BACKGROUND: Chiari malformation type I(CM-I) is one of the most common neurological malformations observed in patients with adolescent idiopathic scoliosis (AIS). It remains uncertain whether AIS patients without neurological symptoms require CM-I decompression before undergoing spinal corrective surgery to prevent cerebellar tonsil herniation and potential neurological complications. This study aims to assess variations in the position of the cerebellar tonsil after posterior spinal fusion (PSF) in neurologically intact patients with AIS. METHODS: We retrospectively evaluated 40 patients with AIS who underwent posterior spinal fusion (PSF) without neurological symptoms. Sagittal hindbrain MRIs were performed before and after spinal surgery. The cerebellar tonsil's position relative to the foramen magnum was measured using the method described by Aboulezz (J Comput Assist Tomogr 1985). We assessed variations in the cerebellar tonsil position concerning spinal correction and elongation after PSF. RESULTS: The average preoperative magnitude of the scoliotic curve was 53.15 degrees (SD 10.46 degrees), and that of thoracic kyphosis was 35.42 degrees (SD 12.38 degrees). In the immediate postoperative period, the average magnitudes were 7.45 degrees (SD 7.33 degrees) and 27.87 degrees (SD 9.03 degrees), respectively. The average correction achieved with PSF was 86% in the coronal plane (p < 0.00001). The average length of the spine in the coronal plane was 44.5 cm (SD 5.25 cm) preoperatively and 48.27 cm (SD 4.40 cm) postoperatively (p < 0.00001). The average length of the vertebral column in the sagittal plane was 50.87 cm (SD 4.47 cm) preoperatively and 55.13 cm (SD 3.27 cm) postoperatively (p < 0.00001). No significant difference was observed in the position of the cerebellar tonsil in the measurements taken before and after spinal deformity correction (p = 0.6042). In 10 out of 40 patients, caudal displacement of the cerebellar tonsil was observed after PSF. In 28 patients, no variation in the position of the cerebellar tonsil was observed, and in 2, cephalic displacement was noted. CONCLUSION: In AIS patients with a normal position of the cerebellar tonsil undergoing posterior spinal fusion, there is no significant change in their position after surgery. Our analysis did not reveal any correlation between curve correction, spine elongation, and variations in the cerebellar tonsil position. SIGNIFICANCE: The position of the cerebellar tonsil remained largely unchanged after PSF in AIS patients without neurological symptoms. Further studies would be necessary to determine whether surgical decompression is required in asymptomatic patients with CM-I before undergoing surgery to correct spinal deformity.

La escoliosis idiopática del Adolescente (EIA) es una deformidad tridimensional de la columna vertebral que aqueja de diversas formas la condición de vida de quien la padece, afectando el desarrollo físico y psicológico del paciente. Proponemos evaluar el resultado clínico, radiológico y el componente de calidad de vida de pacientes tratados quirúrgicamente con EIA, en el servicio de Neurocirugía, Hospital Jesús Yerena del Lídice. Se evaluó patrón de la curva, clasificación según Lenke, ángulo de Cobb pre y postquirúrgico, edad de tratamiento quirúrgico, gradode corrección, calidad de vida según cuestionario SRS-22 pre y postquirúrgica, valoración funcional mediante la escala de discapacidad de Oswestry (ODI) y complicaciones postoperatorias. Se estudiaron 22 casos (95 % femenino), el principal grupo etario fue de 11-14 años (55 %), la escoliosis tipo 1 fue la predominante (60 %), el ángulo de Cobb inicial fue mayor a 50° (50 %) y la madurez esquelética principal fue Risser 3 (55 %). La edad de resolución quirúrgica predominante fue entre 15-18 años (60 %), el grado de corrección postoperatorio fue de 75-100 % (73 %) y la densidad de los tornillos pediculares fue de 1.4-1.7 (64 %). En el cuestionario SRS-22, hubo mayor afectación en las dimensiones de autoimagen con una media de 1.8 y la valoración por ODI mostró un índice de incapacidad mínima, que fue mejorando en los controles sucesivos. Nuestra principal complicación fueron las infecciones (36 %). Concluimos que la cirugía correctiva temprana en la EIA ofrece a una mejoría clínica, radiológica y de la calidadde vida de estos paciente(AU)
Adolescent idiopathic scoliosis (AIS) is a three-dimensional deformity of the spine that affects the living condition of those who suffer from it in various ways, affecting the physical and psychological development of the patient. We propose to evaluate the clinical and radiological outcome and the quality-of-life component of patients treated surgically with EIA, in the Neurosurgery Service, Hospital Jesús Yerena del Lídice. Curve pattern, Lenke classification, pre-and post-surgical Cobb angle, age at surgical treatment, degree of correction, quality of life according to pre-and post-surgical SRS-22 questionnaire, functional assessment using the Oswestry Disability Scale (ODI) and postoperative complications were evaluated. 22 cases were studied (95% female), the main age group was 11-14 years (55%), type 1 scoliosis was predominant (60%), the initial Cobb angle was greater than 50° (50%) and the main skeletal maturity was Risser 3 (55%). The predominant age of surgical resolution was between 15-18 years (60%), the degree of postoperative correction was 75-100% (73%) and the density of pedicle screws was 1.4-1.7 (64%). In the SRS-22 questionnaire, there was greater impairment in the self-image dimensions with a mean of 1.8 and the ODI assessment showed aminimal disability index, which improved in successive controls. Our main complication was infections (36%). We conclude that early corrective surgery in AIS offers clinical, radiological and quality of life improvement for these patient(AU)

