RESUMEN

Objective: To investigate the value of a SnapECG monitoring in diagnosing arrhythmias compared with the conventional management. Methods: In the first phase, the SnapECG and 12-lead electrocardiogram (ECG) were simultaneously adopted to detect arrhythmias in 439 hospitalized patients. The accuracies of the SnapECG in detecting different arrhythmias were assessed. In the second phase, 62 patients with palpitations were randomized to receive the SnapECG monitoring or conventional management for 3 months. The diagnosis rate, time of diagnosis, episodes before diagnosis, associated expenses, and scores of the modified European Heart Rhythm Association (EHRA), Self-rating Anxiety Scale (SAS), and the 36-item short-form health survey questionnaire (SF-36) were compared between groups. Results: In the first phase, the SnapECG monitoring showed a sensitivity of 83.55% and specificity of 96.79% in identifying tachyarrhythmias, and a sensitivity of 95.29% and specificity of 97.54% in identifying bradyarrhythmias. In the second phase, 1642 ECGs were recorded by the SnapECG, among which 290 abnormal ECGs were identified. Compared with the conventional management, the SnapECG monitoring increased the diagnosis rate of symptomatic arrhythmias (70.97% vs. 19.35%, P < 0.05), shortened the time of diagnosis (48.26 ± 36.78 days vs. 71.45 ± 30.01 days, P < 0.05) and consequently reduced the episodes of symptomatic arrhythmias prior to establishing diagnosis. The scores of modified EHRA, SAS, SF-36 significantly improved at 3-month compared with their baseline levels in the SnapECG group. Conclusions: Remote monitoring with the SnapECG can achieve early diagnosis of symptomatic arrhythmias. However, its sensitivity in identifying P-wave-related arrhythmias warrants further improvement.

【Objective】 To explore the role and value of applied muscle tension (AMT) in preventing vasovagal nerve reaction (VVR) in blood donors. 【Methods】 A total of 2 992 people, susceptible to suffer VVR from May 2020 to may 2022, were randomly divided into control group (1406 cases) and observation group (1 586 cases). The control group was not given AMT intervention, while the observation group received AMT intervention at different periods during blood donation. The changes of systolic blood pressure, diastolic blood pressure, heart rate and psychological state of anxiety (self-rating anxiety scale, SAS) of blood donors were monitored in the two groups at each period to compare the occurrence of VVR. 【Results】 There were no statistically significant differences in blood pressure and heart rate between the two groups before blood donation (P>0.05). The parameters were relatively stable in observation group during and after donation, but significantly different from that of the controls(P>0.05). SAS score was similar in two groups before blood donation(P>0.05), while decreased in observation group during and after donation in comparison with the controls(P<0.05). The incidence of VVR in the observation group was 3.09%, which was significantly lower than that in the control group (7.97%)(P<0.05). The incidence of VVR was 2.18% after AMT exercise during blood donation. 【Conclusion】 AMT intervention in different periods of blood donation can significantly reduce the occurrence of VVR.

Objective:To explore influence of dexmedetomidine on Riker sedation -agitation scale (SAS) score and Ramsay sedation score in patients with hypertensive cerebral hemorrhage after borehole drainage . Methods :A total of 96 patients with hypertensive cerebral hemorrhage after borehole drainage treated in our hospital were selected . According to analgesic drugs taken after operation ,they were divided into dexmedetomidine group (n=48) and mid‐azolam group (n= 48) .Riker SAS score ,Ramsay sedation score and adverse reactions after operation were com‐pared between two groups . Results:Compared with midazolam group ,there were significant rise in Ramsay seda‐tion score [6h :(1.59 ± 0.52) scores vs .(2.13 ± 0.15) scores] and significant reduction in Riker SAS score [6h :(4.09 ± 0.82) scores vs .(3.73 ± 0.76) scores] on 6h ,12h and 24h after operation ,P<0.05 or <0.01 ;and signifi‐cant reduction in incidence rate of adverse reactions (37.5% vs .10.4% , P< 0.05 ) in dexmedetomidine group . Conclusion:Dexmedetomidine possesses definite analgesic and sedative effects on patients with hypertensive cerebral hemorrhage after borehole drainage ,it's safe andreliable ,which is worth extending .

Objective To explore the effects of antenatal premium care on the improvement of negative emotions and pregnancy outcomes for patients with gestational hypertension. Methods A total of 128 patients with gestational hyper-tension who were admitted to our hospital from June 2013 to January 2014 were selected as research subjects. They were randomly assigned to two groups, who received antenatal premium care (observation group) and regular nursing care (control group). SAS and SDS scores before and after admission, premature delivery, cesarean section, placental abruption, postnatal hemorrhage, eclampsia, pre-eclampsia, fetal distress, low birth weight, neonatal asphyxia, death, Apgar scores and life quality were compared. Results After antenatal premium care in the observation group, SAS and SDS scores were (41.42±5.16) and (40.29±5.41) respectively;After antenatal premium care in the control group, SAS and SDS scores were (50.27±5.49) and (48.73±5.63) respectively. Before the antenatal premium care, SAS and SDS scores between the two groups were not statistically significant (P>0.05). After the antenatal premium care, SAS and SDS scores in the observation group were significantly lower than those in the control group, and the differences were statis-tically significant (P<0.05). Differences of negative outcomes compared between the two groups of patients were statisti-cally significant (P<0.05). Differences of negative outcomes compared between the two groups of perinatal infants were statistically significant (P<0.05). Scores of life quality of patients in the observation group were all better than those in patients in the control group, and the differences were statistically significant (P<0.05). Conclusion Antenatal premium nursing is able to effectively ease the negative emotion of patients with gestational hypertension and enhance the out-come of mother and infants.

