Background: Mass vaccination is a cornerstone of public health emergency preparedness and response. However, injudicious placement of vaccination sites can lead to the formation of long waiting lines or queues, which discourages individuals from waiting to be vaccinated and may thus jeopardize the achievement of public health targets. Queueing theory offers a framework for modeling queue formation at vaccination sites and its effect on vaccine uptake. Methods: We developed an algorithm that integrates queueing theory within a spatial optimization framework to optimize the placement of mass vaccination sites. The algorithm was built and tested using data from a mass canine rabies vaccination campaign in Arequipa, Peru. We compared expected vaccination coverage and losses from queueing (i.e., attrition) for sites optimized with our queue-conscious algorithm to those obtained from a queue-naive version of the same algorithm. Results: Sites placed by the queue-conscious algorithm resulted in 9-19% less attrition and 1-2% higher vaccination coverage compared to sites placed by the queue-naïve algorithm. Compared to the queue-naïve algorithm, the queue-conscious algorithm favored placing more sites in densely populated areas to offset high arrival volumes, thereby reducing losses due to excessive queueing. These results were not sensitive to misspecification of queueing parameters or relaxation of the constant arrival rate assumption. Conclusion: One should consider losses from queueing to optimally place mass vaccination sites, even when empirically derived queueing parameters are not available. Due to the negative impacts of excessive wait times on participant satisfaction, reducing queueing attrition is also expected to yield downstream benefits and improve vaccination coverage in subsequent mass vaccination campaigns.

BACKGROUND: Case-control studies involving test-negative (TN) and syndrome-negative (SN) controls are reliable for evaluating influenza and rotavirus vaccine effectiveness (VE) during a random vaccination process. However, there is no empirical evidence regarding the impact in real-world mass vaccination campaigns against SARS-CoV-2 using TN and SN controls. OBJECTIVE: To compare in the same population the effectiveness of SARS-CoV-2 vaccination on COVID-19-related hospitalization rates across a cohort design, TN and SN designs. METHOD: We conducted an unmatched population-based cohort, TN and SN case-control designs linking data from four data sources (public primary healthcare system, hospitalization registers, epidemiological surveillance systems and the national immunization program) in a Chilean municipality (Rancagua) between March 1, 2021 and August 31, 2021. The outcome was COVID-19-related hospitalization. To ensure sufficient sample size in the unexposed group, completion of follow-up in the cohort design, and sufficient time between vaccination and hospitalization in the case-control design, VE was estimated comparing 8-week periods for each individual. RESULTS: Among the 191,505 individuals registered in the primary healthcare system of Rancagua in Chile on March 1, 2021; 116,453 met the cohort study's inclusion criteria. Of the 9,471 hospitalizations registered during the study period in the same place, 526 were COVID-19 cases, 108 were TN controls, and 1,628 were SN controls. For any vaccine product, the age- and sex-adjusted vaccine effectiveness comparing fully and nonvaccinated individuals was 67.2 (55.7-76.3) in the cohort design, whereas it was 67.8 (44.1-81.4) and 77.9 (70.2-83.8) in the TN and SN control designs, respectively. CONCLUSION: The VE of a COVID-19 vaccination program based on age and risk groups tended to differ across the three observational study designs. The SN case-control design may be an efficient option for evaluating COVID-19 VE in real-world settings.

Background and Objectives: Since its appearance in 2019, multiple risk factors have been identified for presenting a severe form of COVID-19 and different vaccines have also been developed to prevent severe manifestations. However, despite a vaccination history, some cases progress to complications or even death. The objective of this study was to determine the strength of the association between the severity of COVID-19 and the history of vaccination in patients treated at a public reference hospital in Mexico City. Methods: This was a non-experimental, retrospective, and analytical epidemiological study of cases and controls. The study population was people treated at a concentration hospital for COVID-19 care between July 1, 2021, and June 30, 2022, in Mexico City. Results: 132 participants (44 cases and 88 controls) were included in the study. The risk factors most strongly associated with COVID-19 severity were age greater than or equal to 60 years, presenting 22 breaths per minute at the first medical evaluation, systolic blood pressure greater than or equal to 140 millimeters of mercury, and a history of at least one chronic comorbidity. However, vaccination history was associated with 94% (OR 0.06) lower odds of developing severe COVID-19 compared to those without a history of vaccination, regardless of the presence of associated risk factors. Conclusion: Lacking a history of vaccination and presenting any of the identified risk factors confer higher odds of developing severe forms of the disease.(AU)


Justificativa e Objetivos: Desde o seu aparecimento em 2019, foram identificados múltiplos fatores de risco para a apresentação de uma forma grave de COVID-19 e foram desenvolvidas diferentes vacinas para prevenir o aparecimento de manifestações graves. No entanto, apesar de um histórico de vacinação, alguns casos podem evoluir para complicações ou mesmo para a morte. O objetivo deste estudo foi determinar a força de associação entre a gravidade da COVID-19 e o histórico de vacinação em pacientes atendidos em um hospital público de referência na Cidade do México. Métodos: Estudo epidemiológico não-experimental, retrospectivo e analítico, de casos e controles. A população do estudo foram indivíduos atendidos em um hospital de concentração para atendimento à COVID-19 entre 1 de julho de 2021 e 30 de junho de 2022, na Cidade do México. Resultados: 132 participantes (44 casos e 88 controles) foram incluídos no estudo. Os fatores de risco mais fortemente associados à gravidade da COVID-19 foram idade superior ou igual a 60 anos, apresentar 22 respirações por minuto na primeira avaliação médica, pressão arterial sistólica superior ou igual a 140 milímetros de mercúrio e histórico de pelo menos uma comorbidade crônica. No entanto, histórico de vacinação foi associado a uma probabilidade 94% (OR 0,06) menor de desenvolver COVID-19 grave em comparação com aqueles sem histórico de vacinação, independentemente da presença de fatores de risco associados. Conclusão: A ausência de histórico de vacinação e a presença de algum dos fatores de risco identificados conferem maiores probabilidades de desenvolver formas graves da doença.(AU)


