load, rings appeared as a potential source of bacterial contamination, reinforcing the need to remove jewelry during working hours. Hand hygiene using alcohol, or soap and water significantly decreased the bacterial load on the participants' hands, with handwashing proving to be the most efficient method for removing intentional contamination.

Background: Proper hand hygiene (HH), which includes sanitizing with alcohol-based hand rub (ABHR) (or handwashing with soap and water if ABHR is unavailable), is key for preventing healthcare-associated infections (HCAI), including COVID-19. Understanding drivers of HH is key to improving adherence. Aim: This study aims to explore drivers and barriers to HH practice at two hospitals in the Dominican Republic in the context of the COVID-19 pandemic to inform development of HH behaviour change interventions. Methods: We conducted in-depth interviews with 20 hospital staff during September 2021. We used the COM-B (capability, opportunity, motivation, behaviour) model to explore HH experiences and preferences. Interviews were recorded, transcribed, coded, and analysed using a thematic approach. Results: A total of 11 parent codes and 27 sub-codes were identified, and 1145 coded segments were analysed. Use of handwashing with soap and water and/or sanitizing with ABHR was reported by all participants; handwashing was generally preferred. Participants expressed knowledge of proper HH methods (capability), but inconsistent supplies and lack of time presented HH challenges (opportunity). Interviewees described practicing HH to protect themselves and their families from COVID-19 and other infections (reflective motivation) or out of habit (automatic motivation). Discussion: By understanding and addressing underlying factors affecting HH, hospitals can decrease the risk of HCAIs. Our findings suggest that interventions implemented to improve HH in these hospitals should target motivation and opportunity. These findings informed a multimodal intervention to increase ABHR access and implement message-tested communications campaigns; end-point assessments will provide insights into the intervention's impact.

BACKGROUND: Healthcare-associated infections are prevalent in low- and middle-income countries and may be reduced through proper hand hygiene (HH) adherence during patient care. AIM: We produced and distributed alcohol-based hand rub (ABHR) to 19 public primary- and secondary-level healthcare facilities in Quetzaltenango, Guatemala, and carried out HH observations to assess healthcare workers' (HCWs) HH adherence, and to identify factors associated with this practice. HH adherence was defined as washing hands with soap and water or using ABHR. METHODS: Observations were conducted before (2021, baseline) and after (2022, follow-up) ABHR distribution to evaluate the evolution of HH practices over time. Bivariate comparisons and mixed-effects logistic regression models were used to explore associations between HH adherence and the following independent variables: healthcare facility level, type of contact performed, timing of HH performance, occupational category of HCW and materials present (e.g., water, soap, ABHR). FINDINGS: We observed 243 and 300 patient interactions among 67 and 82 HCWs at each time point, respectively. HH adherence was low for both observation periods (40% at baseline and 35% at follow-up). HCWs were more likely to adhere to HH during invasive contacts, after patient contact, and if the HCW was a physician. CONCLUSION: HH adherence varied by scenario, which underscores the importance of addressing multiple determinants of behaviour change to improve adherence. This requires interventions implemented with a multi-modal approach that includes both increasing access to HH materials and infrastructure, as well as HH education and training, monitoring and feedback, reminders, and promoting a HH safety culture.

Access to water, sanitation, and hygiene (WASH) resources in schools is critical for disease prevention and control, especially during public health emergencies. In Belize, systematic, national data on WASH in schools are needed to inform public health decisions and interventions. From December 2021 to January 2022, a national survey was sent electronically to government and government-aided primary and secondary schools in Belize (N = 308) to gather information on WASH services. From the survey, 12 pilot schools were selected based on the highest self-reported need for WASH resources to participate in additional evaluation and intervention, which included environmental nudges, supplemental supply provision, and hand hygiene education. To understand how the progression of the COVID-19 pandemic may have influenced hand hygiene, facility assessments to evaluate access to hand hygiene resources were conducted in person when most schools reopened for face-to-face learning during the pandemic (March 2022) and 15 months later (June 2023). Among the schools participating in the national survey (N = 221), 55% reported times when water was not available at the schools. Almost 9 in 10 schools (89%) had a functional handwashing station, and 47% reported always having soap for handwashing. Between baseline and follow-up at the 12 pilot schools, we observed decreases in the proportion of functional handwashing access points (-11%), functional handwashing access points accessible for individuals with disabilities (-17%) and small children (-29%), and functional alcohol-based hand rub dispensers (-13%). Despite the ongoing COVID-19 pandemic, we observed gaps in WASH resources in schools in Belize during the onsite assessments at the pilot schools. Schools should be encouraged and provided with WASH resources to maintain vigilance for disease control measures.