PURPOSE: To evaluate whether there is a mismatch between Risser staging and the proximal humerus ossification system (PHOS); and to analyze the correlation in the skeletal maturity stages between the two humeral epiphyses. METHODS: Data from patients aged 10 to 18 years with adolescent idiopathic scoliosis (AIS) seen between 2018 to 2021 were analyzed. In an anteroposterior (AP) spine radiograph the ossification process was evaluated using the Risser classification method and bilateral PHOS (if both humeral epiphyses were visualized). A mismatch between methods was defined as a Risser 0-1 (relatively skeletally immature) with a PHOS 4-5 (skeletally mature), or a Risser 2-5 (relatively skeletally mature) with a PHOS 1-3 (skeletally immature). The McNemar test was used to calculate the significance of the mismatch. RESULTS: A mismatch between Risser and PHOS stages was observed in 28.5% of 105 patients, which was statistically significant (p < 0.001). Of the 49 patients with a Risser 0-1, 55.1% (n = 27) had a PHOS 4-5. None of the patients with a Risser 2-5 had a PHOS 1-3. In the 47 patients in whom both humeri were visualized, the absolute correlation between the left and right PHOS values was 95.7%. CONCLUSION: Of AIS patients who are relatively skeletally immature according to Risser staging, more than half may be skeletally mature when measured with PHOS. In patients with a Risser 0-1, it is recommended to measure skeletal maturity in an AP spine radiograph using the PHOS method, which may more accurately guide treatment decision-making, without the need to visualize both humeral epiphyses in this radiographic projection. PURPOSE: Abnormal leptin bioavailability has play key roles in the etiology of adolescent idiopathic scoliosis (AIS). Both leptin and its receptor levels may be modulated by the presence of genetic polymorphisms. This study aimed to evaluate the role of polymorphisms in the leptin (LEP) and its main receptor (LEPR) genes in the AIS susceptibility in girls. METHODS: A retrospective case-control study was conducted with 189 AIS and 240 controls. LEP rs2167270 and LEPR rs2767485 polymorphisms were genotyped using a TaqMan validated assay. Associations were evaluated by odds ratios (OR) and 95% confidence intervals (CI). RESULTS: The AIS group showed a predominance of girls under 18 years old (n = 140, 74.1%), 148 (78.3%) had low or normal BMI, 111 (58.7%) had Cobb ≥ 45º and 130 (68.7%) were skeletally mature. Minor allele frequencies of rs2167270 and rs2767485 were 35.7% and 18.3%, for AIS and 35.6% and 25.4% for controls, respectively. LEPR rs2767485 T and TC + TT were associated with higher risk of AIS (OR = 1.53; 95% CI = 1.09-2.13 and OR = 1.84; 95% CI = 1.69-2.01, respectively), since CC genotype was only present in the control group. In addition, the LEP rs2167270 GA + AA was more frequent in low weight group (BMI ≤ 24.9) of girls with AIS. There was no significant association between LEP rs2167270 and AIS susceptibility, and LEPR rs2767485 and BMI. CONCLUSION: The LEPR rs2767485 was associated with the genetic susceptibility of AIS and LEP rs2167270 with low BMI. These data can contribute to the identification of genetic biomarkers to improve the diagnosis and treatment.

BACKGROUND: Fibrillin-1 (FBN1) is an extracellular matrix glycoprotein essential to the structural component of microfibrils and FBN1 gene polymorphisms can be associated with adolescent idiopathic scoliosis (AIS) susceptibility. This study aimed to evaluate the potential role of the FBN1 rs12916536 polymorphism in AIS development or severity and the variation in Cobb angle in relation to patient's characteristics. METHODS: DNA from 563 subjects (185 AIS patients and 378 controls) were genotyped using a validated TaqMan allelic discrimination assay. A multivariate logistic regression model evaluated the association between polymorphism and AIS, using the adjusted odds ratios (OR) with their respective 95% confidence intervals (95% CI). A linear regression analysis evaluated the variation in Cobb angle according to the patient's age and body mass index (BMI). RESULTS: Among the AIS group there was a predominance of females (12:1), low or normal BMI (90%), 58% had a Cobb angle greater than 45° and 74% were skeletally mature. Age was a risk factor (4-fold) for curve progression higher than BMI (P < 0.001). The allelic frequency of the rs12916536 G > A polymorphism was 40% in controls and 31% in AIS cases; and this difference was statistically significant (P = 0.004). FBN1 rs12916536 GA + AA genotypes were associated with a lower risk of AIS susceptibility (OR = 0.58 and 95% CI = 0.35-0.98), after adjustment for age, sex and BMI. However, no significant differences were detected in polymorphism distribution with the severity of the disease (Cobb < 45° or ≥ 45°). CONCLUSION: Age was a risk factor for progression of the scoliotic curve and FBN1 rs12916536 polymorphism a protective factor for AIS susceptibility.