Justificación y Objetivos: Desde su aparición en 2019, se han identificado múltiples factores de riesgo para presentar una forma grave de COVID-19 y también se han desarrollado distintas vacunas que previenen la aparición de manifestaciones de gravedad. Sin embargo, a pesar del antecedente de vacunación, algunos casos se complican o incluso fallecen. El objetivo del este estudio fue determinar la fuerza de asociación entre la gravedad de la COVID-19 con el antecedente de vacunación en pacientes atendidos en un hospital público de referencia de la Ciudad de México. Métodos: Estudio epidemiológico no experimental, retrospectivo y analítico, de casos y controles. La población de estudio fueron personas atendidas en un hospital de concentración para la atención de COVID-19 entre el 1 de julio de 2021 y el 30 de junio de 2022 en la Ciudad de México. Resultados: 132 participantes (44 casos y 88 controles) fueron incluidos en el estudio. Los factores de riesgo más fuertemente asociados con la gravedad de la COVID-19 fueron la edad mayor o igual a 60 años, presentar 22 respiraciones por minuto en la primera valoración médica, tensión arterial sistólica mayor o igual a 140 milímetros de mercurio y el antecedente de al menos una comorbilidad crónica. No obstante, el antecedente de vacunación se asoció con 94% (RM 0.06) menos posibilidades de desarrollar COVID-19 grave con respecto a aquellos sin antecedente vacunal, independientemente de la presencia de los factores de riesgo asociados. Conclusión: carecer del antecedente de vacunación y presentar alguno de los factores de riesgo identificados confieren las mayores posibilidades de presentar formas graves de la enfermedad.(AU)

Background: Coronavirus SARS-CoV-2 has been the cause of multiple hospitalizations due to respiratory distress, pneumonia, multi-organic failure and death in all the world. The BioNTech/Pfizer lab was the first to get the approval from the Food and Drug Administration (FDA) for vaccine application, beginning with health workers. However, there are a few studies about adverse effects after applying the COVID-19 vaccine. Objective: To assess the presence and time of onset of adverse effects produced by the COVID-19 vaccine. Material and methods: A retrospective cohort study was carried out, including medical residents from the Hospital de Especialidades No. 1 (Specialties Hospital No. 1) from the Bajío National Medical Center, who received two doses of Pfizer-BioNTech vaccine and decided to participate voluntarily. We obtained the information by a self-assessment tool sent via email. All adverse effects were collected emphasizing those described in the literature. Results: A total of 101 medical residents were included. It was observed that 55.54% presented adverse effects after the first dose, which appeared in the first 24 hours after the vaccine was administered. After the second dose, 54.5% presented symptomatology, 41.6% during first 24 hours after the vaccine was administered. Conclusions: It was observed symptomatology after applying the complete series (two doses) of BioNTech/Pfizer; however, there were mild and of short duration.


Introducción: el coronavirus SARS-CoV-2 ha sido causa de múltiples hospitalizaciones por distrés respiratorio, neumonía, falla multiorgánica y fallecimientos en todo el mundo. El laboratorio de BioNTech/Pfizer fue el primero en conseguir aprobación por la Food and Drug Administration (FDA) para la aplicación a gran escala de su prototipo de vacuna, comenzando la aplicación en el sector salud. Sin embargo, existen pocos estudios sobre los efectos adversos tras la aplicación de dicha vacuna. Objetivo: evaluar la presencia y el tiempo de inicio de efectos adversos producidos por la vacuna contra COVID-19. Material y métodos: se realizó un estudio de cohorte retrospectivo, se incluyeron médicos residentes adscritos al Hospital de Especialidades No. 1 del Centro Médico Nacional del Bajío, quienes recibieron dos dosis de la vacuna BioNTech/Pfizer y que decidieron participar voluntariamente. Se recabó la información mediante un instrumento de evaluación autoaplicable vía electrónica. Se recolectaron todos los eventos adversos y se hizo énfasis en los descritos en la literatura. Resultados: se incluyeron un total de 101 médicos residentes. Se observó que el 55.4% presentó efectos secundarios después de la primera dosis, mismos que aparecieron en las primeras 24 horas tras la aplicación. Posterior a la aplicación de la segunda dosis, el 54.5% presentó sintomatología, el 41.6% en las primeras 24 horas posterior a la aplicación. Conclusiones: se observó la aparición de sintomatología posterior a la aplicación del esquema de vacunación con BioNTech/Pfizer; sin embargo, se trató de síntomas leves de corta duración.

BACKGROUND: Despite the unequivocal benefits of vaccination, vaccine coverage has been falling in several countries in the past few years. Studies suggest that vaccine hesitancy is an increasingly significant phenomenon affecting adherence to vaccines. More recently, during the COVID-19 pandemic, political views have emerged as an additional influencing factor for vaccine hesitancy. METHODS: In this ecologic study, we used information from publicly available databases to investigate the association between political ideology, depicted by the percentage of votes for the right-wing candidate Jair Bolsonaro in the presidential elections of 2018 and 2022, and COVID-19 vaccination in Brazilian municipalities. The primary endpoint was the COVID-19 vaccination index, calculated as the number of COVID-19 vaccine doses administered up to September 2022 divided by the number of inhabitants in each municipality. The analysis was conducted using Pearson correlation coefficients and linear regression models adjusted for HDI, the percentage of male voters, the percentage of voters who were older than 50 years old, and the percentage of voters with a middle school education or less. In addition, we explored whether the effect of the percentage of Bolsonaro voters on the COVID-19 vaccination index was modified in different quartiles of HDI using an interaction term. RESULTS: Five thousand five hundred sixty-three Brazilian municipalities were included in the analysis. For both the 2018 and 2022 elections, the percentage of votes for Jair Bolsonaro was significantly and inversely associated with COVID-19 vaccine uptake after adjustment for the sociodemographic characteristics of the voters (change in mean vaccination index in 2018 for each 1% increase in Bolsonaro voters -0.11, 95% confidence interval [CI] -0.13 to -0.08, p < 0.001; change in mean vaccination index in 2022 for each 1% increase in Bolsonaro voters -0.09, 95% CI -0.11 to -0.07, p < 0.001). We also found a statistically significant interaction between the primary predictor of interest and HDI scores, with a more significantly detrimental effect of the right-wing political stance in municipalities in the lower HDI quartiles (interaction p < 0.001 for the first HDI quartile; p = 0.001 for the second HDI quartile). CONCLUSION: Our findings suggest that political ideologies have influenced COVID-19 vaccine hesitancy in Brazilian municipalities, affecting communities inequitably. The politicization of vaccines is a new challenge for vaccine programs. Strategies to face these challenges should include joint efforts from governments and civil society for a common public health goal.

