RESUMO

Introduction : La tympanoplastie en technique underlay vise à mettre un greffon en dessous des reli quats tympaniques. Le but de l'étude était d'en évaluer les résultats anatomiques et fonctionnels au Bénin. Méthode : il s'est agi d'une étude rétrospective réalisée dans le service d'ORL-CCF de l'Hôpital d'Instruction des Armées de Cotonou de novembre 2016 à octobre 2018. Ont été inclus tous les patients ayant bénéficié d'une tympanoplastie en technique underlay pour une otite moyenne chronique simple à tympan ouvert. Les données anatomiques et fonctionnelles pré et post-opératoires ont été comparées à trois mois post-opératoire. Résultat : En deux ans, 10 patients ont bénéficié d'une tympanoplastie soit une incidence de 5 cas par an. L'âge moyen des patients était de 26 ans avec des extrêmes de 17 et 39 ans et une sex-ratio de 0,66. L'abord chirurgical retro-auriculaire et la technique underlay ont été utilisés pour tous les patients. Aucun patient n'a été opéré des deux oreilles. La fermeture de la perfo ration tympanique a été objectivée chez 8 patients sur 10 avec 2 cas de surinfection post-opératoire. Le gain audiométrique moyen a été de 25 dB. Le rinne audiométrique était supérieur à 20 dB chez tous les patients en pré-opératoire et chez 3 patients en post-opératoire. Conclusion : La tympanoplastie est une chirurgie rare au Bénin. Réalisée par la voie retro-auriculaire et la technique underlay, elle a permis d'obtenir d'excellents résultats en termes de fermeture anatomique du tympan mais aussi un gain auditif significatif. C'est une technique à recommander dans nos contextes au Bénin

RESUMO

Introduction : les pathologies ORL de l'enfant sont multiples et diversifiées. Le cout élevé de leur prise en charge ainsi que l'accessibilité limitée aux soins démontrent la nécessité de renforcer la prévention. Le but de travail était de recenser les aspects épidémio-cliniques des affections ORL de l'enfant et d'en analyser les mesures de prévention primaire. Méthode : l'étude s'est déroulée dans le service d'ORL-CCF du CNHU de Cotonou. Elle a été descriptive avec un recrutement prospectif du 1er Janvier au 30 Juin 2018. Ont été inclus les enfants de 0 à 18 ans reçus en consultation ORL. Les données cliniques ont été analysées. Résultats : les enfants ont représenté 25,79 % des consultants (311 enfants sur 1206 patients). L'âge moyen était de 7,20 ± 5,36 ans et 164 parmi eux (52,73 %) étaient de sexe masculin. Les pathologies observées étaient : infectieuses/inflammatoires 181 cas (58,20 %), traumatiques 56 cas (18 %), fonctionnelles 26 cas (8,36 %), tumorales 20 cas (6,43 %), malformatives 17 cas (5,47 %) et vasculaires 11 cas (3,54 %). Les dix principales pathologies ORL infantiles étaient : otites moyennes aiguës 60 cas (19,29 %), corps étrangers ORL 47 cas (15,11 %), hypertrophies amygdaliennes 36 cas (11,58 %), rhinopharyngites 21 cas (06,75 %), amygdalites 19 cas (6,11 %), hypoacousies isolées 18 cas (5,79 %), rhinites 17 cas (5,47 %), freins de langue 9 cas (2,89 %), adénopathies cervicales 7 cas (2,25 %) et otite externe 5 cas (1,61 %). Conclusion : Les pathologies ORL de l'enfant sont fréquentes en consultation. Elles sont dominées par les infections et les traumatismes. Le suivi rigoureux du calendrier vaccinal et les sensibilisations sur les règles d'hygiène de vie des enfants pourraient contribuer à la réduction de leurs incidences


