RESUMEN

Musculoskeletal pain is a widely experienced public healthcare issue, especially after traumatic muscle injury. Besides, it is a common cause of disability, but this pain remains poorly managed. However, the pathophysiology of traumatic muscle injury-associated pain and inflammation has not been fully elucidated. In this regard, the transient receptor potential ankyrin 1 (TRPA1) has been studied in inflammatory and painful conditions. Thus, this study aimed to evaluate the antinociceptive and anti-inflammatory effect of the topical application of a TRPA1 antagonist in a model of traumatic muscle injury in rats. The mechanical trauma model was developed by a single blunt trauma impact on the right gastrocnemius muscle of Wistar male rats (250-350 g). The animals were divided into four groups (Sham/Vehicle; Sham/HC-030031 0.05%; Injury/Vehicle, and Injury/HC-030031 0.05%) and topically treated with a Lanette® N cream base containing a TRPA1 antagonist (HC-030031, 0.05%; 200 mg/muscle) or vehicle (Lanette® N cream base; 200 mg/muscle), which was applied at 2, 6, 12, 24, and 46 h after muscle injury. Furthermore, we evaluated the contribution of the TRPA1 channel on nociceptive, inflammatory, and oxidative parameters. The topical application of TRPA1 antagonist reduced biomarkers of muscle injury (lactate/glucose ratio), spontaneous nociception (rat grimace scale), inflammatory (inflammatory cell infiltration, cytokine levels, myeloperoxidase, and N-acetyl-ß-D-glucosaminidase activities) and oxidative (nitrite levels and dichlorofluorescein fluorescence) parameters, and mRNA Trpa1 levels in the muscle tissue. Thus, these results demonstrate that TRPA1 may be a promising anti-inflammatory and antinociceptive target in treating muscle pain after traumatic muscle injury.

This study examined the percentage and location of trigger points in police working dogs. Twelve dogs housed at a military police kennel were selected through convenience sampling. Only active dogs with no comorbidities or radiographic changes doing 6 hours of intense physical activity per day were included. After orthopedic and neurological examination, dogs were palpated for the detection of trigger points (TPs), carried out by two independent examiners, with criteria of palpations previously standardized. TPs were recorded using an anatomy reference image according to the corresponding anatomical location. The percentage of TPs was highest in the lumbar portion of the longissimus dorsi muscle (42%), followed by the latissimus dorsi, pectineus, quadriceps femoris, and sartorius (33%) muscles. Most TPs were located on the right side of the body. This study's percentage of TPs in police working dogs was higher in spinal and hind limb muscles, especially on the right side. The major criteria for identifying TPs in dogs were the pain responses to palpation and contractile local response. The findings of this study could be used to refine myofascial pain prevention to reduce early retirement due to musculoskeletal pain and draw attention to this kind of problem that can also affect dogs.

(1) Background: Mitochondrial dysfunction and redox imbalance seem to be involved in fibromyalgia (FM) pathogenesis. The results of our previous studies suggest that whole-body vibration training (WBVT) would improve redox status markers, increase blood irisin levels, and ameliorate the body composition of women with FM. (2) Objective: The current study aimed to investigate WBVT on oxidative stress markers, plasma irisin levels, and body composition in women with FM. (3) Methods: Forty women with FM were randomized into WBVT or untrained (UN) groups. Before and after 6 weeks of WBVT, body composition was assessed by dual-energy radiological absorptiometry (DXA), and inflammatory marker activities were measured by enzymatic assay. (4) Results: Body composition, blood irisin levels, and oxidative stress markers were similar between UN and WBVT groups before the intervention. After 6 weeks of intervention, the WBVT group presented higher irisin levels (WBVT: 316.98 ± 109.24 mg·dL³, WBVT: 477.61 ± 267.92 mg·dL³, p = 0.01) and lower TBARS levels (UN: 0.39 ± 0.02 nmol MDA/mg protein, WBVT: 0.24 ± 0.06 nmol MDA/mg protein, p = 0.001) and visceral adipose tissue mass (UN: 1.37 ± 0.49 kg, WBVT: 0.69 ± 0.54 kg, p = 0.001) compared to the UN group. (5) Conclusions: Six weeks of WBVT improves blood redox status markers, increases irisin levels, and reduces visceral adipose tissue mass, favoring less cell damage and more outstanding oxidative balance in women with FM.