Este estudo teve como objetivo analisar o número de casos e óbitos por Covid-19 em relação a gênero e idade, quantidade total de vacinas aplicadas e relação entre a aplicação das vacinas e número de mortes, de janeiro a novembro de 2021, na cidade e região de Viçosa, Minas Gerais. Os dados foram fornecidos pelo Setor de Vigilância Epidemiológica do município e pelos boletins diários da prefeitura. Em média, houve mais de mil casos de infectados/mês de janeiro a julho, a maior parte adultos do gênero feminino. A mortalidade também foi maior entre as mulheres. Foi registrada queda do número de casos e de mortes coincidente com a aplicação da primeira dose de vacina em pessoas sem comorbidades e com início de aplicação da segunda dose nos grupos prioritários. Tal achado indica a importância da imunização coletiva no controle da pandemia.


This study aimed to analyze the number of cases and deaths from Covid-19 in relation to gender and age, the total vaccines applied, and the relationship between the vaccine application and the number of deaths between January and November 2021, in the municipality and region of Viçosa, state of Minas Gerais. Data were provided by the Epidemiological Surveillance Sector of the municipality of Viçosa and by the Daily Bulletins, published by the city hall. On average, there were more than 1,000 cases per month from January to July, mostly female adults. Mortality was also higher for females. There was a drop in the number of cases and deaths, coincident with the application of the first dose of vaccine in people without comorbidities and the initial period of application of the second dose in priority groups, indicating the importance of collective immunization to control the pandemic.

Las vacunas SOBERANA®02 y SOBERANA® Plus contra el coronavirus tipo 2 causante del síndrome respiratorio agudo severo, recibieron autorización de uso en emergencia por la autoridad reguladora de Cuba, y de inmediato aconteció una campaña de vacunación masiva en población pediátrica, lo que devino en una inminente movilización de centros de vacunación y vigilancia de sus eventos adversos. El Centro de Inmunología Molecular realizó un estudio de farmacovigilancia intensiva que evalúo el cumplimiento del esquema heterólogo con ambas vacunas, su seguridad y la incidencia de casos positivos a COVID-19 en niños y adolescentes después de completar el esquema de inmunización. Desde el 15 de septiembre al 31 de diciembre del 2021, participaron 529 sujetos entre 2 y 18 años de edad, de ambos sexos, sin antecedentes de infección por coronavirus tipo 2 del síndrome respiratorio agudo severo, procedentes de 35 municipios y 12 provincias cubanas, quienes recibieron vacuna SOBERANA®02 (dos dosis) y SOBERANA®Plus (una dosis). Se realizó vigilancia de eventos adversos hasta 30 días después de la última dosis recibida. Se consultó la plataforma informática nacional Higia Andariego para identificar los casos positivos al virus del síndrome respiratorio agudo severo coronavirus 2, hasta 3 meses de haber completado la vacunación. El 98,5 por ciento de los participantes completó el esquema de vacunación y en el 6,6 por ciento se notificó algún evento adverso con relación consistente a la vacunación. Predominaron las reacciones locales (dolor, eritema, inflamación), sobre las reacciones sistémicas (fatiga y febrícula), de intensidad ligera o moderada. Se logró un elevado cumplimiento del esquema de inmunización, con un perfil de seguridad favorable, los sujetos con esquema completo de inmunización no enfermaron de COVID-19(AU)


The severe acute respiratory syndrome coronavirus 2 vaccines, SOBERANA®02 and SOBERANA®Plus, received authorization for emergency use by the Cuban regulatory authority; a massive vaccination campaign was immediately launched in the pediatric population, which led to an imminent mobilization of vaccination centers and surveillance of adverse events. The Molecular Immunology Center conducted an intensive pharmacovigilance study to evaluate compliance of the heterologous scheme with both vaccines, their safety, and the incidence of COVID-19 positive cases in children and adolescents after completing the immunization schedule. From September 15 to December 31, 2021, a total of 529 subjects between 2 and 18 years of age, of both sexes, without a history of infection by severe acute respiratory syndrome coronavirus type 2, from 35 municipalities and 12 Cuban provinces, who received SOBERANA®02 (two doses) and SOBERANA®Plus (one dose) vaccines, were included in the study. Surveillance for adverse events was performed up to 30 days after the last dose received. The national computer platform Higia Andariego was consulted to identify positive cases for severe acute respiratory syndrome coronavirus 2 up to 3 months after completing vaccination. According to the report, 98.5percent of the participants completed the vaccination schedule and 6.6percent of them reported some adverse event consistently related to vaccination. Local reactions (pain, erythema, inflammation) prevailed over systemic reactions (fatigue and fever), of light or moderate intensity. High compliance with the immunization schedule was achieved, with a favorable safety profile; subjects with a complete immunization schedule did not become ill with COVID-19(AU)

Introduction: Social stigmas in people living with Human Immunodeficiency Virus (HIV) have been part of their history and can still be identified in today's care environments. Objective: To analyze the social stigmas in relation to the care of people living with HIV by those who provide the anti Sars-CoV-2 vaccination service. Materials and methods: A descriptive quantitative research were developed that included a census sample of 126 people. The scale of social stigma in the care of people with HIV/AIDS (Acquired immunodeficiency syndrome) was applied. The information was analyzed through SPSS version 25. Results: Most of the participants reported not knowing people with HIV, but they have been treated as users (54%) and considered that they are discriminated (88.1%). Although a low level of stigma was evidenced (71.4%), these are present in 85.7%. Having had contact as a user with people with HIV was associated with the level of stigma (p=0.008). Conclusions: It is necessary to promote training in which the complexity of the phenomenon is addressed and aspects such as social stigmas are identified, incorporating the biopsychosocial paradigm together with a gender perspective in favor of the comprehensive provision of care.


