RESUMO

Objectives: This study aimed to describe the "how-to" details and processes for developing and evaluating a short-term international student research and education program. Methods: This study included two parts: development and implementation, and evaluation of the program. A foreign doctoral nursing student requested to visit the West Virginia University School of Nursing for research training and academic teaching experience. The global nursing program coordinator and the lead mentor met and identified the program basis and team participants. Learning activities were related to this student's interest in older adults and in-home end-of-life care, included four contents: research; educational engagement; practice/clinical engagement; special activities & professional development training and courses. The program evaluation was conducted at the end of the six-month visit using a focus group and an anonymous short survey. Participants (n = 10) included the host university's faculty, graduate students, and one short-term international student. Results: Content analysis of the participant's short survey answers and the focus group discussion resulted in three themes: 1) lessons learned from participating in the program (enhanced diversity in education and research, gaining new global healthcare knowledge, and self-reflection on and promoting cultural awareness); 2) program barriers and challenges (the visa application process, the English language barrier, the lack of public transportation in the town, and scheduling conflicts); and 3) strategies to overcome these barriers and challenges (adding cultural diversity information into curriculum; attending English proficiency with others in the class; using different communication techniques; continuous collaboration with the University's Office of Global Affairs for the Visa application process; providing interactive experiences). All participants rated their engagement in the program as very satisfactory (Mean = 4.67, SD = 0.50), and highly impacted their nursing skills in research, teaching, and clinical practice (Mean = 4.67, SD = 0.50). Conclusions: All the participants have gained some experience in this program, which can be used for future teaching guidance. Further, international nursing students need assistance to adjust and thrive in the host university learning community. Faculty must be culturally sensitive, understand the visiting foreign nursing student' learning needs, and advocate for an environment suitable for enhancing the students' learning.

Among African tribes, Inuit, the Druze, Hinduism, Buddhism, Jainism, and Sikhism, a diversity of attitudes toward suicide exists. The meaning of suicide appears to be largely a reflection of the impact of the death on friends, family, and the broader community; the circumstances of the individual within their community; and the specific religious implications of suicide. The interpersonal impact is seen as consisting of the material impact of suicide; the spiritual and emotional burden endured by the community; and norms related to suicide. Individual factors include the degree of social integration; the social impact of suicide before death; and how the individual anticipates suicide affecting their afterlife. Taken together, the commonly repeated notion that reincarnation beliefs lend themselves to suicidal behavior finds little support, and instead it appears that social and pragmatic issues shape the meaning and interpretation of religious beliefs which in turn buffer or facilitate suicidal behavior.

Consumption of methylmercury (MeHg)-contaminated fish is the primary source of MeHg in humans and poses a hazard to human health. Because of widespread atmospheric deposition of inorganic mercury (IHg), all water bodies in the United States have been contaminated with Hg. In aquatic ecosystems, IHg is converted to MeHg, which biomagnifies, reaching high concentrations in piscivorous fish. It is not possible for governmental agencies to monitor fish from every waterbody to determine if concentrations of MeHg in fish are hazardous to human health. To help government agencies focus their monitoring efforts, it is critical that we develop the ability to predict regions where waterbodies are most likely to contain fish with hazardous concentrations of MeHg. The objective of the present study was to examine the relationship between MeHg contamination of largemouth bass (Micropterus salmoides), a popular piscivorous gamefish, and land cover in 24 ecoregions across 15 states in the southeastern United States. In our study we demonstrate for the first time that 72% of the variance in average concentrations of MeHg in largemouth bass between ecoregions of the southeastern United States can be explained by the percentage coverage by evergreen forests, emergent herbaceous wetlands, and pasture/hay. Land cover determines the sensitivity of freshwater systems to atmospheric IHg deposition, and the present study suggests that at the ecoregion scale, MeHg bioaccumulation in piscivorous gamefish, and ultimately the health hazard that these MeHg-contaminated fish pose to humans, can be in part predicted by land-cover type. Hence, the ISoP Education Committee through its working group dedicated to Central Indexing and knowledge Dissemination has built a database of worldwide educational programs and most common references in Pharmacometrics.

PURPOSE: Patients with chronic pain and/or spasticity who have an intrathecal targeted drug delivery (TDD) pump require frequent needle access procedures to refill the pump's medication reservoir. Some patients find the access procedure painful and/or anxiety provoking. The purpose of this study was to determine if a nursing intervention of providing a distraction with a music relaxation video during the pump access procedure would reduce patients' pain and state anxiety. DESIGN/METHODS: In this longitudinal, pre-test post-test, quasi-experimental design, patients watched a 13-minute calming music video while the nurse performed the needle access procedure. Data collection instruments included a combination of researcher-developed questionnaires, the Defense & Veterans Pain Rating Scale (DVPRS), and the State Trait Anxiety Inventory (STAI). Pain and anxiety were measured pre- and post-pump refill procedures at baseline with no intervention (T1) and at two subsequent pump refills with the music video intervention (T2 and T3). RESULTS: The sample consisted of 31 adults with chronic pain (n = 28) or spasticity (n = 3). All participants had an implanted pump for at least six months. Specialty trained nurses refilled the participants' pump in their home. The findings support the use of a music video as an effective intervention for reducing participants' perception of pain and anxiety during the pump refill procedure. Neither pre-test pain nor pre-test anxiety scores differed across time. Post-test mean pain scores decreased consistently across time (T1 = 5.55, T2 = 4.42, T3 = 4.23; P = .002). Post-test anxiety scores showed a significant decrease (P <.05) between T1 (M = 37.87) and T2 (M = 30.94) which was not sustained at T3 (M = 35.68, P = .284). Overall, the majority of participants indicated liking the music video intervention. They also expressed interest in using the intervention for future refill procedures or other stressful situations. CONCLUSIONS: The music video was effective in producing a sustained reduction over time of participants' pain during the pump refill procedures; it was most effective in initially reducing participants' state anxiety at the initial intervention visit; however, it did not demonstrate a progressively sustained effect. Participants' opinions about the music video experience were positive.