INTRODUÇÃO: O treinamento de força tem sido recomendado na reabilitação clínica, bem como no condicionamento físico de atletas. Não é incomum, em ambos os casos, a presença de dor durante a prática; no entanto, até o momento, não há informação consensual em relação aos efeitos da dor muscular aguda nas adaptações ao treinamento de força. OBJETIVO: O objetivo deste estudo piloto foi avaliar os efeitos da dor induzida experimentalmente na adaptação da força muscular após um período de treinamento de 8 semanas. MÉTODO: O estudo incluiu cinco voluntários saudáveis do sexo masculino e não treinados. Os participantes foram submetidos a um protocolo de treinamento de força (3x/semana durante 8 semanas) para os músculos flexores do cotovelo. A dor muscular aguda foi induzida no início de cada sessão de treinamento, por meio de infusão intramuscular de 2,5 ml de solução salina hipertônica (6%) no ventre do músculo bíceps braquial. A força dinâmica máxima (1RM) e a contração isométrica voluntária máxima (CIVM) foram medidas antes e após quatro e oito semanas de treinamento. RESULTADOS: A força dinâmica máxima aumentou, em média, 37,3% e 78,4% após quatro e oito semanas, respectivamente. Entretanto, pouca ou nenhuma diferença foi encontrada na CIVM (-1,7% e -3,0% após quatro e oito semanas, respectivamente). CONCLUSÃO: Após 24 sessões de treinamento de força, com dor muscular aguda induzida a cada sessão, voluntários saudáveis aumentaram sua capacidade de produzir força dinâmica máxima em mais de 75%; entretanto, a força isométrica apresentou apenas pequenas variações negativas.


INTRODUCTION: Strength training has been recommended in clinical rehabilitation, as well as in the physical conditioning of athletes. It is not uncommon, in both cases, the presence of pain during practice; however, to date, there is no consensual information about the effects of acute muscle pain on strength training adaptations. OBJECTIVE: The aim of this pilot study was to evaluate the effects of experimentally induced pain on muscle strength adaptation after an 8-week training period. METHOD: The study included five untrained, healthy male volunteers. Participants were submitted to a strength training protocol (3x/week for 8 weeks) for the elbow flexor muscles. Acute muscle pain was induced at the beginning of each training session through an intramuscular infusion of 2.5 ml of hypertonic saline (6%) into the biceps brachii muscle belly. Maximal dynamic strength (1RM) and maximal voluntary isometric contraction (MVIC) were measured at pre- and after four and eight weeks of training. RESULTS: Maximal dynamic strength increased, on average, 37.3% and 78.4% after four and eight weeks, respectively. However, little, if any, difference was found in MVIC (-1.7% and ­ 3.0% after four and eight weeks, respectively). CONCLUSION: After 24 strength training sessions, with acute muscle pain induced every session, healthy volunteers increased their ability to produce maximal dynamic strength by more than 75%; however, isometric strength presented only small negative changes.

(1) The evidence points to an increase in oxygen reactive species as one of the possible causes of fibromyalgia (FM). In addition, it is plausible that an imbalance in redox markers can be associated with pain amplification and dynapenia in FM patients. The aim of our study was to investigate possible factors associated with muscle pain and lean body mass in FM patients. (2) Methods: This was a quantitative, exploratory and cross-sectional study of 47 patients with FM (53.45 + 7.32 years). We evaluated self-perceptions of muscle pain, lean body mass, body composition, quality of life, sleep quality, depression index, muscle performance and oxidative stress biomarkers. (3) Results: We observed that lower blood levels of antioxidants and poor quality of life explained 21% of the greater muscle pain. In addition, high blood levels of oxidative stress, worse muscle performance and poor quality of life explained 27% of the lower lean mass in patients with FM. (4) Conclusions: Larger amounts of lipid peroxidation and reductions in antioxidant levels, in addition to lower muscle performance and poor life quality, are possible independent contributors to greater muscle pain and lower lean body mass in FM patients.

