RESUMO

Electrical injuries sometimes cause grave internal lesions that go unnoticed in an initial exploration. Low-tension electrical energy can produce burns of variable depth by flash or conduction, but we rarely find fractures. This case reports a case of bilateral shoulder fractures secondary to low-voltage injury which were undiagnosed in the emergency approach.

RESUMO

The case is presented of a golfer who was struck by lightning while playing golf during a thunderstorm. The patient was found lying unconscious on wet grass with his clothes scorched and his spiked golf shoes torn. He had suffered dermal burns affecting the neck, thorax, abdomen, and upper and lower limbs (10% total body surface area), without any cardiovascular or respiratory disturbances. It may be hypothesized that the lightning current went over the outside of the patient, causing ignition of his clothes. Treatment included monitoring, adequate fluid management, debridement, and topical treatment (silver sulphadiazine). Complete healing of the wounds was achieved in two weeks. After three years' follow-up, the patient had no sequelae.

RESUMO

Presentamos en este trabajo nuestra experiencia en la cirugía de reconstrucción mamaria mediante colgajos locales en doble V. Esta técnica ,descrita por primera vez en el año 1998 por Bolivar de Souza, es sencilla; los colgajos se caracterizan por su facilidad de rotación y trasposición así como por una buena vascularización. Proporciona resultados estéticos satisfactorios asegurando una adecuada forma y posición de la mama gracias a un implante de silicona y favorece la movilidad de la región axilar. Este método, que no pretende sustituir a las técnicas habituales de reconstrucción mamaria, supone una alternativa válida para aquellas pacientes que no presentan graves secuelas debidas a radiodermitis (AU)


We report our experience in breast reconstruction surgery by means of cutaneous flaps in double V. This technique, developed by Bolivar de Souza in 1998, is easy to carry out; the flaps can be easily rotated and transposed, and they are well-vascularized. It achieves good aesthetic result and guarantee an adequate shape and positioning of the breast thanks to a mammary implant, getting good mobility for the axillary region. This technique does not intend to substitute the current techniques used in breast reconstruction, but it represents as an alternative for those patients who don’t suffer from severe cutaneous radiodermitis (AU)