INTRODUCCIÓN: La higiene de manos (HM) es la principal medida para disminuir las IAAS, las que en las Unidades de Cuidados Intensivos (UCI) presentan una alta prevalencia. En Chile no existe información sobre el impacto de la estrategia multimodal de la OMS para la HM en adultos. El objetivo fue evaluar el impacto de la implementación de la estrategia en una UPC. METODOLOGÍA: Estudio longitudinal con evaluación pre y post-intervención, entre los años 2018 y 2021, en la UCI del Hospital del Trabajador (HT), Santiago, Chile. La implementación se evaluó con pautas de cumplimiento de HM, consumo de jabón y productos en base alcohólica (PBA). El impacto se midió con las tasas de neumonía asociada a ventilación mecánica (NAVM), infecciones del torrente sanguíneo asociadas a CVC (ITS- CVC) y del tracto urinario por CUP (ITU-CUP), y la incidencia anual de dermatitis. RESULTADOS: El cumplimiento de pautas aumentó de 91 a 96% (p < 0,05). El consumo total de productos para la HM aumentó de 0,17 a 0,31 L/día/cama y de PBA en 10%. Las tasas de IAAS pre y post-intervención fueron para NAVM de 10,3 y 8,4; ITS-CVC de 0,8 y 1,5 e ITU-CUP de 4,2 y 5,3 por 1.000 días de exposición. La incidencia anual de dermatitis disminuyó en 30% (p < 0,05). CONCLUSIONES: La implementación de la estrategia multimodal se asoció a una disminución de las tasas de NAVM y de dermatitis en la UCI del HT.
INTRODUCTION: Hand hygiene is the main measure to decrease infections related to healthcare and the Intensive Care Unit has a high prevalence. In Chile there aren't reports about the impact of the World Health Organization multimodal hand hygiene improvement strategy. AIM: To assess the implementation impact of this strategy at the ICU. METHODOLOGY: Longitudinal study with pre- and postintervention evaluation during the years 2018-2021 at ICU. The implementation was assessed against hand hygiene compliance guidelines, soap consumption and alcohol-based products. The impact was evaluated with the rates of ventilator-associated pneumonia (VAP), catheter related bloodstream infection (CRBSI) and catheter associated urinary tract infection (CAUTI) and the annual dermatitis incidence. RESULTS: The guidelines compliance increased from 91% to 96% (p < 0.05). The total product consumption increased from 0.17 to 0.31 Liters/day/bed. The use of alcohol-based products increased by 10%. HAI rates pre- and post-intervention were for VAP 10.3 and 8.4, CRBSI 0.8 and 1.5 and CAUTI 4.2 and 5.3. The annual dermatitis incidence decreased by 30.8% (p < 0.05). CONCLUSIONS: The strategy implementation benefited the decrease of VAP and the dermatitis prevention in ICU.

BACKGROUND: Hand hygiene (HH) is an important strategy for preventing health-care-associated infections (HAIs). Few programs focus on HH for family members and primary caregivers but fewer for patients. This study aimed to estimate the frequency with which hospitalized pediatric patients have hand contact with hospital surfaces. METHODS: We conducted a cross-sectional descriptive observational study consisting of three phases: the first was the creation of an observation and data collection tool, the second was the training of the monitors, and the third was the observational study of hand contact and HH opportunities in hospitalized pediatric patients. RESULTS: Over 3600 minutes of observation, 2032 HH opportunities were detected, averaging 33.8/h (SD 4.7) as determined by hand contact with hospital surfaces of hospitalized pediatric patients. In our study, infants and preschool children had the highest frequency of hand contact. CONCLUSION: The high frequency of hand contact of hospital surfaces by children suggests that hourly hand disinfection of patients and caregivers, objects and surfaces around the patients may be prevention measures that could be incorporated to reduce HAIs in pediatric hospitals.
INTRODUCCIÓN: La higiene de manos es una estrategia importante para la prevención de infecciones asociadas a la atención sanitaria. Existen pocos programas centrados en la higiene de manos para los familiares y cuidadores primarios, y aún menos para el paciente. El objetivo de este estudio fue cuantificar la frecuencia con la que los pacientes pediátricos hospitalizados tienen contacto manual con superficies hospitalarias. MÉTODOS: Se llevó a cabo un estudio observacional descriptivo transversal que constó de tres fases: la primera fue la creación de una herramienta de observación y registro de datos; la segunda fue la capacitación de los monitores y la tercera fue el estudio observacional del contacto manual y de las oportunidades de higiene de manos en pacientes pediátricos hospitalizados. RESULTADOS: Durante los 3600 minutos de observación, se detectaron 2032 oportunidades, con una media de 33.8 (DE 4.7) por hora de oportunidades de higiene de manos establecidas por contacto manual con superficies de pacientes pediátricos hospitalizados. Los lactantes y los niños en edad preescolar presentaron la mayor frecuencia de contacto manual. CONCLUSIONES: La alta frecuencia de contacto manual por parte del niño indica que medidas como la desinfección de las manos cada hora del paciente y del cuidador, así como de los objetos y superficies alrededor del paciente, podrían ser medidas útiles que deberían incluirse para prevenir las infecciones asociadas a la atención de la salud en los hospitales pediátricos.