STUDY DESIGN: A retrospective, comparative study. OBJECTIVE: To determine the radiological behavior of the lumbar curve in selective fusions in premenarchal girls with adolescent idiopathic scoliosis (Lenke 1 B/C). METHODS: A retrospective, comparative study was conducted. Selective fusion was performed in 21 patients younger than 18 years. The patients were divided into 2 groups: group A, after menarche (n = 12) and group B, before menarche (n = 9). Angles (preoperative, and at 1 and 2 years postoperatively) of the fused thoracic curves and the corresponding lumbar curves were measured and compared. For statistical analysis, the t test was used with a significance level of P < .05. RESULTS: Mean preoperative angle value of the proximal/main thoracic curve was 61° in group A and 57° in group B (P = .44), and 21° and 20°, respectively, in the first year postoperatively (P = .61). Mean preoperative angle value of the lumbar curve was 43° in group A and 42° in group B (P = .87), while at 1 year after surgery, this curve was 19° in both groups (P = 0.91), and at 2 years postoperatively, the curve was 16° in group A and 17° in group B (P = .75). CONCLUSIONS: Over a 2-year follow-up, we did not find significant radiological differences in lumbar curves between patients who underwent surgery before and after menarche.

OBJECTIVE: To assess the effectiveness of preoperative magnetic resonance imaging (MRI) in adolescent idiopathic scoliosis (AIS) patients with unremarkable history and physical examination. METHODS: The imaging data of consecutive patients with presumed AIS treated with a posterior spinal fusion between 2010 and 2016 were reviewed. The presence of traditional risk factors, atypical curve patterns, and its association with relevant abnormalities on MRI were investigated. The number needed to diagnose (NND) and the number needed to misdiagnose (NNM) were calculated to measure MRI effectiveness. RESULTS: A total of 198 consecutive patients were identified and divided according to the presence of MRI findings. Both groups predominantly consisted of females, with a mean age of 15 years and right thoracic curvature. Neural axis abnormalities were detected in 25 patients, and the groups had a similar proportion of atypical findings, as curve magnitude, thoracic kyphosis, curve direction, and sex. The NND was 7.9 patients and NNM was 66 patients, meaning that the management was changed before the spine fusion in 12% of patients with neural axis abnormalities. None of the traditional risk factors could predict a higher incidence of neural axis abnormalities in asymptomatic AIS patients. CONCLUSION: Traditional risk factors may not be predictive of patients with a higher risk of changes in MRI. Both NND and NNM are representations easily understood by clinicians. Using these indexes to define if a patient should be submitted for additional imaging tests may facilitate the decision of using MRI as a preoperative screening tool in AIS patients. ABSTRACT Objective To determine if there is a statistically significant difference in the flexibility of the curves in the adolescent idiopathic scoliosis (AIS) by using lateral inclination radiographs in supine or prone decubitus. Methods We evaluated 19 patients with AIS, waiting for surgery. Radiographs of the patients were performed in orthostatic anteroposterior incidences and right and left lateral inclinations in prone and supine decubitus. The comparison between prone and supine decubitus was performed through the flexibility rates of the curves measured in each position. Results The mean flexibility rates measured in lateral inclination radiographs with the patient in the supine position were 54.4% ± 38.8% in the proximal thoracic curve, 45.8% ± 15.6% in the main thoracic curve, and 80.5% ± 20.7% in the thoracolumbar / lumbar curve. When the lateral inclination radiographs were performed with the patient in the prone position, we observed mean flexibility rates of 66.4% ± 34.3% in the proximal thoracic curve, 50.1% ± 12.8% in the main thoracic curve, and 80.6% ± 19.0% in the thoracolumbar / lumbar curve. Conclusion This present study did not find a statistically significant difference between the flexibility rates of the curves in the prone and supine positions, suggesting that the two radiographic methods analyzed are similar in the evaluation of the flexibility of the curves in adolescent idiopathic scoliosis. RESUMO Objetivo Determinar se há diferença estatisticamente significativa na flexibilidade das curvas na escoliose idiopática do adolescente (EIA), ao utilizar radiografias em inclinação lateral em decúbito supino ou prono. Métodos Foram avaliados 19 pacientes com EIA, em espera de cirurgia. Os pacientes realizaram radiografias em incidências anteroposterior em ortostase, inclinações laterais direita e esquerda em decúbito prono e supino. Realizou-se a comparação entre o decúbito prono e supino através das taxas de flexibilidade das curvas aferidas em cada posição. Resultados A taxa de flexibilidade média aferida, quando realizadas as inclinações laterais com o paciente em posição