Introducción: Los estigmas sociales en personas que viven con VIH han sido parte de su historia y aún se puede identificar en los entornos actuales de atención. Objetivo: Analizar los estigmas sociales en relación con la atención de las personas que viven con VIH por parte de quienes otorgan el servicio de vacunación anti Sars-CoV-2. Materiales y métodos: Se desarrolló una investigación cuantitativa descriptiva que incluyó una muestra censal de 126 personas. Se aplicó la Escala de estigma social en la atención de personas con VIH/SIDA. La información fue analizada a través del SPSS versión 25. Resultados: La mayoría de participantes refirió no conocer a personas con VIH, pero sí les han atendido como usuarios (54%), y consideró que son discriminadas (88,1%). Pese a que se evidenció un nivel bajo de estimas (71,4%), éstos están presentes en el 85,7%. El haber tenido contacto como usuario con personas con VIH se asoció con el nivel de estigma (p=0.008). Conclusiones: Se requiere fomentar capacitaciones en las que se aborde la complejidad del fenómeno y se identifiquen aspectos como los estigmas sociales, incorporando el paradigma biopsicosocial y con perspectiva de género en pro de la prestación integral de los cuidados.

Resumen Objetivo: Caracterizar y optimizar el flujo de pacientes dentro de un centro de vacunación, para los casos donde debe tomarse en cuenta que existe una capacidad finita en las colas frente a las estaciones. Materiales y Métodos: Se asume que los sistemas de vacunación se comportan como una red cerrada de colas con capacidad finita; para caracterizar el flujo de pacientes, se aplica simulación y un diseño experimental Box-Bhenken, donde las variables son las capacidades en las colas (buffer); posteriormente se obtienen los metamodelos del tiempo de ciclo y del número de pacientes vacunados, finalmente se calcula la asignación óptima de los lugares en las colas aplicando programación matemática. Resultados: Las colas de llenado de formatos, entrega de información y vacunación son las de mayor efecto sobre el tiempo de estadía; las colas de vacunación y el Triage son las de mayor efecto sobre la cantidad de pacientes vacunados.Si se maximiza la salida de pacientes, la mayor cantidad de lugares deben asignarse a la estación de vacunación y el resto de los espacios se distribuyen en las demás estaciones; por otra parte, si se minimiza el tiempo de estadía, entonces la mayor parte de los lugares se asignan a la estación Triage y a continuación a la estación de Vacunación, el resto de los espacios se asignarán dependiendo de la capacidad total del sistema. Conclusiones: Los responsables de administrar esta clase de sistemas deben definir el criterio bajo el cual deben cuantificar el desempeño del sistema de vacunación y a partir de este, gestionar y controlar el proceso. Cuando la demanda supera las expectativas y no es viable incrementar la capacidad, entonces la alternativa es optimizar el flujo controlando la cantidad de personas dentro del sistema.


Abstract Objective: Characterize and optimize the flow of patients within a vaccination center, for cases where it must be consider that there is a finite capacity in the queues in front of the stations. Materials and Methods: Vaccination systems are assumed to behave as a closed queue network with finite capacity; To characterize the flow of patients, simulation and a Box-Bhenken experimental design are applied, where the variables are the capacities in the queues (buffer); Subsequently, the metamodels of the cycle time and the number of vaccinated patients are obtained. Finally, the optimal allocation of places in the queues is calculated by applying mathematical programming. Results: The queues for filling out forms, delivery of information and vaccination are the ones with the greatest effect on the length of stay; the vaccination and triage queues have the greatest effect on the number of vaccinated patients. If the output of patients is maximized, the greatest number of places should be assigned to the vaccination station and the rest of the spaces are distributed in the other stations; On the other hand, if the length of stay is minimized, then most of the places are assigned to the Triage station and then to the Vaccination station, the rest of the spaces will be assigned depending on the total capacity of the system. Conclusions: Those responsible for managing this class of systems must define the criteria under which they must quantify the performance of the vaccination system and, based on this, manage and control the process. When demand exceeds expectations and increasing capacity is not feasible, then the alternative is to optimize flow by controlling the number of people within the system.

Objetivo Evaluar el efecto de las fuentes de información sobre COVID-19, los beneficios y las barreras percibidas de la vacunación, y la actitud hacia la vacunación sobre la intención de vacunarse contra COVID-19 en la población de 40 años en adelante en México. Métodos Se desarrolló una investigación cuantitativa, transversal y explicativa. Se aplicó una encuesta electrónica a 703 personas de 40 años en adelante que residen en México. El análisis de resultados se realizó a través de la técnica de ecuaciones estructurales por medio del método de mínimos cuadrados parciales. Resultados Las fuentes de información sobre COVID-19 influyen positivamente sobre los beneficios percibidos de la vacuna y disminuyen las barreras percibidas. Además, los beneficios percibidos influyen positivamente sobre la actitud hacia la vacunación de COVID-19; sin embargo, las barreras no afectan la actitud. Adicionalmente, esta última influye en la intención de vacunarse contra COVID-19 e, indirectamente, las fuentes de información y los beneficios percibidos influyen sobre la intención a vacunarse mediados por la actitud.