Back pain and inflammation of the epaxial musculature is a significant problem in all equine athletes. Treatment of back pain can be challenging and often requires a multimodal approach. In humans, bio-electromagnetic energy regulation therapy (BEMER) has been reported to be effective in pain modulation. With its increased use in people comes a similar robust application in veterinary medicine unfortunately, there is unsubstantiated evidence for this type of therapy in horses. Objectives of this study were to assess analgesic responses and biomechanical outcome variables using a bio-electromagnetic energy regulation therapy blanket, and to evaluate serum biomarkers as a method to monitor the treatment effects in horses with thoracolumbar epaxial muscle pain. Cohort study of 8 horses treated for 3 consecutive days. Horses with naturally-occurring thoracolumbar epaxial muscle pain were used in this study. Objective outcome variables were recorded daily for 5 days, which included spinal evaluation, mechanical nociceptive thresholds, electromyography, kinematics, kinetics, and serum biomarkers. BEMER blanket therapy significantly improved thoracolumbar epaxial muscle nociceptive thresholds. Center of pressure displacement as a measure of postural stability was significantly improved as well as significant gains in spinal flexibility were demonstrated at study completion. A significant treatment effect was not appreciated in measures of muscle tone, ground reaction forces or serum biomarkers. Limitations include the lack of a control group and a definitive structural diagnosis of thoracolumbar epaxial muscle pain. The BEMER blanket produced significant clinical and biomechanical effects in horses with back pain.

Abstract Molecular mechanisms involved in the development of muscle pain induced by static contraction are not completely elucidated. This study aimed to evaluate the involvement of the transient receptor potential vanilloid 1 (TRPV1) and the transient receptor potential ankyrin 1 (TRPA1) receptors expressed in peripheral and central terminals of primary afferents projected to gastrocnemius muscle and spinal cord in muscle pain induced by static contraction. An electrical stimulator provided the contraction of rat gastrocnemius muscle and mechanical muscle hyperalgesia was quantified through the pressure analgesimeter Randall-Selitto. AMG9810 and HC030031 were used. When administered in ipsilateral, but not contralateral gastrocnemius muscle, drugs prevented mechanical muscle hyperalgesia induced by static contraction. Similar results were obtained by intrathecal administrations. We propose that, in an inflammatory muscle pain, peripheral and central TRPV1 and TRPA1 work together to sensitize nociceptive afferent fibers, and that TRPV1 and TRPA1 receptors are potential target to control inflammatory muscle pain.

Background: The ability to produce force is critical to several daily activities. Strategies to reduce delayed onsent muscle soreness (DOMS) and restore force-generating properties that occur in response to training and impair physical performance have been proposed. Acupuncture has been proposed to accelerate recovery and optimize muscle functioning. It was to determine the effects of acupuncture on DOMS and on the muscle force production capacity. Methods: Thirty physically active volunteers (26.3 ± 3.1 years old, 1.70 ± 0.08 m, 66.5 ± 12.6 kg) were allocated in a control (CG; n = 10), placebo (PG; n = 10), and acupuncture (AG; n = 10) groups. The groups were subjected to an exercise protocol to induce DOMS and were assessed for muscle force, threshold, and rate of perceived pain and activation of the biceps brachialis muscle before (PRE) and 20 minutes after (POST) a single acupuncture session using a mixed model analysis. Results: The threshold and rate of perceived pain were reduced in the AG (P < 0.05) but remained unchanged in the other groups (P > 0.05). Muscle activation also showed larger changes in the AG in comparison with the other groups (P < 0.05). No changes in the ability to produce force were found after acupuncture in any groups (P > 0.05). Conclusions: Acupuncture may reduce discomfort and improve muscle activation but was not effective to promote force improvement and/or restore the force-generating properties. It may be speculated that muscle disruption may have played a role and did not allow participants to restore their muscular performance. The participation of the peripheral opioid and cannabinoid endogenous systems in modulating muscle pain and inflammation has not been fully explored. Thus, the aim of this study was to investigate the involvement of these endogenous systems during muscular-tissue hyperalgesia induced by inflammation. Hyperalgesia was induced by carrageenan injection into the tibialis anterior muscles of male Wistar rats. We padronized an available Randal-Sellito test adaptation to evaluate nociceptive behavior elicited by mechanical insult in muscles. Western blot analysis was performed to evaluate the expression levels of opioid and cannabinoid receptors in the dorsal root ganglia. The non-selective opioid peptide receptor antagonist (naloxone) and the selective mu opioid receptor MOP (clocinnamox) and kappa opioid receptor KOP (nor-binaltorphimine) antagonists were able to intensify carrageenan-induced muscular hyperalgesia. On the other hand, the selective delta opioid receptor (DOP) antagonist (naltrindole) did not present any effect on nociceptive behavior. Moreover, the selective inhibitor of aminopeptidases (Bestatin) provoked considerable dose-dependent analgesia when intramuscularly injected into the hyperalgesic muscle. The CB1 receptor antagonist (AM251), but not the CB2 receptor antagonist (AM630), intensified muscle hyperalgesia. All irreversible inhibitors of anandamide hydrolase (MAFP), the inhibitor for monoacylglycerol lipase (JZL184) and the anandamide reuptake inhibitor (VDM11) decreased carrageenan-induced hyperalgesia in muscular tissue. Lastly, MOP, KOP and CB1 expression levels in DRG were baseline even after muscular injection with carrageenan. The endogenous opioid and cannabinoid systems participate in peripheral muscle pain control through the activation of MOP, KOP and CB1 receptors.