supino, foi de 54,4% ± 38,8% na curva torácica proximal, 45,8% ± 15,6% na curva torácica principal e 80,5% ± 20,7% na curva toracolombar/lombar. Quando realizadas as inclinações laterais com o paciente em posição prono, observou-se taxa de flexibilidade média de 66,4% ± 34,3% na curva torácica proximal, 50,1% ± 12,8% na curva torácica principal e 80,6% ± 19,0% na curva toracolombar/lombar. Conclusão O presente estudo não encontrou diferença com significância estatística nas taxas de flexibilidade das curvas nas posições prono e supino, sugerindo que os dois métodos radiográficos analisados são semelhantes na avaliação da flexibilidade das curvas na escoliose idiopática do adolescente. No significant differences in postoperative trunk shift and coronal balance were found (p =.69 and p =.74, respectively). The HD group had higher blood loss (p =.02), number of implants (p <.001), levels fused (p =.002), and surgical time (p <.001). The HD group had a higher prevalence of hypokyphosis from before operation to follow-up (p <.001). No significant differences were observed in the SRS-30 scores before operation and at 2 years' follow-up. The matched groups had similar preoperative major curves (p =.56), ages (p =.75), and BMIs (p =.61). Significantly longer surgical time (p =.009), higher density (p <.001), and better correction (p =.0001) were found in the HD group at 2 years' follow-up. No significant differences were found in the SRS-30 scores before operation and at 2 years' follow-up. CONCLUSION: LD constructs included fewer segments fused, lower intraoperative estimated surgical blood loss, and shorter operation time, and potentially decreasing complication risks due to fewer implants. OBJETIVO: Comparar os desfechos radiográficos e cirúrgicos da escoliose Lenke 1B e 1C. MÉTODOS: Cento e vinte pacientes com escoliose Lenke 1B e 1C foram agrupados de acordo com a densidade do implante, como segue: baixa densidade (BD) de ≤ 1,4 e alta densidade (AD) de > 1,4. Foram analisados os grupos pareados (30 pacientes cada) com base na idade, magnitude da curva e índice de massa corporal (IMC). Os parâmetros radiográficos foram avaliados antes da cirurgia, no pós-operatório imediato (POI) e no acompanhamento de dois anos. O questionário SRS-30 foi administrado antes da cirurgia e no acompanhamento de dois anos. RESULTADOS: As principais curvas dos grupos BD (n = 82) e AD (n = 38) foram respectivamente 59,1° e 65,6° antes da operação (p < 0,001), 26,3° e 22,9° no POI (p = 0,05) e 29,9° e 19,8° aos 2 anos de acompanhamento (p < 0,001). Não foram encontradas diferenças significantes no desvio do tronco e no balanço coronal no pós-operatório (p = 0,69 e p = 0,74, respectivamente). O grupo AD teve mais perda sanguínea (p = 0,02), número de implantes (p < 0,001), níveis de fusão (p = 0,002) e tempo de cirurgia (p < 0,001). O grupo AD teve maior prevalência de hipocifose do período anterior à cirurgia até o acompanhamento (p < 0,001). Não houve diferenças significantes nas pontuações do SRS-30 antes da operação e aos 2 anos de acompanhamento. No pré-operatório, os grupos pareados tinham curvas principais (p = 0,56), idade (p = 0,75) e IMC (p = 0,61) semelhantes. Constatou-se tempo cirúrgico expressivamente maior (p = 0,009), maior densidade (p < 0,001) e melhor correção (p = 0,0001) no grupo AD aos 2 anos de acompanhamento. Não foram encontradas diferenças significantes nas pontuações do SRS-30 antes da cirurgia e no acompanhamento de 2 anos. CONCLUSÃO: As estruturas de BD incluíram menos segmentos fundidos, menor perda de sangue intraoperatória estimada, menor tempo de cirurgia e menos risco de complicações, com possibilidade de redução, por causa do menor número de implantes. Métodos: Cento e vinte pacientes com escoliose Lenke 1B e 1C foram agrupados de acordo com a densidade do implante, como segue: baixa densidade (BD) de ≤ 1,4 e alta densidade (AD) de > 1,4. Foram analisados os grupos pareados (30 pacientes cada) com base na idade, magnitude da curva e índice de massa corporal (IMC). Os parâmetros radiográficos foram avaliados antes da cirurgia, no pós-operatório imediato (POI) e no acompanhamento de dois anos. O questionário SRS-30 foi administrado antes da cirurgia e no acompanhamento de dois anos. Resultados: As principais curvas dos grupos BD (n = 82) e AD (n = 38) foram respectivamente 59,1° e 65,6° antes da operação (p < 0,001), 26,3° e 22,9° no POI (p = 0,05) e 29,9° e 19,8° aos 2 anos de acompanhamento (p < 0,001). Não foram encontradas diferenças significantes no desvio do tronco e no balanço coronal no pós-operatório (p = 0,69 e p = 0,74, respectivamente). O grupo AD teve mais perda sanguínea (p = 0,02), número de implantes (p < 0,001), níveis de fusão (p = 0,002) e tempo de cirurgia (p < 0,001). O grupo AD teve maior prevalência de hipocifose do período anterior à cirurgia até o acompanhamento (p < 0,001). Não houve diferenças significantes nas pontuações do SRS-30 antes da operação e aos 2 anos de acompanhamento. No pré-operatório, os grupos pareados tinham curvas principais (p = 0,56), idade (p = 0,75) e IMC (p = 0,61) semelhantes. Constatou-se