RESUMO

La esquistosomiasis es una de las enfermedades parasitarias más extendidas en el mundo. La infección por Schistosoma haematobium es habitual en inmigrantes procedentes de áreas endémicas. El hombre se infecta por las larvas que penetran a través de la piel al realizar actividades relacionadas con el agua contaminada. La sintomatología más frecuente que aparece es una hematuria, incluso macroscópica. No debemos olvidar la existencia de enfermedades inusuales en nuestro medio. Presentamos el caso de un varón inmigrante (Mali) con hematuria por esquistosomiasis vesical. El estudio parasitológico en orina detectó huevos de Schistosoma. Actualmente, el paciente está asintomático tras tratamiento con Praziquantel (AU)


Schistosomiasis is one of the most widespread parasitic diseases in the world. Schistosoma haematobium infection is usually found in immigrants from endemic areas. Schistosoma species is acquired by exposure to fresh water that harbours cercariae released by infected snails and penetrating the skin. Haematuria is the most common symptom. We must not forget the existence of unusual diseases in our environment. We present a case of an immigrant male (from Mali) with haematuria caused by bladder schistosomiasis. Parasitological study reported Schimosoma´s eggs. The patient is currently asymptomatic after being treated with Praziquantel (AU)

RESUMO

Aplasia cutis congenita is a rare disorder characterized by localized absence of skin at birth. Type V in Frieden's classification, which is associated with fetus papyraceous or placental infarcts, occurs as a large cutaneous defect on the trunk and extremities. The patient we report had a lesion affecting the trunk and extremities symmetrically, with no family history of the disorder or chromosomal abnormalities. In our opinion, despite the absence of fetus papyraceous or placental infarct, this patient's condition can be classified as type V.

After acute administration of ammonium chloride, infants 1 to 16 months of age were similar to older children in their capacity to acidify their urine. The infants had a higher rate of excretion of titratable acid and a lower rate of excretion of ammonium but were similar in their rate of excretion of total hydrogen ion.Bicarbonate titrations performed in infants during the first year of life demonstrated a threshold ranging from 21.5 to 22.5 mmoles per L, maximal rate of reabsorption from 2.6 to 2.9 mmoles per 100 ml glomerular filtrate, and marked titration splay. A nephronic frequency distribution curve of the ratio of glomerular filtration rate to tubular reabsorptive capacity demonstrated both heterogeneity and skewing to the right, suggesting the presence of significant numbers of nephrons with low tubular transport capacity relative to filtration rate.It is suggested that the "physiologic acidosis" of the infant is due neither to a limited renal capacity to excrete hydrogen ion nor to a reduced capacity for reabsorption of bicarbonate, but rather to a low renal plasma bicarbonate threshold. Although the level of the threshold may relate to the kinetics of bicarbonate reabsorption during this period, it appears to be due at least in part to functional and morphologic heterogeneity of nephrons.