BACKGROUND: In the adult population, the work environment and physical fitness levels are directly related to the onset of musculoskeletal pain, repetitive strain injuries, and decreased blood circulation. Although low levels of muscle strength and flexibility may lead to a higher prevalence of pain, specific anatomic regions are poorly addressed. OBJECTIVE: To investigate the prevalence of musculoskeletal pain and the association between strength or flexibility and pain in university staff. METHODS: The sample was composed of 110 members of staff from a university in Guarapuava-PR. Body mass and height values were obtained, from which the BMI was calculated. The pain evaluation was performed by means of a questionnaire, analyzing the intensity, frequency, and anatomical region. The subjects were then submitted to strength (right and left hand grip, lumbar traction, lower limb traction) and flexibility tests (sit and reach test). RESULTS: The anatomical region with the highest prevalence of pain was the lumbar region (43.4%). When the association between the presence of pain and flexibility was performed, only the lumbar traction presented significant results, with the weakest group demonstrating pain (OR: 3.47 [1.27 - 9.49]). CONCLUSION: The results demonstrate that low levels of strength in the lumbar region are associated with the presence of painful symptomatology.

Regular physical exercise has been described as a good strategy for prevention or reduction of musculoskeletal pain. The Peroxisome Proliferator-Activated Receptor Gamma (PPARγ) has been investigated as a promising target for the control of inflammatory pain. Therefore, the aim of this study was to evaluate whether activation of PPARγ receptors is involved in the reduction of acute muscle pain by chronic exercise and, in this case, whether this process is modulated by inflammatory cytokines. To this end, Wistar rats were submitted to swimming physical training for a period of 10 weeks, 5 days per week, 40 min/day, in an intensity of 4% of the body mass. Muscle hyperalgesia was measured by Randall Selitto test and pro-inflammatory cytokines were quantified by ELISA. The results showed that swimming physical training prevented the onset of acute mechanical muscle hyperalgesia and the increase in muscle levels of Cytokine-induced neutrophil chemoattractant 1 (CINC-1) induced by carrageenan into gastrocnemius muscle. In addition, local pre-treatment with the selective PPARγ receptors antagonist GW9662 reversed the mechanical muscle hypoalgesia and the modulation of CINC-1 levels induced by swimming physical training. These data suggest that swimming physical training prevented the onset of acute mechanical muscle hyperalgesia by a mechanism dependent of PPARγ receptors, which seems to contribute to this process by modulation of the pro-inflammatory cytokine CINC-1, and highlight the potential of PPARγ receptors as a target to control musculoskeletal pain and to potentiate the reduction of musculoskeletal pain induced by exercise.

