Meticulous history taking and careful examination should always be performed in all patients because a common symptom, such as sore throat, could be a presentation of an unusual problem. Sore throat from an ingested foreign body can perforate the mucosa and migrate extraluminally penetrating vital structures of the neck. Rare documentation of such cases involves sharp objects and, if untreated, may result in life-threatening complications. This article presents a case of a patient with a 3-week history of sore throat, and diagnostic work-up revealed a sharp metallic foreign body (needle pin) in the hypopharynx, which migrated extraluminally through the posterior pharyngeal wall. There were no signs or lesions on endoscopy to help point the exact entry point and location of the foreign body. Intraoperative imaging using C-arm was used to identify the exact position of the foreign body, which was at the level of C4, 0.5 cm deep to the right lateral posterior pharyngeal wall mucosa, and oriented lateral to the vertebra. The needle pin was subsequently extracted.

Side population (SP) cells mediate chemoresistance in leukemia. However, chemical inhibition approach to target SP cells has been poorly studied. Herein, we report the discovery of isatin derivatives of nicotinic acid amide as potent side population cell inhibitors. The selected derivatives showed superior potency over the reference drug verapamil. Furthermore, the treatment increased chemosensitivity and inhibited the cell proliferation on three different leukemic cell lines, K562, THP-1 and U937, suggesting that both SP and the bulk of leukemic cells are affected. Moreover, treatment with the most potent compound Nic9 reduced the expression of ABCG2, demonstrating that side population inhibition effect of the target derivatives is at least via ABCG2 inhibition. Importantly, the target derivatives induced erythrocyte/dendritic differentiation to leukemic cells mainly through Musashi/Numb pathway modulation.

Four new complexes of Hg (II), Pb (II), Sn (II) and Bi (III) with indomethacin drug ligand (IMC) were synthesized and characterized by using infrared, electronic, 1H-NMR spectral, thermogravimetric and conductivity measurements. The IMC was found to act as bidentate chelating agent. IMC complexes coordinate through the oxygen of the carboxyl group. The molar ratio chelation is 1:2 (M2+:IMC) with general formula [M (IMC) 2], nH2O for Hg (II), Pb(II) and Sn(II), but 1:3 for Bi(III) ions. Antibacterial screening of these heavy metal complexes against Escherichia coli (Gram-ve), Bacillus subtilis (Gram +ve) and anti-fungi (Asperagillus oryzae, Asperagillus niger, Asperagillus Flavus) were investigated. In the present study, we found evidence suggesting that Bi+3/IMC possesses the capacity to protect the stomach, sperm, testes, cellular ATP, cellular NAD, INSL3, PGD2, PGE2 and antioxidant enzymes from deleterious actions of IMC.

RESUMO

A novel poly (vinyl chloride) PVC membrane sensor for Fe(2+) ions is described. The sensor is based on the use of newly synthesized chiral 2,6-bis-(carboxamide methyl ester)pyridine derivative as neutral ionophore in plasticized PVC membrane. The sensor display a fast, stable and near-Nernstian response over a relative wide ferrous concentration range (1 × 10(-3) to 6 × 10(-6) M), with cationic slope of 31.5 ± 0.5, mV per concentration decade over a pH range of 5.0-9.0. The direct determination of 0.25-56.0 µg/ml of ferrous in aqueous solution shows an average recovery of 98.5% and a mean relative standard deviation of 1.5% at 20.0 µg/ml. The sensor displays long life-span, long-term stability, high reproducibility, and short response time. Selectivity coefficients for Fe(II) relative to a number of interfering substances were investigated. The sensor shows high significantly for Fe(2+) over Fe,(3+) Cu,(2+) Zn,(2+) Cd,(2+) Hg,(2+) Pb,(2+) Ni,(2+) Co,(2+) Mn,(2+) Al,(3+) alkaline earth and alkali metal ions. The sensor is successfully applied for measurement of ferrous in drug formulations. The results obtained for the determination of ferrous using the proposed sensor are comparable favourably with those obtained using the spectrophotometric method. The voltammetric behaviour of montelukast (MKST) was studied using cyclic voltammetry, direct current (DCt), differential pulse polarography (DPP) and alternating current (ACt) polarography. MKST exhibited well-defined cathodic waves over the range pH range 1-5. No anodic waves were produced over the same pH range. At pH 1, the analytical pH; the diffusion current constant (Id) was 2.2+/-0.01 microA l mmol-1. The current concentration plot was rectilinear over the range 2-20 microg ml-1 with correlation coefficient (n=10) of 0.9943. The lower limit of detection (S/N=2) was 0.2 microg ml-1 (3.41x10(-7) M). The wave has been characterised as being diffusion-controlled, although adsorption phenomenon played a limited role in the electrode reaction. The proposed method was successfully applied to the determination of MKST in commercial tablets, and results were in agreement with those given with a reference HPLC method. The method was further extended to the in vitro determination of the drug in spiked human plasma. The mean % recovery (n=5) was 101.38+/-3.85. The number of electrons transferred in the reduction process could be accomplished and a proposal of the electrode reaction was proposed.

