RESUMO

The impact of "Pasechnyi" spa of the Shmakovka health resort on the circadian urinary output, renal excretion of magnesium, calcium, nonorganic phosphorus, oxalates, uric acid, phospholipids, acido- and ammoniogenases, daily fluctuations of urinary pH was studied for the first time in 65 children with chronic pyelonephritis. In the presence of spa treatment the authors revealed a 75-100 per cent increase in the circadian urinary output, urinary excretion of magnesium, uric acid, ammonia, titrated acids, a decrease in the levels of calcium, oxalates, nonorganic phosphorus and acidification of the urine at 9 o'clock in the morning mainly in children with primary pyelonephritis and at 9 and 6 o'clock in the morning in patients with concurrent uricosuria. Other parameters were not significantly different from those in the controls. Acidification of the urine in the presence of high uricosuria resulted in crystalluria of urates and oxalates in 26.31 per cent of patients with the concurrent urate diathesis. The water of Shmakovka mineral springs is recommended for patients with primary pyelonephritis, phosphaturia and calcium oxalate crystalluria with alkaline reaction of the urine and unjustified for those who suffered from urate diathesis.

RESUMO

Changes in subpopulations of OKT 3, OKT 4, OKT 8, HNK-I, B-IgM, and B-IgG-producing lymphocytes in carriers of HIV antibodies were studied. A decrease in OKT 4 helpers/inducers, an increase in OKT 8-suppressors of cytotoxic cells, HNK-I, Ia-positive and B-IgM-producing lymphocytes was demonstrated. The ratio of OKT 4/OKT 8 lymphocytes was below 1.0 in most of the subjects examined. Most marked changes in lymphocyte subpopulations were observed in anti-HIV carriers with lymphadenopathy.

The study included 119 children and 138 adults suffering from viral hepatitis A. The controls consisted of 114 adult patients with hepatitis B. Examinations of the total antibody to hepatitis. A virus revealed a rise in titres from 1 : 450 in the first days of the disease to 1 : 4 400 in the convalescent period. In the acute period the antibodies were represented by immunoglobulins M which circulate in the blood up to 4-5 months of convalescence. At 3-4 weeks after the onset of the disease, synthesis of immunoglobulins G begins which are anamnestic antibodies. The importance of specific immunoglobulins M in virus elimination from the host and their role in the early diagnosis of the disease have been demonstrated.

Excretion of hepatitis A virus (HAV) in the time course of the infectious process is described. Among 197 fecal specimens collected from 45 patients HAV was found in 40 (20.5%); in 51% of the patients the virus was isolated in the 1st week of the icteric period, in 25% in the 2nd week and in 4% in the 3rd week. The buoyant density in cesium chloride gradient of virus specimens obtained on the 1st-2nd day of jaundice was represented by one peak of 1.30-1.35 g/cm3, whereas the material collected at late periods of the disease had heterogeneous buoyant density in zones from 1.24 to 1.43 g/cm3. The immune electron microscopic studies showed the fecal specimens collected early in the disease to be richer in virus than those collected at later intervals.

The presence of HBe-antigen and antibody to it was determined in 233 subjects, including patients with acute hepatitis B, chronic hepatitis, and asymptomatic carriers of HBs-antigen. Preliminary concentration of blood sera was shown to increase the frequency of the e antigen detection in patients with acute hepatitis B to 33.3%, and in carriers to 1.7%. In parallel studies, no antibody to the e antigen was found in the acute period of hepatitis B. A direct correlation was found to exist between e-antigenemia and the severity of hepatitis B, with the exception of fatal outcomes. No significant differences in the frequency of the e antigen detection by sex, age of the patients, and titers of HBs-antigen in acute hepatitis B were established.