BACKGROUND: Shoulder injuries are common in individuals who use wheelchairs. OBJECTIVES: This study investigated the presence of mechanical pain hypersensitivity and trigger points in the neck-shoulder muscles in elite wheelchair basketball players with/without shoulder pain and asymptomatic able-bodied elite basketball players. METHODS: Eighteen male wheelchair basketball players with shoulder pain, 22 players without shoulder pain, and 20 able-bodied elite male basketball players were recruited. Pressure pain thresholds were assessed over C5-C6 zygapophyseal joint, deltoid muscle, and second metacarpal. Trigger points in the upper trapezius, supraspinatus, teres minor, infraspinatus, teres major, latissimus dorsi, subscapularis, pectoralis minor, pectoralis major and deltoid muscles were also examined. RESULTS: Wheelchair basketball players with shoulder pain showed lower pressure pain thresholds over the C5-C6 joint and second metacarpal than elite wheelchair basketball players without pain (between-groups differences: 1.1, 95%CI 0.4, 1.8 and 1.8, 95%CI 0.8, 2.8, respectively) and able-bodied basketball players without pain (between-groups differences: 0.8, 95%CI 0.4, 1.2; 1.6, 95%CI 0.8, 2.4, respectively). The mean number of myofascial trigger points for wheelchair basketball players with unilateral shoulder pain was 4.8±2.7 (2±1 active, 2.9±2.2 latent). Wheelchair basketball players and able-bodied basketball players without shoulder pain exhibited a similar number of latent trigger points (2.4±2.0 and 2.4±1.8, respectively). Wheelchair basketball players with shoulder pain exhibited higher number of active myofascial trigger points than those without pain (either with or without wheelchair), but all groups had a similar number of latent trigger points (P<0.05). CONCLUSIONS: The reported mechanical pain hypersensitivity suggests that active trigger points may play a role in the development of shoulder pain in elite male wheelchair basketball players.

Es común que los asocien a un dolor referido a la articulación témporomandibular, inclusive a un dolor dental de origen no odontogénico, donde se ve incluida esta estructura.


Abstract A lot of the patients present primary headaches and most of the time, they are not connected in the general practice of dentistry, it´s common that the patient relates the pain to the symptom of TMJ referral pain, or another dental pain non odontogenic, muscular or others.

BACKGROUND AND PURPOSE: The lack of clear knowledge about the etiology of nonspecific neck pain (NS-NP) strengthens the need for other mechanisms, still poorly described in the literature, to be investigated. Therefore, a quantitative analysis of two cases of NS-NP in subjects with functiona dyspepsia was conducted in order to verify the immediate and seven-day postintervention effects of visceral manipulation (VM) to the stomach and liver on neck pain, cervical range of motion (ROM), and electromyographic (EMG) activity of the upper trapezius muscle. CASE DESCRIPTION: Case A was an 18-year-old female with a complaint of nonspecific neck pain for one year, with reported pain on waking, momentary intermittent pain, and occasional symptoms of paresthesia in the upper limbs. Case B was a 25-year-old female with a complaint of cervical pain for one year, accompanied by pain in the unilateral temporomandibular joint, and medial thoracic region. Both cases presented functional dyspepsia. OUTCOMES: The results demonstrated (subjects A and B, respectively) a general increase in cervical ROM (range: 12.5% to 44.44%) and amplitude of the EMG signal (immediately postintervention: 57.62 and 20.78; post seven days: 53.54% and 18.83%), and an increase in muscle fiber conduction velocity immediately postintervention (4.44% and 7.44%) and a decrease seven days postintervention (25.25% and 21.18%). For pain, a decrease was observed immediately postintervention (23.07% and 76.92%) and seven days postintervention (100% for both subjects). DISCUSSION: A single VM provided important clinical improvement in neck pain, cervical spine range of motion, and EMG activity of the upper trapezius muscle, immediately and seven days postintervention in two NS-NP subjects with functional dyspepsia.

BACKGROUND: Quillaja saponaria Mol. bark contains a high concentration of triterpene saponins that have been used for centuries as a cleansing, antiinflammatory and analgesic agent in Chilean folk medicine. In earlier studies, in mice, both the anti-inflammatory as well as the antinociceptive effect of the major sapogenin, quillaic acid have been demonstrated (QA). OBJECTIVE: To determine the antihyperalgesic effect of QA one and seven days after itpl administration of complete Freund's adjuvant (CFA) in male mice using the hot plate test in the presence of complete Freund's adjuvant (HP/CFA) as an acute and chronic skeletal muscle pain model. METHODS: The present study evaluated the antihyperalgesic activity of QA against acute and chronic skeletal muscle pain models in mice using the hot plate test in the presence of complete Freund's adjuvant (HP/CFA), at 24 h (acute assay) and 7 days (chronic assay) , with dexketoprofen (DEX) as the reference drug. RESULTS: In acute and chronic skeletal muscle pain assays, QA at 30 mg/kg ip elicited its maximal antihyperalgesic effects (65.0% and 53.4%) at 24 h and 7 days, respectively. The maximal effect of DEX (99.0 and 94.1 at 24 h and 7 days, respectively) was induced at 100 mg/kg. CONCLUSION: QA and DEX elicit dose-dependent antihyperalgesic effects against acute and chronic skeletal muscle pain, but QA is more potent than DEX in the early and late periods of inflammatory pain induced by CFA.

