RESUMO

In an aphakic eye with corneal edema, performing Descemet's stripping automated endothelial keratoplasty (DSAEK) combined with implantation of intraocular lens can be a challenge. This case report describes a surgical technique for postsurgical aphakia with endothelial decompensation in a 42-year-old female with Marfan syndrome and subluxated lens. This technique comprised implanting DSAEK with fibrin glue-assisted sutureless posterior chamber intraocular lens. The donor lenticels were formed on a 60-kHz femtosecond laser platform (IntraLase®). Two partial-thickness scleral flaps and sclerotomies were created, and then, the Descemet's membrane was scored and stripped. A posterior chamber intraocular lens was implanted, and its haptics was pulled out through the sclerotomies and tucked beneath the flaps. The flaps were then apposed with fibrin glue. The donor lenticule was introduced to the anterior chamber and unfolded. Air tamponade was used to stabilize and center it. This technique significantly improved the uncorrected and best-corrected visual acuities of the patient, and no donor dislocations were reported. This case corroborates the findings of few similar cases that have found combined use of glued intraocular lens with DSAEK to be beneficial in such cases.

RESUMO

PURPOSE: To assess intra-correlations between lens density based-on Scheimpflug Imaging System, power used during surgery, surgery duration, and endothelial cell loss in eyes with nuclear cataract. SETTING: Department of Ophthalmology, King Fahd Hospital of the University, Alkhobar, Saudi Arabia. DESIGN: Prospective cross-sectional observational study. METHODS AND MATERIAL: The objective lens density and endothelial cell density were measured using the Scheimpflug system and specular microscopy, respectively. Intra-operatively, all phacodynamic parameters and duration of the surgery were documented. RESULTS: This study of 62 patients (71 eyes) with a mean age of 58.56 ±â€¯10.4 years. The mean Scheimpflug-measured lens density was 13.93 ±â€¯3.27. The mean phacodynamic parameters, namely, power, ultrasound time, and elliptical motion were 13.63 ±â€¯6.38, 1.27 ±â€¯1.12, and 50.56 ±â€¯50.06, respectively. There were a positive linear correlations between the Scheimpflug-measured lens density and phacodynamic parameters, power (AVG %), ultrasound time, and elliptical motion (r = 0.501, r = 0.620, and r = 0.641, respectively; all P < .001), amount of endothelial loss (r = 0.445, P < .001), and surgery duration (r = 0.346; P < .01). Phacodynamic parameters were positively correlated with degree of endothelial loss (P < .01). The length of the surgery failed to show any correlation with damage happened to the endothelium (r = 0.210, P > .05). CONCLUSION: A positive correlations were observed between Scheimpflug-measured lens density with phacodynamic parameters, surgery duration, and endothelial loss. A strong correlation was observed between the degree of endothelial cell loss and phacodynamic parameters but not with the length of the procedure.

BACKGROUND AND OBJECTIVES: Keratoplasty services in Saudi Arabia have progressed steadily in the past few decades. We sought to identify the leading indications and types of keratoplasty performed in the Eastern Province of Saudi Arabia over a six-year period and to compare these indications with published data. DESIGN AND SETTING: This was a retrospective descriptive analysis of the records of patients who underwent keratoplasty at four ophthalmology centers in the Eastern Province between 2008 and 2013. PATIENTS AND METHODS: All keratoplasty procedures were included in the analysis. The primary surgical indication and type of surgery were identified for each case. RESULTS: Keratoplasties included 570 penetrating keratoplasty, 217 deep lamellar keratoplasty, 80 triple procedures, 24 descemet stripping automated endothelial keratoplasty and 12 Boston keratoprosthesis implantations. The mean age of all patients was 28.8 years (range 14-72 years), and 58.9% of the patient were males. The lead.ing indication for keratoplasty was keratoconus 53.10%, bullous keratopathy 13.80%, corneal scarring 10.7%, regrafts 9.1%, and stromal dystrophies 4.9%. CONCLUSIONS: In this study, the leading indications for keratoplasty were keratoconus, bullous keratopathy, corneal scarring, regrafts and stromal dystrophies. A significant increasing trend for descemet's stripping automated endothelial keratoplasty surgeries was observed in spite of the number of cases.