Background and Objectives: to identify the perceptions of nursing professionals who worked during the covid-19 pandemic regarding Healthcare-Associated Infections (HAIs) and Hand Hygiene (HH), categorizing them by profession and region in Brazil. Method: An observational study was conducted from November 2020 to December 2021, involving 493 nursing professionals from all regions of Brazil. The Google Forms® platform, disseminated through social media was used. A questionnaire titled "Basic Questionnaire on Healthcare Professionals' Perception of Healthcare-Associated Infections and Hand Hygiene" was administered. The results were analyzed descriptively, presenting absolute and relative frequencies, divided by groups of nursing professionals (nurses, technicians, and assistants) and by regions of Brazil. Results: The results showed that 43.9% of nurses reported a significant impact of HAIs on the clinical progression of patients, whereas only 26.7% of nursing technicians and assistants shared this perception. Regarding HH, 50.8% of nurses considered a substantial effort necessary to perform it adequately, while 68.9% of nursing technicians and assistants agreed with this statement. Conclusion: most nursing professionals had a high perception of HAIs and HH, considering their profession and geographic region. These findings can contribute to the development of future strategies aimed at improving HH practices in nursing care, particularly during outbreaks of infectious diseases such as covid-19.(AU)
Justificativa e Objetivos: identificar as percepções dos profissionais de enfermagem que atuaram durante a pandemia de covid-19 em relação às Infecções Relacionadas à Assistência à Saúde (IRAS) e à Higienização das Mãos (HM), classificando-os por profissão e regiões brasileiras. Método: estudo observacional foi conduzido de novembro/2020 a dezembro/2021, com a participação de 493 profissionais de enfermagem de todas as regiões do Brasil. Utilizou-se o formulário do Google Forms®, divulgado em redes sociais. Foi aplicado um questionário intitulado "Questionário básico sobre a percepção de profissionais de saúde sobre infecções relacionadas à assistência à saúde e à higienização das mãos". Os resultados foram analisados de forma descritiva, apresentando frequências absolutas e relativas, divididos por grupos de profissionais de enfermagem (enfermeiros, técnicos e auxiliares) e por regiões do Brasil. Resultados: Os resultados mostraram que 43,9% dos enfermeiros relataram um impacto muito alto das IRAS na evolução clínica dos pacientes, enquanto apenas 26,7% dos auxiliares e técnicos de enfermagem compartilharam essa percepção. Em relação à HM, 50,8% dos enfermeiros consideraram que é necessário um grande esforço para realizá-la adequadamente, enquanto 68,9% dos auxiliares e técnicos de enfermagem concordaram com essa afirmação. Conclusão: a maioria dos profissionais de enfermagem apresentou uma alta percepção sobre HM e IRAS, levando em consideração a profissão e a região geográfica. Esses resultados podem contribuir para o desenvolvimento de estratégias futuras com o objetivo de aprimorar as práticas de HM na assistência de enfermagem, principalmente durante surtos de doenças infecciosas, como a covid-19.(AU)
Justificación y Objetivos: identificar las percepciones de los profesionales de enfermería que trabajaron durante la pandemia de COVID-19 en relación con las Infecciones Relacionadas con la Atención de la Salud (IRAS) y la Higiene de las Manos (HM), clasificándolos por profesión y región. Métodos: se llevó a cabo un estudio observacional desde noviembre/2020 hasta diciembre/2021, con la participación de 493 profesionales de enfermería de las 5 regiones de Brasil. El formulario de Google® fue difundido en redes sociales. Se aplicó un cuestionario: "Cuestionario básico sobre la percepción de los profesionales de la salud sobre infecciones relacionadas con la atención de la salud y la higiene de las manos". Los resultados se analizaron de manera descriptiva, presentando frecuencias absolutas y relativas, divididos por enfermeros, técnicos y auxiliares y por regiones. Resultados: 43,9% de los enfermeros informaron impacto muy alto de IRAS en la evolución de los pacientes, mientras que solo 26,7% de los auxiliares y técnicos compartieron esta percepción. En cuanto a la HM, 50,8% de los enfermeros consideraron que se requiere gran esfuerzo para llevarla a cabo adecuadamente, mientras que 68,9% de los auxiliares y técnicos de enfermería estuvieron de acuerdo con esta afirmación. Conclusión: la mayoría de los profesionales de enfermería tuvo una percepción alta sobre las IRAS y la HM, teniendo en cuenta la profesión y la región. Background and Objectives: hand hygiene (HH) is a crucial safety practice, but lack of knowledge can hinder compliance. The objective was to measure and assess the knowledge of nursing professionals who provided care during the COVID-19 pandemic regarding HH and to verify the association between HH knowledge and professional category and regions in Brazil. Methods: an observational study was conducted from November 2020 to December 2021 involving 493 nursing professionals from all regions of Brazil. Data collection was carried out using Google Forms® and social media platforms. The Hand Hygiene Knowledge Test for Healthcare Professionals was used, and results were analyzed descriptively. Pearson's chi-square test (x2) and Fisher's exact test were employed to assess associations. Results: among the participants, the majority (74.7%) had limited or subpar knowledge of HH. Nurses had a higher level of knowledge compared to nursing assistants and licensed practical nurses. Associations were found between professional category and correct answers regarding microorganism destruction time and the type of HH to be used. Conclusion: Brazilian nursing professionals had limited knowledge of HH, with nurses displaying a higher level of knowledge compared to nursing assistants and licensed practical nurse. Continuous education and guidance are necessary to improve HH practices among the nursing staff.(AU)