tempo cirúrgico expressivamente maior (p = 0,009), maior densidade (p < 0,001) e melhor correção (p = 0,0001) no grupo AD aos 2 anos de acompanhamento. Não foram encontradas diferenças significantes nas pontuações do SRS-30 antes da cirurgia e no acompanhamento de 2 anos. Conclusão: As estruturas de BD incluíram menos segmentos fundidos, menor perda de sangue intraoperatória estimada, menor tempo de cirurgia e menos risco de complicações, com possibilidade de redução, por causa do menor número de implantes. Resumo Escoliose idiopática do adolescente (EIA) atinge de 2% a 4% de jovens no Brasil. Repetidas exposições aos exames radiológicos, no acompanhamento desta deformidade, podem ser danosos à saúde. O objetivo deste estudo é apresentar um protocolo de fotogrametria, como método não ionizante para quantificação da escoliose, e relacioná-lo ao método radiológico de Cobb. Dezesseis indivíduos portadores de escoliose idiopática (21,4 ± 6,1 anos de idade e 19,8±0,2 de índice de massa corporal) foram submetidos à radiografia posteroanterior do tronco, de pé e, posteriormente, fotografia do tronco posterior, após receberem marcadores anatômicos nos processos espinhosos das vértebras C7 até L5. As imagens foram encaminhadas para análise independente de dois examinadores treinados na quantificação da escoliose para o tipo de imagem recebida. A média angular torácica de Cobb e de fotogrametria foram 36,14° e 36,43°, respectivamente. A diferença média entre os métodos foi de 4,1°. Não houve diferença estatisticamente significante (p-valor < 5%) entre eles. A fotogrametria, por ser não ionizante, ter baixo custo e ser portátil, poderá representar uma alternativa ao método radiológico. Novos estudos são necessários no aprimoramento das técnicas não ionizantes no rastreamento da EIA.
Abstract Adolescent idiopathic scoliosis (AIS) affects 2% to 4% of young people in Brazil. Repeated exposures to radiation used in the monitoring of the deformity can be harmful to the health. This study aimed to present a photogrammetry protocol as a non-ionizing method to quantify scoliosis and relate it to the Cobb radiological method. Sixteen individuals with idiopathic scoliosis (age: 21.4 ± 6.1 years, body mass index: 19.8 ± 0.2 kg/m2) underwent standing posteroanterior X-ray examination of the trunk. Additionally, markers were placed on the spinal processes of the C7 to L5 vertebrae, and posterior trunk photographs were taken. All images were sent for independent analysis by two examiners who were trained in the quantification of scoliosis. The average of the thoracic curvature evaluated through the photogrammetry and Cobb methods were 36.43° and 36.14°, respectively. With an average difference of 4.1°, the methods were not statistically different (p < 0.05). As a non-ionizing method that is low cost and portable, photogrammetry may represent a suitable alternative to the radiological method. Further studies are needed for the improvement of non-ionizing techniques in AIS screening.

BACKGROUND: Bracing is used as a valid non-surgical treatment for adolescent idiopathic scoliosis (AIS) to avoid progression of the deformity and thereby surgery. The effect of bracing treatment on quality of life of patients with AIS has been a topic of interest in the international literature. The aim of this study was to evaluate the quality of life and patient satisfaction during bracing treatment for AIS of a pediatric hospital. MATERIAL AND METHOD: We assessed a total of 43 non-consecutive female patients (mean age at questionnaire, 13 years and 1 month and 10 years and 8 months to 14 years and 5 months; mean period of usage of brace, 1 year and 7 months), with adolescent idiopathic scoliosis (AIS), older than 10 years of age until skeletal maturity, with a Risser sign less than 3 and scoliosis between 20 and 45°, treated with thoracolumbosacral orthosis (TLSO) for a period longer than 6 months, and without other comorbidities or previous surgeries, were evaluated. The patients were administered a previously validated to Spanish questionnaire on quality of life (Brace Questionnaire (BrQ); Grivas TB et al.). BrQ is a validated tool and is considered a disease-specific instrument; its score ranges from 20 to 100 points, and higher BrQ scores are associated with better quality of life. RESULTS: The patients reported using the brace for a mean of 17.6 h daily and for a mean period of 1 year and 7 months at the time of the study. Overall, 72% of the study population reported to be in some way psychologically affected by the brace wearing, 56% felt their basic motor activities were affected, 54% felt socialization with their environment was affected, 46% considered their quality of life deteriorated due to pain, and 40% reported conflicts in the school environment. CONCLUSION: Patients with AIS treated with bracing reported a negative impact (53.5% overall) on quality of life and treatment satisfaction in terms of psychological, motor, social, and school environment aspects. An interdisciplinary approach would be important for the integrated psychosocial care of these patients.

