The rapid acceleration of technology development is outpacing our ability to assess it objectively and competently. Developments in gastrointestinal endoscopy technology are occurring at an exponential rate. Driven by a better understanding of nature, better tools for imaging, measuring, sensing, modeling, and analyzing, as well as more sophisticated researchers and research techniques, developments that used to take decades occur in a fraction of the time. This is as true in medicine as it is in other areas of science; it is especially true in gastrointestinal endoscopy.

RESUMO

The use of dilators to enlarge esophageal strictures is an old and widely used technique. New technology and designs permit reduced trauma and complications and may ultimately extend the durability of the procedure. Understanding the physics of dilators and their use is essential for clinicians to achieve maximum therapeutic benefit for their patients.

RESUMO

Clinical experience with 6 and 9 Fr ultrasound imaging catheters (UICs) reveals that several transducer and catheter tip varieties are needed for optimum imaging of diseased intravascular sites. Our UIC design has combined established catheter design and very high frequency ultrasound imaging technology to create a versatile, user configured system for intravascular ultrasound imaging. Optimum use requires proper strategic selection of transducer and catheter sizes, frequencies of operation, and interventional accessories.

RESUMO

Knowledge of the physical principles of balloon dilatation is essential to the safe and efficacious implementation of percutaneous transluminal angioplasty. Choosing the correct size syringe and the correct size for the lesion, using and monitoring the appropriate pressure, and knowing when to stop influence the success of the procedure. Differences in the characteristics of different balloon materials not only result in a difference in feel to the user (pressure-volume relationship), but in a significantly greater dilating force being applied to the lesion at the same pressure. Further studies are required to determine optimum inflation techniques, appropriate balloon size for a specific lesion, and possibly to characterize lesions before and during dilatation.
