RESUMO

PURPOSE: To compare contrast material-enhanced T1-weighted and fluid-attenuated inversion-recovery (FLAIR) magnetic resonance (MR) images in depicting leptomeningeal metastases. MATERIALS AND METHODS: Malignant lesions detected at cytologic examination of cerebrospinal fluid in 70 patients were reviewed. There were 58 studies in which both FLAIR and contrast-enhanced T1-weighted spin-echo MR images were available. A senior neuroradiologist reviewed the images from each sequence individually and separately for signs of leptomeningeal metastases and assigned a diagnostic rating of positive, indeterminate, or negative. RESULTS: Leptomeningeal metastases were depicted in 38 cases on contrast-enhanced T1-weighted spin-echo images and in 20 cases on FLAIR images. In three cases, leptomeningeal metastases were detected by using only FLAIR images. In 20 cases, leptomeningeal metastases were detected by using only contrast-enhanced T1-weighted spin-echo images. FLAIR imaging has a sensitivity of 34% for cytologically proved leptomeningeal metastases. Gadolinium-enhanced MR imaging has a sensitivity of 66%. CONCLUSION: Used alone, contrast-enhanced T1-weighted images are better than FLAIR images for detecting leptomeningeal metastases. This is particularly true for cases in which leptomeningeal metastases manifest primarily or solely as cranial nerve involvement.

BACKGROUND AND PURPOSE: Small case series have reported potential benefit from thrombolysis after spontaneous intraventricular hemorrhage (IVH). Our objective was to review our experience using intraventricular urokinase (UK) in treating selected patients with IVH. METHODS: Using medical records, we identified all patients who received ventriculostomies for CT-confirmed nonaneurysmal nontraumatic spontaneous IVH from December 1992 through November 1996. We reviewed charts and CT images and examined the data for associations with specific outcomes. RESULTS: We identified 40 patients, 18 treated with ventriculostomy alone and 22 receiving adjunctive intraventricular UK. The initial Glasgow Coma Scale (GCS) scores of the two groups were similar (P = 0.5). While there was a trend for patients with any intraparenchymal hemorrhage (IPH) to receive UK (P = 0.07), the mean size of IPH in those who received ventriculostomy alone was larger than in those who received adjunctive UK (P = 0.002). There was lower mortality in the group treated with UK (31.8 versus 66.7%; P = 0.03), but there was only a trend toward an increase in favorable outcome (22.2% versus 36.4%; P = 0.3). Overall, the most significant association with outcome was neurological condition at presentation (GCS >5 versus < or = 5; P = 0.003). Receiving UK did not increase the occurrence of complications or hospital length of stay for survivors (P = 0.5). CONCLUSIONS: Intraventricular UK remains a safe and potentially beneficial intervention. While it appeared to lower mortality, a randomized, placebo-controlled trial is needed to explore whether the therapy can increase the incidence of favorable outcomes.

Frozen, hydrated acrosomal bundles from Limulus sperm were imaged with a 400 kV electron cryomicroscope. Segments of this long bundle can be studied as a P1 crystal with a unit cell containing an acrosomal filament with 28 actin and 28 scruin molecules in 13 helical turns. A novel computational procedure was developed to extract single columns of superimposed acrosomal filaments from the distinctive crystallographic view. Helical reconstruction was used to generate a three-dimensional structure of this computationally isolated acrosomal filament. The scruin molecule is organized into two domains which contact two actin subunits in different strands of the same actin filament. A correlation of Holmes' actin filament model to the density in our acrosomal filament map shows that actin subdomains 1, 2, and 3 match the model density closely. However, actin subdomain 4 matches rather poorly, suggesting that interactions with scruin may have altered actin conformation. Scruin makes extensive interactions with helix-loop-beta motifs in subdomain 3 of one actin subunit and in subdomain 1 of a consecutive actin subunit along the genetic filament helix. These two actin subdomains are structurally homologous and are closely spaced along the actin filament. Our model suggests that scruin, which is derived from a tandemly duplicated gene, has evolved to bind structurally homologous but non-identical positions across two consecutive actin subunits.
