RESUMO

Study of the cytochemical characteristics of enzymatic activity of peripheral blood cells in the newborns with a history of medium-severe-chronic intrauterine hypoxia revealed shifts in the levels of succinate dehydrogenase and a-glycerophosphate dehydrogenase. These data imply that changed enzymatic activity of dehydrogenases is an unfavorable prognostic sign indicating failure of adaptation and deterioration of reactivity in the early neonatal period.

RESUMO

Eighty-seven parturients were examined to disclose the function of the thyroid during narcotic exposure in labor and in cesarean section in cases of high risk of giving birth to infants with perinatal abnormalities. Neurotropic agents were found to inhibit the fetal hypophyseo-thyroid system in cases with a history of intrauterine hypoxia, thus saving this system from exhaustion. Employment of various types of general anesthesia in cesarean section in this patient population provides an adequate maternal neurovegetative protection from surgical aggression.

RESUMO

Myomectomy has been performed during cesarean section in 205 women with a uterine myoma. Guidelines for surgical and anesthesiological management, fluid and transfusion therapy during and after the operation were obtained for this population of women. A stepwise approach was determined to prevent postoperative complications of interventions of a greater scope during cesarean section in women with uterine myoma.

RESUMO

During cesarean delivery, the isoenzymic spectrum of lactate dehydrogenase and lactic acid levels were examined in 109 parturients with chronic fetal hypoxia. For initial anesthesia, sombrevin, kalipsol, hexenal were applied, basal anesthesia was performed with neuroleptic analgetics and combined electric anesthesia. The findings make it possible to choose the optimal anesthetic agents in this group of parturients in order to make the status of a fetus and a neonate stable and better.

The protective effect of the narcoanalgetics, viadril (hydroxydione sodium succinate) and administered intraperitoneally in doses of 5 and 20 mg/kg was studied in the experiment on 146 pregnant noninbred white rats with simulated hemic hypoxia in the late pregnancy (the 19th day). The aforementioned rats were studied for the composition of the peripheral blood, their weight gain oxidative isoenzymes (LDH), pH and pO2 in the blood. Wilson's technique was used for the assessment of the fetal status. The offspring was studied with regard to the weight increment, the rate of postnatal mortality, behavioral reactions, etc. It was stated that viadril strengthened the resistance of pregnant animals to hypoxia and contributed to the alleviation of late sequela of posthypoxic changes in the fetus and newborn.

Cortisol was measured in 50 parturients with low birthweight (LBW) fetuses in the course of vaginal delivery (n-18) and cesarean section (n-32). Twenty normal parturients whose pregnancy and labor were uncomplicated were taken as controls. In the main group, low birthweights corresponded to gestational age in 30 parturients, and fetal growth retardation (FGR) was diagnosed in 20. In the course of vaginal delivery, maternal cortisol level increased significantly, the rise being higher in parturients with the FGR syndrome. In both groups, blood cortisol levels were higher in the umbilical artery as compared to umbilical vein; in FGR cases, umbilical-artery cortisol level approached the respective values in control newborns. Following cesarean section, LBW fetuses showed considerably lower umbilical cortisol levels. Separate examination of cortisol fractions has demonstrated that free cortisol goes up in the maternal blood both at vaginal and abdominal delivery. The effect of corticosteroids, used to prevent respiratory distress syndrome, on maternal adrenal activity and LBW fetuses has been evaluated.

The activity of some enzymes was examined in blood serum and lymphocytes of women with chronic salpingo-oophoritis in combination with tumourous processes of the genital organs. The objects of the study were 96 women, divided into three groups: first group (control)-20 healthy women, second group-37 women with chronic salpingo-oophoritis and third group-39 women, who suffered from chronic salpingo-oophoritis together with tumours and tumourous processes of genital organs-uterine myoma, endometriosis, cysts and cystomas of the ovaries. The studies showed that in women with continuous course of inflammatory processes there were phenomena in the lymphocytes characteristic of tension of adaptation-compensatory reactions, but inhibition and lowering of the activity of energy metabolism was observed in women with tumours and tumourous processes at the same time. Lymphocyte acid phosphatase, asparataminotransferase, hydroxy-butiratdehydrogenase and lactatdehydrogenase appears to be a prognostic sign for early discovery of features of tumourous process in the female organism on the background of chronic inflammatory process of genital organs.

Some types of anesthesia, used usually in labor anesthesia (sodium hydroxybutyrate, lexir) or in abdominal delivery (neuroleptanesthesia, cetalar narcosis, electroanesthesia) did not affect distinctly the activity of amine oxidases in blood sera of women in labor and of fetus. In some groups of women in labor the neonates were found, blood serum of which exhibited high activity of diamine oxidase, not observed in normal state. A slight decrease in deamination of benzylamine (20%) and 4-nitrobenzylamine (30%) was observed in patients with gynecological diseases within 2 and 3 days after operations. If intestinal paresis developed in the patients within the postoperational period deamination of these substrates was decreased by 75-80% in blood sera. After 2-4 courses of hyperbaric oxygenation a slight but statistically distinct decrease (by 22-25%) in the rate of deamination of benzylamine and 4-nitrobenzylamine was found in blood sera of the patients.
