RESUMO

OBJECTIVES: This cross-sectional epidemiological study aimed at determining the prevalence of cardiovascular risk factors (CVRF; including obesity, dyslipidaemia, hypertension, diabetes and smoking), among patients from the Algerian sub-population of the "Africa/Middle East Cardiovascular Epidemiological" study attending general practitioners at primary healthcare facilities, and stratified according to their environment (rural/urban), sex and age. PATIENTS AND METHODS: The study sites, located in 10 wilayas (administrative regions), were situated in urban and rural areas (rural populations defined as living at least 50km away from urban centres, or lacking access to suburban transport). RESULTS: Four hundred and ten subjects (262 female, 148 male) were enrolled; 287 subjects were from an urban environment and 123 from a rural environment. Mean age was 50.4 years. Ninety one point eight percent of patients had ≥1 CVRF; 48.2% had ≥3 CVRF. Prevalence for the different CVRF was: 61.7% for dyslipidaemia; 39.5% for hypertension; 25.0% for diabetes; 10.0% for smoking, 70.0% for abdominal obesity and 32.0% for a body mass index ≥30kg/m2. CONCLUSION: The high prevalence of all CVRF observed in the Algeria sub-group, especially among the rural population, should encourage us to develop a carefully planned strategy for primary prevention, opportunistic screening and early management, in both urban and rural settings, and with particular attention to young adults. These actions should involve all state bodies and those active in civil society, in order to guarantee full achievement of set goals. The ACE trial is registered under NCT01243138.

RESUMO

Descreveu-se um surto de yersiniose em uma criação de pampo Trachinotus marginatus estudado no Laboratório de Piscicultura Estuarina e Marinha da Universidade Federal do Rio Grande. A enfermidade manifestou-se por transtornos natatórios, exoftalmia com panoftalmite e pela clássica boca vermelha, nome pelo qual se denomina "enfermidade da boca vermelha" (EBV). Na necropsia, foram observados focos de hemorragia peritoneal, esplenomegalia e hepatomegalia. Microscopicamente, foi observada panoftalmite com infiltrados inflamatórios densos que afetavam quase todas as estruturas oculares. Esses infiltrados estavam constituídos por granulócitos, linfócitos, macrófagos e células granulares eosinofílicas. No exsudado, observaram-se estruturas pequenas, pouco coradas, de aspecto bacteriano Gram negativo. O estudo imuno-histoquímico, que se utilizou de um anticorpo monoclonal anti-Yersinia ruckeri, resultou positivo. Este é o primeiro surto conhecido de yersiniose em Trachinotus marginatus no Brasil.


In the "Laboratório de Piscicultura Estuarina e Marinha da Universidade Federal do Rio Grande" the rearing of Trachinotus marginatus (pompano) is studied, and in one of these rearing a yersioniosis outbreak occured. The disease was manifested by swimming disturbances, exophthalmiawith panophthalmitis and the classic red mouth, (red mouth disease (RMD). The necropsies revealed focus of peritoneal hemorrhage, esplenomegalyand hepatomegaly.Microscopically a panophthalmitis with dense inflammatory infiltrates was observed, which affected almost all ocular structures. These infiltrates were constituted by granulocytes, lymphocytes, macrophages and eosinophylicgranular cells (ECG), in the exudatessmall structures less colored with Gram negative bacteria aspect were observed. The immunohistochemical, which used a monoclonal antibody anti- Yersinia ruckeri waspositive. The RMD is caused by enterobacteriaGram negative, which affects both freshwater and saltwater fish, with predominance in salmonids. This is the first known yersiniosis outbreak in Trachinotus marginatus in Brazil and it is necessary to keep this disease in mind at the moment of rearing fish of this species.

RESUMO

We found intense myoliquefaction in vivo among specimens of wild and farmed South American flatfish Paralichthys orbignyanus Valenciennes, 1839. The soft flesh condition was attributed to Myxobolus sp. since the fish were not infected by myxosporeans which are usually associated with this condition (Kudoa spp.), and other causes (bacteria, fungi) were not found. The histopathology of the infection is described.

Myxobolus platanus n. sp. infecting the spleen of Mugil platanus Günther, 1880 (Osteichthyes, Mugilidae) from Lagoa dos Patos, Brazil is described The parasites formed round or slightly oval whitish plasmodia (about 0.05-0.1mm in diameter) on the surface of the organ. The spores were round in frontal view and oval in lateral view, 10.7µm (10-11) long, 10.8µm (10-11) wide and 5µm thick, and presented four sutural marks along the sutural edge. The polar capsules, equal in size, were prominent, surpassing the mid-length of the spore, and were oval with the posterior extremity rounded, and converging with their anteriorly tapered ends. They were 7.7µm (7-8) long and 3.8µm (3.5-4) wide. A small intercapsular appendix was present. The polar filament formed five to six coils obliquely placed to the axis of the polar capsule. No mucous envelope or distinct iodinophilous vacuole were found


