RESUMEN

Gender relations are a set of socially determined norms and rules that assign values, characteristics, and expectations to individuals based on their biological sex. These aspects also influence the clinician-patient relationship, since it has been for a long time based on cisheteronormativity. However, this attitude alienated the LGBTQIA+ community from health services. Global and specific gynecologic care needs to be offered to the LGBTQIA+ population, which has demands for sexual and reproductive health care. In this narrative review, we bring conceptual aspects, gender identity and expression, sexual history, screening for cancer and other care to the community.

We conducted a systematic review to explore the relationship between perceived risk for HIV acquisition and sexual HIV exposure among sexual and gender minorities. We included 39 studies divided into (i) correlations or associations, (ii) models using sexual HIV exposure as the outcome, and (iii) models using perceived risk for HIV acquisition as the outcome. The sample size range was from 55 to 16,667 participants, primarily cisgender men who have sex with men (73.3%) and White (51.3%). Sexual HIV exposure and perceived risk for HIV acquisition assessments and recall time frames across studies differed markedly. Most of studies (84.6%) found significant correlations, comparisons, or associations between different levels of perceived risk for HIV acquisition and high sexual HIV exposure. In addition, 51.3% of studies reported other variables associated with high sexual HIV exposure (i.e., misuse of substances or alcohol) or with high perceived risk for HIV acquisition (i.e., younger age). In conclusion, the association between perceived risk for HIV acquisition and sexual HIV exposure has shown to be consistent. However, the assessment for perceived risk for HIV acquisition should include more components of perception (i.e., an affective component), or for sexual HIV exposure should consider the different estimated sexual per-acts probability of acquiring HIV.

RESUMEN

In recent years, there have been international references to the vocal approach for the specific group of transgender individuals, although the Latin American literature is still very timid on this matter. The purpose of this article is to identify the current norms, statistics, and vocal approach towards transgender individuals in Chile and Argentina, considering the experience of two speech and language pathologists with more than twenty years of experience on voice therapy. Reflections were made on the transgender reality in these countries, the limitations in the implementation of the depathologization of the transgender group were outlined, some current and unreliable statistics were presented, some innovative actions in the public system were highlighted, and the lack of knowledge about the benefits of vocal work for transgender men and women was discussed. The identified aspects could benefit from multicenter research that strengthens speech therapy actions with this group, contributing to depathologization and positive approach.


Desde los últimos años es posible encontrar referencias internacionales sobre el abordaje vocal al grupo específico de las personas transgénero, aunque la literatura latinoamericana sigue muy tímida en este asunto. La propuesta de este artículo es identificar las normas vigentes, estadísticas y abordaje vocal hacia las personas transgénero en Chile y Argentina, considerando la experiencia de dos fonoaudiólogas con más de veinte años de experiencia en terapia vocal. Se hicieron reflexiones sobre la realidad transgénero en los países citados, se delinearon las limitaciones en la puesta en práctica de la despatologización del grupo transgénero, se expusieron algunas estadísticas -vigentes y poco confiables-, se plasmaron algunas acciones novedosas en el sistema público y el desconocimiento sobre los beneficios del trabajo vocal en hombres y mujeres transgénero. Los aspectos detectados podrían beneficiarse de investigaciones multicéntricas que fortalezcan acciones fonoaudiológicas con este grupo, contribuyendo a la despatologización y el abordaje positivo.

BACKGROUND: Masculinizing mammoplasty is a surgical procedure frequently performed in transmale individuals. Despite providing a positive impact for the patient, this surgery has high rates of complications and revisions. In cases requiring a double incision, there are advantages in using an inferior pedicle when possible. METHODS: This was an observational study. Outcomes and complications were analyzed in 104 patients operated on by the author using three techniques: concentric periareolar surgery, double incision with an areola graft, and double incision with an inferior areolar pedicle. The Breast-Q questionnaire adapted for transgender males was applied. RESULTS: The responses of the adapted Breast-Q questionnaires demonstrated high scores regarding quality of life and satisfaction. The rates of complications and surgical revisions were 24.3 and 25.6%, respectively, with no significant difference between the techniques. The most common complication was hematoma (13.6%), which was positively associated with the use of testosterone. Ischemia of the nipple-areola complex was present in 8.7% of the operated breasts. When using the inferior pedicle, areola ischemia occurred more often when the nipple-fold distance was greater than 8 cm. CONCLUSIONS: Quality of life, satisfaction, complications, and revisions were comparable to those observed in the literature. Hematoma was the most frequent complication, and an association with the use of testosterone was observed. When a double incision is indicated, the inferior pedicle is more advantageous than the areola graft; however, it should be used when the distance between the nipple and the inframammary fold is 8 cm or less. OBJECTIVE: To verify breathiness in the cisgender and transgender men and women's voices, compare values of acoustic and perceptual indicators of breathiness and fundamental frequency (f0) between groups, and compare them between the voices attributed as female and male. STUDY DESIGN: Cross sectional retrospective study. METHODS: The study was approved by the Research Ethics Committee (4,937,140). Sustained vowel /a/ and continuous speech recordings of 21 cisgender men (CISM), 31 transgender men (TM), 32 cisgender women (CISW), and 31 transgender women (TW) were analyzed. Three judges conducted a perceptive-auditory analysis regarding the degree breathiness, using a visual analog scale, and attributed gender (female or male). The ABI (Acoustic Breathiness Index) was extracted using the PRAAT software (6.1.16). The f0, Harmonic-Noise Ratio (HNR), Voice Turbulence Index (VTI), and Soft Phonation Index (SPI) were analyzed using the Multi-Dimensional Voice Program (KayPentax). RESULTS: The ABI value for CISM was lower than for TM and CISW. CISW had a higher f0 than; TM had a higher f0 than CISM; and TW had a higher f0 than CISM. The groups did not differ for HNR and VTI. Regarding the SPI, CISM had higher values than CISW. Regarding the auditory perception, TM presented more intense breathiness than CISM in the vowel. Regarding gender attribution by voice, the voices CISM and CISW were 100% identified as male and female. On the other hand, in the vowel analysis, 45.2% of the TM voices were perceived as female, and 59.4% of TW voices as male. CONCLUSION: Breathiness occurs differently between groups and the voices perceived as male and female. Even when TM is submitted to the use of testosterone and undergoes vocal changes, the transglottal airflow remains, which is a female characteristic of phonation.