Fish consumption advisories are used to inform citizens in the United States about noncommercial game fish with hazardous levels of methylmercury (MeHg). The US Environmental Protection Agency (USEPA) suggests issuing a fish consumption advisory when concentrations of MeHg in fish exceed a human health screening value of 300 ng/g. However, states have authority to develop their own systems for issuing fish consumption advisories for MeHg. Five states in the south central United States (Arkansas, Louisiana, Mississippi, Oklahoma, and Texas) issue advisories for the general human population when concentrations of MeHg exceed 700 ng/g to 1000 ng/g. The objective of the present study was to estimate the increase in fish consumption advisories that would occur if these states followed USEPA recommendations. The authors used the National Descriptive Model of Mercury in Fish to estimate the mercury concentrations in 5 size categories of largemouth bass-equivalent fish at 766 lentic and lotic sites within the 5 states. The authors found that states in this region have not issued site-specific fish consumption advisories for most of the water bodies that would have such advisories if USEPA recommendations were followed. One outcome of the present study may be to stimulate discussion between scientists and policy makers at the federal and state levels about appropriate screening values to protect the public from the health hazards of consuming MeHg-contaminated game fish.

To retrospectively evaluate prognostic accuracy of subjective assessment of right ventricle (RV) enlargement on CT pulmonary angiography (CTPA) images in comparison with objective measures of RV enlargement in patients with acute pulmonary embolism (PE). For 200 consecutive patients with acute PE, two readers blinded to patient outcomes subjectively determined whether the maximum RV diameter was greater than that of the left ventricle (LV) using axial CTPA images. For the objective measurements, RV/LV diameter ratios were calculated using axial images and 4-chamber reformatted images. For all assessments, sensitivities and specificities for predicting PE-related death within 30-days and a composite outcome including PE-related death or the need for intensive therapies were compared. The agreement between two readers was 91.5% (kappa = 0.83) and all other assessments had pair-wise agreement over 75% (kappa = 0.53-0.72). There was no significant difference in sensitivity between the subjective and objective methods for predicting both outcomes. The specificity for subjective RV enlargement (55.4-67.7%) was significantly higher than objective measures (45.8-53.1%), except for the 4-chamber views where, for one reader, the specificity of the subjective evaluation was higher but did not reach statistical significance. Complex measurements of RV/LV diameter ratios may not be needed to maximize the prognostic value from CTPA. The radiologist who interprets the CTPA images should report RV enlargement when the RV diameter subjectively appears larger than the LV.

The purpose of this study was to estimate dose reduction after implementation of asymmetrical cone beam processing using exposure differences measured in a water phantom and a small cohort of clinical coronary CTA patients. Two separate 320 × 0.5 mm detector row scans of a water phantom used identical cardiac acquisition parameters before and after software modifications from symmetric to asymmetric cone beam acquisition and processing. Exposure was measured at the phantom surface with Optically Stimulated Luminescence (OSL) dosimeters at 12 equally spaced angular locations. Mean HU and standard deviation (SD) for both approaches were compared using ROI measurements obtained at the center plus four peripheral locations in the water phantom. To assess image quality, mean HU and standard deviation (SD) for both approaches were compared using ROI measurements obtained at five points within the water phantom. Retrospective evaluation of 64 patients (37 symmetric; 27 asymmetric acquisition) included clinical data, scanning parameters, quantitative plus qualitative image assessment, and estimated radiation dose. In the water phantom, the asymmetric cone beam processing reduces exposure by approximately 20% with no change in image quality. The clinical coronary CTA patient groups had comparable demographics. The estimated dose reduction after implementation of the asymmetric approach was roughly 24% with no significant difference between the symmetric and asymmetric approach with respect to objective measures of image quality or subjective assessment using a four point scale. When compared to a symmetric approach, the decreased exposure, subsequent lower patient radiation dose, and similar image quality from asymmetric cone beam processing supports its routine clinical use.

OBJECTIVE: The objective of our study was to evaluate the difference in coronary enhancement provided by 60 versus 80 mL of contrast medium (370 mg I/mL) for prospectively ECG-gated single-heartbeat axial 320-MDCT. MATERIALS AND METHODS: We retrospectively evaluated 108 consecutive 320-MDCT angiography studies. Group 1 (n = 36) received 60 mL of an iodinated contrast medium and group 2 (n = 72), 80 mL. All patients were imaged with a standardized protocol: iopamidol 370 followed by 40 mL of saline, both administered at a rate of 6 mL/s. Two imagers subjectively assessed image quality throughout the coronary arteries. Region-of-interest attenuation (HU) measurements were performed in the aorta plus the proximal and distal coronary arteries. RESULTS: Subjective analysis of all coronary segments showed slightly better image quality for group 2. Patients in group 1 had significantly (p < 0.05) lower mean attenuation values for the individual coronary vessels. Nevertheless, 96.7% of all coronary segments in the group 1 patients had an attenuation of greater than 300 HU; when analysis was limited to group 1 patients with a body mass index of greater than 30, 92.8% of the segments were more than 300 HU, and all segments measured more than 250 HU. CONCLUSION: An injection protocol based on 60 mL of iopamidol (370 mg I/mL) for prospectively ECG-gated wide-area detector single-heartbeat coronary CT angiography (CTA) has less coronary enhancement than a protocol based on 80 mL. However, using 60 mL, more than 96% of coronary segments had sufficient enhancement (i.e., > 300 HU), supporting the general use of 60-mL protocols for clinical wide-area detector coronary CTA.