Justificativas e Objetivos: a higiene das mãos (HM) é uma prática crucial para a segurança, mas a falta de conhecimento pode prejudicar a adesão. Este estudo avaliou o conhecimento dos profissionais de enfermagem que prestaram cuidados durante a pandemia de Covid-19 em relação à HM e explorou a associação entre o conhecimento de HM, categoria profissional e regiões no Brasil. Métodos: um estudo observacional foi conduzido de novembro de 2020 a dezembro de 2021 envolvendo 493 profissionais de enfermagem de todas as regiões do Brasil. A coleta de dados foi realizada usando o Google Forms® e plataformas de mídia social. Foi utilizado o Teste de Conhecimento de Higiene das Mãos para Profissionais de Saúde, e os resultados foram analisados descritivamente. O teste qui-quadrado de Pearson (x2) e o teste exato de Fisher foram empregados para avaliar as associações. Resultados: entre os participantes, a maioria (74,7%) tinha conhecimento limitado ou insuficiente sobre HM. Os enfermeiros apresentaram um nível mais elevado de conhecimento em comparação com os auxiliares e técnicos de enfermagem. Foram encontradas associações entre a categoria profissional e respostas corretas sobre o tempo de destruição de microrganismos e o tipo de HM a ser utilizado. Conclusão: os profissionais de enfermagem brasileiros possuíam conhecimento limitado sobre HM, sendo que os enfermeiros apresentaram um nível mais elevado de conhecimento em comparação com os auxiliares e técnicos de enfermagem. Educação contínua e orientação são necessárias para melhorar as práticas de HM entre a equipe de enfermagem.(AU)
Justificación e Objetivos: la higiene de manos (HM) es una práctica de seguridad crucial, pero la falta de conocimiento puede dificultar el cumplimiento. Este estudio evaluó el conocimiento de los profesionales de enfermería que brindaron atención durante la pandemia de Covid-19 en relación con la HM y exploró la asociación entre el conocimiento de HM, la categoría profesional y las regiones en Brasil. Métodos: se realizó un estudio observacional desde noviembre de 2020 hasta diciembre de 2021 que incluyó a 493 profesionales de enfermería de todas las regiones de Brasil. La recopilación de datos se realizó utilizando Google Forms® y plataformas de redes sociales. Se utilizó el Test de Conocimiento de Higiene de Manos para Profesionales de la Salud, y los resultados se analizaron de manera descriptiva. Se emplearon la prueba chi-cuadrado de Pearson (x2) y la prueba exacta de Fisher para evaluar las asociaciones. Resultados: entre los participantes, la mayoría (74,7%) tenía conocimiento limitado o deficiente de HM. Las enfermeras tenían un nivel de conocimiento más alto en comparación con los auxiliares y técnicos de enfermería. Se encontraron asociaciones entre la categoría profesional y las respuestas correctas sobre el tiempo de destrucción de microorganismos y el tipo de HM que debía utilizarse. Conclusión: los profesionales de enfermería brasileños tenían un conocimiento limitado de HM, siendo que las enfermeras mostraban un nivel de conocimiento más alto en comparación con los auxiliares y técnicos de enfermería. Objective: To analyze hand hygiene (HH) adherence indicators among healthcare professionals in a medical day care (MDC) and present strategies used to encourage this practice. Method: A cross-sectional and descriptive study was conducted in an MDC located in Salvador, Bahia. Data collection consisted of document analysis and internal audit data on HH practice from 2016 to 2023. Results: HH adherence rates ranged from 39.4 to 81.4%, with an average of 63.9%. Among the evaluated healthcare professionals, nurses showed the highest HH adherence (74.3%), followed by nursing technicians (71.1%) and physicians (50%). Multimodal strategies to encourage HH in this service included HH training, with systematic evaluation con-ducted by nursing coordinators. Conclusion: The HH adherence rates presented are higher than those reported in the literature but below the standar-dized goal of 70% in the MDC studied, confirming that the implementation of this apparently simple practice is a complex, multi-causal issue that requi-res coordination between management policies as well as scientific knowledge in building a culture in favor of this practice in healthcare organizations. Objetivo: Analizar los indicadores de adherencia a la higiene de las manos (HM) entre profesionales de la salud de un hospital de día (HD) y pre-sentar las estrategias utilizadas para incentivar esa práctica. Método: Investigación transversal y descriptiva, realizada en un HD ubicado en Salvador, Bahía. La recolección de datos consistió en el análisis documental y de los datos de las auditorías internas de la práctica de HM entre 2016 y 2023. Resultados:Se identificaron porcentajes de adherencia a la HM que oscilaron entre el 39,4% y el 81,4%, con un promedio de 63,9%. De los profesionales de la salud evaluados, los enfermeros presentaron la mayor adherencia a la HM (74,3%), seguidos de los técnicos de enfermería (71,1%) y los médicos (50%). Las estrategias multimodales para estimular la HM adoptadas en este servicio incluyeron la capacitación en HM, con evaluación sistemática realizada por los coordinadores de enfermería. Conclusión: Los porcentajes de adherencia a la HM presentados aquí son superiores a los reportados en la literatura, pero están por debajo del objetivo del 70% estandarizado en el HD estudiado, lo que confirma que la implementación de esta práctica, aparentemente