We compared in-task affect to HIIE and MICE, and its relationship with time spent at different metabolic domains, perceived exertion (RPE), self-efficacy, enjoyment, and future intention of exercise in overweight inactive men. Muscle damage and soreness, and inflammation were assessed post-exercise. Fifteen participants (28.9 ±â€¯5.0 yr; 29.2 ±â€¯3.8 kg/m2) completed a HIIE (10 × 1 min at 100% Vmax, 1 min recovery) and MICE (20 min at 55-59% VO2reserve) session. Affect, alertness, RPE, and self-efficacy were assessed in-task, and enjoyment and future intention post-task. At baseline, 24 and 48 h, creatine kinase, lactate dehydrogenase, interleukin-6 and -10, tumor necrosis factor alpha, and muscle soreness were assessed. Affect (-3.1 ±â€¯1.8 vs. 0.8 ±â€¯1.8, P < .001) and self-efficacy (70 ±â€¯15 vs. 90 ±â€¯15%, P < .001) were lower, while RPE and alertness were higher in HIIE compared to MICE (Ps ≤ .02). Affect was negatively correlated with RPE in HIIE (r = -0.90) and MICE (r = -0.72), and time spent above respiratory compensation point in HIIE (r = -0.59). Affect was positively correlated with self-efficacy in MICE (r = 0.74). Enjoyment, future intention, muscle damage and soreness, and inflammation were similar between HIIE and MICE post-exercise. Therefore, in-task HIIE was experienced as unpleasant compared to MICE, but the psychological and physiological responses post-task were similar in overweight inactive men.

Introducción. En la práctica deportiva, la fatiga muscular es un factor de riesgo para lesión muscular, constituyéndose en un problema que aparece cuando la carga mecánica a la cual se somete un músculo o grupo muscular es superior a la resistencia de este. Objetivo. Describir las intervenciones con crioterapia en Fatiga Muscular post esfuerzo, en individuos que realizan práctica deportiva, a través de una Revisión Sistemática. Método. Se realizó la búsqueda en bases de datos PUBMED/MEDLINE, EMBASE, LILACS, SCIELO, SCIENCE DIRECT, Registro Cochrane Central de Ensayos Controlados con la estrategia de búsqueda: Sports Medicine, Cryotherapy, Muscle Fatigue, Recovery of Function, Physical Exertion, Muscle Damage. La identificación y detección de las referencias fueron exportadas al gestor de referencias MENDELEY. La evaluación del Riesgo de Sesgo se realizó con el uso del software Review Manager y la calidad metodológica con lo reportado por la Base de Datos PEDro. Resultados. Se evaluaron 19 ensayos clínicos aleatorizados en los cuales se evidencia que la crioterapia reporta cambios en la fatiga muscular desde la disminución del dolor y biomarcadores. La inmersión en agua fría fue la modalidad más efectiva para acelerar la cinética de recuperación 72 horas después del ejercicio; demostró menor dolor y mayores niveles de recuperación, percibidos a lo largo de 24 a 48 horas después del ejercicio. Conclusión. De la totalidad de modalidades de crioterapia, la inmersión en agua fría es la que reporta mejores resultados en el proceso de recuperación de la fatiga muscular.


Introduction. In sports practice, muscle fatigue is a risk factor for muscle injury becoming a problem that appears when the mechanical load to which a muscle or muscle group is subjected is greater than its resistance. Objective. Describe post-effort Muscular Fatigue in active Men and Women who perform sports practice against interventions with cryotherapy, through a Systematic Review. Method. PUBMED / MEDLINE, EMBASE, LILACS, SCIELO, SCIENCE DIRECT, Central Cochrane Registry of Controlled Trials with the search strategy were searched: Sports Medicine, Cryotherapy, Muscle Fatigue, Recovery of Function, Physical Exertion, Muscle Damage The identification and detection of references were exported to the MENDELEY reference manager. The Bias Risk assessment was performed with the use of Review Manager software and methodological quality as reported by the PEDro Database. Results. 19 randomized clinical trials were evaluated in which it is evident that cryotherapy reports changes in muscle fatigue since the decrease in pain, and biomarkers. Cold water immersion was the most effective modality to accelerate recovery kinetics 72 hours after exercise; showed less pain and higher levels of recovery perceived over 24 to 48 hours after exercise. Conclusion. Of all the cryotherapy modalities, the immersion in cold water is the one that reports the best results in the process of recovery of muscular fatigue.