ENT pathologies in children are multiple and diverse. The high cost of their care and the limited accessibility to care demonstrate the need to strengthen prevention. The aim of the work was to identify the epidemiological and clinical aspects of ENT disorders in children and to analyze their primary prevention measures. Method: The study was conducted in the Department of Ear-Nose-Throat Head and Neck Surgery (ENT HNS-CFS) of the National University Hospital Center of Cotonou. It was a descriptive study realized through a prospective enrollment. We included in this study, children aged from 0 to 18 years that were received in the ENT's consultation during the period from January 1st to June 30th, 2018. Socio-demographic and clinical data were analyzed. Results: Children represented 25.79% of the consultants (311 children among 1206 patients). The mean age was 7.20 ± 5.36 years and 164 among them (52.73%) were male. The pathologies identified were: the infectious / inflammatory pathologies with 181 cases (58.20%), the traumatic ones with 56 cases (18%), functional with 26 cases (8.36%), tumoral with 20 cases (6,43%), malformations with 17 cases (5.47%), and vascular pathologies with 11 cases (3.54%). The ten most common childhood ENT diseases were: acute otitis media with 60 cases (19.29%), ENT foreign bodies with 47 cases (15.11%), tonsil hypertrophies with 36 cases (11.58%), Rhinopharyngitis with 21 cases (06.75%), tonsillitis with 19 cases (6.11%), isolated hearing loss with 18 cases (5.79%), rhinitis with 17 cases (5.47%), tongue brakes with 9 cases (2.89%), cervical lymphadenopathies with 7 cases (2.25%) and external otitis with 5 cases (1.61%). Conclusion: The ENT pathologies of the child are frequent in consultation. They are dominated by infections and trauma. Rigorous monitoring of the immunization schedule and raising awareness of healthy lifestyle rules for children could help reduce their impacT

OBJECTIVE: To study rhinologic signs associated with nasal tobacco (snuff) intake in Parakou, northern Benin. MATERIALS AND METHODS: A cross-sectional descriptive comparative study included 300 tobacco snuff takers and 300 subjects who did not use tobacco at all. The sampling technique was a stratified 4-stage random sample for non-users and a convenience non-random sample for snuff takers. RESULTS: The sex-ratio was 0.92 in non-users and 41.9 in snuff takers. Duration of snuff taking was more than 20 years in 24.3% of cases. The symptoms studied were significantly more frequent in snuff takers than non-users (P<0.05). Snoring was reported by 58.3% of snuff takers, versus 5.7% of non-users (P=0.000). Nasal obstruction and rhinorrhea were reported by respectively 26.3% and 22.7% of snuff takers, versus 6.3% and 5.3% of non-users (P=0.000). Hyposmia was reported by 14% of snuff takers, versus 1.3% of non-users (P=0.000). Anterior rhinoscopy found abnormalities in 81.7% of snuff takers, compared to 10.67% of non-users. Nasal hypertrichosis was more frequent in snuff takers than non-users: 40% versus 5% (P=0.000). CONCLUSION: Nasal intake of tobacco is responsible for morbidity in snuff takers compared to non-users of tobacco.

RESUMO

Des études récentes ont montré que l'élévation des aminotransferases et l'infection par Helicobacter pylori aggravaient l'évolution de l'infection par le virus de l'hépatite B chez les porteurs chroniques. Le but de la présente étude était de rapporter les facteurs associés à une élévation des ALAT chez des patients porteurs d'une infection chronique par le VHB. PATIENTS ET METHODES : Cette étude transversale était menée de mai à aout 2017 dans la clinique Universitaire d'Hépato gastroentérologie du CNHU de Cotonou (Benin). Etaient inclus tous les patients porteurs chroniques d'Ag HBs reçus en consultation au cours de cette période. Tous les patients inclus avaient un examen physique, une exploration fonctionnelle hépatique, incluant les ALAT (N<40UI/L), la recherche de l'antigène de l'Helicobacter pylori dans les selles et la détermination de l'ADN du VHB par PCR. RESULTATS : Sur les 121 patients porteurs chroniques d'Ag HBs (M/F 84/37, sex ratio 2.11, âge moyen 42. 3±11.6 ans [18-74], 70 (57.9%) étaient infectés par Helicobacter pylori. La valeur moyenne des ALAT (en UI/L) était de 70.7 ± 99.9 [13 - 665]. Etaient associés à une élévation des Amino transférases, la présence d'une infection par Hp (89.1 ± 15.3) vs (43.48 ± 5), p = 0.01, l'âge (entre 20 et 60 ans vs âge <20 ans ou >60 ans (p=0.000). N'étaient pas statiquement associés, le sexe, l'indice de masse corporelle p=(0.12), la consommation d'alcool (p = 0, 81), ni le niveau de la charge virale ADNVHB (p=0.07). CONCLUSION : L'infection par Hp et l'âge sont des facteurs indépendants d'élévation des ALAT chez ces porteurs chroniques de l'Ag HBs. Une recherche systématique d'infection par Hp et son éradication devraient être indiquées avant le traitement éventuel de l'hépatite chronique B