RESUMO Objetivo realizar a adaptação transcultural dos instrumentos Vocal Congruence Scale (VCS) e o Transgender Scale Congruence (TSC) para o português brasileiro. Método o estudo foi desenvolvido em duas etapas: adaptação transcultural e pré-teste. 1. Adaptação transcultural: foi composta por uma equipe de dois fonoaudiólogos e dois não-fonoaudiólogos, sendo responsáveis pela tradução para o português (um fonoaudiólogo e um não-fonoaudiólogo nativos do português brasileiro - PB e falantes do inglês), de modo independente, com posterior consenso realizado pelos pesquisadores; retrotradução para o inglês (um fonoaudiólogo e um não-fonoaudiólogo nativos do inglês e falantes do PB); análise da versão final por um comitê (um tradutor, um metodologista, e três fonoaudiólogos). 2. Etapa de pré-teste: os instrumentos foram aplicados em 38 indivíduos transgêneros (29 mulheres trans, 2 travestis e 7 homens trans), sendo acrescido na chave de resposta a opção "não aplicável". Os dados foram analisados de forma descritiva e inferencial. Resultados No processo de adaptação transcultural do VCS houve ajustes em cinco itens do questionário, quatro deles quanto à forma e um quanto ao conteúdo. Para o TSC também foram necessários ajustes quanto a forma em cinco itens. No pré-teste, para todos os itens dos instrumentos, a opção não aplicável apresentou proporção significativamente menor que as opções da chave de resposta dos instrumentos. Por fim, foi obtida uma versão traduzida e adaptada para o português dos dois instrumentos. Conclusão Os instrumentos foram transculturalmente adaptados para o português brasileiro e nomeados como Escala de Congruência Vocal e Escala de Congruência da Pessoa Transgênero.


ABSTRACT Purpose to carry out the cross-cultural adaptation of the Vocal Congruence Scale (VCS) and the Transgender Scale Congruence (TSC) instruments into Brazilian Portuguese. Methods the study was developed in two stages: cross-cultural adaptation and pre-test. 1. Cross-cultural adaptation: it was composed of a team of two speech therapists and two non-speech therapists, being responsible for the translation of the instruments into Portuguese (a speech therapist and a non-speech therapist native to Brazilian Portuguese - BP and English speakers, independently, with subsequent consensus achieved by the researchers; back-translation of the instruments into English (a speech therapist and a non-speech therapist who are native speakers of English and speakers of BP); analysis of the final version by a committee (a translator, a methodologist, and three speech therapists). Data were analyzed descriptively and inferentially. Results In the cross-cultural adaptation process of the VCS there were adjustments in five items of the questionnaire, four of them in terms of form and one in terms of content. necessary adjustments regarding form in five items. In the pre-test, for all VCS and TSC items, the non-applicable option had a significantly lower proportion than the instrument response key options (p<0.001, for all). Finally, a translated and adapted version for Brazilian Portuguese of the Vocal Congruence Scale (VCS) and the Transgender Scale Congruence (TSC) instruments was obtained. Conclusion The VCS and TSC were transculturally adapted to Brazilian Portuguese and named as Vocal Congruence Scale and Transgender Person Congruence Scale.

Resumo As pessoas trans estão expostas a diferentes situações de vulnerabilidade que se entrecruzam, causando impactos negativos no processo saúde-doença-cuidado. Para melhor compreender essa questão, buscamos conhecer as experiências de sete estudantes trans de uma universidade pública de Minas Gerais, a partir de entrevistas norteadas por roteiro semiestruturado. As falas foram analisadas pela perspectiva da análise de conteúdo, e as categorias finais foram tensionadas com estudos das ciências sociais e da saúde coletiva. De forma geral, as narrativas se diferenciaram daquelas que compõem a maioria das pesquisas sobre o tema no Brasil, sobretudo em relação à manutenção de vínculos familiares e ao acesso à universidade pública. Nesse sentido, formulamos a hipótese de que a interação desses elementos com outros marcadores de diferenças pode contribuir positivamente para o estabelecimento de alianças e estratégias, a fim de lidar com o regime de inteligibilidade das identidades de gênero e com as dificuldades de acesso à educação, ao trabalho e à saúde. Compreender essa dinâmica e suas limitações, em uma perspectiva em que o individual se enlaça ao coletivo, possibilitou evidenciar o funcionamento da (hétero)normatividade nos processos de segregação de pessoas trans, assim como refletir sobre ações políticas que possam transformar essa realidade social efetivamente.


Abstract Transgender people are exposed to different situations of vulnerability that intersect, having negative impacts on the health-illness-care process. To better understand this issue, we sought to know the experiences of seven transgender undergraduates at a public university in Minas Gerais, from semi-structured interviews. The narratives were analyzed on the perspective of content analysis, and the final categories juxtaposed with studies of social sciences and public health. In general, the narratives are different from those which account for most of the research on the theme in Brazil, especially regarding maintaining family bonds and the access to public universities. Hence, we formulated the hypothesis that the interaction between these elements and other markers of differences can positively contribute to establishing alliances and strategies to deal with the regime of gender identity intelligibility and the constraints of access to education, work, and health. Understanding these dynamics and their limitations, under the perspective in which the individual and the collective are intertwined, enabled us to highlight the role of (hetero)normativity in the segregation processes of transgender people, as well as reflecting on the political actions that might effectively transform this social reality.