Objective To study and explore the influence of humanized nursing on psychological state and life quality in patients with chronic glomerulonephritis. Methods From January 2013 to January 2014 in our hospital 60 patients with chronic glomerulonephritis according to the different nursing methods were divided into observation group (n=30) and control group (n=30). The patients in the control group treated by conventional methods of care. The observation group with humanistic nursing, the efficacy of the two groups, and two groups of the patient's quality of life score, SAS score, SDS score were compared. Results The total effective rate in the observation group was 90.00% (27/30), was sig-nificantly higher than that in the control group 70.00%(21/30), the difference was statistically significant (P<0.05). The SAS score, SDS score of intervention group and the control group after the intervention were significantly lower than be-fore intervention, and the intervention group was significantly lower than the control group, the difference was statisti-cally significant(P<0.05), Energy, pain, emotional reactions, sleep, social isolation and somatic activity score of the in-tervention group and the control group after the intervention were significantly reduced compared with those before in-tervention, and the intervention group was significantly lower than the control group, the difference was statistically sig-nificant (P<0.05). Conclusion The humanized nursing intervention can improve the effect and quality of life in chronic glomerulonephritis, anxiety, depression, so as to enable them to actively cooperate with treatment and nursing.

RESUMEN

Objective To investigate the clinical effect of magnetic combined with acupuncture in treatment of osteo-porosis low back pain. Methods A total of 80 cases of patients with osteoporosis low back pain from May 2013 to July 2015 were selected and randomly divided into two groups, with 40 cases in each group. The control group were given a simple acupuncture for treatment, on the basis of the control group, the study group were combined with super magnetic therapy, the clinical efficacy of the two groups was compared. Results The total efficiency of the treatment of the study group was significantly higher than the control group, the symptom scores and signs scores were significantly higher than the control group, bone mineral density after treatment, the WHOQOL-BREF scores condition were significantly increased compared with before treatment and the control group after treatment, the SAS scores after treatment of two groups were significantly lower than before treatment, and compared with the control group after treatment, the study group SAS score was decreased more significantly, the rate of adverse events was significantly lower than the control group, the satisfaction was more significantly higher than the control group, there were significant difference between the two groups' datas comparison (P<0.05). Conclusion The effect of magnetic therapy combined with acupuncture in treat-ment of osteoporosis low back pain is significant, and with more targeted、security、control,it can improve the patients' bone density, with high safety,and it is worthy of promotion and application in clinical.

RESUMEN

Objective To explore the effect of health education on the compliance and mental health in patients of the Department of cardiology. Methods Selected 154 patients from the department of cardiology from April 2013 to April 2014 in the first people's hospital of Chunan county. These patients were randomly divided into the observation group and the control group. Normal nursing was given to the patients in the control group. Besides normal nursing, health education was conducted on the observation group. The SAS score, SDS score and the compliance were recorded and compared between those two groups. Results After health education, SAS score and SDS score of the patients in the ob-servation group and the control group were significantly lower than before health education(t=78.48, P<0.01; t=57.04, P<0.01), and the SAS score and SDS score of patients in control group were also significantly lower than before(t=26.99, P<0.01;t=24.4, P<0.01). While, after health education, the SAS score and SDS score of the patients in the obser-vation group were significantly lower than that of the patients in the control group(t=45.79, P<0.01; t=41.27, P<0.01). The compliance rate of the observation group was significantly higher than that of the control group(x2=37.762,P<0.01), and the compliance of the patients in the observation group was significantly better than that of the control group(Z=-6.456, P<0.01). Conclusion Health education can reduce the incidence of anxious or depression for patients in depart-ment of Cardiology, improve the treatment compliance. And it is worthy of promotion and application.

RESUMEN

Objective To investigate the effect of humanized nursing on laparoscopicmyomectomy psychological and complications in patients undergoing. Methods 60 cases of laparoscopic myomectomy patients according to different nursing methods were randomly divided into intervention group and control group, with 30 cases in each, the two groups underwent laparoscopic myomectomy, the control group received routine nursingintervention group, emphatically hu-mane nursing intervention, the observed indexes were compared. Results The anal exhaust time, postoperativepatients of the intervention group had postoperative ambulation time,hospitalization time were significantly shorter than the control group(P<0.05). Intervention group SAS score was(43.57±1.32), SDS was(37.48±8.46), respectively, lower than those in the control group (t=6.534, 8.371, P<0.05).Bleeding,subcutaneous emphysema,shoulder and back pain, infection, gastrointestinal reaction occurred after the patients in the intervention group were 6.7%, 6.7%, 20.0%, 0, 13.3%, respec-tively, significantly lower than the control group(χ2=6.235, 6.124, 5.673, 4.237, 4.839, P<0.05). Conclusion The ap-plication of humanized nursing on laparoscopic uterine fibroids resection, helps to improve the patients' anxiety de-pression psychological,reduce the incidence of complications, promote the patient postoperative recovery.