Objective To evaluate the effect of information sources on COVID-19, the benefits and perceived barriers of vaccination, and the attitude towards vaccination on the intention to get vaccinated against COVID-19 in the population aged 40 and over in Mexico. Methods A quantitative, cross-sectional and explanatory investigation was developed. An electronic survey was applied to 703 people aged 40 and over who reside in Mexico. The analysis of results was carried out through the technique of structural equations by means of the method of partial least squares. Results The sources of information on COVID-19 positively influence the perceived benefits of the vaccine and reduce the perceived barriers. Furthermore, the perceived benefits positively influence the attitude towards COVID-19 vaccination; however, barriers do not affect attitude. Additionally, the latter influences the intention to be vaccinated against COVID-19 and indirectly the sources of information and the perceived benefits influence the intention to be vaccinated mediated by the attitude.

INTRODUCTION: Currently, more than 4.5 billion doses of SARS-CoV-2 vaccines have been applied worldwide. However, some developing countries are still a long way from achieving herd immunity through vaccination. In some territories, such as the Colombian Amazon, mass immunization strategies have been implemented with the CoronaVac® vaccine. Due to its proximity to Brazil, where one of the variants of interest of SARS-CoV-2 circulates. OBJECTIVE: To determine the effectiveness of the CoronaVac® vaccine in a population of the Colombian Amazon. METHODS: Between February 24, 2021, and August 10, 2021, a descriptive observational study was carried out in which a population of individuals over 18 years of age immunized with two doses of the CoronaVac® vaccine was evaluated. The study site was in the municipality of Mitú, Vaupés, in southeastern Colombia, a region located in the Amazon bordering Brazil. RESULTS: 99% of the urban population of the Mitú municipality were vaccinated with CoronaVac®. To date, 5.7% of vaccinated individuals have become ill, and only 0.1% of these require hospitalization. One death was attributable to COVID-19 has been reported among vaccinated individuals, and the vaccine has shown 94.3% effectiveness against mild disease and 99.9% against severe infection. CONCLUSIONS: The herd immunity achieved through mass vaccination in this population has made it possible to reduce the rate of complicated cases and mortality from COVID-19 in this region of the Colombian Amazon. HIGHLIGHTS: CoronaVac® has shown 94.3% effectiveness against mild disease and 99.9% against severe infection in this indigenous population. CoronaVac® reduces the mortality rate from 2.2% in 2020 to 0.22% in 2021. The herd immunity was achieved through mass vaccination in this region of the Colombian Amazon.

This study investigated the prevalence of the coronavirus disease (COVID-19) and vaccination among dental students from public and private institutions, as well as verified the level of influence of the pandemic and vaccination on physical, mental health and academic performance. In this cross-sectional study an online questionnaire using Google Forms® was applied to dental students from a public and a private university on epidemiological data, COVID-19, vaccination and the impact of social distancing on health. The rate of COVID-19 infection in both groups was 36.8% (n=46), and when asked about the period of infection, 82.6% of these (n=38) contracted it before the first dose, but there were no statistically significant differences (χ2=2.667; p=0.102) between the groups. There was a difference between the groups tested, since 56.3% of the students from G1 were vaccinated with a second dose, compared to 29.6% from G2 (χ2= 16.554; p= 0.0009). Students were asked about the existence of risks in the dentistry course, and 76.8% considered that there are risks (n=96), with a statistically significant difference (χ2= 10.216; p= 0.001). Regarding the vaccine, 64% of the participants think it is safe (n=80), with a statistically significant difference between the groups (χ2= 4.374; p= 0.036). Most students contracted COVID-19 before the first dose, were vaccinated with the second dose by AstraZeneca and Pfizer immunizers. The academic performance and the physical and mental health of the students were significantly affected, with the students of advanced periods and those of public universities suffering more negative impacts (AU).


Este estudo investigou a prevalência da COVID-19 e vacinação entre estudantes de odontologia de instituições públicas e privadas, bem como verificar o nível de influência da pandemia e da vacinação na saúde física, mental e no desempenho acadêmico. Em um estudo transversal, foi aplicado um questionário online utilizando o Google Forms® a estudantes de odontologia de uma universidade pública e privada sobre dados epidemiológicos, COVID-19, vacinação e o impacto do distanciamento social na saúde. A taxa de infecção por COVID-19 em ambos os grupos foi de 36,8% (n=46), e quando questionados sobre o período de infecção, 82,6% destes (n=38) contraíram antes da primeira dose, mas não houve significância estatística diferenças (χ2=2,667; p=0,102) entre os grupos. Houve diferença entre os grupos testados, pois 56,3% dos escolares do G1 foram vacinados com uma segunda dose, comparado a 29,6% do G2 (χ2= 16,554; p= 0,0009). Os alunos foram questionados sobre a existência de riscos no curso de odontologia, e 76,8% consideraram que existem riscos (n=96), com diferença estatisticamente significativa (χ2= 10,216; p= 0,001). Em relação à vacina, 64% dos participantes a consideram segura (n=80), com diferença estatisticamente significativa entre os grupos (χ2= 4,374; p= 0,036). A maioria dos alunos contraiu COVID-19 antes da primeira dose, foram vacinados com a segunda dose pelos imunizadores AstraZeneca® e Pfizer®. O desempenho acadêmico e a saúde física e mental dos alunos foram significativamente afetados, com os alunos de períodos avançados e os de universidades públicas sofrendo mais impactos negativos (AU).

