RESUMO

Histoplasmosis is a deep mycosis caused by Histoplasma capsulatum, which has been found in soil with accumulated excreta of bats and birds. This disease has variable clinical findings with only pulmonary or systemic involvement. Upper aerodigestive lesions are found mainly associated with systemic disease, affecting particularly patients with immunosuppression conditions mainly caused by HIV. However, it is uncommon in immunocompetent patients. This report describes a case of oro-laryngeal-esophageal histoplasmosis in a HIV-seronegative patient without detectable systemic involvement.

The aim of the present study was to describe the clinical and histopathological findings of 53 cases of sialoliths retrieved from the Oral Pathology Service, School of Dentistry of Piracicaba, State University of Campinas, São Paulo. They were most frequently found in females and adult patients with mean age of 45 years. Submandibular gland was the more affected, followed by minor salivary and parotid glands. The sialoliths were most frequently described as a single (93.3%), nodule (92.3%) and hard consistency (65.2%). Microscopically, the sialoliths showed lamellated pattern of calcification in 84.9% of cases. Dilated pericalcular ducts were observed in 11 cases and squamous metaplasia was noted in 10 of them (90.9%). Areas of periductal chronic inflammation were observed in 81.8% of the cases. In 10 out of 53 cases salivary glands were available for analysis and the more common finding was mild chronic inflammation associated with acinar atrophy.

Sialolithiasis is one of the most common disorders of particular major salivary glands. It is caused by sialolith within the ducts or parenchyma of particularly major salivary glands. Although sialolithasis is not uncommon, it often is clinically misdiagnosed when minor salivary glands are affected. This article describes the clinical and microscopic findings of nine cases of sialolithiasis of minor salivary glands. The lesions frequently appeared as single and asymptomatic nodules in middle-aged patients. Only three sites were affected: four lesions were found in the upper lip, three in the buccal mucosa, and two in the lower lip. The most common clinical hypotheses for diagnosis were mucocele, sialoadenitis, and benign salivary gland tumor. Sialolithiasis of minor salivary glands was misdiagnosed frequently. It should be considered as a possible diagnosis when swelling of the oral tissues is observed.