BACKGROUND: Currently, there is little consensus on how or when to discontinue bracing in adolescent idiopathic scoliosis (AIS). An expert spine surgeon national survey could aid in elucidate discontinuation of the brace.Few data have been published on when and how to discontinue bracing treatment in patients with AIS resulting in differences in the management of the condition. The aim of this study was to characterize decision-making of surgeons in the management of bracing discontinuation in AIS. METHODS: An original electronic survey consisting of 12 multiple choice questions was sent to all the members of the National Spine Surgery Society (497 surveyed). Participants were asked about their type of medical practice, years of experience in the field, society memberships, type of brace they usually prescribed, average hours of daily brace wearing they recommended, and how and when they indicated bracing discontinuation as well as the clinical and/or imaging findings this decision was based on. Exclusion criteria include brace discontinued because of having developed a curve that warranted surgical treatment. RESULTS: Of a total of 497 surgeons, 114 responded the survey (22.9%). 71.9% had more than 5 years of experience in the specialty, and 51% mainly treated pediatric patients. Overall, 95.5% of the surgeons prescribed the thoracolumbosacral orthosis (TLSO), indicated brace wearing for a mean of 20.6 h daily. Regarding bracing discontinuation, indicated gradual brace weaning, a decision 93.9% based on anterior-posterior (AP) and lateral radiographs of the spine and physical examination, considered a Risser ≥ IV and ≥ 24 months post menarche. CONCLUSIONS: The results of this study provide insight in the daily practice of spine surgeons regarding how and when they discontinue bracing in AIS. The decision of bracing discontinuation is based on AP/lateral spinal radiographs and physical examination, Risser ≥ IV, regardless of Tanner stage, and ≥ 24 months post menarche. Gradual weaning is recommended.

ANTECEDENTES: La escoliosis idiopática del adolescente se define como una deformidad tridimensional de la columna vertebral que se presenta entre los 10 y los 18 años, y que se manifiesta con una curvatura vertebral en el plano coronal mayor de 10°. Esta deformidad afecta al 2-3% de la población general, pero solo el 10% del total requerirá en algún momento tratamiento quirúrgico. El método de elección para el manejo es el uso de tornillos transpediculares y barras desrotadoras. OBJETIVO: Realizar un análisis descriptivo de los pacientes que recibieron manejo quirúrgico en nuestro instituto con tornillos transpediculares y barras. MÉTODO: Se trata de un estudio observacional, retrospectivo, analítico, abierto, de muestreo no probabilístico, en el que se incluyeron los pacientes tratados con manejo quirúrgico entre 2012 y 2013. Las deformidades se estratificaron de acuerdo con la clasificación de Lenke. El ángulo de corrección de la deformidad, los niveles instrumentados, el sangrado transquirúrgico y la presencia de complicaciones fueron las variables analizadas. RESULTADOS: La mayoría de los pacientes presentaron curvas Lenke IBN, Nash Moe III, Cobb un promedio de 59.4° y cifosis de 47.8. En promedio se siguió a los pacientes por 35.84 meses, detectando un aumento de la curvatura coronal de 2.28° y un aumento de la curvatura sagital de 2.8°. CONCLUSIÓN: Al comparar estos resultados y la literatura mundial se concluyó que el tratamiento de la escoliosis idiopática del adolescente es un método seguro y reproducible que ofrece una mayor ventaja biomecánica y biológica sobre el uso de instrumentación mixta utilizada anteriormente. BACKGROUND: Adolescent's idiopathic scoliosis is defined as a three-dimensional deformity of the spine, which occurs between 10 and 18-year-old, has a spinal curvature >10° in the coronal plane. This deformity affects 23% of the general population, however, only 10% of the total will require surgery at some point. The method of choice for management is the use of pedicle screws and rods derotational. OBJECTIVE: To perform a descriptive analysis of patients who received surgical treatment in our institute with pedicle screws and rods. METHODS: This is an observational, retrospective, analytical, open study, non-probability sampling, in which patients requiring surgical treatment at our institute between 2012 and 2013 were included, the deformities were stratified according to the classification of Lenke. The angle of deformity correction, instrumented levels, amount of bleeding, presence of complications were the variables analyzed. RESULTS: Lenke classifying mostly IBN, Moe Nash III, an average of 59.4° Cobb and kyphosis of 47.8. On average it was followed patients for 35.84 months, detecting an increase 2.28° coronal curvature and sagittal curvature increase of 2.8°. CONCLUSION: Comparing these results and world literature concluded that the treatment of adolescent's idiopathic scoliosis is a safe and reproducible method that provides greater biomechanical and biological advantage over the use of mixed instrumentation used previously.

BACKGROUND: Pre-donation of autologous blood prior to spine fusion for adolescent idiopathic scoliosis (AIS) has been used in deformity surgery. The effect of pre-donation on pre-operative hematocrit (Hct) remains debated. Multiple factors may influence pre-operative Hct including intravascular volume status, patient factors, and timing of pre-operative blood donation. The purpose of this study was to determine if pre-donation significantly lowers pre-incision Hct in AIS patients. METHODS: A retrospective cohort study of a Level-1 prospective randomized trial was conducted. 125 patients from the homogeneous population were included. AIS patients undergoing a posterior only spinal fusion for AIS were separated into two groups based on their pre-operative blood donation history. Demographic variables, pre-incision Hct, and transfusion rates were compared between the two groups using the Student's T-test. RESULTS: Pre-donation and non pre-donation groups had 28 and 97 patients, respectively. Pre-donation group was 75% female (21F, 7M) and non pre-donation group was 78% female (76F, 21M). There was no difference between pre-donation and non pre-donation groups in mean age (15.6 ± 2.2 vs 14.8 ± 2.2, p = 0.081), BMI (23.1 ± 4.2 vs 21.7 ± 5.3, p = 0.219), and pre-incision Hct (32.8 ± 3.4 vs 33.8 ± 3.1, p = 0.628). The overall transfusion rates were equivalent (32.1± 48.0% vs 25.8 ± 44.0%, p = 0.509), however, the rate of allogenic transfusion for the pre-donation group was significantly lower (3.6 ± 18.9% vs 25.8 ± 44.0%, p = 0.011). CONCLUSIONS: This study supports the use of pre-donation for AIS, without a significant drop in pre-incision Hct. Patients that donate are also much less likely to be exposed to allogenic blood. There may be a surgeon bias to recommend pre-donation in patients with a larger BMI and older age. Future studies are needed from a larger population of patients including those with non-AIS pathology. LEVEL OF EVIDENCE: Level III.