Avaliar a trajetória das taxas de mortalidade por COVID-19 no Brasil e comparar as taxas extremas de 2022 e de 2021, em distintos grupos etários. Estudo ecológico com óbitos por síndrome respiratória aguda grave por COVID-19, tendo o Brasil como unidade de análise. Os dados foram obtidos do Sistema de Informação da Vigilância Epidemiológica da Gripe. Foram avaliados os óbitos no período de 10 de janeiro de 2021 a 12 de fevereiro de 2022, agrupado em Semanas Epidemiológicas (SE). A análise dos dados foi conduzida no software R, utilizando modelos Poisson para estimar as taxas de mortalidade. O nível de significância estatística foi 5%. Foram avaliados 408.180 óbitos, sendo 0,34% de indivíduos menores de18 anos e 64,6% daqueles com 60 anos e mais. De um lado, nas faixas etárias de 0-1, 2-4 e 5-11 anos, observaram-se maiores taxas de mortalidade nas SE 4-6/2022, em comparação às maiores de 2021. De outro, nos indivíduos de 12-17 anos, estimou-se taxa inferior no grupo de SE 4-6/2022, em comparação ao grupo de SE 11-13 de 2021, com razão de mortalidade 0,60 (IC95%: 0,38-0,94). Detectaram-se padrões opostos na mortalidade por COVID-19 no Brasil entre crianças e indivíduos incluídos na campanha nacional de vacinação. Entre os primeiros, observaram-se taxas de mortalidade iguais ou piores do que em fases anteriores da epidemia, contrapondo-se ao registro de queda consistente e forte dos últimos, reforçando a efetividade da vacina contra COVID-19.


This ecological study evaluated the trajectory of COVID-19 mortality rates in Brazil and compared the extreme rates of 2022 and 2021, in different age groups. Data on deaths due to severe acute respiratory syndrome by COVID-19 were obtained from the Influenza Epidemiological Surveillance Information System. Deaths were evaluated from January 10, 2021 to February 12, 2022, grouped into Epidemiological Weeks (EW). Data analysis was conducted in the R software, using Poisson models to estimate mortality rates. Statistical significance level was set at 5%. A total of 408,180 deaths were evaluated, 0.34% of whom were under 18 years old, and 64.6% of whom were 60 years old and over. On the one hand, in the 0-1, 2-4 and 5-11 age groups, higher mortality rates were observed in EW 4-6/2022, compared to the higher ones in 2021. On the other, in the 12-17 age group, a lower rate was estimated in the EW 4-6/2022 group compared to the EW 11-13 group in 2021, with a mortality ratio of 0.60 (95%CI: 0.38-0.94). Opposing patterns were detected in COVID-19 mortality in Brazil among children and individuals included in the national vaccination campaign. Among the former, mortality rates equal to or worse than in previous phases of the epidemic were observed, contrasting with the consistent and strong decline registered in the latter, reinforcing the effectiveness of COVID-19 vaccines.


Los objetivos fueron evaluar la trayectoria de las tasas de mortalidad por COVID-19 en Brasil y comparar las tasas extremas del 2022 y del 2021, en diferentes grupos de edad. Estudio ecológico con muertes por síndrome respiratorio agudo Severo por COVID-19, teniendo a Brasil como unidad de análisis. Los datos se obtuvieron del Sistema de Información de Vigilancia Epidemiológica de la Gripe. Se evaluaron las defunciones del 10 de enero del 2021 al 12 de febrero del 2022, agrupadas en Semanas Epidemiológicas (SE). El análisis de datos se realizó en el software R, utilizando modelos Poisson para estimar las tasas de mortalidad. El nivel de significancia estadística fue del 5%. Se evaluaron un total de 408.180 defunciones, el 0,34% fue de individuos menores de 18 años y el 64,6% fue de individuos con 60 años o más. Por una parte, en los rangos de edad 0-1, 2-4 y 5-11 se observaron mayores tasas de mortalidad en SE 4-6/2022, en comparación con las más altas del 2021. Por otra parte, en los individuos de 12-17 años se estimó una tasa inferior en el grupo SE 4-6/2022, en comparación con el grupo de SE 11-13 del 2021, con una razón de mortalidad de 0,60 (IC95%: 0,38-0,94). Se detectaron estándares opuestos en la mortalidad por COVID-19 en Brasil entre niños e individuos incluidos en la campaña nacional de vacunación. Entre los primeros, se observaron tasas de mortalidad iguales o peores que en etapas anteriores de la epidemia, en contraste con el registro de una reducción consistente y fuerte de los últimos, lo que reforzó la efectividad de la vacuna contra la COVID-19.

A avaliação do impacto da vacinação contra a COVID-19 em idosos é escassa, sobretudo em um cenário com predomínio da variante Gama. O objetivo deste estudo foi avaliar a cobertura vacinal e sua relação com mudanças no padrão de internações e óbitos por COVID-19 em idosos de Manaus, Amazonas, Brasil. Este é um estudo ecológico com dados de internações e óbitos do Ministério da Saúde que avaliou a cobertura vacinal, mediante esquema com duas doses, além de dois regimes de vacinação associados a significativo efeito protetor, um parcial (35 ou mais dias após a primeira dose da vacina Oxford/AstraZeneca) e outro completo (14 dias ou mais após a segunda dose da vacina Sinovac-CoronaVac). A partir da data dos primeiros sintomas, padrões de internação e óbito por COVID-19 foram avaliados, comparativamente, em idosos de 60-69 e de 70 anos ou mais, em dois grupos de Semanas Epidemiológicas (SE) de 2020 (não vacinados) e 2021 (vacinados). Taxas de internação e óbito foram estimadas pelo modelo Poisson. Entre 60-69 anos e naqueles com 70 anos ou mais, a cobertura por vacina foi 41,8% e 54,8%, bem como 53,5% e 90,1% nos grupos de SE 18-20/2021 e 21-23/2021, respectivamente. Em ambos os grupos de SE de 2021, observou-se substancial mudança nos padrões de internações e óbitos por COVID-19, com aumento no risco de internação e óbito nos mais jovens não vacinados, e importante redução no número de idosos vacinados, sobretudo naqueles com 60-69 anos, além de redução global de 62% (IC95%: 52-69) e 63% (IC95%: 43-75) nas taxas de internação e óbitos, respectivamente. Nossos resultados reforçam a importância da vacinação em massa, especialmente em contexto epidêmico como o de Manaus, marcado por elevada circulação da variante Gama.