Introdução. Na prática esportiva, a fadiga muscular é um fator de risco para a lesão muscular, tornando- se um problema que ocorre quando a carga mecânica à qual um músculo ou grupo muscular é submetido é superior à resistência do músculo. Objetivo. Descrever as intervenções com crioterapia na fadiga muscular pós-esforço, em indivíduos que realizam prática esportiva, por meio de uma revisão sistemática. Método. Foi realizada uma busca nas bases de dados PUBMED/MEDLINE, EMBASE, LILACS, SCIELO, SCIENCE DIRECT, Registro Central Cochrane de Ensaios Controlados com a estratégia de busca: Medicina Esportiva, Crioterapia, Fadiga Muscular, Recuperação da Função, Esforço Físico, Dano Muscular. A identificação e detecção das referências foram exportadas para o gerenciador de referência do MENDELEY. A avaliação do risco de viés foi realizada com o uso do software Review Manager de revisão e qualidade metodológica com a relatada pelo banco de dados PEDro. Resultados. Dezenove ensaios clínicos randomizados foram avaliados nos quais se evidencia que a crioterapia relata alterações na fadiga muscular desde a diminuição da dor e dos biomarcadores. A imersão em água fria foi a modalidade mais eficaz para acelerar a cinética de recuperação 72 horas após o exercício; demonstrou menos dor e níveis mais altos de recuperação, percebidos entre 24 e 48 horas após o exercício. Conclusão. De todas as modalidades de crioterapia, a imersão em água fria é a que proporciona os melhores resultados no processo de recuperação da fadiga muscular.

ABSTRACT BACKGROUND AND OBJECTIVES: Muscle pain is one of the causes of the leave of absence in physical activities, as well as limiting factor in sports performance. The objective of this study was to evaluate the early and late response to the pain perception threshold after acupuncture in a single application, in subjects submitted to repetitive strain. METHODS: Clinical-experimental quantitative trial, with control group and blind, with 47 healthy women, aged between 18 and 55 years (36.3±10.6), divided deterministically by sequential alternation to the groups: acupuncture (GACP n=16), Sham (Gsham n=16) and control (CGRT n=15). The repetitive strain was performed in the Reformer equipment of the Pilates method, before and after the intervention, with 0,25x40mm needles. The GACP received the needling at point ST36 (Zusanli). The Gsham received superficial needling out of the acupuncture point, and the CGRT only performed the exercise. All patients had the needles for 20 minutes, except for the CGRT who remained only at rest. A digital pressure algometer was used at point BL57 (Chengshan). The algometry was conducted before the intervention and the exercise, immediately after, and after 24 hours. RESULTS: The GACP showed a significant increase in the pain perception threshold when compared to Gsham (p =0.021) and to CGRT (p<0.001) and after 24 hours, with CGRT (p=0.006). The results were coherent with the acupuncture theoretical foundations, suggesting the release of opioid analgesics and applicability in the recovery of post-exercise sore muscles. CONCLUSION: A single acupuncture intervention was able to increase the Pain Perception Threshold immediately and after 24 hours.


RESUMO JUSTIFICATIVA E OBJETIVOS: A dor muscular é uma das causas de afastamento de práticas de atividades físicas e fator limitante no desempenho desportivo. O objetivo deste estudo foi avaliar a resposta imediata e tardia do limiar de percepção dolorosa após estímulo de acupuntura em uma única aplicação, em sujeitos submetidos a esforço repetitivo. MÉTODOS: Ensaio clínico-experimental, quantitativo, com grupo controle e encoberto, 47 mulheres sadias, com idades entre 18 e 55 anos (36,3±10,6), separadas por alocação determinista por alternância sequencial em grupos: acupuntura (GACP n=16), Sham (Gsham n=16) e controle (GCRT n=15). Realizou-se o esforço repetitivo no equipamento Reformer do método Pilates, antes e após a intervenção com agulhas 0,25x40mm. O GACP recebeu agulhamento no ponto E36 (Zusanli). O Gsham agulhamento superficial, fora do ponto de estímulo de acupuntura e o GCRT só realizou o exercício. Todos permaneceram 20 min com agulhas e o GCRT só em repouso. Utilizou-se um algômetro de pressão digital no ponto B57 (Chengshan). A algometria foi realizada antes da intervenção e do exercício, imediatamente após, e após 24 horas. RESULTADOS: O GACP teve aumento significativo do limiar de percepção dolorosa no momento imediato, quando comparado ao Gsham (p=0,021) e ao GCRT (p<0,001), e após 24 horas com o GCRT (p=0,006). Houve coerência dos resultados com as bases teóricas do estímulo da acupuntura, sugerindo liberação de opioides analgésicos e aplicabilidade em recuperação de dores musculares pós-exercícios. CONCLUSÃO: Uma única intervenção de acupuntura foi capaz de aumentar o limiar de percepção dolorosa de imediato e após 24 horas.