simple, es un tema complejo, multicausal y que requiere articulación entre las políticas de gestión, así como conocimiento científico en la construcción de una cultura a favor de esta práctica en las organizaciones de salud. Dos profissionais de saúde avaliados, os enfermeiros apresentaram a maior adesão à HM (74,3%), seguidos dos técnicos de Enfermagem (71,1%) e dos médicos (50%). As estratégias multimodais para incentivo à HM adotadas nesse serviço incluíram habilitação em HM, com avaliação sistematizada realizada pelas coordenações de Enfermagem. Conclusão: Os percentuais de adesão à HM apresenta-dos são maiores do que os reportados na literatura, mas abaixo da meta de 70% padronizada no HD estudado, ratificando que a implementação dessa prática, aparentemente simples, é tema complexo, multicausal e que requer articulação entre as políticas de gestão, bem como conhecimento científico na construção de uma cultura em prol dessa prática nas organizações de saúde. Abstract Background: Hand hygiene (HH) is an important strategy for preventing health-care-associated infections (HAIs). Few programs focus on HH for family members and primary caregivers but fewer for patients. This study aimed to estimate the frequency with which hospitalized pediatric patients have hand contact with hospital surfaces. Methods: We conducted a cross-sectional descriptive observational study consisting of three phases: the first was the creation of an observation and data collection tool, the second was the training of the monitors, and the third was the observational study of hand contact and HH opportunities in hospitalized pediatric patients. Results: Over 3600 minutes of observation, 2032 HH opportunities were detected, averaging 33.8/h (SD 4.7) as determined by hand contact with hospital surfaces of hospitalized pediatric patients. In our study, infants and preschool children had the highest frequency of hand contact. Conclusion: The high frequency of hand contact of hospital surfaces by children suggests that hourly hand disinfection of patients and caregivers, objects and surfaces around the patients may be prevention measures that could be incorporated to reduce HAIs in pediatric hospitals.
Resumen Introducción: La higiene de manos es una estrategia importante para la prevención de infecciones asociadas a la atención sanitaria. Existen pocos programas centrados en la higiene de manos para los familiares y cuidadores primarios, y aún menos para el paciente. El objetivo de este estudio fue cuantificar la frecuencia con la que los pacientes pediátricos hospitalizados tienen contacto manual con superficies hospitalarias. Métodos: Se llevó a cabo un estudio observacional descriptivo transversal que constó de tres fases: la primera fue la creación de una herramienta de observación y registro de datos; la segunda fue la capacitación de los monitores y la tercera fue el estudio observacional del contacto manual y de las oportunidades de higiene de manos en pacientes pediátricos hospitalizados. Resultados: Durante los 3600 minutos de observación, se detectaron 2032 oportunidades, con una media de 33.8 (DE 4.7) por hora de oportunidades de higiene de manos establecidas por contacto manual con superficies de pacientes pediátricos hospitalizados. Los lactantes y los niños en edad preescolar presentaron la mayor frecuencia de contacto manual. Conclusiones: La alta frecuencia de contacto manual por parte del niño indica que medidas como la desinfección de las manos cada hora del paciente y del cuidador, así como de los objetos y superficies alrededor del paciente, podrían ser medidas útiles que deberían incluirse para prevenir las infecciones asociadas a la atención de la salud en los hospitales pediátricos.
RESUMEN
BACKGROUND: Healthcare-associated infections are among the most common complications during hospitalization. These infections increase morbidity and mortality and they increase length of hospital stay and the cost of healthcare. The aims of our study were to monitor hand hygiene (HH) compliance, HH technique quality and factors related to HH practice among health professionals in a COVID-19 Intensive Care Unit (ICU). METHODS: An observational, prospective study. Between September and December 2021, we observed 69 healthcare professionals in an eight-bed ICU for patients with COVID-19 in midwestern Brazil. We used the WHO observation form to collect data. The dependent variable was HH compliance and independent variables were professional category, sex, HH quality (3-step technique for at least 15 s), number of HH opportunities observed, observation shift and inappropriate glove use. RESULTS: We observed 1185 HH opportunities. The overall compliance rate was 26.4%, but only 6.5% were performed with the correct 3-step technique for the minimum time. HH compliance was considerably lower for moments "before" tasks (6.7%; 95% CI 4.8%, 9.2%) compared with moments "after" tasks (43.8%; 95% CI 39.9%, 47.8%). The logistic model found that inappropriate glove use, night shift and physicians (p < 0.001) were associated with low HH compliance. The infrastructure analysis found that the unit had an insufficient number of alcohol-based handrub (ABHR) dispensers at the point of care and that the mechanism for activating them was poorly designed. CONCLUSIONS: HH compliance was very low. Inappropriate glove use was associated with low compliance and the unit's infrastructure did not support good HH practice. The fact that healthcare professionals were more likely to do HH after tasks, suggests that they use HH to protect themselves rather than the patients. Adequate infrastructure and ongoing health education with a focus on HH while caring for patients in contact precautions are essential for improving HH compliance and patient safety.