The impact of COVID-19 vaccination in the elderly has received relatively little attention, particularly in a scenario predominated by the gamma variant. The aim of this study was to assess vaccination coverage and its relationship to changes in the pattern of COVID-19 hospitalizations and deaths in the elderly in Manaus, Amazonas State, Brazil. This was an ecological study with Brazilian Ministry of Health data on hospitalizations and deaths, assessing vaccination coverage based on a two-dose regimen, in addition to two vaccination regimens associated with a significant protective effect, one partial (35 days or more after the first dose of the Oxford/AstraZeneca vaccine) and the other complete (14 days or more after the second dose of the Sinovac-CoronaVac vaccine). Based on the date of initial symptoms, patterns of COVID-19 hospitalizations and deaths were assessed comparatively in elderly 60-69 years and 70 years or more in two groups of Epidemiological Weeks (EW) in 2020 (unvaccinated) and 2021 (vaccinated). Hospitalization and death rates were estimated with Poisson regression. In the groups 60-69 and 70 years or more, vaccination coverage rates were 41.8% and 54.8%, as well as 53.5% and 90.1%, in the EW groups 18-20/2021 and 21-23/2021, respectively. Both EW groups in 2021 showed a substantial change in the patterns of COVID-19 hospitalizations and deaths, with an increase in the risk of hospitalization and death in unvaccinated younger individuals and an important reduction in vaccinated elderly, especially those 60-69 years of age, besides overall reductions of 62% (95%CI: 52-69) and 63% (95%CI: 43-75) in hospitalization and death rates, respectively. Our results emphasize the importance of mass vaccination, especially during an epidemic such as in Manaus, marked by high circulation of the gamma variant.


La evaluación del impacto de la vacunación contra la COVID-19 en ancianos es escasa, sobre todo en un escenario con predominio de la variante Gamma. El objetivo de este estudio fue evaluar la cobertura de vacunación y su relación con cambios en el patrón de internamientos y óbitos por COVID-19 en ancianos de Manaos, Amazonas, Brasil. Este es un estudio ecológico con datos de internamientos y óbitos del Ministerio de Salud, que evaluó la cobertura de vacunación, mediante un esquema con dos dosis, además de dos regímenes de vacunación, asociados a un significativo efecto protector, uno parcial (35 o más días tras la primera dosis de la vacuna Oxford/AstraZeneca) y otro completo (14 días o más tras la segunda dosis de la vacuna Sinovac-CoronaVac). A partir de los datos de los primeros síntomas, se evaluaron patrones de internamiento y óbito por COVID-19, comparativamente, en ancianos de 60-69 y de 70 años o más, en dos grupos de Semanas Epidemiológicas (SE) de 2020 (no vacunados) y 2021 (vacunados). Se estimaron tasas de internamiento y óbito mediante el modelo Poisson. Entre 60-69 años y en aquellos con 70 años o más, la cobertura por vacuna fue 41,8% y 54,8%, así como 53,5% y 90,1% en los grupos de SE 18-20/2021 y 21-23/2021, respectivamente. En ambos grupos de SE de 2021, se observó un cambio sustancial en los patrones de internamiento y óbitos por COVID-19, con un aumento en el riesgo de internamiento y óbito en los más jóvenes no vacunados e importante reducción en los ancianos vacunados, sobre todo en aquellos con 60-69 años, además de una reducción global de 62% (IC95%: 52-69) y 63% (IC95%: 43-75) en las tasas de internamiento y óbitos, respectivamente. Nuestros resultados refuerzan la importancia de la vacunación en masa, especialmente en un contexto epidémico como el de Manaos, marcado por una elevada circulación de la variante Gamma.

Analisar através de evidências cientificas, o nível de conhecimento da população acerca dos benefícios da imunização infantil. Método: Trata-se de uma revisão integrativa da literatura acerca dos benefícios da imunização infantil, publicada nos últimos 7 anos, dentro das seguintes bases de dados: SciELO, Science direct, biblioteca virtual em Saúde e PubMed. Resultados: Foram analisados 495 artigos, dos quais 7 artigos foram selecionados para esta revisão integrativa, a qual foi realizada de maio a junho de 2020. Foi possível perceber que há uma porcentagem ainda grande da população mundial que opta pela não imunização de suas crianças, onde a grande maioria se dá em países subdesenvolvidos, por falta de visibilidade da mídia e baixa escolaridade, principalmente. Conclusão: Campanhas de vacinação e esclarecimento sobre vacinas devem ser realizadas de forma sistemática.(AU)


Objective: To analyze through scientific evidence, the population's level of knowledge about the benefits of childhood immunization. Method: This is an integrative literature review on the benefits of childhood immunization, published inM the last 7 years, in the following databases: SciELO, Science direct, Virtual Health Library and PubMed. Results: 495 articles were analyzed, of which 7 articles were selected for this integrative review. It was possible to notice that there is a still large percentage of the world population that chooses not to immunize their children, where the vast majority takes place in underdeveloped countries, mainly due to lack of media visibility and low education level. Conclusion: Vaccination campaigns and information about vaccines must be carried out systematically.(AU)

Método: Se trata de una revisión bibliográfica sobre los benefícios de la imunización infantil, publicada en los últimos 7 años, dentro de las seguientes bases de datos: SciELO, Science direct, Virtual Health Library e PubMed. Resultados: Se analizaron 495 artículos, de los cuales se seleccionaron 7 artículos para esta revisión integradora. Se pudo notar que todavia hay um gran porcentaje de la población mundial que opta por no vacunar a sus hijos, donde la gran mayoría se da em países subdesarrollados, debido a la falta de visibilidad en los medios y la baja educación, especialmente. Conclusión: Las campañas de vacunación y la información sobre vacunas deben realizarse de forma sistemática.(AU)

RESUMEN El objetivo de este estudio fue analizar las variaciones espaciotemporales de la mortalidad por COVID-19 en adultos mayores y de la vacunación contra la COVID-19 en esta población. Se utilizaron datos de defunciones por COVID-19 y de personas de 70 a más años inmunizadas con la primera dosis de vacunas contra esta enfermedad, en partidos de la provincia de Buenos Aires, desde el 29 de diciembre del 2020 al 30 de junio de 2021. Se emplearon técnicas de escaneo espaciotemporal para detectar conglomerados. Los partidos del Gran Buenos Aires que tuvieron mayor vacunación con la primera dosis de la vacuna Sputnik V en población de 70 a más años, entre mediados de marzo y principios de abril de 2021, coincidieron mayormente en registrar una disminución de la mortalidad en esta población, entre finales de abril y finales de junio de 2021. Este estudio mostró algunos indicios del impacto positivo de la aplicación de la primera dosis de la vacuna Sputnik V en el Gran Buenos Aires.