In the present work, we investigated the antinociceptive effect of gabapentin in a chronic myositis model and its interference in spinal glial cells. Chronic myositis was induced by injection of Complete Freund Adjuvant (CFA) into the right gastrocnemius (GS) muscle of rats and tests for evaluating mechanical hyperalgesia, thermal hyperalgesia and tactile allodynia were performed. Pharmacological treatment with gabapentin was administrated intrathecally and 100µg and 200µg doses were tested. For analyzing astrocytes and microglia in the spinal cord, immunochemistry assay was performed. It was found that gabapentin 200µg reverted CFA-induced chronic muscle pain bilaterally, in all applied tests and it was able to attenuate microglial but not astrocytes activation in the dorsal horn of spinal cord. In conclusion, gabapentin was able to inhibit hyperalgesia and allodynia in chronic myositis and also to attenuate spinal microglial activation. Therefore, gabapentin could be used as treatment for targeting chronic muscle pain.

Introducción, Los recicladores en su mayoría desarrollan sus actividades en precarias condiciones sanitarias y de seguridad ocupacional. Los síntomas de los trastornos musculoesqueléticos (TME) es un problema que afecta a todos los sectores y ocupaciones, Objetivos, Conocer la percepción de síntomas de los TME, Diseño, Estudio descriptivo transversal. Lugar, Lima Metropolitana, Participantes, Recicladores de cualquier edad y género, Intervenciones, Aplicación del Cuestionario Nórdico, Principales medidas de resultados, Presencia de síntomas de TME por regiones anatómicas, Resultados, De los 131 participantes, 114 cumplieron con los criterios de inclusión, 58,8% (n=57) fueron varones y 41,2% (n=47) mujeres, con una media de 41,2 años (DE: 14,51; min: 18, máx: 77), La región anatómica con más síntomas lue la región lumbar, y lue la dolencia que más interferencia tuvo con las tareas de la casa o el trabajo, En el grupo etario de 20 a 39 años, los síntomas musculoesqueléticos más frecuentes se presentaron en la región cervical (p=0,012), mientras que en los mayores de 50 años se presentaron en las rodillas (p=0,040), Conclusiones, La percepción de síntomas de los TME en los recicladores se presentó predominantemente en la región lumbar y fue la dolencia que más interferencia tuvo con las tareas de casa o el trabajo


Introduction: Recyclers mostly operate in poor sanitary conditions and low occupational satety. The symptoms of musculoskeletal disorders (MSD) represent a problem affecting all the sectors and occupations. Objective, To determine the perception of MSD symptorns. Desiqn, Cross-sectional study, Setting, Metropolitan Lima, Participants, Recyclers of any age and gender, Interventions, Implementation of Nordic Queslionnaire. Main outcome measures, Presence of MSD symptoms according to anatomical reqions. Results, Out of 131 selected participants, 114 met the inclusion criteria of which 58,8% (n = 67) were male and 41.2% (n = 47) female with an average age of 41.2 years (SD: 14,51; min: 18, max: 77), The anatomical region where most of the symptoms occurred was the lumbar region where the disease interlered with most of the household chores or work. In the age group 20-39 years, the most frequent musculoskeletal symptoms occurred in the cervical region (p = 0,012), whereas among people older than 50 years they occurred in the knees (p = 0,040), Conclusions, The perception of the symptoms of musculoskeletal disorders among recyclers was predominantly present in the lumbar region and it was the disease that had the most interlerence with household chores or work.