Objective: To verify the relationship between smoking, age, schooling, and the vocal self-perception of trans women. Methods: Cross-sectional observational quantitative study conducted with 24 trans women over 18 years old, living in their affirmed gender for a minimum of 6 months. Data collection involved selected questions from the translated and authorized Portuguese version of the Trans Woman Voice Questionnaire (TWVQ) and information regarding age, education, occupation, and smoking status. All variables were analyzed descriptively, and the association with smoking was assessed using Pearson's Chi-square, Fisher's Exact, Student's T, and Mann-Whitney tests, with a significance level of 5%. Results: The mean age of trans women was 28.2 +/- 6.5 years (range: 21 - 48). Most participants (41.6%) had completed their high school education and pursued diverse careers. Regarding smoking habits, 58.3% of women were either current smokers or had smoked at least 100 cigarettes in their lifetime. A statistically significant association was found between smoking and age (p = 0.001), with smokers having a lower average age (24.9 years) compared to nonsmokers (32.9 years). However, no statistical significance was observed between smoking, education, and self-perception of vocal femininity. Only 9 (37.5%) trans women perceived their voices as feminine, while 17 (70.7%) desired a more feminine-sounding voice. Limitation: The limited sample size in this study may have constrained the ability to detect significant differences between the analyzed groups using statistical tests. Conclusion: The smoking prevalence was notably high among younger trans women. However, there was no statistically significant difference in vocal self-perception between those who smoked and those who did not. The majority of trans women expressed a desire for their voices to sound more feminine, particularly among smokers.


Objetivo: Verificar la relación entre el tabaquismo, edad, escolaridad y la autopercepción vocal de mujeres trans. Métodos: Estudio observacional transversal cuantitativo realizado con 24 mujeres trans mayores de 18 años, que viven en su género afirmado durante 6 meses. Para la recolección de datos se utilizaron preguntas seleccionadas de la versión portuguesa traducida y autorizada del Trans Woman Voice Questionnaire (TWVQ) y datos sobre edad, educación, ocupación y tabaquismo. Todas las variables se analizaron descriptivamente y la asociación con el tabaquismo se realizó mediante las pruebas Chi-cuadrado de Pearson, Exacta de Fisher, T de Student y Mann Whitney, con un nivel de significación del 5%. Resultados: La edad promedio de las mujeres trans fue de 28,2 +/- 6,5 (rango 21 - 48). La mayoría de las participantes (41,6%) había terminado la escuela secundaria con carreras muy diversas. Respecto al tabaquismo, el 58,3% de las mujeres fuman actualmente o han fumado al menos 100 cigarrillos en su vida. Hubo una asociación estadísticamente significativa entre el tabaquismo y la edad (p = 0,001), en la que la edad promedio entre los fumadores (24,9 años) fue menor que la de los no fumadores (32,9 años). No hubo significación estadística entre el tabaquismo, la educación y la autopercepción vocal. Solo 9 (37,5%) mujeres trans actualmente consideran su voz femenina y 17 (70,7%) dijeron que la voz ideal podría sonar más femenina. Limitación: La pequeña casuística puede haber limitado la identificación de diferencias entre los grupos analizados a través de pruebas estadísticas. Conclusión: El tabaquismo fue alto entre las mujeres trans, especialmente las más jóvenes. La autopercepción vocal no fue estadísticamente diferente entre los grupos de fumadores y no fumadores. La mayoría de las mujeres trans dijeron que sus voces podrían sonar más femeninas, especialmente las fumadoras.

The transgender and gender-diverse (TGD) population holds significant relevance concerning exercise and sports practices. Specific challenges are highlighted, such as physical inactivity driven by internal and external reasons, including discrimination in sports environments. Psychological aspects and hormonal therapy effects create implications for hindering participation in sports competitions. Disparities in cardiovascular risk factors are also mentioned, emphasizing the need for inclusive sports policies based on scientific evidence. The article underscores the importance of addressing social, psychological, and medical aspects to promote the physical and mental health of the TGD population, particularly in the professional sports area.


La población trans y de género diverso (TGD) tiene una gran relevancia en relación con la práctica de ejercicio y deporte. Se destacan desafíos específicos, como la inactividad física motivada por razones internas y externas, incluida la discriminación en entornos deportivos. Existen aspectos psicológicos y efectos de la terapia hormonal que generan implicaciones para la dificultar participación en competiciones deportivas. También se menciona la disparidad en los factores de riesgo cardiovascular y se plantea la necesidad de políticas deportivas inclusivas basadas en evidencia científica. El artículo enfatiza la importancia de abordar los aspectos sociales, psicológicos y médicos para promover la salud física y mental de la población TGD, especialmente en el ámbito deportivo profesional.

An incompatible voice with one's own identity is a theme frequently pointed out by the transgender population in speech therapy sessions. Speech therapy is a technique that allows the adjustment of the speaker's voice within a range of possibilities. The speech-language pathologist's role is to provide training, guidance, and counseling to this population, taking into account the specificities of each individual. In light of this, this study aims to report the experience of undergraduate students and speech-language professionals in providing care to the transgender population in a communication laboratory. Initially, key concepts, such as the differences between sex, gender, gender identity, and sexual orientation, are presented. Topics addressed include the violence suffered by the transgender population, the pursuit of vocal adaptation, the creation and development of the communication clinic, the support and assistance provided by professionals to the transgender population and their families, as well as the procedures adopted by the clinic. Among the conclusions, it is highlighted that speech therapy has demonstrated the importance of individual-centered care, legitimizing the guarantee of promoting the users' health. Furthermore, the importance of the speech-language professional in the vocal and communicative improvement of this population, which is often stigmatized, is emphasized (AU).


