Long term hemodialysis spondylarthropathy: an atlanto-occipital location.

A 41 years old patient on hemodialysis for 17 years with a clinical and radiological picture of osteoarticular beta 2 microglobulin amyloidosis, is presented. The patient had skeletal involvements, included cystic lesions of the left hip and both humeral heads and a C5/C6 spondylarthropathy. She had also associated carpal tunnel syndrome. Radiological exploration of the atlanto-occipital articulation demonstrated destructive changes. Histology documented osseous and periarticular deposition of beta 2 microglobulin derived amyloid. This is an unusual location of destructive spondylarthropathy in a patient on chronic hemodialysis.

We report two cases, one of a long-standing epididymitis with a testicular infarct, the second of testicular tumor with torsion. Scrotal ultra-sound is a quick, easily applied, accurate method to follow cases of subacute epididymitis with atypical evolution. In case of testicular torsion, secondary to tumor, it helps to rule out scrotal exploration and to decide of an orchidectomy with a high located surgical approach.

Metatarsal localisation of osteoblastoma is rare. We report a case of a sixteen years old girl. The foot was swollen but unpainful. The plain film showed an extensive, osteolytic tumor of the whole of the first metatarsal bone. The radiological diagnosis could be discussed with an aneurysmal cyst, a chondroma, a desmoplastic fibroma, a fibrous dysplasia, an angioma. The total resection was possible without opening of the tumor and reconstruction was possible with a tibial graft. The pathological diagnosis was benign osteoblastoma. Recovery was uneventful.