RESUMO

Le schwannome malin est une tumeur des gaines nerveuses développée aux dépens des cellules de Schwann. Il est extrêmement rare dans la région cervico-faciale, et surtout à la mandibule. L'intérêt de cette présentation est qu'elle a permis de mettre en évidence les difficultés de prise en charge de cette tumeur. Nous rapportons le cas d'une patiente de 60 ans qui avait subi en avril 2018 à la clinique mutualiste de Cotonou une résection interruptrice de l'hémimandibule droite pour un schwannome malin du nerf dentaire inférieur droit. Il y a eu récidive 4 mois après la résection. La revue de littérature faite à l'occasion de la présentation de ce cas avait mis en exergue les réelles difficultés diagnostiques et thérapeutiques de cette tumeur. C'est une tumeur agressive avec un taux élevé de métastases et de récidives locales. Le schwannome malin du nerf dentaire inférieur est de pronostic sombre car, en dépit de la résection chirurgicale, seule option thérapeutique dans notre contexte, il y a eu récidive au bout de 4 mois

Introduction : Le goitre "bénin" est une hypertrophie diffuse de la thyroïde normo-fonctionnelle, non-inflammatoire et non-cancéreuse. Le but de la présente étude était d'analyser les particularités de prise en charge des goitres pluri-nodulaires relevant d'une indication chirurgicale. Matériel et méthode : Cette étude de cohorte rétrospective a été réalisée du 1er janvier 2013 au 1er janvier 2018 à l'Hôpital d'Instruction des Armées - Centre Hospitalier Universitaire de Cotonou, Bénin. Ont été inclus les patients opérés pour goitre pluri nodulaire bénin. Ont été exclus les patients atteints de pathologies thyroïdiennes non nodulaires ou de cancer thyroïdien. Les données étudiées ont été : l'âge, le sexe, la profession, le lieu de résidence, le motif de consultation, les signes cliniques, les signes échographiques, la prise en charge chirurgicale, les complications per et post-opératoires et l'histologie définitive de la pièce d'exérèse. Ces données ont été colligées à l'aide du logiciel Excel 2016. Résultats : Pendant la période d'étude 2,46% des consultations ORL ont bénéficié d'une prise en charge chirurgicale pour goitre pluri-nodulaire bénin (19 cas/an) : 71 femmes (72,45%) et 27 hommes (27,55%), moyenne d'âge de 36 ans (17 à 44 ans). D'un point de vue socio-économique : 50 patients exerçaient une profession libérale et 35 patients étaient sans emploi fixe. Les motifs de consultation étaient une masse cervicale (67 patients), des signes d'hyperthyroïdie (20 patients) ou des signes de compression de l'axe viscéral du cou (11 patients). La durée moyenne avant consultation ORL était de 7,62 ans (1 à 18 ans). L'examen clinique avait objectivé une glande thyroïde "ferme" (70 patients ; 71,4%) pluri-nodulaire (76 patients ; 77,55%) sans adénopathie cervicale palpable (100%). Deux tiers des patients avaient un goitre de grade 3 de l'OMS. Les gestes chirurgicaux étaient une thyroïdectomie subtotale (52 ; 53,1%), une thyroïdectomie totale (30 patients ; 30,6%) et une lobectomie unilatérale avec isthmectomie (16 patients ; 16,3%). L'examen anatomo-pathologique a été réalisé pour toutes les pièces de thyroïdectomie objectivant un goitre macro-folliculaire chez 51 patients (52,04%). Aucun cas de décès n'a été enregistré. Les suites opératoires ont été marquées par un taux de paralysie récurrentielle de 2,78 % en termes de nerf à risque ainsi qu'une reprise chirurgicale pour drainage d'hématome compressif (2,04%). Conclusion : Le traitement de choix du goitre pluri-nodulaire bénin est théoriquement la thyroïdectomie totale. Cependant, le recours à une l'opothérapie et la nécessité d'un long suivi post-opératoire limite ses indications chez les patients de niveau socio-économique défavorable. Les techniques chirurgicales de thyroïdectomies partielles ou subtotales gardent tout leur intérêt dans ces indications