Voz incompatível com a própria identidade é um tema frequentemente apontado pela população transgênera em atendimentos fonoaudiológicos. A fonoterapia é uma técnica que permite a adequação da voz do falante, dentro de um campo de possibilidades. Ao fonoaudiólogo cabe o treinamento, a orientação e o aconselhamento dessa população, levando em conta as especificidades de cada indivíduo. Diante disso, este estudo tem como objetivo relatar a experiência de estudantes de graduação e profissionais de Fonoaudiologia no atendimento voltado à população transgênera em um laboratório de comunicação. Inicialmente são apresentados conceitos-chave, tais como as diferenças entre sexo, gênero, identidade de gênero, e orientação sexual. São abordados temas como a violência sofrida pela população trans, a busca pela adequação vocal, a criação e o desenvolvimento do ambulatório de comunicação, o acolhimento e o suporte prestado pelos profissionais à população trans e a seus familiares, além dos procedimentos adotados pelo ambulatório. Dentre as conclusões, destaca-se que a assistência fonoaudiológica tem mostrado a importância do cuidado centrado no indivíduo, legitimando a garantia da promoção de saúde dos usuários. Ademais, é destacada a importância do profissional de fonoaudiologia no aprimoramento vocal e comunicativo dessa população que é frequentemente estigmatizada (AU).

Transgênero (trans) é um termo que alberga toda a diversidade de gênero. A incongruência de gênero faz parte desse espectro e refere-se à pessoa cuja identidade de gênero é oposta ao sexo que lhe foi atribuído no nascimento. A terapia hormonal de afirmação de gênero, bem como a cirurgia de afirmação de gênero, é necessária para adequar o corpo ao gênero ao qual a pessoa se identifica. Os homens trans necessitam da terapia com testosterona, que visa reduzir as concentrações de estradiol e incrementar a testosterona circulante para níveis fisiológicos masculinos, resultando em masculinização. A mulher trans receberá o estradiol, associado ou não a um antiandrogênico, visando reduzir a testosterona e incrementar o estrogênio para níveis femininos, resultando em feminização. A cirurgia de afirmação de gênero é, frequentemente, requerida para completar as modificações fenotípicas para o homem e a mulher trans. O ginecologista e obstetra tem um papel crucial no provimento de cuidados a essa população. O presente artigo visa sistematizar algumas ações que o ginecologista e obstetra pode oferecer e que têm potencial para melhorar a qualidade de vida dos homens e mulheres trans. (AU)


Transgenero (trans) is an umbrella term that encompasses all gender diversity. Gender Incongruity is part of this spectrum and refers to the person whose gender identity is opposed to the sex assigned to them at birth. Gender-affirming hormone therapy as well as gender-affirming surgery are necessary to adapt the body to the gender to which the person identifies. Trans men require testosterone therapy to reduce estradiol concentrations and increase circulating testosterone to male physiological levels resulting in masculinization. Trans women will receive estradiol associated or not with an antiandrogenic to reduce testosterone and increase estrogen to female levels resulting in feminization. gender-affirming surgery is often required to complete phenotypic modifications for trans men and women. The gynecologist and obstetrician plays a crucial role in to provide care to this population. This article aims to systematize some actions that the gynecologist and obstetrician can offer to improve the quality of life of trans men and women. (AU)

RESUMEN INTRODUCCIÓN El acceso a la salud para las personas trans ha sido históricamente obstaculizado por el estigma y la discriminación institucional. A pesar de los avances legales y sociales en materia de reconocimiento de derechos de los últimos años, todavía se identifican barreras para su efectivo cumplimiento. Este trabajo tuvo como objetivo describir las principales barreras en la accesibilidad a los consultorios inclusivos (CI) y analizar las necesidades específicas de la población trans que se atiende en la provincia de Buenos Aires (PBA). MÉTODOS Se presenta un subanálisis de un estudio cualitativo más amplio realizado en 2017, cuyo objetivo fue relevar información sobre demandas sanitarias, accesibilidad y calidad de atención de los servicios de salud específicos para población de lesbianas, gays, bisexuales y trans (LGBT) en ocho efectores de la PBA. La muestra fue intencional, conformada por 29 entrevistas semiestructuradas a referentes y profesionales y 2 grupos focales con masculinidades y feminidades trans, respectivamente. Se realizó un análisis temático. RESULTADOS Se identificaron barreras organizacionales y simbólicas, sobre todo en los CI que funcionan en hospitales. Entre las simbólicas, se evidencia la persistencia de prácticas patologizantes en salud mental. DISCUSIÓN Los CI brindan una respuesta satisfactoria, pero transitoria. Es necesario transversalizar la perspectiva de género a todas las intervenciones en salud.


ABSTRACT INTRODUCTION Access to health for trans people has historically been hampered by institutional stigma and discrimination. Despite the legal and social advances achieved in recent years in terms of recognition of rights, barriers to effective enforcement are still identified. The objective of this work was to describe the main barriers to access inclusive healthcare services and analyze the specific needs of trans people receiving healthcare in the province of Buenos Aires (PBA). METHODS A subanalysis of a wider qualitative study conducted in 2017 is presented, its objective was to collect information on healthcare needs, accessibility and quality of care of health services specific for lesbian, gay, bisexual and transgender (LGBT) population in eight healthcare providers of PBA. The sample was intentional, including 29 semi-structured interviews to social organization leaders and professionals, and 2 focus groups with trans masculinities and femininities, respectively. Data were analyzed using thematic analysis. RESULTS Organizational and symbolic barriers were identified, especially in inclusive healthcare services functioning in hospitals. One of the symbolic ones was the persistence of pathologizing practices in mental healthcare. DISCUSSION Inclusive healthcare services provide a satisfactory, but temporary response. It is necessary to adopt a cross-cutting gender perspective for all healthcare interventions.