Descreve-se Myxobolus platanus n. sp. infectando o baço de Mugil platanus Günther, 1880 (Osteichthyes, Mugilidae) da Lagoa dos Patos, Brasil. Os parasitas formavam plasmódios brancos redondos ou ligeiramente ovais (diâmetro de cerca de 0,05-0,1mm) à superfície do órgão. Os esporos eram circulares em observação frontal e ovais em obervação lateral, medindo, em média, 10,7µm (10-11) de comprimento, 10,8µm (10-11) de largura e 5µm de espessura, e tinham quatro marcas suturais ao longo da linha de sutura. As cápsulas polares eram grandes e do mesmo tamanho ultrapassando a zona média do esporo. Eram de forma oval, tendo a extremidade posterior arredondada, e convergiam pelas extremidades anteriores afiladas, medindo 7,7µm (7-8) de comprimento por 3,8µm (3,5-4) de largura. Um pequeno apêndice intercapsular estava presente. O filamento polar formava cinco a seis dobras colocadas obliquamente em relação ao eixo da cápsula. Não havia envelope mucígeno nem vacúolo iodofílico

Previous work using two-point linkage analysis showed that performing a lod score (LOD) analysis twice, once assuming dominant and once assuming recessive inheritance, and then taking the larger of the two values (designated MMLS) usually has more power to detect linkage than any other method tested. Using computer simulation for a variety of complex inheritance models, we demonstrated power for the MMLS comparable with analysis assuming the true model. However, reports in the literature suggested that the MMLS approach might fail to detect linkage using multipoint analysis due to genetic model misspecification. Here, we tested the robustness of the MMLS approach under multipoint analysis. We simulated data under complex inheritance models, including heterogeneity, epistatic, and additive models. We examined the expected maximum LOD, LOD assuming heterogeneity (HLOD), and nonparametric linkage statistics and the corresponding estimated position in a chromosomal interval of 10 markers with 10% recombination between markers. The mean estimates of position were generally good for all three statistics except when heterogeneity existed, where the LOD and the NPL did not perform as well as the HLOD. The MMLS approach was as robust using multipoint as using two-point linkage analysis. LOD and/or the HLOD generally had more power to detect linkage than NPL across a variety of generating models, even after correcting for the multiple tests. For finding linkage to one locus of several contributing to disease expression, assuming the dominant and recessive models with reduced penetrance is a good approximation of the mode of inheritance at that locus.

Several methods have been proposed for linkage analysis of complex traits with unknown mode of inheritance. These methods include the LOD score maximized over disease models (MMLS) and the "nonparametric" linkage (NPL) statistic. In previous work, we evaluated the increase of type I error when maximizing over two or more genetic models, and we compared the power of MMLS to detect linkage, in a number of complex modes of inheritance, with analysis assuming the true model. In the present study, we compare MMLS and NPL directly. We simulated 100 data sets with 20 families each, using 26 generating models: (1) 4 intermediate models (penetrance of heterozygote between that of the two homozygotes); (2) 6 two-locus additive models; and (3) 16 two-locus heterogeneity models (admixture alpha = 1.0,.7,.5, and.3; alpha = 1.0 replicates simple Mendelian models). For LOD scores, we assumed dominant and recessive inheritance with 50% penetrance. We took the higher of the two maximum LOD scores and subtracted 0.3 to correct for multiple tests (MMLS-C). We compared expected maximum LOD scores and power, using MMLS-C and NPL as well as the true model. Since NPL uses only the affected family members, we also performed an affecteds-only analysis using MMLS-C. The MMLS-C was both uniformly more powerful than NPL for most cases we examined, except when linkage information was low, and close to the results for the true model under locus heterogeneity. We still found better power for the MMLS-C compared with NPL in affecteds-only analysis. The results show that use of two simple modes of inheritance at a fixed penetrance can have more power than NPL when the trait mode of inheritance is complex and when there is heterogeneity in the data set.

It is well known that maximizing the maximum LOD score over multiple parameter values or models (i.e., the method of mod scores, or MMLS), will inflate type I error, compared with assuming only one parameter value/model in the linkage analysis. On the other hand, a mod score often has greater power to detect linkage than does a LOD score (Z) calculated under a wrong genetic model. Therefore, it is of interest to determine the actual magnitude of type I error in realistic genetic situations. Simulated data sets with no linkage were generated under three dominant and three recessive single-locus models, with reduced penetrance (f = .8, .5, and .2). Data sets were analyzed for linkage by (1) maximizing over penetrance only, (2) maximizing over "dominance model" (i.e., dominant versus recessive), and (3) maximizing over both penetrance and dominance model simultaneously. In (1), the resultant significance levels were approximately doubled, compared with baseline values if one had not maximized over penetrances (i.e., compared with a one-sided chi2(1)). In (2), significance levels were increased somewhat less, and, in (3), they were increased by approximately two to three times (but not more than four times) over those of the one-sided chi2(1). This means that, for a given size of test alpha, an investigator would need to increase the Z used as a test criterion, by approximately 0.30 LOD units for analyses as in (1) or (2) and by 0.60 Z units for analyses as in (3). These guidelines, which are valid up to approximately Z = 3.0, are conservative for (1) and are very conservative for (2) and (3). By quantifying the increase in significance level (or, correspondingly, the increase in Z), our findings will enable users to rationally assess the advantages versus the disadvantages of mod scores.