Resumen La escoliosis idiopática del adolescente (EIA) consiste en una deformidad tridimensional de la estructura de la columna vertebral en los planos coronal, sagital y rotacional, que afecta a sujetos sanos a partir de los 10 años de edad hasta la madurez ósea o al término del crecimiento. Esta enfermedad puede comprometer el estado físico, emocional y, por lo tanto, la calidad de vida de quienes la padecen, por lo que debe ser considerada un problema de salud significativo. La EIA se puede sospechar con un examen físico de columna, mediante el cual se evalúa el nivel de los hombros, el nivel de las escápulas, el nivel de la pelvis, la prueba de Adams y el signo de Pitres. A pesar de que en México no se han establecido oficialmente programas para la detección temprana de la enfermedad, la prueba de Adams puede ser la pauta para identificar y prevenir el desarrollo de esta deformidad. La aplicación de esta prueba de tamizaje está al alcance de todos. A pesar de que se podría considerar que su detección oportuna desde etapas tempranas sería la mejor estrategia para combatirla a tiempo, existen grupos de investigación a nivel internacional que opinan distinto, lo que ha colocado a la detección temprana como un tema controversial. Por lo anterior, el objetivo de este trabajo es ofrecer al lector un panorama general y actualizado de las diferentes perspectivas del tema, esperando que sea de utilidad para la práctica profesional.
Abstract Adolescent idiopathic scoliosis (EIA), consists of a three-dimensional deformity of the structure of the spine in the coronal, sagittal and rotational planes affecting healthy subjects ranging from 10 years of age until skeletal maturity or term of growing. This disease can compromise physical and emotional state, therefore, the quality of life of those who suffer it, so the EIA must be considered as significant health problem. The EIA can be suspected by a physical examination of the spine on whichshoulders level, shoulder blades level, pelvis level, Adams Test and Pitres sign are evaluated. Even though, it hasn't been officially established a program for an early detection of the disease in Mexico, evidence through Adams Test can be a guideline to identify and prevent the development of this deformity. The application of this screening test is available to all. Although it could be considered that early detection of this deformity at young ages would be the best strategy to get on time for treatment, there are international research groups that have a different opinion, which has placed early detection of EIA as a controversy topic. Therefore, the aim of this paper is to provide a general and updated overview of the different perspectives of this issue, hoping it will be useful for the professional practice.

STUDY DESIGN: A retrospective cohort study with IRB approval. INTRODUCTION: Significant blood loss remains an important concern in terms of the performance of the posterior spinal fusion in adolescent idiopathic scoliosis. Several strategies have been reported to minimize blood loss during surgery. In order to address the need to minimize blood loss without sacrificing the quality of the fusion, in our hospital, we adopted a two-step surgical approach. This surgical approach consist of the exposure and instrumentation of the lumbar region prior to and followed by an extension of the surgical incision to the thoracic region for its subsequent instrumentation. The main purpose of this study was to compare a two-step surgical approach with the one-step (standard) approach. METHODS: This study was a review of all the data on consecutive posterior spinal fusion surgeries performed by a specific two-surgeon team during 2004-2013. Demographics, surgical variables, radiographic findings, and outcomes regarding blood loss, morbidity, and the duration of the procedure were evaluated. RESULTS: Eighty-five patients underwent the standard surgical exposure, and 41 patients underwent the two-step surgical technique. With the exception of BMI, neither group showed any statistically preoperative variable significant differences. None of the postoperative outcome variables were statistically significant between both surgical approaches. CONCLUSIONS: No differences were detected in terms of using a two-step surgical approach versus the one-step standard surgical approach regarding perioperative blood loss, surgical time, or complications. Even with current techniques and instrumentation, complications can occur after operative treatment of adolescent idiopathic scoliosis. The most dreaded complications-neurologic deficits-are relatively infrequent, occurring in 1% or less of patients. Nonneurologic deficits, such as infection, pseudarthrosis, curve progression, and proximal junctional kyphosis, are more frequent, but are much less likely to require reoperation or to cause poor functional outcomes. Understanding the potential complications of surgical treatment of pediatric spinal deformity is essential for surgical decision-making.