Resumen El presente estudio tuvo por objetivo analizar las características de artículos científicos publicados durante el último quindenio respecto a la atención en salud a personas trans en América Latina y el Caribe. Se desarrolló una revisión sistematizada de artículos publicados en cuatro bases de datos, entre los años 2005 y 2020, en la que se encontraron 20 estudios que fueron objeto de análisis. En su mayoría, estos dan cuenta de las percepciones que tienen las comunidades trans y, en menor frecuencia, las experiencias de los profesionales de la salud sobre la atención en salud. Los resultados permiten identificar barreras que influyen en los procesos de atención y los desafíos para mejorarla. Sobresale la formación deficiente e insuficiente en los profesionales sobre identidades trans y los enfoques diferenciales y afirmativos del género, y la necesidad de fortalecer los abordajes integrales entre las comunidades académicas, de la salud y la generación de políticas públicas transformativas.


Abstract Health care is important for the transitions of people with trans life experiences and to strengthen the processes of health and disease. Health professionals are educating and accompanying agents; however, the literature frequently reports that there are barriers perceived by professionals regarding their competencies to accompany such processes, in addition to others related to social representations regarding trans identities, the limited scientific evidence regarding the processes of gender reaffirmation and representing the communities themselves according to their historical and sociocultural characteristics. The aim of this study was to analyze the characteristics of scientific articles published during 15 years regarding health care for trans persons in Latin America and the Caribbean. A systematized review of articles published in four databases between 2005 and 2020 was carried out. 20 studies were analyzed in the research. Most of them report the perceptions of trans communities and, less frequently, the experiences of health professionals on health care. The results allow the identification of barriers that influence the processes of care and the challenges to improve it. The deficient and insufficient training of professionals on trans identities and differential and affirmative gender approaches stand out, as well as the need to strengthen comprehensive approaches among the academic and health communities and the generation of trans affirmative public policies. On the other hand, the results evidence a construction of science with respect to gender that is cisnormative and invisibilizes gender diversities. In addition, many of the studies establish vertical relationships between participants, including researchers, which generates research dynamics in which trans communities are considered more as informants and less as expert peers, community researchers or participants in a collaborative and co-creative research process. These findings support the need and renew the urgency to generate processes of knowledge construction with and for trans communities that allow nurturing educational, social, public policy and health spaces that represent their needs for gender transitions, in case the person considers so, and that welcome in an integral and humanized way the processes of health and disease. Research on health and health care in trans communities both in Colombia and in the rest of Latin America and the Caribbean does not show the implementation of health policies focused on the community and that respond to their health needs. Health professionals, around the world, should accompany from care actions focused on the processes of self-identification and self-determination of gender. Being recognized and made visible based on gender identities and expressions has a positive impact on the mental health of trans people and their well-being. In professional practice, it is important and essential to overcome the barriers of social inequality, promoting a critical social vision and understanding of the reality of this population. It is necessary to expand research from gender diversities and separate from the LGBTIQ+ umbrella given the complex social, cultural, biological, and psychological diversities faced by each community. As for health professionals, it is necessary to deepen the measurement and understanding of attitudes, knowledge, beliefs, skills and gaps identified by all staff, from doctors and nurses to institution managers. This in order to articulate and triangulate information and develop intervention programs aimed at comprehensive and accessible care, especially for their reports about the little information on sexual and gender diversity received during their professional training process and organizational culture. Finally, each country should develop clinical guidelines based on local, national, and international evidence that responds to the psychological and sociocultural needs and experiences of trans communities that include the process of gender transitions from hormone therapy, affirmative surgeries and other non-binary processes of gender expressions, family, psychosocial support, and community accompaniment.

OBJETIVO: Analisar a percepção de pessoas transexuais sobre as situações preconceituosas vivenciadas no seu cotidiano. MÉTODO: Estudo descritivo, com abordagem qualitativa, realizado presencialmente entre os meses de junho e dezembro de 2018, com 25 pessoas transexuais dos municípios de Petrolina/PE e Juazeiro/BA. A coleta de dados ocorreu por meio de entrevistas semiestruturadas, que foram gravadas e transcritas na íntegra e esses dados analisados por meio da Análise de Conteúdo temática. RESULTADOS: Verificou-se que as pessoas transexuais percebem a ocorrência de situações de violência em seu cotidiano, evidenciada de diversas formas. Ao indagar sobre a violência e o preconceito sofrido, foi mencionado o medo, a visão diferente e preconceituosa das pessoas, as dificuldades no uso de banheiros públicos e a falta de informação quanto à diferenciação dos subgrupos da sigla LGBTQIA+. Quanto às dificuldades enfrentadas, foram citadas as barreiras de acesso à saúde, educação, inserção no mercado de trabalho e relacionamento familiar. CONCLUSÃO: Assim, considerando a complexidade dessa problemática na região estudada e no país como um todo, torna-se fundamental a participação política desses indivíduos em movimentações sociais da diversidade sexual e de gênero, de forma a se buscar a conscientização da sociedade a respeito dessa diversidade e, a partir disso, propor o planejamento e execução de ações que visem diminuir as dificuldades de pessoas transexuais em relação à garantia dos direitos fundamentais.