ABSTRACT The aim of this study was to analyze the spatiotemporal variations of COVID-19 mortality and vaccination against COVID-19 in older adults. We used data from deaths due to COVID-19 and persons aged 70 years and older immunized with the first dose of vaccines against this disease, from districts of the province of Buenos Aires, between December 29, 2020, and June 30, 2021. Spatiotemporal scanning techniques were used to detect clusters. The parties of Greater Buenos Aires that had the highest vaccination rate with the first dose of the Sputnik V vaccine in population aged 70 years and older, between mid-March and early April 2021, also registered a decrease in mortality in this population, between the end of April and the end of June 2021. This study showed some signs of the positive impact associated with the application of the first dose of Sputnik V vaccine in Greater Buenos Aires.

OBJECTIVE: To identify factors associated with COVID-19 vaccine hesitancy and acceptance among the Mexican population. METHODS: In a web-based nationwide survey in early December 2020, respondents were inquired about their sociodemographic characteristics and their willingness to accept a hypothetical COVID-19 vaccine given a 50% or 90% effectiveness. A logistic regression model was used to identify the factors associated with hesitancy and acceptance. RESULTS: A total 3 768 responses were analyzed. A 90% effective vaccine was accepted by 85% of respondents, while only 46% would accept being vaccinated with a 50% effective vaccine. In univariate analysis, each age group (40-49, 50-59, and ≥60) was strongly associated with vaccine hesitancy for a 90% effective vaccine (OR 0.48, 95% CI 0.38, 0.63; OR 0.33, 95 CI 0.26, 0.41; and OR 0.28, 95 CI 0.21, 0.38, respectively) compared to the 18-39 age group. After multivariable adjustment, similar magnitudes of association were observed. Being female and higher socioeconomic status were also associated with higher vaccine hesitancy. CONCLUSIONS: Vaccine hesitancy represents a major public health problem in Mexico and is driven by multiple factors. RESUMEN

To the date of publication of this editorial, we have gone through over 20 months of facing the complex and challenging SARS-CoV-2 pandemic since the first case was reported in late 2019. The consequences globally have been significant not only due to the unprecedented morbidity and mortality throughout the world, but also as effect of the drastic changes taking place in the usual dynamics in the individual, family, and collective settings, given the overall interruption of habitual functions and operations in the distinct contexts of daily life, sustaining only those activities considered essential.


A la fecha de publicación de este editorial ya hemos transitado más de 20 meses enfrentando la compleja y desafiante pandemia por SARS-CoV-2 desde que se reportara el primer caso a finales de 2019. Las consecuencias en el mundo han sido significativas no solo por la morbilidad y mortalidad sin precedentes sino, además, como efecto de los drásticos cambios ocurridos en las dinámicas usuales en lo individual, familiar y colectivo dada la interrupción generalizada de las funciones y operaciones habituales en los distintos contextos de la vida cotidiana, por lo cual se mantuvieron solamente aquellas actividades consideradas esenciales.

Resumo O objetivo deste artigo é analisar o uso da inteligência artificial espacial no contexto da imunização contra COVID-19 para a seleção adequada dos recursos necessários. Trata-se de estudo ecológico de caráter transversal baseado em uma abordagem espaço-temporal utilizando dados secundários, em Unidades Básicas de Saúde do Brasil. Foram adotados quatro passos analíticos para atribuir um volume de população por unidade básica, aplicando algoritmos de inteligência artificial a imagens de satélite. Em paralelo, as condições de acesso à internet móvel e o mapeamento de tendências espaço-temporais de casos graves de COVID-19 foram utilizados para caracterizar cada município do país. Cerca de 18% da população idosa brasileira está a mais de 4 quilômetros de distância de uma sala de vacina. No total, 4.790 municípios apresentaram tendência de agudização de casos de Síndrome Respiratória Aguda Grave. As regiões Norte e Nordeste apresentaram o maior número de Unidades Básicas de Saúde com mais de 5 quilômetros de distância de antenas de celular. O Plano nacional de vacinação requer o uso de estratégias inovadoras para contornar os desafios do país. O uso de metodologias baseadas em inteligência artificial espacial pode contribuir para melhoria do planejamento das ações de resposta à COVID-19.


Abstract This article explores the use of spatial artificial intelligence to estimate the resources needed to implement Brazil's COVID-19 immu nization campaign. Using secondary data, we conducted a cross-sectional ecological study adop ting a time-series design. The unit of analysis was Brazil's primary care centers (PCCs). A four-step analysis was performed to estimate the popula tion in PCC catchment areas using artificial in telligence algorithms and satellite imagery. We also assessed internet access in each PCC and con ducted a space-time cluster analysis of trends in cases of SARS linked to COVID-19 at municipal level. Around 18% of Brazil's elderly population live more than 4 kilometer from a vaccination point. A total of 4,790 municipalities showed an upward trend in SARS cases. The number of PCCs located more than 5 kilometer from cell towers was largest in the North and Northeast regions. Innovative stra tegies are needed to address the challenges posed by the implementation of the country's National COVID-19 Vaccination Plan. The use of spatial artificial intelligence-based methodologies can help improve the country's COVID-19 response.