Resumo Objetivo Investigar as opiniões e atitudes dos estudantes de enfermagem no papel de pacientes ou familiares de pacientes, a respeito do comportamento de higiene das mãos dos profissionais de saúde e da participação dos pacientes na campanha de higiene das mãos. Métodos Estudo transversal prospectivo realizado entre 2021-2022 no Nursing Department, Faculty of Health Sciences, de duas universidades turcas. A amostra do estudo foi composta por 330 alunos. Os dados foram coletados por meio de questionário autoaplicável. A taxa de resposta do questionário foi de 89,43%. O teste qui-quadrado foi utilizado na análise dos dados. Resultados A média de idade dos estudantes foi de 19,80±1,30 anos, 76,1% eram do sexo feminino, 50,9% afirmaram ter recebido instrução sobre Infecções Associadas aos Cuidados de Saúde (IACS). Enquanto 30,1% dos estudantes relataram realizar a higiene das mãos "9 a 11 vezes" em sua vida diária, 54,6% relataram "12 a 15 vezes" no hospital, e 96,4% dos estudantes se perguntaram se os profissionais de saúde realizavam a higiene das mãos antes de fornecer cuidados durante as internações. De acordo com 30,5% dos estudantes, lembretes dos pacientes e seus familiares sobre a realização da higiene das mãos antes do contato com os pacientes os deixariam satisfeitos. Houve diferença estatisticamente significativa entre a instrução anterior dos estudantes sobre IACS e a higiene das mãos como cuidado importante a pacientes hospitalizados (p<0,05). Conclusão Os estudantes de enfermagem apresentaram conhecimento suficiente sobre a higiene das mãos e uma atitude positiva frente aos comportamentos de higiene das mãos dos profissionais de saúde. Estudantes de enfermagem como pacientes e familiares dos pacientes podem ser incluídos nas campanhas de higiene das mãos dos profissionais de saúde, desde que as etapas do programa sejam bem planejadas.

Puede incluirse a los estudiantes de enfermería como pacientes y familiares de los pacientes en las campañas de higiene de manos de los profesionales de la salud, siempre que las etapas del programa estén bien planificadas.
Abstract Objective To investigate the views and attitudes of nursing students, as patients or relatives, on healthcare professionals' hand hygiene behavior and patient participation hand hygiene campaign. Methods This prospective cross-sectional study was conducted in the nursing departments of the health and science faculties at two Turkish universities between 2021-2022. The study sample comprised 330 students. Data were collected using a self-administered questionnaire. The response rate of the questionnaire was 89.43%. Chi-square test was used in data analysis. Results The mean age of students was 19.80±1.30 years, 76.1% were female, 50.9% stated they had received education regarding healthcare-associated infections (HAI). While 30.1% of students reported they performed hand hygiene "9-11 times" in their daily lives, 54.6% reported performing "12-15 times" in the hospital, and 96.4% of students expressed wondering if healthcare professionals performed hand hygiene before offering care during hospitalizations. Among students, 30.5% stated that reminders from patients and their relatives about performing hand hygiene before contact with patients would make them happy. There was a statistically significant difference between students' previous training in HAIs and hand hygiene as an important inpatient care (p<0.05). Conclusion Nursing students had sufficient knowledge of hand hygiene and a positive attitude towards hand hygiene behaviors of healthcare professionals. Nursing students, such as patients and their relatives, can be included in hand hygiene campaigns for healthcare professionals, provided that the program steps are well planned.

Resumo Objetivo Verificar a adesão e a qualidade da técnica de higiene das mãos realizada por profissionais de saúde nos momentos preconizados e identificar os fatores impactantes. Métodos Estudo observacional, com 41 profissionais de Unidade de Terapia Intensiva Adulto de um hospital de ensino brasileiro. A coleta de dados foi de setembro a dezembro de 2021, utilizando questionário sociodemográfico e formulário de observação da Organização Mundial da Saúde. Foram aplicados testes estatísticos de análise binomial e modelo de regressão logística de efeitos mistos e adotou-se nível de significância de 0,05. Resultados Um total de 1.055 oportunidades de Higiene das Mãos foram observadas, a taxa de adesão foi de 23,98% e pode-se verificar que os profissionais não executaram a técnica preconizada. O teste binomial verificou maior adesão aos momentos "após" quando comparados aos momentos "antes" (p< 0,001) e foi constatado impacto negativo do uso de luvas (p< 0,001). O modelo logístico reforçou a maior adesão nos momentos "após". Ainda que a adesão esteja baixa entre todas as categorias profissionais, o modelo logístico demonstrou mais chances de higiene das mãos pelos enfermeiros. Conclusão A adesão à higiene das mãos foi consideravelmente baixa principalmente considerando a técnica adequada. Foi verificado impacto negativo do uso de luvas na adesão higiene das mãos. Os profissionais demonstraram maiores chances de aderirem a higiene das mãos nos momentos "após" quando comparados aos momentos "antes".