OBJECTIVE: To analyze the perception of transgender people about the prejudiced situations they experience in their daily lives. METHOD: Descriptive study, with a qualitative approach, carried out in person between June and December 2018, with 25 transgender people from the municipalities of Petrolina/PE and Juazeiro/BA. Data collection took place through semi-structured interviews, which were recorded and transcribed in full and these data were analyzed using thematic Content Analysis. RESULTS: It was found that transgender people perceive the occurrence of situations of violence in their daily lives, evidenced in different ways. When inquiring about the violence and prejudice suffered, fear, the different and prejudiced view of people, the difficulties in using public restrooms and the lack of information regarding the differentiation of subgroups of the acronym LGBTQIA+ were mentioned. As for the difficulties faced, barriers to access to health, education, insertion in the labor market and family relationships were cited. CONCLUSION: Thus, considering the complexity of this problem in the region studied and in the country as a whole, the political participation of these individuals in social movements of sexual and gender diversity becomes essential, in order to seek society's awareness of this diversity and, from this, propose the planning and execution of actions that aim to reduce the difficulties of transgender people in relation to the guarantee of fundamental rights.


OBJETIVO: Analizar la percepción de las personas transgénero sobre las situaciones de prejuicio que viven en su vida cotidiana. MÉTODO: Estudio descriptivo, con enfoque cualitativo, realizado de forma presencial entre junio y diciembre de 2018, con 25 personas transgénero de los municipios de Petrolina/PE y Juazeiro/BA. La recolección de datos ocurrió a través de entrevistas semiestructuradas, que fueron grabadas y transcritas en su totalidad y estos datos fueron analizados mediante el Análisis de Contenido temático. RESULTADOS: Se constató que las personas trans perciben la ocurrencia de situaciones de violencia en su cotidiano, evidenciado de diferentes formas. Al indagar sobre la violencia y los prejuicios sufridos, se mencionó el miedo, la mirada diferente y prejuiciosa de las personas, las dificultades para utilizar los baños públicos y la falta de información respecto a la diferenciación de subgrupos de las siglas LGBTQIA+. En cuanto a las dificultades enfrentadas, se mencionaron las barreras de acceso a la salud, la educación, la inserción en el mercado laboral y las relaciones familiares. CONCLUSIÓN: Así, considerando la complejidad de esta problemática en la región estudiada y en el país en su conjunto, la participación política de estas personas en los movimientos sociales de la diversidad sexual y de género se vuelve fundamental, con el fin de buscar la conciencia de la sociedad sobre esta diversidad y, a partir de ello, proponer la planificación y ejecución de acciones que apunten a reducir las dificultades de las personas transgénero en relación a la garantía de los derechos fundamentales.

OBJECTIVE: The impact of gender-affirming hormone therapy (GAHT) on cardiovascular (CV) health is still not entirely established. A systematic review was conducted to summarize the evidence on the risk of subclinical atherosclerosis in transgender people receiving GAHT. METHODS: A systematic review was performed following Preferred Reporting Items for Systematic Reviews and Meta-analyses guidelines, and data were searched in PubMed, LILACS, EMBASE, and Scopus databases for cohort, case-control, and cross-sectional studies or randomized clinical trials, including transgender people receiving GAHT. Transgender men and women before and during/after GAHT for at least 2 months, compared with cisgender men and women or hormonally untreated transgender persons. Studies reporting changes in variables related to endothelial function, arterial stiffness, autonomic function, and blood markers of inflammation/coagulation associated with CV risk were included. RESULTS: From 159 potentially eligible studies initially identified, 12 were included in the systematic review (8 cross-sectional and 4 cohort studies). Studies of trans men receiving GAHT reported increased carotid thickness, brachial-ankle pulse wave velocity, and decreased vasodilation. Studies of trans women receiving GAHT reported decreased interleukin 6, plasminogen activator inhibitor-1, and tissue plasminogen activator levels and brachial-ankle pulse wave velocity, with variations in flow-mediated dilation and arterial stiffness depending on the type of treatment and route of administration. CONCLUSIONS: The results suggest that GAHT is associated with an increased risk of subclinical atherosclerosis in transgender men but may have either neutral or beneficial effects in transgender women. The evidence produced is not entirely conclusive, suggesting that additional studies are warranted in the context of primary prevention of CV disease in the transgender population receiving GAHT. This systematic review and meta-analysis investigated the effectiveness of strategies and types of tests on HIV testing uptake by men who have sex with men (MSM) and transgender women (TGW), and in reaching PLWH. Articles published up to July 2020 were identified from major electronic databases and grey literature. Data were extracted and assessed for risk of bias. Estimates were pooled using random-effect meta-analysis while heterogeneity was evaluated by Cochran's Q test and I2. This study is registered with PROSPERO (CRD42020192740). Of 6820 titles, 263 studies (n = 67,288 participants) were included. The testing strategies reported in most studies were community- (71.2%) and facility-based (28.8%). Highest uptake, with facility-based testing, occurred and reached more PLWH while with standard laboratory tests, it occurred with the highest HIV prevalence among MSM. However, urine test showed a highest rate of new HIV infection. Multiple test combinations had the highest uptake and reached more PLWH among TGW. Various testing strategies, considering barriers and regional differences, and different test types, need be considered, to increase uptake among MSM and TGW.


RESUMEN: Esta revisión sistemática y metaanálisis tuvo como objetivo investigar la efectividad de las estrategias y los tipos de pruebas para la realización de la prueba del VIH entre hombres que tienen sexo con hombres (HSH) y mujeres transgénero (MT), y su alcance a personas que viven con el VIH (PVV). Los artículos publicados hasta julio de 2020 fueron identificados utilizando base de datos y literatura gris; se extrajeron los datos de los artículos y se evaluó el riesgo de sesgo. Las estimaciones se agruparon mediante un metaanálisis de efectos aleatorios, mientras que la heterogeneidad fue evaluada utilizando la prueba Q de Cochran y la prueba I2. Este estudio está registrado en PROSPERO (CRD42020192740). De 6820 títulos, fueron incluidos 263 estudios (n = 67,288 participantes). En la mayoría, las estrategias reportadas fueron comunitarias (71.2%) y en establecimientos de salud (28.8%). Las pruebas realizadas en establecimientos de salud tuvieron mayor captación y mayor alcance de PVV mientras que con las pruebas de laboratorio estándar se detectó una mayor prevalencia del VIH entre los HSH. Sin embargo, la prueba de orina mostró una tasa más elevada de nuevas infecciones por el VIH. La combinación de múltiples pruebas tuvo mayor captación y tuvo alcance a PVV entre MT. Es necesario considerar las diferentes estrategias, tomando en cuenta las barreras y diferencias regionales, así como los diferentes tipos de pruebas, para incrementar la realización de pruebas del VIH entre HSH y MT.

