RESUMO

OBJECTIVE: The aim of the study is the increasing the efficiency and improving the quality of dental surgical care for patients with tumor diseases of the blood system. MATERIAL AND METHODS: For the period from 2020 to 2022 The authors examined and treated 15 patients with tumor diseases of the blood system who were hospitalized at the National Medical Research Center for Hematology of the Ministry of Health of the Russian Federation. Of these, 11 provided dental surgical benefit. There were 5 men (33%), women - 10 (67%). The mean age of the patients was 52 years. A total of 12 surgical operations were performed: biopsies - 5; opening of the infiltrate - 3; imposition of secondary sutures - 1; bougienage of the salivary gland ducts - 1, removal of the salivary gland -1 and amputation of the tooth root - 1. Four patients underwent conservative treatment. RESULTS: The use of methods of local hemostasis made it possible to minimize the number of hemorrhagic complications. Thus, the development of external bleeding from the postoperative wound was observed in 1 (20%) (out of 5) patients in the group of acute leukemia. Hematoma was diagnosed in 2 patients. The sutures were removed on the 12th day. Finally, the wounds were epithelialized by an average of 17 days. CONCLUSION: The authors believe that the most common surgical intervention in patients with tumorous blood diseases is a biopsy, which should be performed with partial resection of the tissue surrounding the tumor. In the course of dental interventions, hematological patients may develop complications caused by suppression of immunity and fatal bleeding.

RESUMO

Reperfusion damage to the cellular structures of tissues in the early post-ischemic period is a consequence of the restoration of blood flow and reoxygenation. Currently, there is no effective treatment for reperfusion metabolic disorders in clinical practice. Over the past decades, biological studies of hypoxia and the role of hypoxia-inducible factor-1α (HIF-1), potentiating succinatoxidase oxidation by signal from the succinate-dependent receptor (GPR91), have significantly improved the understanding of oxygen homeostasis during the period of recovery of blood flow. HIF-1 plays a key role in postischemic damage and is an oxygen-sensitive transcription factor that mediates adaptive metabolic responses to hypoxia and hyperoxia during reperfusion and reoxygenation. Activation of HIF-1 by succinate improves cell survival in hypoxic and posthypoxic (hyperoxygenated) environment, altering energy metabolism, proliferation, angiogenesis and vascular remodeling. The role of succinate oxidation in the period of ischemia / reperfusion and reoxygenation suggests the widespread use of infusion succinate as a protector that reduces the degree of tissue damage by reactive oxygen species (ROS) and restores the usual oxygen homeostasis.

RESUMO

THE AIM OF THE STUDY: To familiarize dentists with the features of the clinical course of acute purulent mumps occurring in patients with COVID-19. MATERIAL AND METHODS: Four patients aged 73 to 89 years with acute bilateral purulent mumps, which occurred against the background of COVID-19, were under observation. The patients were treated for COVID-19, and acute purulent mumps was also treated. RESULTS: As a result of treatment, two patients were discharged in satisfactory condition on the 9th and 10th days under medical supervision at their place of residence. Their mumps phenomena have completely subsided. Two patients died from acute respiratory failure, cerebral edema, septic condition, acute renal and hepatic insufficiency. From the parotid glands, acute inflammatory phenomena subsided. CONCLUSION: It was found that one of the complications of COVID-19 may be the development of acute bilateral purulent mumps, which aggravated the course of the underlying disease. Inflammation of the parotid glands proceeded according to the type of acute postoperative mumps against the background of a sharp decrease in the secretory activity of the salivary glands according to the type of asialia. Timely comprehensive treatment of acute mumps allows in some cases to avoid a fatal outcome.

RESUMO

The operation of complex tooth extraction in patients with hemorrhagic syndrome is a difficult task. The authors developed a method of complex tooth extraction in their retention in patients with hemorrhagic syndrome. The study found that the proposed method allowed to reduce the risk of hemorrhagic complications and optimize the healing of post-extraction wounds. The procedure was carried-out in 11 patients with teeth retention and impaired eruption. The paper presents advantages and restrictions of the method.

RESUMO

The influence of the significantly nonstationary Raman contribution to nonlinear susceptibility on the dynamics of femtosecond optical solitons interaction and decay is theoretically analyzed. The theoretical results are in good quantitative agreement with the recent experiments of Mitschke and Mollenauer [Opt. Lett. 11, 659 (1986); 12, 355 1987)].

RESUMO

We theoretically examine the interaction between two solitons in a double-mode optical fiber. The bound state between two solitons of different modes is investigated, including both dark and bright solitons. It is shown that interaction between dark solitons as well as between bright solitons is always attractive, but the interaction between bright and dark solitons may be repulsive. Analytical results are in agreement with numerical ones.