Resumen Objetivo Verificar la adhesión y la calidad de la técnica de higiene de manos realizada por profesionales de la salud en los momentos recomendados e identificar los factores impactantes. Métodos Estudio observacional con 41 profesionales de una unidad de cuidados intensivos adulta de un hospital universitario brasileño. La recopilación de datos se realizó de septiembre a diciembre de 2021, mediante un cuestionario sociodemográfico y un formulario de observación de la Organización Mundial de la Salud. Se aplicaron pruebas estadísticas de análisis binominal y el modelo de regresión logística de efectos mixtos, y se adoptó un nivel de significación de 0,05. Resultados Se observó un total de 1.055 oportunidades de higiene de manos. El índice de adhesión fue del 23,98 % y se pudo verificar que los profesionales no ejecutan la técnica recomendada. La prueba binominal verificó una mayor adhesión en los momentos "después" comparado con los momentos "antes" (p<0,001) y se constató un impacto negativo del uso de guantes (p<0,001). El modelo logístico reforzó la mayor adhesión en los momentos "después". Aunque la adhesión sea baja en todas las categorías profesionales, el modelo logístico demostró mayor probabilidad de higiene de manos por parte de los enfermeros. Conclusión La adhesión a la higiene de manos fue considerablemente baja, sobre todo si se considera la técnica adecuada. Se verificó un impacto negativo del uso de guantes en la adhesión a la higiene de manos. Los profesionales demostraron mayores probabilidades de adhesión a la higiene de manos en los momentos "después" comparado con los momentos "antes".
Abstract Objective To verify adherence and quality of hand hygiene techniques performed by health professionals at recommended times and identify impacting factors. Methods This is an observational study, with 41 professionals from the Adult Intensive Care Unit of a Brazilian teaching hospital. Data collection took place from September to December 2021, using a sociodemographic questionnaire and observation form from the World Health Organization. Statistical tests of binomial analysis and mixed-effects logistic regression model were applied, and a significance level of 0.05 was adopted. Results A total of 1,055 hand hygiene opportunities were observed; the adherence rate was 23.98%; and it can be seen that professionals did not perform the recommended technique. The binomial test verified greater adherence to moments "after" when compared to moments "before" (p< 0.001) and a negative impact of using gloves was found (p< 0.001). The logistical model reinforced greater adherence in moments "after". Even though adherence is low among all professional categories, the logistical model demonstrated more chances of hand hygiene by nurses. Conclusion Adherence to hand hygiene was considerably low, especially considering the appropriate technique. A negative impact of using gloves on hand hygiene adherence was verified. Professionals demonstrated greater chances of adhering to hand hygiene in the moments "after" when compared to moments "before".

Abstract Objective: to investigate factors associated with digital health literacy, hand hygiene knowledge and behavior among Brazilian adolescents during the COVID-19 pandemic. Method: cross-sectional study with 473 adolescents aged 15 to 19. Data were collected virtually between June and August 2021 through a questionnaire on sociodemographic characteristics, digital health literacy, knowledge and behavior regarding hand hygiene. Variables were adjusted using multiple linear regression models with normal response. Results: the average digital health literacy score was 29.89 ±5.30; for hand hygiene knowledge and behavior was 13.1 ±1.5 and 11.1±2.7, respectively. Higher knowledge and behavior scores for hand hygiene were associated with having completed high school, compared to those attending college, among those who attend health courses, seek information about the pandemic and in scientific articles. Higher digital health literacy scores in adolescents who did not wear masks when playing sports (p= 0.017). There was an association between digital health literacy and knowledge (p = 0.000) and behavior (p = 0.000) regarding hand hygiene. Conclusion: there is an association between higher digital health literacy scores and hand hygiene knowledge and behavior.

Os dados foram obtidos virtualmente, entre junho e agosto de 2021, por meio de questionário sobre características sociodemográficas, letramento digital em saúde, conhecimento e comportamento para higienização das mãos. Variáveis foram ajustadas por modelos de regressão linear múltipla com resposta normal. Resultados: a pontuação média de letramento digital em saúde foi de 29,89 ±5,30. Para o conhecimento e comportamento na higienização das mãos foi de 13,1 ±1,5 e 11,1±2,7, respectivamente. Maiores pontuações de conhecimento e comportamento na higienização das mãos se associaram ao ensino médio finalizado, isto se comparados aos que cursam faculdade, aos que frequentam curso em saúde, aos que buscam informações sobre a pandemia e em artigos científicos. Além disso, houve maiores pontuações do letramento digital em saúde em adolescentes que não utilizavam máscaras quando praticavam esporte (p= 0,017). Houve associação entre letramento digital em saúde e conhecimento (p = 0,000) e comportamento (p = 0,000) para higienização das mãos. Conclusão: existe associação entre maiores escores de letramento digital em saúde e o conhecimento e comportamento para a higienização das mãos.