AIMS: Allergic rhinitis (AR) is an extremely common disease. This study aimed to determine the prevalence and severity of AR symptoms and potential associated factors, in Parakou, a city in Benin. MATERIALS AND METHODS: A cross-sectional study was conducted between April and July 2016, involving inhabitants of the city aged between 6 and 65 years. Cluster sampling was carried out. AR symptoms were defined as the simultaneous presence of rhinorrhea, nasal obstruction and sneezing without evidence of respiratory infection. Data were analyzed on EpiData Analysis v2.2.2.183 and then on R.3.2.1. RESULTS: Three hundred and ninety-five inhabitants were questioned, with a male/female ratio of 1.32, and median age of 19 years. One hundred and forty-one (35.7%) had AR symptoms, 47 (33.3%) had family history of atopy, and 87 (61.7%) had allergic conjunctivitis. The main triggers were house dust (139; 98.5%) and air pollution (111; 78.7%). On multivariate analysis, associated factors were age<18 years (P<0.05), exposure to house dust (P<0.001), and personal history of urticaria (P<0.001) or allergic conjunctivitis (P<0.01). Twenty-eight subjects (19.9%) had persistent symptoms. Forty-three (30.5%) had moderate/severe symptoms, associated with age≥18 years (P<0.01), family history of atopy (P<0.01), personal history of atopic eczema/dermatitis (P<0.01), and presence of a dog or cat at home (P=0.01). CONCLUSION: The prevalence of AR symptoms in Parakou was high. A non-negligible proportion of symptoms were severe. This disease merits special attention in the city.

OBJECTIVES: Cervicofacial cellulitises are dominated by dental affections. The aim of this study is to describe etio-epidemiological and therapeutic aspects of cervicofacial cellulitis of dental cause at the CNHU-HKM in Cotonou (Benin). METHODS: It's a retrospective study from 1998 to 2007 included patients with inflammatory cervicofacial tumefaction of dental cause. A glycaemia, a HIV serology and a blood formula numeration are realized. Some patients practiced standard oral and dental radio. Patients were hospitalized and received mono, bi or triple antibiotic. Lancing with swab of pus for cytobacteriological and antibiogram examination was realized for some of patients. Dental cares were been systematic. RESULTS: 68 cases whose 53 dental cause where been collected 77.94%. Bracket concern patients between 0-20 and 20-40 years old (64.15%). Males were, more concerned. Dental traumatisms find again (18.88%), tooth decay (56.60%) and 24.53% patients practiced dental extraction. Anti inflammatory practiced by 67.92% patients associated to antibiotic in 41.51% cases. The three latest molar were concerned to 26 patients. Among those last 22 had attack of mandibular teeth. Treatment consisted in lancing drainage to 2/3 patients in 83.40% cases associated to tri antibiotic. Evolution has been favourable with recovery to 86.67%; 6 decreases to 13.33% cases. CONCLUSIONS: Cervicofacial cellulitises are dangerous, rapidly responsible of deceases by extension. A better oral and dental hygiene would improved prognosis.

OBJECTIVE: Sinonasal inverted papilloma is a rare benign tumor with a high recurrence rate and potential malignant transformation. The purpose of this study was to analyze the clinical and radiological aspects and to identify the suitable surgical approaches to be used in developing countries. METHODOLOGY: In 3 years, 7 cases have been analyzed in a retrospective study. Patients presented with unilateral nasal obstruction and epitasis, a whitish unilateral polypoid mass, sinonasal opacity with osteolysis on CT scan or on sinus X-ray. Using these data we adopted the Krouse staging which classifies the disease in four stages and accordingly the appropriate surgical approach was used. The diagnosis was confirmed by histological examination of the biopsied specimen. RESULTS: Average age 48.28 years, predominant sex male. Symptom: unilateral nasal obstruction 5, bilateral nasal obstruction 1, epistaxis 5. The average time-delay before consultation was 73 months. The combined endonasal endoscopic and vestibular was the most commonly used approach (4 cases). The results were satisfying. After a minimal follow up of 2 years, one case of recurrence was detected after 4 years. CONCLUSION: CT scan is an essential tool for the diagnosis and detecting the extension of sinonasal inverted papillomas. Better results were obtained with a surgical approach that combined a vestibular approach and an endonasal endoscopic approach in our region.