ABSTRACT Objective: to understand what undergraduate Nursing students from a university in southern Brazil know about trans people. Method: a descriptive study with a qualitative approach based on interviews conducted with 19 undergraduate Nursing students enrolled from the eighth to the tenth period of the course. Data collection took place in October and November 2021. Three thematic categories emerged from the thematic analysis, namely: the undergraduate course has to speak about this!; I don't know what it is about; and What is known and said about trans people. Results: the students showed insecurity in the care of trans people because this content is not regularly taught in the curriculum. They denote not knowing how to welcome, care for, respect specificities and treat them by their social name, as well as difficulties taking care of this population group, both in the present time and in the future. In addition to that, few of them had the opportunity of undergoing this experience in the academic practice fields. The content is taught in a single academic discipline that deals with sexuality in general. Conclusion: lack of knowledge about sexual diversity is a limiting factor in the health care provided to this population group. Certain weakness is identified in the teaching-learning process when not relating the reality of the trans population to their health demands.


RESUMEN Objetivo: comprender el conocimiento de los estudiantes de grado de Enfermería de una universidad del sur de Brasil en relación a las personas trans. Método: estudio descriptivo de enfoque cualitativo a partir de entrevistas realizadas con 19 estudiantes de grado de Enfermería inscritos en el octavo a décimo período del curso. La recolección de datos tuvo lugar en octubre y noviembre de 2021. Surgieron 3 categorías del análisis temático: ¡En la carrera de grado se debe hablar de esto!; No sé de qué se trata; y Lo que se sabe y dice sobre las personas trans. Resultados: los estudiantes se muestran inseguros en la atención de las personas trans debido a que este contenido no se ofrece regularmente en el plan de estudio. Denotan desconocimiento sobre como recibir a las personas trans, atenderlas, respetar sus especificidades y tratarlas por su nombre social, además de dificultades en el cuidado de este grupo poblacional, tanto en el presente como en el futuro. Además, pocos participantes tuvieron la oportunidad de vivir esta experiencia en los campos de práctica académica. El contenido solo se dicta en una disciplina que trata el tema dela sexualidad en general. Conclusión: el desconocimiento acerca de la diversidad sexual es un factor limitante en la atención de la salud que se proporciona a este grupo poblacional. Se identifica cierta fragilidad en el proceso de enseñanza-aprendizaje ya que no se relaciona la realidad de la población trans con sus necesidades en materia de salud.


RESUMO Objetivo: compreender o conhecimento dos estudantes de graduação de enfermagem em uma universidade do sul do Brasil no tocante às pessoas trans. Método: estudo descritivo com abordagem qualitativa a partir de entrevistas realizadas com 19 estudantes de graduação em enfermagem matriculados do oitavo ao décimo período do curso. A coleta de dados ocorreu nos meses de outubro e novembro de 2021. Da análise temática emergiram 3 categorias: o curso de graduação precisa falar disso!; eu não sei do que se trata; e o que se sabe e se diz sobre as pessoas trans. Resultados: os estudantes demonstram insegurança no cuidado das pessoas trans devido ao não oferecimento regular deste conteúdo no currículo. Denotam desconhecimento de como acolher, cuidar, respeitar as especificidades, tratar pelo nome social, e também dificuldades em cuidar desta população no presente e no futuro. Além disso, poucos tiveram a oportunidade de vivenciar esta experiência nos campos de prática acadêmica. O conteúdo é oferecido em uma única disciplina que trata da sexualidade em geral. Conclusão: o desconhecimento acerca da diversidade sexual é um fator limitador na assistência à saúde a esta população. Identifica-se uma fragilidade no ensino-aprendizagem ao não relacionar a realidade da população trans com suas demandas de saúde.

O objetivo deste estudo foi avaliar os sintomas ansiosos em indivíduos transexuais atendidos em um ambulatório de gênero e sexualidade. Caracteriza-se como um estudo descritivo, de corte transversal e abordagem quantitativa, adotando amostra por conveniência (n=46). Foi realizado nas dependências de um Hospital Universitário Nordestino mediante aplicação do Inventário de Ansiedade de Beck (BAI) e formulário sociodemográfico, os dados coletados foram trabalhados no EpiInfo. Os resultados revelaram trans em sua maioria jovens, de cor parda, solteiros, com ensino médio completo, estudantes, residiam com pais e/ou parentes, não religiosos, e metade com vínculo empregatício. A ansiedade leve esteve presente em 21,7% e mínima em 34,8% dos pacientes, enquanto, a moderada em 28,3% e a grave 15,2%. Identificou-se relevância estatística na associação entre ansiedade e idade (p= 0,0072), e de ansiedade com vínculo empregatício (p=0,0039). Portanto, demonstrou-se que a maioria dos entrevistados apresentaram com menor severidade sintomas ansiosos, podendo ter relação com seu maior grau de instrução, presença de suporte social e presença de vínculo de trabalho.


