RESUMO

Backgrounds People living with human immunodeficiency virus (HIV) are at a greater risk of chronic kidney disease (CKD) compared to people not having HIV. Evaluating the prevalence of CKD in people living with HIV in Dammam, Saudi Arabia was the main objective of this study. Methods This cross-sectional study included adult HIV patients who were followed at Dammam Medical Complex. The patients' demographic data, comorbid conditions, and HIV history were reviewed from their electronic medical records. Results A total of 729 patients were counted. The glomerular filtration rate (GFR) of 235 patients could not be estimated. The data for the remaining 494 patients were analyzed. The cohort consisted of 406 male patients (82.19%) and 88 female patients (17.81%). The mean ± standard deviation for the patients' age and HIV duration were 39.08±10.93 years and 4.37±3.15 years, respectively. Ten patients (2.02%) had a GFR of <60 mL/min/1.73 m2. Among 136 patients who had an estimated GFR of ≥60 mL/min/1.73 m2 and were tested by a urine examination, 27 patients (19.85%) had albuminuria. Combining the two figures resulted in an estimated prevalence of CKD in HIV patients of 21.47%. Only one patient (0.02%) was receiving dialysis. Conclusions The prevalence of CKD in people living with HIV in Dammam, Saudi Arabia was higher than the general population. The findings highlight the elevated risk of CKD among people living with HIV and emphasize the importance of regular monitoring and early detection of kidney dysfunction in this population.

RESUMO

Colonic intramural hematomas are rarely encountered clinically. They are most commonly caused by blunt trauma to the abdomen. Diagnosis is usually reached by a combination of a detailed history, physical examination, and radiological investigations. A 14-year-old female patient presented to the emergency department complaining of abdominal pain with a history of a go-karting accident. Upon physical examination, the patient was tachycardic and hypertensive, with right-side abdominal tenderness and fullness. After going through routine radiological investigations, a computed tomography scan showed a large intramural hematoma of the ascending colon measuring around 7.7 x 8.4 x 2 cm. The patient was admitted for conservative management. Throughout her admission, serial examinations were performed, which showed improvement in the patient's condition and the size of the hematoma. The patient was discharged in a stable condition after showing good recovery. Following up with the patient a month later, she was in good condition with no active complaints, and an ultrasound was done that revealed complete resolution. To our understanding, this report of colonic intramural hematoma caused by the unusual etiology of the go-karting accident, which was successfully managed conservatively, adds significantly to the literature.

RESUMO

Introduction Selfies are most commonly posted on social media, where their popularity has led to intensified awareness by the subjects of their appearance. This, led to higher demand for cosmetic procedures, intending to enhance how the subject appears in future selfies. Aim This study aims to evaluate the influence of high expectations and selfie photographs on satisfaction level results of facial plastic surgery in Eastern Province, Saudi Arabia. Subject and methods This cross-sectional study was conducted among Eastern Province residents of Saudi Arabia. A self-administered questionnaire was sent to the residents using an online survey. The questionnaire included demographic characteristics (i.e., age, gender, nationality, etc.) and specific questions regarding the influence of selfie photographs and high expectations on the satisfaction rates among patients who had facial plastic surgery. Results Of the 192 participants, 79.2% were females, and 44.8% were aged between 18 to 30 years old. The most commonly sought cosmetic surgery was rhinoplasty (38.5%). 71.4% were taking selfies, while 39.1% perceived better expectations with plastic surgery. High expectations with plastic surgery were more prevalent in older and married participants. It is important to note that an increased satisfaction rate after cosmetic procedures was more associated with high expectations of plastic surgery. Conclusion High expectations directly influence satisfaction with appearance after the cosmetic procedure, but taking selfies does not. Respondents of older age and who were married demonstrated better expectations of plastic surgery compared to the rest of the groups. More investigations are required to confirm the relationship between expectation and taking selfies in terms of satisfaction with self-appearance after cosmetic surgery.

RESUMO

Introduction The prevalence of obesity has experienced a significant global increase in recent years, emerging as a prominent worry affecting numerous individuals throughout various countries, including Saudi Arabia. Bariatric surgery, a common treatment, often leads to excess skin. Despite its benefits, few patients choose body contouring surgery. A cross-sectional study aims to identify barriers, including socioeconomic and psychological factors. Methodology This is a cross-sectional study conducted in Saudi Arabia. Participants included those who underwent bariatric surgery. Data were collected through questionnaires and analyzed by Statistical Product and Service Solutions (SPSS, version 29) (IBM SPSS Statistics for Windows, Armonk, NY). Results Our study involved 662 Saudi participants with post-bariatric surgery, primarily females (386, 58.3%), aged 19-29 (44.3%). Most had undergone bariatric surgery (558, 84.3%), mainly gastric sleeve (485, 73.3%). Excess skin was a common issue (311, 47.0%). Difficulties included rashes and emotional distress (e.g., depression). About 8.3% had body-contouring surgery, including body lifts (13, 23.6%) and liposuction (19, 34.5%). Factors influencing surgery decisions included self-confidence (123, 18.6%) and cost (9.9%). Barriers for 32.2% considering surgery included cost (80.2%) and fear of a second surgery (45.6%). Females (67.1%), Saudis (85.4%), and employed individuals (49.3%) were more likely to consider surgery (p < 0.05). Conclusion Our study highlights the complexity of body-contouring decisions after bariatric surgery in Saudi Arabia. Cost and fear were barriers; females, Saudis, and employed individuals were more likely to consider surgery. A patient-centered approach, addressing barriers, and offering support are crucial for informed choices and improved well-being.

RESUMO

Choanal atresia is a rare developmental condition that is defined as a narrowing or complete blockage of the nasal passages. Rapid surgical management is crucial in cases of bilateral choanal atresia since it may develop into a life-threatening emergency. We present the case of a full-term female newborn who developed mild respiratory distress soon after birth. The pediatrician was not able to insert a feeding tube through the nostrils despite repeated attempts. Cranial computed tomography confirmed the diagnosis of bilateral choanal atresia with an ectopic nostril. Furthermore, echocardiography demonstrated moderate atrial septal defect. The newborn underwent a successful correction of this anomaly via the trans-nasal surgical approach.