Introducción. El trabajo de cuidados ha sido el núcleo fundamental de análisis y objeto de estudio tradicional de la Enfermería. Las investigaciones que durante mucho tiempo se han realizado se han centrado mayoritariamente en la producción de cuidados desde la mirada cientificista y hegemónica de la ciencia una ciencia de la enfermería llena de conocimientos técnicos y mecánicos desde una visión parcial de estos, una visión eurocentrista y anglosajona con "valor" académico, dejando al margen los trabajos que se realizan fuera de estos ámbitos y que no están plagados de números y cosas "tangibles". Metodología. Se trata de una investigación histórico cultural, que pretende deconstruir y cristalizar el conocimiento femenino en torno a la higiene y al lavado de manos, por medio de la transdisciplinariedad, haciendo uso de la heurística y la hermenéutica analógica. Estudiar estos fenómenos como sustento de la la historia del cuidado en la sociedad mexicana gracias al pueblo nahua, es una fuente de información que robustece el cuerpo histórico académico de la profesión de enfermería en México y Latinoamérica Conclusiones. La participación de las mujeres en la educación y en otros ámbitos ha tenido un realce inconmensurable, equiparable con el conocimiento hegemónico proveniente de Europa, las mujeres histórica y universalmente son quienes se han encargado de ilustrar y enseñar desde el seno del hogar las normas básicas de convivencia y civilidad, y aunque ha sido un papel de cierta forma impuesto, no se debe dejar de lado que el conocimiento empírico de todas estas mujeres es valioso y digno de darse a conocer
Introduction. The work of care has been the fundamental nucleus of analysis and traditional object of study of nursing. The research that has been carried out for a long time has focused mainly on the production of care from the scientificist and hegemonic viewpoint of science, a nursing science full of technical and mechanical knowledge from a partial view of this, a Eurocentric and Anglo-Saxon view with academic "value", leaving aside the work that is carried out outside these fields and which is not full of numbers and "tangible" things. Methodology. This is cultural-historical research, which aims to deconstruct and crystallize feminine knowledge around hygiene and hand washing, by means of transdisciplinarity, making use of heuristics and analogical hermeneutics. Findings. Nahua women in Ancient Mexico, says Clementina Battcock in the Seminar on the History of Women in Ancient Mexico (Battcock, 2018, 2021), that the history of women in Ancient Mexico is a silenced history, "bajita", for which historians and historians have been building a deep debate on the contributions of women. BACKGROUND: Hand hygiene (HH) is an important practice that prevents transmission of infectious diseases, such as COVID-19. However, in resource-limited areas, where water and soap are not always available, it can be difficult to practice HH correctly and at appropriate moments. The purpose of this study was to assess HH knowledge and behaviors among students from six elementary schools in Quetzaltenango, Guatemala to identify gaps that could later inform interventions to improve HH. METHODS: We conducted knowledge, attitude, and practices (KAP) surveys among primary school students during the COVID-19 pandemic in July 2022. We also observed students' HH practices at three different moments during the day, making note of the use of the HH station and materials, duration of handwashing, presence of a HH assistant, and the students' sex. We also used the Quantitative Personal Hygiene Assessment Tool (qPHAT), to measure hand dirtiness before eating, after restroom use, and upon arriving to school. RESULTS: We surveyed 109 students across six schools. Mean scores were 4 out of 5 for knowledge, 8 out of 8 for attitudes, and 6 out of 7 for HH practices. Most students identified "before eating" as a critical moment for HH (68.8%), fewer identified "after restroom use" (31.2%), and no students mentioned HH being necessary "after coughing or sneezing". We observed 326 HH opportunities of which 51.2% performed correct HH (used water and soap for at least 20 s or used alcohol-based hand rub, where materials were available). We collected 82 qPHAT hand swabs. A Kruskal Wallis test revealed a significant difference in hand dirtiness between entering the school and after restroom use (p = 0.017), but no significant difference before eating and after entering the school (p = 0.6988). CONCLUSIONS: The results from the KAP survey show high scores, however correct identification of key moments for HH was relatively uncommon, especially after restroom use and after coughing or sneezing. Additionally, half of HH opportunities observed had correct HH practices and on average, hands were dirtiest when arriving at school. These findings will inform interventions to improve HH practices and behaviors, which will be evaluated with follow-up data collection.

Water, sanitation, and hygiene (WASH) services in schools are essential to reduce infectious disease transmission, including that of COVID-19. This study aimed to establish a baseline of WASH services in six public elementary schools in Guatemala, with a focus on hand hygiene. We used the WHO/UNICEF Joint Monitoring Programme (JMP) report indicators to assess the WASH infrastructure at each school. We collected water samples from easily accessible water points (pilas, or bathroom sinks) at each school to test for the presence of total coliforms and E. coli. In-depth interviews were carried out with teachers to understand hand hygiene practices and systems at school. Results indicate that all schools had water available at the time of the survey. All water samples at four schools tested positive for total coliforms and at one school, positive for E. coli. All schools had sanitation facilities, but services were limited. Only 43% of handwashing stations at schools had soap available. No school had disability-inclusive WASH services. Financial constraints and a lack of appropriate WASH infrastructure were the main barriers reported by teachers to meet hand hygiene needs at school. Appropriate access to WASH infrastructure and supplies could increase hand hygiene practices and improve learning conditions for students.