The objective of this study was to evaluate anxious symptoms in transgender individuals attended in an outpatient clinic of gender and sexuality. It is characterized as a descriptive, cross-sectional and quantitative study, adopting a sample for convenience (n=46). It was carried out in the premises of a Northeastern University Hospital by applying the Beck Anxiety Inventory (BAI) and sociodemographic form, the data collected were worked on at EpiInfo. The results revealed that the majority of them were young, of a brown color, single, with a full high school education, students, resided with parents and/or relatives, non-religious, and half with employment. Mild anxiety was present in 21.7% and minimal in 34.8% of the patients, while moderate in 28.3% and severe in 15.2%. Statistical relevance was identified in the association between anxiety and age (p= 0.0072), and anxiety with employment (p=0.0039). Therefore, it was demonstrated that the majority of the interviewees presented with less severity anxious symptoms, which may have a relationship with their higher degree of education, presence of social support and presence of a work bond.


El objetivo de este estudio fue evaluar los síntomas ansiosos en los individuos transgénero atendidos en una clínica ambulatoria de género y sexualidad. Se caracteriza por un enfoque descriptivo, transversal y cuantitativo, tomando una muestra para su conveniencia (n=46). Se realizó en las instalaciones de un Hospital Universitario del Noreste, aplicando el Inventario de Ansiedad Beck (BAI) y el formulario sociodemográfico, los datos recogidos se trabajaron en EpiInfo. Los resultados revelaron transexuales en la mayoría de los jóvenes, de color marrón, solteros, estudiantes de secundaria, que vivían con padres y/o parientes, no religiosos y la mitad con un vínculo laboral. La ansiedad leve fue del 21,7% y mínima del 34,8% de los pacientes, mientras que la moderada fue del 28,3% y la grave del 15,2%. Se identificó la relevancia estadística en la asociación entre ansiedad y edad (p= 0,0072) y ansiedad con relación al empleo (p=0,0039). Por lo tanto, se demostró que la mayoría de los entrevistados presentaban síntomas menos ansiosos, que podían tener relación con su nivel de educación superior, la presencia de apoyo social y la presencia de un vínculo de trabajo.

Resumen La población trans y de género diverso (TGD) tiene una gran relevancia en relación con la práctica de ejercicio y deporte. Se destacan desafíos específicos, como la inactividad física motivada por razones internas y externas, incluida la discriminación en entornos deportivos. Existen aspectos psicológicos y efectos de la terapia hormonal que generan implicaciones para la dificultar participación en competiciones deportivas. También se menciona la disparidad en los factores de riesgo cardiovascular y se plantea la necesidad de políticas deportivas inclusivas basadas en evidencia científica. El artículo enfatiza la importancia de abordar los aspectos sociales, psicológicos y médicos para promover la salud física y mental de la población TGD, especialmente en el ámbito deportivo profesional.


Abstract The transgender and gender-diverse (TGD) population holds significant relevance concerning exercise and sports practices. Specific challenges are highlighted, such as physical inactivity driven by internal and external reasons, including discrimination in sports environments. Psychological aspects and hormonal therapy effects create implications for hindering participation in sports competitions. Disparities in cardiovascular risk factors are also mentioned, emphasizing the need for inclusive sports policies based on scientific evidence. The article underscores the importance of addressing social, psychological, and medical aspects to promote the physical and mental health of the TGD population, particularly in the professional sports area.

Objetivo: compreender os pontos positivos e negativos na assistência à saúde mental às mulheres transgêneros na atenção psicossocial. Método: pesquisa descritiva e exploratória qualitativa, com participação de três mulheres transgênero assistidas por três Centros de Atenção Psicossocial Álcool e Drogas (CAPSad) de um município da região central do Brasil. Realizaram-se entrevistas individuais semiestruturadas, submetidas à análise de conteúdo temática. Resultados: pontos positivos são: bom relacionamento interpessoal, variedade de atividades terapêuticas, aceitação, atenção e respeito à singularidade das usuárias, a continuidade do cuidado em liberdade e o encaminhamento para o processo de transexualização. Os pontos negativos são: relacionamento interpessoal terapêutico ineficaz de alguns profissionais, tempo de acolhimento noturno reduzido e poucas atividades manuais. Considerações finais: a pesquisa evidencia que apesar das potencialidades do cuidado a mulheres transgênero, os pontos negativos reforçam que os profissionais que atuam em CAPSad necessitam de educação permanente em saúde pautada na lógica de cuidado psicossocial.


Objective: to understand the positive and negative aspects of mental health care for transgender women in psychosocial care. Method: descriptive and exploratory qualitative research, with the participation of three transgender women assisted by three Psychosocial Care Centers for Alcohol and Drugs (CAPSad) in a city in the central region of Brazil. Semi-structured individual interviews were carried out, submitted to thematic content analysis. Results: positive points are: good interpersonal relationships, variety of therapeutic activities, acceptance, attention and respect for the uniqueness of users, continuity of care in freedom and referral to the transsexualization process. The negative points are: ineffective therapeutic interpersonal relationship of some professionals, reduced night care time and few manual activities. Final considerations: the research shows that despite the potential of care for transgender women, the negative points reinforce that professionals working in CAPSad need permanent health education based on the logic of psychosocial care.

Método: investigación cualitativa descriptiva y exploratoria, con la participación de tres mujeres transgénero asistidas por tres Centros de Atención Psicosocial para Alcohol y Drogas (CAPSad) en una ciudad de la región central de Brasil. Se realizaron entrevistas individuales semiestructuradas, sometidas a análisis de contenido temático. Resultados: los puntos positivos son: buenas relaciones interpersonales, variedad de actividades terapéuticas, aceptación, atención y respeto a la singularidad de los usuarios, continuidad de la atención en libertad y derivación al proceso de transexualización. Los puntos negativos son: relación interpersonal terapéutica ineficaz de algunos profesionales, tiempo reducido de atención nocturna y pocas actividades manuales. Consideraciones finales: la investigación muestra que a pesar del potencial de atención a mujeres transgénero, los puntos negativos refuerzan que los profesionales que actúan en el CAPSad necesitan educación permanente en salud basada en la lógica de la atención psicosocial.
