RESUMO

Introduction: Inguinal hernias are common and typically include a portion of abdominal organs. However, there have been reports of additional peculiar content. Case presentation: The authors present the case of a 68-year-old man with sigmoid colon cancer presenting as a left inguinal hernia. Discussion: Colorectal cancer is a unique component that can be identified within inguinal hernias and is a prevalent problem among affected individuals because such a presentation is unusual. Conclusion: Surgeons should be aware of this risk when operating on inguinal hernias in order to prevent ineffective care. The best course of action may be appropriate exploration and oncological excision when underlying colon cancer is suspected after a hernial procedure.

INTRODUCTION: Hernias of the posterior rectus sheath are very rare abdominal wall hernias with only around 15 reported cases to date. CLINICAL PRESENTATION: This case report examines a 27-year-old female who is presented with epigastric abdominal pain and vomiting. An Abdomen CT scan was done and showed signs of SBO and herniation of the small bowel at the posterior rectus sheath. The patient underwent exploratory laparotomy that showed right-sided posterior rectus sheath obstructed hernia, which was repaired with primary closure. Postoperatively, the patient was doing well and was discharged on postoperative day 3 in good general condition. CONCLUSION: The patient had no complaints during her follow-up at one month. Due to its rarity and potential complications, it is also important to report this case to enhance the evidence base for posterior rectus sheath hernia and to familiarise this uncommon condition to radiologists, clinicians, and surgeons.

RESUMO

Bezoar occurs due to the ingestion of inedible material. The most common bezoar is a phytobezoar, which results from the ingestion of indigestible food particles found in vegetables and fruits. Other types include trichobezoar, which involves hair, lactobezoar, which involves milk products, pharmacobezoar, which involves medication, and in unusual cases, bezoar may involve different materials such as metals, plastics, and paper. We are presenting a case of a 19-year-old patient, a known case of Prader-Willi syndrome, who presented with difficulty breathing and tachypnea after aspiration of grape particles, and then he started to complain of melena and vomiting of dark content. He was admitted for urgent bronchoscopy and endoscopy, which showed a bezoar composed of grapes and threads. He was managed endoscopically by removing most of the threads and grape particles and releasing the tangled threads to facilitate its migration distally. Follow-up endoscopy showed complete resolution of the previously noticed content. We reported this case to discuss the endoscopic management of unusual bezoar involving threads.

Introduction: Recurrency of Pancreatic adenocarcinoma after resection can be as high as 85%. Most of the recurrences happen within two years of pancreatic resection and may be local or present as a metastatic disease. Clinical presentation: Herein, we report a patient who presented with metastatic pulmonary adenocarcinoma after 20 years of curative resection of pancreatic adenocarcinoma. Histopathology of the pulmonary mass confirms the diagnosis as a metastatic adenocarcinoma of gastrointestinal origin. Conclusion: Despite the length of the disease-free period, lung metastasis of pancreatic cancer is the most likely diagnosis according to the clinical course, histopathology and biochemical tumor marker. Tumor marker Ca19-9 is very sensitive and reliable test to detect recurrency of pancreatic cancer even if the imaging modalities cannot detect the tumor.

RESUMO

Mucormycosis is a rare fungal infection mostly encountered in immunosuppressed patients. Other major risk factors are diabetes mellitus, solid organ transplant and chronic use of glucocorticoids. Early diagnosis should be obtained as soon as possible due to the infection's aggressive behavior and high probability of dissemination. Here we present a case of pulmonary mucormycosis in a non-diabetic patient, known to have systemic lupus erythematous and had a renal transplant recently presented with shortness of breath and was treated with antibiotic as a case of chest infection with minimal improvement. Then, after full investigations, he seemed to have mucormycosis which was successfully treated with combined liposomal amphotericin B and resection of the infectious lesion.

BACKGROUND: Thymoma is a rare neoplasm, which may be associated with autoimmune disorders, the occurrence of hyperthyroidism in the patients with thymoma is rare. CLINICAL PRESENTATION: Here we present an extremely rare case of a 56-year-old female patient who was discovered to have malignant thymoma with associated myasthenia gravis and hyperthyroidism due to toxic multinodular goiter. Our patient started to complain of difficulty breathing and swallowing, Chest CT scan was done and revealed an anterior mediastinal mass, measures about 4.1 × 3.1 × 2.2 cm with enlarged lymph node mostly representing thymoma. Complete thymectomy was performed via uniportal video-assisted thoracoscopic surgery, and the patient's postoperative recovery was uneventful. Microscopic histopathology findings corresponded to thymoma type B1. CONCLUSION: The coexistence of thymoma, MG and toxic multinodular goiter is extremely rare. MG should always be suspected in a patient with thymoma. VATS thymectomy has been increasingly used to treat thymoma as it has several advantages over open surgery.
