RESUMO

The study aims to estimate precisely the different levels of mental loop and foramina in the Mosul population of Nineveh governorate in Iraq. It is a cross-sectional study of a previously saved database in the cone-beam computed tomography device. The study sample was 357 patients selected randomly from the database of patients at the Oral Radiology Unit at the Maxillofacial Department in Al-Salam Teaching Hospital. The study variables are mental foramen (MF) position, shape, radiograph appearance, and mental loop presence and direction. The results show that the age group from 19 to 30 years shows the highest percentage (40.1%). Females show more than half of the cases (59.7%). The MF position's highest percentage is shown in 2 areas with the slight differences. These are the long axis of first premolar and the area between first premolar and second premolar (42.6%, 40.9). According to shape variation round shapes are the uppermost percent (53.8%). More than 60% of cases (66.1%) show continuity with the inferior alveolar canal and 50% show no loop presence, and the mental nerve leaves the foramen in a perpendicular direction. In conclusion localization of the MF, nerve, and its loop precisely in both horizontal and vertical directions, both hard and soft tissues in a continuous view. Before arranging any surgical procedure, anatomic markers are crucial. To reduce the danger of harm to the foramen region, the authors thus advised cone-beam computed tomography before scheduling implant surgery, particularly in the mental region of the jaw.

AIMS: The study aimed at analyzing patient's case sheets in regard to the incidence of skeletal discrepancy present in cases and its relation with the demographic profile of the sample. All these are chronicled for more than 10 years periods. METHODOLOGY: This is a retrospective study analysis of the orthognathic case sheets for more than 10 years period. The total numbers of patients are 678. Patient's case sheet was already prepared by the Multi-Disciplinary Team Orthognathic Surgery Clinic in Al-Salam Teaching Hospital, which is the only authorized committee in Nineveh Health Directorate. RESULTS: The highest age percentage is between (18 and 27 y/76%). Angle class III cases are the uppermost cases (36%) from the total. A significant P value is clear at the level ≤0.05 and ≤0.01 between surgery type and discrepancy in anterior segmental osteotomies (upper and lower) which is performed in bi-maxillary protrusion cases and Angle class II cases (0.01**). Similarly, anterior segmental osteotomies (upper jaw only) which are indicated in both open bite and Angle Class II cases documented as a statistically significant P value (0.02*). The positive correlation is shown in all variables with the disharmony or facial discrepancies. Esthetic and beauty as causes for treatment recorded more than function in relation to time series. CONCLUSION: This study documents that patients with skeletal class III accounted for the largest percentage (64%) in the study group. A high increase in patients number seeking treatment for their discrepancy is obvious with time from 2009 till 2022.

Background The outcome of alveolar grafting with synthetic bone substitute (Osteon III) in various bone defect volumes is highlighted. Methods A prospective study was accomplished on 55 patients (6-13 years of age) with unilateral alveolar bone cleft. Osteon III, consisting of hydroxyapatite and tricalcium phosphate, is used to reconstruct the defect. Alveolus defect diameter was calculated before surgery (V1), after 3 months (V2), and finally after 6 months (V3) postsurgery. In the t -test, a significant difference and correlation between V1, V2, and V3 are stated. A p- value of 0.01 is considered a significant difference between parameters. Results The degree of cleft is divided into three categories: small (9 cases), medium (20 patients), and large (26 cases).The bone volume of the clefted site is divided into three steps: volume 1: (mean 18.1091 mm 3 ); step 2: after 3 months, volume 2 resembles the amount of unhealed defect (mean 0.5109 mm 3 ); and the final bone volume assessment is made after 6 months (22.5455 mm 3 ). Both show statistically significant differences in bone volume formation. Conclusion An alloplastic bone substitute can also be used as a graft material because of its unlimited bone retrieval. Osteon III can be used to reconstruct the alveolar cleft smoothly and effectively.

INTRODUCTION: The clinical importance of ankylosis in children is concerning with its massive effect and disturbance on the mandibular future growth causing gross deformity apart from the limited mouth opening. Trauma is the most common cause of bony and fibrous ankylosis. History, physical examination and radiographical examination of each patient with ankylosis of TMJ is mandatory in arriving to a final diagnosis, severity, involvement of adjacent structures and ultimately to plan the treatment. CASE PRESENTATION: Ten years old girl presented with her family seeking solution for a severely limited mouth opening. From the history, the child has suffered from height fall at the age of 4 years. She had complained from swelling and pain near the ear, treated by analgesics, gradually subsided and neglected. This limitation affects on feeding and had an impact on the child's health, seeking for management for the condition became mandatory. Clinical examination, radiographical examination is prepared in beside that ethical approval with full discussion with the parents done. Blind nasal intubation done. DISCUSSION: Patients with true bilateral ankylosis are considered as the aggressive type. It's not just because of ankylosis. Surgeons should keep in their minds that well experienced anesthetist is important; also extension of the ankylosed bone with amount of cutting is important and re ankylosis as a complication must be avoided. CONCLUSION: The surgeons agree with the statement that success in the preventing reankylosis after TMJ gap arthroplasty. Its primarily refers to the early postoperative physiotherapy, maintained on a long term.

RESUMO

Salvage of fractured abutment screw is one of the critical complications, which can be solved in different ways. In this study, an innovative approach is introduced to solve this problem. It is a relatively simple, non-invasive technique, which would provide an excellent result when executed with ultimate care and precision.