OBJECTIVES: To present in a retrospective report a contemporary series of patients aged 14 years and younger who were treated for stones with ureteroscopy at our institution from 1991 to 2002. With the improvement and miniaturization of ureteroscopes and ancillary instruments, the endoscopic treatment of renal and ureteral calculi in children has become more feasible. METHODS: A retrospective chart review was performed of 23 patients aged 14 years and younger who had undergone ureteroscopy for the treatment of ureteral or renal calculi at our institution. RESULTS: A total of 27 stones were treated in 23 patients. Of the 27 stones, 18 were in the distal ureter, 5 in the mid ureter, 2 in the proximal ureter, and 2 in the renal pelvis. Ureteral dilation was performed in 4 (17.4%) of the 23 patients. The lithotripsy modalities used were holmium:yttrium-aluminum-garnet laser in 16 (69.6%), electrohydraulic lithotripsy in 3 (13%), a combination of holmium laser and electrohydraulic lithotripsy in 2 (8.7%), and basket extraction alone in 2 (8.7%) of 23 patients. Ureteral stents were placed in 21 (91.3%) of 23 patients. The average operative time was 46.9 minutes (range 15 to 92). In 21 (91.3%) of 23 patients, postoperative imaging was available and revealed that 20 (95.2%) of the 21 patients were rendered stone free. Two patients were lost to follow-up. No intraoperative complications occurred. One patient was treated postoperatively with intravenous antibiotics for transient fever. CONCLUSIONS: Ureteroscopy is safe and effective in the management of ureteral and renal calculi in children. In our institution, it has emerged as a valid first-line therapy for the treatment of pediatric urolithiasis.

The voltammetric behaviour of danazol DZ (antigonadotropin) was studied using cyclic voltammetry, direct current, differential pulse polarography (DPP) and alternating current polarography. Danazol exhibited irreversible cathodic waves over the pH range of 1-5 in Britton Robinson buffers. At pH 1 (the analytical pH), a well-defined wave with E1/2 of -1.04 V versus Ag/AgCl reference electrode was obtained. The diffusion current constant (Id) was 4.8+/-0.14 microA.L.m mole(-1) and the current-concentration plot was rectilinear over the range from 5 x 10(-6) to 1 x 10(-4) M with correlation coefficient (n = 11) of 0.995. The calculated detection limit was 1 x 10(-6) M using the DPP mode. The wave was characterized as being irreversible, diffusion-controlled although adsorption phenomenon played a limited role in the electrode process. The proposed method was applied to commercial capsules and the average percentage recovery was in agreement with that obtained by the official USP method. The method was extended to the in vitro determination of DZ in spiked human urine and plasma samples, the percentage recoveries were 96+/-4 and 97+/-5, respectively. A proposal of the electrode reaction was postulated.

Chlordane, is one of many environmental pollutants that reach human or animal body through food and water consumption, its presence may frequently be associated with other metals such as lead. In this work, the toxicity of chlordane and lead to reproductive tissues in Swiss mice was studied. Oral daily doses of 75 and 275 mg/kg b.w. were administered to male mice, for 35 days, animals were sacrificed at the 2nd, 3rd, 4th, and 5th week. The results show that chloradane had a damaging effect on testicular tissues, the effect was obvious through reduction in diameter of the seminiferous tubules, number of spermatogonia, and primary and secondary spermatocytes and spermatids. The effect was remarkably increased by the presence of lead, which needs more investigation in order to determine whether it's a synergistic or additive effect.

The Escherichia coli genome carries seven rRNA (rrn) operons, each containing three rRNA genes. The presence of multiple operons has been an obstacle to many studies of rRNA because the effect of mutations in one operon is diluted by the six remaining wild-type copies. To create a tool useful for manipulating rRNA, we sequentially inactivated from one to all seven of these operons with deletions spanning the 16S and 23S rRNA genes. In the final strain, carrying no intact rRNA operon on the chromosome, rRNA molecules were expressed from a multicopy plasmid containing a single rRNA operon (prrn). Characterization of these rrn deletion strains revealed that deletion of two operons was required to observe a reduction in the growth rate and rRNA/protein ratio. When the number of deletions was extended from three to six, the decrease in the growth rate was slightly more than the decrease in the rRNA/protein ratio, suggesting that ribosome efficiency was reduced. This reduction was most pronounced in the Delta7 prrn strain, in which the growth rate, unlike the rRNA/protein ratio, was not completely restored to wild-type levels by a cloned rRNA operon. The decreases in growth rate and rRNA/protein ratio were surprisingly moderate in the rrn deletion strains; the presence of even a single operon on the chromosome was able to produce as much as 56% of wild-type levels of rRNA. We discuss possible applications of these strains in rRNA studies.
