This case demonstrates the safe step by step approach to treatment of pediatric patients with muscle spasm headache. If there are any neurologic signs or the LiteSplint is not effective, then a laboratory orthopedic appliance therapy may not be effective and a neurologic referral is necessary. It is always required to review the latest physical exam with the parent and physician if the symptoms do not improve in an orderly sequence. The LiteSplint acts as a screening and diagnostic aid in determining the source of head pain. For very young patients (three to six years of age) who may not be able to easily tolerate an appliance, an extra heavy coating of flowable composite that can act as a sealant on the primary molars, e.g. Revolution, may open the bite enough to alleviate headache or earache symptoms. Dental clinicians can perform a valuable service for their patients if headaches from deep bite malocclusions can be diagnosed and treated at an early age.

This author was challenged by the idea of finding a cost-effective way of providing diagnostic appliance therapy for the great number of patients with temporomandibular joint pain. His answer was remoldable craniomandibular appliances. These appliances provide an increase in both vertical dimension and mandibular AP position. He outlines the uses of these appliances for temporomandibular disk displacement, myofascial pain, and bruxism patients.

This article introduces a new appliance for the orthopedic correction of craniofacial disorders. The development of a remoldable craniomandibular (RCM) appliance is the result of four years of clinical research. The remoldable craniomandibular appliance is designed as an inexpensive and time efficient method of arriving at a differential diagnosis for some patients with temporomandibular joint related headache. In addition, clinical observation indicates that it may be helpful in treatment for patients suffering from internal derangement of the temporomandibular joint. Although no specific claims for the appliance are being made at this time, research studies currently designed and implemented may be able to confirm the potential for this appliance. Independent studies are encouraged.