From October 1st, 2003 to March 31, 2004 about six months, 45 patients with ENT manifestations of HIV have been followed as well at the ENT service of CNHU of Cotonou and at the departmental hospital of Porto-Novo. 26 among then have HIV serologic positif. From this prospective study, we retain that the patients from 20 to 49 years old represented 84,6%. Male has predominated: 57,7%. The socio-professional strata with low revenue and those with itinerant professions are the greatest target: 87,7%. The transmission mode has been heterosexual in 80%. The HIV1 has been isolated in 88,5%. As a result of clinical observation, 50,3% of patients have recurring infections of superior aero-digestive tract : otitis, sinusitis and oropharyngeal mycosys. The facial paralysis has been noted in 10,8%, chronic cervical adenopathies 10,8% and parotid tumors 8,1%.

INTRODUCTION: Cemento-ossifying fibroma is a rare benign tumor which can affect both jaw bones, particularly the mandible. CASE REPORTS: We report two cases of cemento-ossifying fibroma which illustrate the radiological and anatomic features of these tumors. Differential diagnosis between cementifying fibroma and ossifying fibroma was particularly difficult. The cementifying fibroma was treated by surgical resection followed by immediate reconstruction with an iliac bone graft. The ossifying fibroma was treated by enucleation-resection. DISCUSSION: The clinical, radiological and histological features of cementifying and ossifying fibroma allow distinction. Surgical treatment is achieved by enucleation resection for small-sized ossifying fibromas and mono-bloc resection with bone reconstruction for large-sized cementifying and ossifying fibromas.

RESUMO

OBJECTIVE: Report of 2 cases of sphenochoanal polyp. MATERIAL AND METHOD: We present two cases of sphenochoanal polyp aged of 22 and 53. They were operated by endonasal endoscopic approach. They are still free of symptoms and no recurrence are observed after 13 and 56 months of follow-up. CONCLUSION: The clinical, radiological and pathological features of the sphenochoanal polyp are reviewed. Different etiopathogenic hypothesis are discussed and particular the gastroesophageal reflux. The treatment is surgical.

Haemorrhagic cysts of the parathyroid gland are rare. Our case report concerns a presentation with a constrictive cervical haematoma, confirmed by ultrasound-guided aspiration. The haematoma, rapidly recurred, indicating exploration of the neck and haemostasis. Histological examination of excised tissue showed the haemorrhagic nature of the cyst.

RESUMO

Isolated inflammatory sphenoid sinusitis, associated with complications are rare. Authors report 23 cases in 8 years. The left sphenoid is the most often involved, in 50 years or more old females. Neurological complications are the most frequent (75% of the cases): isolated headache, neuro-ophtalmologic paralysis, meningitis, before respiratory complications (20%). A bacteriological etiology was found in 60% of the cases, and a fungal etiology in 25%. The treatment is based on a large endoscopic marsupialization of the sphenoid. Recovery is the rule, except for neuro-ophtalmologic paralysis which recovery is inconstant.

RESUMO

Endonasal microsurgery, which has widely proved its efficacy, is not free of complications. They can be due to the technical act itself. In this case they result of surgery mistake and they induce an effraction of the operatory cavity walls. They can also be due to excessive healing of the nasal mucosa. Then they represent the real cause of failure in this type of surgery. The authors present their experience that they compare with similar cases of the literature.

RESUMO

The authors reports the case of a 34 years old patient with a cicatricial pemphigoid whose diagnosis was made on a nasal symptomatology, isolated, persistent and with an increasing aggravation over the five last years. This auto-immune bullous disease is quite unusual and has got the trend of a synechia characteristic. The mucous membrane O.R.L. attacks are scarce not to say exceptional in their nasal aspect. The treatment based on Disulone has quickly allowed a progress over a trouble caused by the lesions but had to be interrupted for problems of tolerance.