Objetivo: Analizar, con resonancia magnética, las variaciones de posición de la amígdala cerebelosa, antes de corregir la deformidad espinal mediante una artrodesis vertebral posterior instrumentada y después, en pacientes con escoliosis idiopática del adolescente sin sintomatología neurológica. Materiales y Métodos: Se evaluaron retrospectivamente 40 pacientes con escoliosis idiopática del adolescente, sin síntomas neurológicos y sometidos a una artrodesis vertebral posterior instrumentada para corregir la deformidad espinal. A todos se les realizó una resonancia magnética de la fosa cerebral posterior y de la columna cervical, antes de la cirugía espinal y después de ella. Resultados: La magnitud preoperatoria promedio de la curva escoliótica fue de 53,15° y la de la cifosis torácica fue de 35,42º. En el posoperatorio inmediato, la magnitud promedio fue de 7,45º y de 27,87º,respectivamente. El valor promedio de la longitud de la columna en el plano coronal fue de 44,5 cm en el preoperatorio y de 48,27 cm en el posoperatorio. El valor promedio de la longitud de la columna vertebral en el plano sagital fue de 50,87 cm en el preoperatorio y de 55,13 cm en el posoperatorio. No se observó unadiferencia significativa respecto de la posición de la amígdala cerebelosa en las mediciones antes de corregir la deformidad espinal y después (p = 0,6042). Conclusión: No se observó una variación significativa en la ubicación de la amígdala cerebelosa respecto del agujerooccipital en pacientes con escoliosis idiopática del adolescente que fueron sometidos artrodesis vertebral posterior instrumentadapara corregir la deformidad espinal.
Objective: To evaluate variations in cerebellar tonsil position after posterior spinal fusion in neurologically intact patients with adolescent idiopathic scoliosis. Methods: We retrospectively evaluated 40 patients with adolescent idiopathic scoliosis and no neurological symptoms that underwent posterior spinal fusion. Anteroposterior and sagittal standing radiographs, and sagittal hindbrain MRI wereperformed in all patients before and after spinal surgery. We evaluated variations in cerebellar tonsil position in relation to spinal correction and spinal elongation after posterior spinal fusion. Results: Mean preoperative magnitude of the curve was 53.15° and thoracic kyphosis was 35.42º. Mean postoperative valueswere 7.45º and 27.87º, respectively. The average length of the spine in the coronal plane was 44.5 cm in preoperative x-rays and 48.27 cm in postoperative x-rays. The average length in the sagittal plane was 50.87 cm in preoperative x-raysand 55.13cm in postoperative x-rays. There was no significant difference in theposition of the cerebellar tonsil before and after spinal correction (p = 0.6042). Conclusion: Position of the cerebellar tonsil did not change with posterior spinal fusion in patients with adolescent idiopathic scoliosis.

Introducción: En nuestro Centro, es una práctica frecuente realizar controles clínicos y radiológicos durante el primer año posoperatorio de la escoliosis idiopática del adolescente, habitualmente en el posoperatorio inmediato, a los 3, 6, 9 y 12 meses. Se conoce el impacto negativo de estas radiaciones sobre el cuerpo humano y no hay guías claras sobre cuál sería el esquema óptimo de seguimiento radiológico. Materiales y Métodos: Se realizó un estudio retrospectivo de 55 pacientes operados por vía posterior con tornillos pediculares, sin síntomas de radiculopatía ni pérdida de la correcciónclínica. Se analizaron los hallazgos radiológicos durante el primer año posoperatorio. Se evaluaron las intercurrencias y su impacto en la conducta terapéutica. Resultados: Media de la edad: 15.1; media del índice de Risser: 3,7 (rango 0-5); porcentaje de pedículos instrumentados en la curva: 81,7%; media de la corrección de la curva principal: 74%; media de los espinogramas posoperatorios de la población 3,7 ± 0,93 (cada espinograma consta de un frente y un perfil). No se modificó la conducta terapéutica sobre la base de los hallazgos radiológicos en ningún paciente. Conclusión: En los pacientes con diagnóstico de escoliosis idiopática del adolescente operados con tornillos pediculares y sin síntomas de radiculopatía, dolores persistentes ni pérdida clínica de la corrección, se podrían reducir los controles radiológicos a tres durante el primer año posoperatorio.
Introduction: In our center, clinical and radiological controls are a common practice in the first year after surgery for adolescent idiopathic scoliosis, usually they are performed at the immediate post-surgery period, and at 3, 6, 9 and 12 months. The negative impact of these radiationson the human body is a well-known consequence and there are not clear clinical guides about what would be the optimal radiological monitoring approach. Methods: Fifty-five patients with pedicles screws and no symptoms or loss of clinical correction were retrospectively evaluated. Radiological findings in the first year post-surgery were assessed, as well as the intercurrent processes and their impact on the therapeutic behavior. Results: Mean age: 15.1; mean Risser: 3.7 (range 0-5); percentage of instrumented pedicles: 81.7%; mean correction in the principal curve: 74%; mean X-rays 3.7 ± 0.93 (each X-ray consists of a front and a side face). Clinical behavior was not modified based on the radiological findingsin any patient. Conclusion: In patients with adolescent idiopathic scoliosis who underwent surgery with pedicle screws and without symptoms of radiculopathy, persistent pain or loss of clinical correction, the X-ray controls could be reduced to three in the first year after surgery.
