BACKGROUND: Proton pump inhibitors (PPIs) are the mainstay of treatment for acid peptic diseases (APDs), but are often irrationally prescribed in clinical practice. Appropriate prescription of PPIs is needed to optimize outcomes, and minimize risks and cost burden on the healthcare system. OBJECTIVE: To review available literature on efficacy and safety of proton pump inhibitors (PPIs) and give recommendations for rational use of PPIs from an Indian perspective. METHODS: Twelve healthcare professionals (9 gastroenterologists, 1 cardiologist, 1 orthopedist, 1 clinical pharmacologist) comprised the expert group; members disclosed conflicts of interest. The creation of the expert review was through a process that included meetings (in-person, online, telephone) where each professional contributed their experiences with regards to efficacy and safety of PPIs. Articles published between the years 2000 and 2017 were reviewed for evaluation of safety and efficacy of PPIs in treatment of various APDs. CONCLUSION: This expert review provides key recommendations for decision making in order to minimize the irrational use of PPIs. Some significant recommendations include: patients with GERD and acid-related complications should take a PPI for minimum 12 weeks for healing of esophagitis, and for maximum up to 48 weeks for symptom control. Patients with Barrett's esophagus should take long-term PPI. Patients at high risk for ulcer-related bleeding from NSAIDs including aspirin should take a PPI if they continue to take NSAIDs. Best practice recommendations are meant to merely assist with decision making in conjunction with patients' clinical history, and are not intended to dictate mandatory rules.

INTRODUCTION: Total femur arthroplasty (TFA) is a rare salvage limb procedure which serves as an effective alternative to limb amputation. Most commonly, it is indicated for oncologic orthopedic cases, and very few cases of a purely traumatic history for TFA have been documented. The decision to perform this complex procedure involves paying careful attention to reconstructive needs and functional expectations of the patient. Alternatives to this procedure are quite detrimental and include hip disarticulation and above-the-knee amputation. CASE REPORT: A 66-year-old woman with a history of hypertension, and parkinsonism was brought into the outpatient department of a tertiary care hospital. She has undergone a procedure for proximal femur nailing on her left hip in 2013 due to a trauma-related fracture. However, after the development of a series of complications, she underwent revision surgery on her left hip 6 times by different surgeons. This included surgeries for implant removal and fixation of a custom-made bipolar hemi-replacement hip, followed by a cemented bipolar hip hemi- replacement with plating and cerclage wires followed by infection, the treatment of which entailed implant removal and placement of an antibiotic cement. This was followed by a long stem constrained cemented total hip arthroplasty which also failed. The surgeon then made the decision to perform a TFA. 12-month follow-up post-operatively showed neither peri-prosthetic infection or inflammation nor any leg length discrepancy. The functional outcome on lower extremity function scale showed improvement from 0 pre-operatively to 31 at 12-month follow- up. CONCLUSION: TFA serves as a viable technique for salvage of the lower extremity in cases of infected non-unions. However, a thorough evaluation of the particular case in the hand should be made before reaching a conclusion.

We are presenting a case of cervical (C4) aneurysmal bone cyst in a 13-year-old girl, came to the outpatient department with neck pain and stiffness since 6 months and normal neurology. We did an en bloc excision of locally aggressive tumor through anterior plus posterior approach and stabilization by lateral mass screw fixation and anterior cervical instrumentation. Involvement of several adjacent cervical vertebrae by an aneurysmal bone cyst is rare, and conventional treatment with curettage and bone grafting is most likely to carry a high rate of recurrence and spinal instability. We recommend complete excision of the tumor and instrumentation in a single stage to avoid instability.

AIM: This study was conducted to evaluate the antimicrobial activity of 5.25% sodium hypochlorite (NaOCl), 2% chlorhexidine (CHX) and BioPure MTAD when used as a fnal rinse against Enterococcus faecalis. MATERIALS AND METHODS: Sixty single-rooted premolars were biomechanically prepared, inoculated with E. faecalis and divided into various groups. These were then irrigated with the test irrigants and tested microbiologically for growth of E. faecalis immediately after irrigation and after 48 hours. RESULTS: Statistical analysis showed that there was a signifcant difference between the antibacterial activities of BioPure MTAD, 2% CHX and 5.25% NaOCl at 5 minutes; however, the antibacterial activities of the three irrigants were comparable after 2 days of irrigation. CONCLUSION: The present study concludes that BioPure MTAD is as effective against E. faecalis as 5.25% NaOCl and more effective than 2% CHX. CLINICAL SIGNIFCANCE: E. faecalis is one of the most resistant intracanal species and a possible cause of root canal failure. Many authors have stressed the importance of using antimicrobial irrigants during chemomechanical preparation to ensure complete disinfection. Therefore, various irrigating solutions have been used during and immediately after root canal preparation to remove debris and necrotic pulp tissue and to eliminate microorganisms that cannot be reached by mechanical instrumentation.

RESUMO

Tuberculosis of the spine is one of the most common spine pathology in India. Over last 4 decades a lot has changed in the diagnosis, medical treatment and surgical procedures to treat this disorder. Further developments in diagnosis using molecular genetic techniques, more effective antibiotics and more aggressive surgical protocols have become essential with emergence of multidrug resistant TB. Surgical procedures such as single stage anterior and posterior stabilization, extrapleral dorsal spine anterior stabilization and endoscopic thoracoscopic surgeries have reduced the mortality and morbidity of the surgical procedures. is rapidly progressing. It is a challenge to treat MDR-TB Spine with late onset paraplegia and progressive deformity. Physicians must treat tuberculosis of spine on the basis of Culture and sensitivity.

RESUMO

This is a report of a case in which, for the first time, direct lightning strike had caused burn injuries associated with internal soft-tissue bruising and visceral injury subjacent to lac jewellery on the chest. The burn injury depicted a symmetrical pattern, which was almost 5 times larger than the size of the jewellery. Surprisingly, there was bruising of soft tissue beneath the burn, associated with extravasation of blood in anterior mediastinum, contusion of anterior border of lungs, contusion to heart and rent by burn to the blouse of the victim over the front and back, all in one line, showing very unusual injury of lightning strike.
